ABSTRACT

Lactococcus lactis cremoris is a facultative anaerobic, gram-positive coccus whose natural host is bovine livestock. It may form part of the normal human bacterial flora found in the oropharynx, the gastrointestinal tract and the vagina. This bacterium is essential in the food industry, where it is used in milk fermentation to obtain cheese, yoghurt, etc. Exposure to unpasteurised dairy products has thus been recognised as a risk factor for infection by this organism. It is generally considered to be non-pathogenic, although it appears that pathogenicity may be emerging. We present an atypical case of necrotising pneumonia caused by L. lactis cremoris.

ABSTRACT

Realizamos un análisis retrospectivo del perfil bacteriológico de los aislamientos de uropatógenos de una población pediátrica procesados en el Hospital Central de Asturias en los últimos cinco años. Escherichia coli fue el patógeno aislado con mayor frecuencia (57 por ciento), seguido de Enterococcus faecalis (11 por ciento), Pseudomonas aeruginosa (8 por ciento) y Proteus mirabilis (6 por ciento). La determinación de la sensibilidad a los antibióticos mostró que, en nuestro medio, las cefalosporinas de primera, segunda y tercera generación, al igual que la nitrofurantoína, deben ser consideradas la mejor opción para el tratamiento oral empírico de las infecciones de orina en la edad pediátrica, representando la fosfomicina una alternativa válida en determinados casos. En los niños hospitalizados las cefalosporinas de tercera generación se muestran como la opción más recomendable, pudiendo considerarse la asociación de gentamicina cuando exista riesgo evidente de complicaciones. En cualquier caso, el tratamiento debe ser revisado en función de la información bacteriológica y la respuesta al tratamiento (AU)

ABSTRACT

A case of endocarditis caused by Coxiella burnetii in a patient with an aortic Starr-Edwards prosthesis is described. The diagnosis was made by detecting high titres of antibody against Coxiella burnetii phase I antigens. After 15 weeks of chemotherapy with tetracycline and cotrimoxazole, aortic prosthetic valve replacement was carried out because of haemodynamic deterioration and he died 4 days later. The histologic picture of the excised valve was consistent with endocarditis. Coxiella burnetii should be considered in the differential diagnosis of culture-negative endocarditis in our country, especially when the patient has not receiving antibiotic treatment in the last 2 weeks.
