ABSTRACT

As úlceras venosas são lesões de membros inferiores causadas pela insuficiência venosa crônica e se caracterizam como um problema de saúde pública, sobretudo pela sua transcendência e vulnerabilidade. Alguns autores consideram que a convivência com uma úlcera venosa pode ocasionar mudanças e dificuldades em diversos aspectos na vida da pessoa acometida, com interferências negativas na qualidade de vida, paralelamente a sentimentos relacionados ao comprometimento da capacidade funcional e das relações sociais. No entanto, o sofrimento definido pelas próprias pessoas com úlceras venosas é pouco analisado na literatura. O objetivo do estudo foi compreender o sofrimento na perspectiva de pacientes com úlceras venosas, com enfoque em suas representações sobre o agravo. Trata-se de pesquisa qualitativa, fundamentada na Teoria das Representações Sociais, na perspectiva de Alain Giami. Foram realizadas entrevistas individuais abertas com 12 pacientes com úlceras venosas, que realizam curativos e acompanhamento em unidades básicas de saúde, no município de Belo Horizonte. Os dados foram interpretados com base na Análise Estrutural da Narração e desvelaram quatro categorias: contexto de vida; impacto da existência da ferida no cotidiano; representações sobre a ferida: causas, tratamento, cuidados, curativo; expectativas em relação à cura e tratamento da ferida. A análise dessas representações mostrou que o sofrimento, muitas vezes, é reflexo de uma assistência deficitária em vários aspectos, concordando com o que outros autores afirmam sobre a repercussão da assistência inadequada aos pacientes com úlceras venosas. Esta pode levar a que uma lesão permaneça anos sem cicatrizar, acarretando alto custo social e emocional. Venous ulcers are lesions of the lower limbs caused by chronic venous insufficiencyand are characterized as a public health problem. Some authors consider that livingwith a venous ulcer can cause changes and difficulties in several aspects in the life ofthe affected person, with negative interferences in the quality of life, in parallel withfeelings related to impairment of functional capacity and social relations. However, thepresence of suffering defined by the people themselves with venous ulcers is littleanalyzed in the literature. The objective of the study was to understand the sufferingfrom the perspective of patients with venous ulcers, focusing on their representationsabout the disease. This is a qualitative research, based on the Theory of Social Representations, from the perspective of Alain Giami. Individual interviews werecarried out with 12 patients with venous ulcers, who performed dressings and followup at Basic Health Units in the city of Belo Horizonte. The data were interpreted basedon the Structural Analysis of Narrative and revealed four categories: Context of life;Impact of the existence of the wound in daily life; Representations about the wound:causes, treatment, care, dressing; Expectations regarding wound healing andtreatment. The analysis of these representations showed that the suffering is often areflection of deficient assistance in several aspects, agreeing with what other authorsaffirm about the repercussion of the inadequate assistance to the patients with venousulcers that can lead to an injury to remain years without Healing, resulting in a highsocial and emotional cost. ABSTRACT

BACKGROUND: The best treatment option for patients with Graves' disease (GD) depends on each person's situation and how the differences between the treatment options matter to them in bringing resolution to their illness. The objective of this study was to develop and test an encounter decision tool (GD Choice) for patients and clinicians to engage in shared decision making about the treatment of GD. METHODS: GD Choice was developed using an iterative process based on the principles of interaction design and participatory action research. To evaluate the impact of the tool, a controlled before-after study was conducted, assessing the use of GD Choice versus usual care (UC). RESULTS: Sixty-eight patients were enrolled, 37 to UC and 31 to GD Choice. At baseline, the groups were similar. Treatment discussion length was similar in both arms. After their visit, patients in both groups had similar knowledge about the options, except for GD Choice patients knowing significantly more about the complications of treatment (correctly answered by 83% vs. 55%; p = 0.04). Compared with UC, patients in the GD Choice arm had greater involvement in decision making observed on video recordings of clinical encounters (mean OPTION scale score, 35% vs. 30%; p = 0.02), but reported similar levels of decisional comfort and participation in shared decision making. CONCLUSIONS: GD Choice increases engagement in the decision-making process and knowledge regarding intervention complications without increasing the length of consultation. These promising results support the conduct of a randomized trial of GD Choice versus UC in a large multicenter trial.

