ABSTRACT

BACKGROUND Natalizumab is an anti-integrin monoclonal antibody used as an alternative treatment regimen for patients with autoimmune disorders, especially multiple sclerosis and Crohn's disease. Natalizumab-induced liver injury has been rarely reported and may follow the first dose (with increases in liver enzymes usually after 6 or more days), or after multiple doses. In general, it is non-severe acute hepatitis (with a hepatocellular pattern) and autoantibodies can be positive, mainly anti-nuclear and anti-smooth muscle antibodies. CASE REPORT We are reporting the case of a 60-year-old woman diagnosed with multiple sclerosis previously treated with interferon-beta, dimethyl fumarate, and fingolimod, who presented jaundice 1 day after the first infusion of natalizumab. She had an early-onset acute hepatitis with aminotransferases levels higher than 1000 IU/L and total bilirubin almost 41 mg/dL. Anti-nuclear and anti-smooth muscle antibodies were positive and the histopathological analysis of the liver showed intrahepatic cholestasis associated with moderate necroinflammatory activity (subacute cholestatic hepatitis) and mild diffuse perisinusoidal fibrosis, which could be compatible with the hypothesis of drug-induced liver injury. The scenario of an autoimmune-like hepatitis led the medical team to start oral prednisone and she progressively improved in clinical and laboratory features. Serum levels of liver enzymes and bilirubin were normal within 3 months and there was no further increase after discontinuation of corticosteroid therapy. CONCLUSIONS Physicians should be aware of the risk of early-onset acute hepatitis in patients starting natalizumab, especially women with multiple sclerosis. Treatment with corticosteroid for a few months may be beneficial.

ABSTRACT

Objetivo: Analisar a prática e importância atribuída ao envolvimento do paciente na segurança do cuidado durante internação hospitalar, na percepção dos profissionais de saúde. Método: Estudo exploratório, descritivo, com abordagem qualitativa. Dados coletados em setembro de 2018, por meio de entrevista com 34 profissionais de saúde, em hospital público do Distrito Federal, Brasil. Análise norteada pela técnica de análise de conteúdo, modalidade temática, proposta por Bardin. Estudo aprovado pelo Comitê de Ética em Pesquisa. Resultados: Emergiram três categorias temáticas: Desenvolvendo a autonomia do paciente e família para o cuidado seguro, Práticas facilitadoras para envolver o paciente e familiar no cuidado seguro e Envolvimento do paciente: desafios para a consolidação no cenário hospitalar. Considerações finais: A prática e importância atribuída ao envolvimento do paciente hospitalizado no cuidado, aos profissionais de saúde, convergem com a literatura existente, porém apresenta limitações considerando a segurança do paciente. Objective: To analyze the practice and importance attributed to the involvement of the patient in the safety of care during hospitalization, in the perception of health professionals. Methods: Exploratory, descriptive study with a qualitative approach. Data collected in September 2018, through an interview with 34 health professionals, in a public hospital in the Federal District, Brazil. Analysis guided by the technique of content analysis, thematic modality, proposed by Bardin. Study approved by the Research Ethics Committee. Results: Three thematic categories emerged: Developing patient and family autonomy for safe care, Facilitating practices to involve the patient and family in safe care and Patient involvement: challenges for consolidation in the hospital setting. Conclusion: The incorporation of patient involvement for the safety of care in the practice of professionals revealed challenges related to the training of health professionals, adequacy of human and material resources and health literacy. Objetivo: Analizar la práctica e importancia atribuida a la implicación del paciente en la seguridad del cuidado durante la hospitalización, en la percepción de los profesionales sanitarios. Métodos: Estudio exploratorio descriptivo con abordaje cualitativo. Datos recolectados en septiembre de 2018, a través de una entrevista a 34 profesionales de la salud, en un hospital público del Distrito Federal, Brasil. Análisis guiado por la técnica de análisis de contenido, modalidad temática, propuesta por Bardin. Estudio aprobado por el Comité de Ética en Investigación. Resultados: Surgieron tres categorías temáticas: Desarrollar la autonomía del paciente y la familia para una atención segura, Facilitar prácticas para involucrar al paciente y su familia en la atención segura y Participación del paciente: desafíos para la consolidación en el ámbito hospitalario. Conclusión: La incorporación de la participación del paciente para la seguridad del cuidado en la práctica de los profesionales reveló desafíos relacionados con la formación de los profesionales de la salud, la adecuación de los recursos humanos y materiales y la alfabetización en salud. RESUMEN Objetivos: analizar la percepción de los profesionales de la salud sobre el significado y la práctica del involucramiento del paciente en la seguridad de la atención en Atención Primaria de Salud. Métodos: estudio exploratorio, cualitativo, desarrollado con 22 profesionales del Distrito Federal, Brasil. Se realizó una entrevista semiestructurada entre octubre y noviembre/2018. Se realizó análisis de contenido, según Bardin. Resultados: participaron enfermeras, médicos, dentistas, entre otros. Se revelaron las siguientes categorías: Significado de la participación del paciente en la seguridad de la atención; Factores que intervienen en la implicación del paciente en la seguridad asistencial; Estrategias para la participación del paciente en la seguridad de la atención; Calificación para la participación del paciente en la seguridad de la atención. Consideraciones Finales: el significado de la implicación del paciente para la seguridad asistencial se asoció con la corresponsabilidad y la atención centrada en el paciente. La práctica de los profesionales reveló factores intervinientes y el uso de estrategias de participación. Se identificó una brecha en la capacitación sobre participación del paciente en la seguridad de la atención.


RESUMO Objetivos: analisar a percepção dos profissionais de saúde sobre o significado e a prática do envolvimento do paciente na segurança do cuidado na Atenção Primária à Saúde. Métodos: estudo exploratório, qualitativo, desenvolvido com 22 profissionais no Distrito Federal, Brasil. Realizou-se entrevista semiestruturada entre outubro e novembro/2018. Realizou-se análise de conteúdo, conforme Bardin. Resultados: participaram enfermeiros, médicos, odontólogos, dentre outros. Revelaram-se as categorias: Significado do envolvimento do paciente na segurança do cuidado; Fatores intervenientes ao envolvimento do paciente na segurança do cuidado; Estratégias de envolvimento do paciente na segurança do cuidado; Qualificação para o envolvimento do paciente na segurança do cuidado. Considerações Finais: o significado do envolvimento do paciente para a segurança do cuidado foi associado à corresponsabilização e ao cuidado centrado no paciente. A prática dos profissionais revelou fatores intervenientes e uso de estratégias de envolvimento. Identificou-se lacuna na formação sobre envolvimento do paciente na segurança do cuidado.
