ABSTRACT

Durante la época nazi se llevaron a cabo de forma sistemática experimentos con seres humanos y la eliminación de individuos considerados como no productivos para la sociedad. En dichos actos se vieron involucrados médicos y dermatólogos cuyo nombre está vinculado de una forma u otra a nuestra especialidad, algunos de ellos muy conocidos, como Reiter. Con esta revisión queremos dar a conocer quién hubo detrás de muchos de los nombres que utilizamos para denominar enfermedades, signos clínicos, histológicos y síndromes en nuestra especialidad (AU)

ABSTRACT

Laser systems and other energy sources for home use are being developed and will have an exponential growth in the coming years. It is in the field of laser hair removal and hair regrowth where these lasers and intense pulsed light systems for home use are gaining most ground. There are few studies supporting their efficacy and safety and there are still no long term studies. Currently these systems do not require medical prescription and can be purchased in the same way as any over-the-counter product. Although they are not a substitute for professional medical systems, they will play an important role in the treatment of medical and cosmetic conditions, and dermatologists should know about them. Technological progress and commercial and market pressures will encourage the development of more sophisticated devices and broaden the cosmetic and medical indications. Appropriate use and rigorous long-term studies would be desirable to better define their safety and efficacy.

ABSTRACT

Los sistemas láser y otras fuentes energéticas de uso domiciliario se están desarrollando y van a tener un crecimiento exponencial durante los próximos años. Es en el campo de la depilación láser y la estimulación capilar donde mayor auge están teniendo estos sistemas láser y de luz pulsada intensa de uso domiciliario. Son escasos los estudios que avalan su eficacia y seguridad y no disponemos aún de estudios a largo plazo. Actualmente estos sistemas no requieren prescripción médica y se pueden adquirir como cualquier producto de venta sin receta. Aunque no sustituyen a los sistemas médicos profesionales, van a desempeñar un importante papel en el tratamiento de las patologías médico-estéticas, por lo que es conveniente el conocimiento de las mismas por parte de los dermatólogos. El desarrollo y avance tecnológico, así como la presión comercial y de los mercados, hará que aparezcan nuevos sistemas más sofisticados y para otras indicaciones dermoestéticas. Su correcta utilización y el desarrollo de estudios rigurosos a largo plazo sería deseable para establecer su eficacia y seguridad de forma más precisa (AU)

ABSTRACT

Creutzfeldt-Jakob disease is a prion spongiform encephalopathy that causes a rapidly progressive dementia whose neuropathological features are neuronal loss, proliferation of glial cells, and the presence of small vacuoles, which produces a spongiform appearance. Two cases detected in the year 2005 in the <> Hospital are presented. The first one is the case of a man who presented with intense malaise and weight loss, developing dementia in 4 months. The second one was a woman who presented with diplopia, headache and gait disturbances who developed severe neurological impairment with dementia in three months. In both cases the 14.3.3 protein in the cerebrospinal fluid was positive and the EEG showed a characteristic pattern. The genetic studies were negative and both were homozygous for methionine at codon 129. Given that the incidence of this disease in Castilla la Mancha is 0.6 cases/100000 inhabitants/year, the detection of 2 cases in less than a year in Talavera de la Reina (whose population is 150,000) surprised us, leading us to intensify surveillance.

ABSTRACT

La enfermedad de Creutzfeldt-Jakob (ECJ) es una encefalopatía espongiforme producida por priones que provoca una demencia progresiva cuyas características neuropatológicas son la pérdida neuronal, la proliferación de células gliales y las vacuolas, lo que origina el aspecto de esponja. Se presentan dos casos detectados en el año 2005 en el Hospital Nuestra Señora del Prado. El primero es el de un hombre que comenzó con intensa astenia y cuadro constitucional, desarrollando una demencia en 4 meses; y el segundo caso es el de una mujer que comenzó con diplopía, cefalea y alteraciones de la marcha, desarrollando un deterioro neurológico con demencia en tres meses. En ambos casos la proteína 14.3.3 del líquido cefalorraquídeo fue positiva y el electroencefalograma mostró un patrón característico. Los estudios genéticos fueron negativos, y ambos presentaban polimorfismo del codón 129, siendo homocigotos para la metionina. Nos ha sorprendido la aparición de dos casos en menos de un año en Talavera de la Reina y nos ha hecho aumentar la vigilancia