ABSTRACT

Esse estudo teve por objetivo refletir sobre o processo regulatório existente entre a Unidade Básica de Saúde e o Centro de Referência de Saúde do Trabalhador, tendo como linha mestra as ações de saúde previstas na Rede Nacional de Atenção Integral à Saúde do Trabalhador. A partir de uma revisão não sistemática da literatura, a autora procurou apresentar a trajetória da saúde do trabalhador até sua incorporação aos princípios do Sistema Único de Saúde, a fim de entender os principais desafios existentes entre o fluxo para o atendimento ao trabalhador, a partir de seu ingresso na atenção primária à saúde e o atendimento nos serviços disponibilizados pelo Centro de Referência de Saúde do trabalhador. Assim, tendo como base a literatura e as normas operacionais do sistema de saúde, a autora concluiu que a adoção da Regulação de Acesso se apresenta como uma ferramenta facilitadora do processo de articulação das ações de saúde existentes entre a atenção primária e o Centro de Referência de Saúde do Trabalhador.

ABSTRACT

Objetiva-se descrever como a integração ensino-serviço pode contribuir para a promoção de modos saudáveis de vida na Atenção Primária à Saúde (APS) no âmbito do Programa de Educação para o Trabalho pela Saúde, em uma Unidade Básica de Saúde (UBS). Trata-se de um relato de experiência pautado em estudo longitudinal descritivo, com abordagem qualitativa, realizado com usuários da UBS, efetivado em três etapas: sensibilização, incentivo à participação em grupos operativos e oficina culinária. A sensibilização possibilitou aos acadêmicos um contato direto com a comunidade, proporcionando aproximação entre população-acadêmicos-profissionais do local. Nas demais etapas, verificou-se interesse dos participantes, com relatos sobre os benefícios das atividades desenvolvidas. Concluiu-se que as ações realizadas oportunizaram a integração ensino-serviço e favoreceram a promoção de modos saudáveis de vida entre os usuários da APS


This study aimed to describe how the teaching-service integration can contribute to the promotion of healthy ways of life in Primary Health Care (PHC) in the of Educational Working Program for Health in a Basic Health Unit (BHU). This is an experience report guided longitudinal study with qualitative approach conducted with users of BHU, effected in three stages: awareness, encourage participation in operative groups and culinary workshop. The awareness allowed academics to a direct contact with the community, providing a closer approach between the academics, the professionals and the community of the place. During other stages there was interest from participants, with the reports on the benefits of the activities, that the participants showed interest. It was concluded that the actions taken optimized teaching-service integration and favored the promotion of healthy ways of life among users of the PHC.

INTRODUCTION: The present study was motivated by the need to systematically assess the research productivity of the Heart Institute (InCor), Medical School of the University of São Paulo, Brazil. OBJECTIVE: To explore methodology for the assessment of institutional scientific research productivity. MATERIALS AND METHODS: Bibliometric indicators based on searches for author affiliation of original scientific articles or reviews published in journals indexed in the databases Web of Science, MEDLINE, EMBASE, LILACS and SciELO from January 2000 to December 2003 were used in this study. The retrieved records were analyzed according to the index parameters of the journals and modes of access. The number of citations was used to calculate the institutional impact factor. RESULTS: Out of 1253 records retrieved from the five databases, 604 original articles and reviews were analyzed; of these, 246 (41%) articles were published in national journals and 221 (90%) of those were in journals with free online access through SciELO or their own websites. Of the 358 articles published in international journals, 333 (93%) had controlled online access and 223 (67%) were available through the Capes Portal of Journals. The average impact of each article for InCor was 2.224 in the period studied. CONCLUSION: A simple and practical methodology to evaluate the scientific production of health research institutions includes searches in the LILACS database for national journals and in MEDLINE and the Web of Science for international journals. The institutional impact factor of articles indexed in the Web of Science may serve as a measure by which to assess and review the scientific productivity of a research institution.

