ABSTRACT

In the pleural empyema (PE) treatment, not depending on introduction of multiple operative procedures and the medicinal preparations application, some issues remain unsolved, including the infection agents verification, the most rapid bronchial fistula elimination and the lung volume restoration. The EP infection agents spectrum, their sensitivity to preparations were revealed, as well as the enhanced rate of the methicillin-resistant stamms (MRSA) and the microorganisms associations verification. A reduction of the infection agents sensitivity towards "simple" antibacterial preparations was established, so the physicians, treating PE, must prescribe "hard" antibiotics, what enhances its cost.

ABSTRACT

In was analyzed diagnostics and treatment results of 32 victims with duodenal injuries. The authors used 3% hydrogen peroxide solution for duodenal rupture diagnosis. Suggested surgical technique includes intestine intersection where it was broken and anastomosis with a loop of small intestine by using of Roux's method. Also it was done duodenal passage temporary shutdown by using of catgut purse-string suture on pyloric part of stomach.

The method of surgical treatment of duodenum traumatic damage was proposed, which prevent insufficiency anastomoses sutures, duodenostasis for elimination of damage section and fool temporary duodenum switch in early postoperative period provided.

ABSTRACT

Duodenal damages occurs rarely and constitutes 0.9-5% of all traumas of abdominal organs. The problem is actual because of the condition complicated diagnosis, the absence of standardized surgical tactics and high lethality (from 10-25 up to 40%). As a rule, duodenal damages are revealed intraoperatively. Specific symptoms, characteristic for duodenal damages, are absent. Results of treatment of 28 patients, suffering traumatic duodenal damages, were analyzed and low information value of modern diagnostic methods was noted. There was proved the efficacy of measures for diagnosis of duodenal trauma, using 3% solution of hydrogen peroxide.