ABSTRACT

Four cases of malaria in patients who had travelled to Equatorial Guinea the previous weeks and had not received prophylaxis are presented. There were three men and one woman, two natives of Equatorial Guinea who had been living in Spain for a long time and the other two Spanish. Following a 15-30 day trip in Equatorial Guinea they all presented with fever, shivering, headache and diarrhea( two cases) five to ten days after their return. Only one patient presented splenomegaly. The thick blood smear showed plasmodia in all patients and P. falciparum was identified in only two patients. All of them were treated with doxycycline and quinine sulfate with a favourable outcome. Only one of the patients needed hospitalization. Following some aspects of the epidemiology, symptoms, diagnosis and treatment of this emerging disease are presented.

ABSTRACT

A case of a 30-year-old man is presented. He had a past medical history of asthma and presented with cough, shortness of breath, fever and chest pain. The chest X-ray showed cardiomegaly and a interstitial pattern. The echocardiogram revealed a severe pericardial effusion that required performing a pericardiocentesis. A CT scan showed mediastinal adenopathies and reticulonodular interstitial pattern. A bronchoscopy could not be completed because of non-tolerance. Finally a lung biopsy allowed us to reach a final diagnosis of lung adenocarcinoma. Following, some aspects of lung cancer in young people are commented, especially its incidence, histology, symptoms and a prognosis. A few aspects of the interstitial lung disease are also addressed and the differential diagnosis with lymphangitic carcinomatosis.

ABSTRACT

Presentamos cuatro casos de paludismo en pacientes que habían viajado en las semanas previas a Guinea Ecuatorial, sin haber realizado profilaxis. Eran 3 varones y una mujer, dos naturales de Guinea Ecuatorial, pero residentes en España desde hace largo tiempo y otros dos de nuestro país. Tras realizar un viaje de una duración entre 15 y 30 días por su país, presentaron entre los días 5 y 10 de su regreso, un cuadro de fiebre, escalofríos, cefalea y en dos casos diarreas. Se palpó esplenomegalia en un solo paciente. Se observaron plasmodium en la extensión de sangre periférica en los cuatro enfermos, identificándose Plasmodium falciparum, en dos pacientes. Todos fueron tratados con sulfato de quinina y doxiciclina, evolucionando favorablemente. Un paciente fue tratado en régimen de hospitalización. Posteriormente comentamos algunos aspectos epidemiológicos, clínicos, diagnósticos y de tratamiento de esta enfermedad emergente

ABSTRACT

We present a case of a 49 year old man who presented with a 2 month history of fever, nausea vomiting and weight loss (10 kg). He was admitted to the Hospital with the diagnosis of fever of unknown origin. A CT scan showed a wide thickening of the colonic wall (transverse colon and hepatic flexure) suggesting a colon tumor, and a solid mass in the left liver lobe. A colonoscopy was performed showing a circumferential neoplasm and biopsies were taken. The histopathology was informed as a diffuse large B cell lymphoma. Primary colon lymphoma is a rare gastrointestinal tumor that represents less than 1% of the gastrointestinal lymphomas. Based on this case, we reviewed the most recent articles about gastrointestinal lymphomas.

ABSTRACT

Presentamos el caso de un varón de 49 años que presentó un cuadro de fiebre de dos meses de evolución, acompañado de nauseas, vómitos y perdida de 10 kilogramos de peso. Con el diagnostico de fiebre de origen desconocido ingresó. En la TAC abdominal se objetivó un extenso engrosamiento de la pared de colon transverso y ángulo hepático, sugestivo de neoplasia de colon y una gran lesión sólida en lóbulo hepático izquierdo. Se realizó una colonoscopia observándose una neoplasia circunferencial, tomándose biopsias. La anatomía patológica fue informada de linfoma de células grandes difuso tipo B. El linfoma primario de colon es un tumor muy poco frecuente del tracto gastrointestinal, no superando el 1% en las series consultadas. Basándonos en este caso se hizo una revisión de los artículos más recientes sobre linfoma gastrointestinal

ABSTRACT

Apert's syndrome is an uncommon disease characterized by synostosis of extremities, vertebrae and skull. A clear association between Apert's syndrome and acne vulgaris with resistance to usual acne treatments has been described. A case of Apert's syndrome treated with oral isotretinoin with good results is reported and the pathogenic mechanisms of acne in this syndrome are discussed.