O presente trabalho apresenta as recomendações do International Committee of Medical Journal Editors (ICMJE) e da Organização Mundial da Saúde (OMS) para os registros de ensaios clínicos. Discute as conseqüências para as publicações científicas e o papel da BIREME para a inclusão da América Latina e Caribe nessas iniciativas. Descreve o portal de acesso à Plataforma Internacional de Registro de Ensaios Clínicos (ICTRP), coordenada pela OMS, e as principais iniciativas existentes, destacando as da América Latina e Caribe.


This document discusses the recommendations of the International Committee of Medical Journal Editors (ICMJE) and of the World Health Organization for clinical trials registering. The consequences for scientific publications and the role of BIREME for joining Latin America and Caribbean countries in these initiatives are discussed. The International Clinical Trials Registry Platform (ICTRP) coordinated by WHO as well as the existent initiatives are described, highlighting those from Latin America and the Caribbean.

ABSTRACT

O estudo aborda o câncer na mídia. É baseado na hipótese de que, há 10 anos, o câncer era considerado pela imprensa como um problema de interesse apenas de uma parcela da população. Atualmente, a partir de ações de comunicação do Instituto Nacional de Câncer (INCA) e das diversas instâncias envolvidas na questão, o câncer já seria considerado como uma questão de saúde pública. Assim, de forma a contribuir para guiar as ações de Comunicações no Instituto, seria importante checar se a percepção da imprensa sobre o câncer estaria em consonância com atual política do Ministério da Saúde. Essa política considera o câncer como uma questão de saúde pública. Também era necessário investigar a importância que a imprensa dá ao tema e se a cobertura da mídia em relação ao assunto evoluiu. Com esse objetivo, foram entrevistados 13 jornalistas dos principais meios de comunicação do país, todos com experiência na área de saúde. Paralelamente, foram analisadas 360 matérias publicadas em alguns dos mais importantes jornais e revistas do país, nos períodos de junho de 1997 a julho de1998, e junho de 2006 a julho de 2007.

OBJETIVO: Analisar as instruções aos autores contidas em revistas científicas nacionais que publicam artigos da área cirúrgica, buscando traçar um panorama da situação, bem como fazer recomendações para editores e autores. MÉTODOS: Foram analisadas as Instruções aos Autores de 20 revistas e classificadas segundo os Requisitos de Vancouver. Foram consultadas na SciELO, nos sites das revistas ou no último fascículo impresso disponível na coleção da BIREME. Os resultados foram analisados descritivamente, sendo considerada a freqüência de cada variável. RESULTADOS: 75 por cento recomendam a adoção dos Requisitos de Vancouver, embora nem sempre indiquem a versão atualizada; 90 por cento das revistas fizeram menção ao cumprimento de princípios éticos de pesquisa, 80 por cento ao processo de revisão por pares e 70 por cento ao conflito de interesses e à cessão dos direitos autorais. Os idiomas estrangeiros aceitos com maior freqüência foram inglês (80 por cento) e espanhol (30 por cento). Todas as revistas publicam artigos originais, seguidos pelos de revisão (90 por cento), relatos de caso (80 por cento), cartas ao editor (70 por cento) e artigos de atualização (55 por cento). A nomenclatura adotada para as seções das revistas é muito variada. CONCLUSÃO: Ainda que a liberdade editorial e independência de cada editor devam ser respeitadas, há critérios aceitos internacionalmente que devem ser observados. As tendências atuais, visando à priorização das revistas eletrônicas em acesso aberto, implicarão em modificações no processo de editoração das revistas científicas.

All journals publish original papers, followed by reviews (90 percent), case reports (80 percent), letters to the editor (70 percent), and clinical updates or continuing education (55 percent). The nomenclature for the sections varied among journals. CONCLUSION: Even though publishing freedom and independence of each publisher must be respected, there are internationally accepted criteria that must be observed. The current trends, towards the prioritization of open access electronic journals, will lead to important changes in the process of publishing scientific journals.

ABSTRACT

This study explores the distribution of international, regional and national scientific output in health information and communication, indexed in the MEDLINE and LILACS databases, between 1996 and 2005. A selection of articles was based on the hierarchical structure of Information Science in MeSH vocabulary. Four specific domains were determined: health information, medical informatics, scientific communications on healthcare and healthcare communications. The variables analyzed were: most-covered subjects and journals, author affiliation and publication countries and languages, in both databases. The Information Science category is represented in nearly 5% of MEDLINE and LILACS articles. The four domains under analysis showed a relative annual increase in MEDLINE. The Medical Informatics domain showed the highest number of records in MEDLINE, representing about half of all indexed articles. The importance of Information Science as a whole is more visible in publications from developed countries and the findings indicate the predominance of the United States, with significant growth in scientific output from China and South Korea and, to a lesser extent, Brazil.

ABSTRACT

Este artigo apresenta estudo exploratório sobre a distribuição da produção científica internacional, regional e nacional na área de informação e comunicação em saúde, referenciada nas bases de dados MEDLINE e LILACS, de 1996 a 2005. Para a seleção dos artigos, foi usada a estrutura hierárquica do vocabulário DeCS na área de Ciência da Informação. Foram determinados quatro domínios específicos: informação em saúde, informática médica, comunicação científica em saúde e comunicação em saúde. As variáveis analisadas foram: os assuntos e periódicos mais representativos, a produção por país de afiliação dos autores e por país de publicação e os idiomas, em ambas as bases de dados. Cerca de 5 por cento dos artigos correspondeu à categoria Ciência da Informação. Os quatro domínios tiveram aumento relativo anual em MEDLINE. O domínio Informática Médica foi o que apresentou maior número de registros em MEDLINE, representando pouco mais da metade de todos os artigos indexados. A importância da Ciência da Informação é mais visível nas publicações dos países desenvolvidos e os resultados confirmaram a predominância dos Estados Unidos e o crescimento significativo da produção científica da China e Coréia do Sul e, em menor escala, do Brasil.

ABSTRACT

Communication flow of scientific information has been restructured with the development of new technologies and the Internet and their impact on social relations worldwide. The production of scientific knowledge has also been influenced by these cultural, social and economic changes and has contributed to new patterns of scientific communication. The objective of the study was to present the traditional scientific communication model and its evolution to electronic scientific communication stimulated by the use of electronic media and Internet and networking. While the traditional model is based on printed publications, the new one focuses on electronic publishing and open unlimited access to published literature. The challenges faced are in using all the potential of electronic media for improving traditional communication flow of scientific information and defining policies to support the new model of scientific communication to ensure quality, preservation and dissemination of information as a common good.

ABSTRACT

Com o advento da Internet e das novas tecnologias houve uma reestruturação do fluxo da comunicação científica, assim como das relações sociais no mundo. A produção de conhecimento científico também foi influenciada por essas mudanças culturais, econômicas e sociais e contribuiu para transformar os padrões de comunicação científica. O objetivo do estudo é apresentar o modelo de comunicação tradicional e sua evolução para a comunicação científica eletrônica, estimulada pelo uso de meios eletrônicos e da Internet e pelo compartilhamento em rede. Enquanto o modelo tradicional está voltado para a publicação impressa, o novo modelo propõe a publicação eletrônica e o acesso livre e irrestrito ao material publicado. Os desafios consistem na utilização de todas as potencialidades dos meios de comunicação eletrônicos para aprimorar aspectos positivos do fluxo de comunicação científica tradicional e na definição de políticas que sustentem a nova estrutura do fluxo de comunicação científica, garantindo a qualidade, preservação e disseminação da informação como bem público.
