ABSTRACT

RESUMO Objetivo analisar as evidências de validade de conteúdo da história em quadrinho "Tenho diabetes e agora?". Métodos pesquisa metodológica na produção de tecnologia em saúde no formato de história em quadrinhos para apoio a familiares e crianças internadas, com diagnóstico de Diabetes Mellitus tipo 1. Após elaboração da tecnologia educativa, ocorreu a etapa de validação do conteúdo com seis crianças e sete familiares, na qual se aplicou a escala Likert, com indicadores analisados por meio do Índice de Validade de Conteúdo. Resultados a tecnologia educativa apresentou bons índices de validação conforme avaliação das crianças (0,833) e familiares (1,000). Conclusão as crianças se identificaram com o personagem da historinha e concordaram ser uma ferramenta para o entendimento da doença, e os familiares afirmaram ser oportuno propagar a tecnologia em outros serviços de saúde. Contribuições para a prática há situações que permeiam aspectos relacionados ao cuidado com crianças com diabetes, portanto, a história em quadrinhos poderá contribuir para a qualidade da assistência multiprofissional a essa população acometida pelo Diabetes Mellitus.


ABSTRACT Objective to analyze the evidence of content validity of the comic strip "I have diabetes, what now?" Methods methodological research into the production of health technology in the form of a comic book to support family members and hospitalized children diagnosed with type 1 diabetes mellitus. After developing the educational technology, the content was validated with six children and seven family members using a Likert scale, with indicators analyzed using the Content Validity Index. Results the educational technology showed good validation indices according to the evaluation of the children (0.833) and family members (1.000). Conclusion the children identified with the character in the story and agreed that it was a tool for understanding the disease, and family members said that it would be useful to disseminate the technology in other health services. Contributions to practice there are situations that permeate aspects related to the care of children with diabetes, so the comic book could contribute to the quality of multi-professional care for this population affected by Diabetes Mellitus.

ABSTRACT

OBJETIVO: Avaliar o nível de burnout em Centro de Queimadura da capital federal brasileira. MÉTODO: Estudo transversal com aplicação de questionário sociodemográfico e da escala Maslach Burnout Inventory (MBI) à equipe de enfermagem com atuação em centro de queimadura. RESULTADOS: Participaram 30 profissionais, enfermeiros e técnicos, 80,0% com carga horária de até 40 horas semanais de trabalho. Os escores da síndrome de burnout foram de 46,7% quanto ao nível médio de Reduzida Realização Profissional e nível baixo de Despersonalização (46,7%), seguidos pela Exaustão Emocional, com 40% de nível alto e baixo, respectivamente. CONCLUSÕES: Verificou-se que dois (6,6%) profissionais apresentaram manifestações de burnout e que 60% tinha alto risco de burnout. Os achados são importantes para propor ações e estratégias para melhoria do atendimento e qualidade de vida para esses profissionais.


OBJECTIVE: To evaluate the level of Burnout in a Burn Center in the Brazilian federal capital. METHODS: Cross-sectional study with the application of a sociodemographic questionnaire and the Maslach Burnout Inventory (MBI) scale to the nursing team working in a burn center. RESULTS: 30 professionals, nurses and technicians participated, 80.0% with a workload of up to 40 hours/week of work. Burnout syndrome scores were 46.7% regarding the average level of Low Professional Achievement and low level of Depersonalization (46.7%), followed by Emotional Exhaustion with 40% of high and low levels, respectively. CONCLUSIONS: It was found that two (6.6%) professionals had manifestations of burnout and that 60% had a high risk of burnout. The findings are important to propose actions and strategies to improve care and quality of life for these professionals.

Métodos: estudo de caso-controle, comparando idosos com e sem diabetes. 218 pacientes entrevistados em clínicas ambulatoriais: geriatria e endocrinologia. Instrumentos validados foram utilizados para avaliação cognitiva e funcional. Resultados: pessoas com diabetes tiveram pontuação baixa em testes cognitivos. HbA1c e educação tiveram relação com perda de memória, comprometendo as atividades básicas e instrumentais do cotidiano. Conclusão: Este estudo permitiu avaliar que altos valores glicêmicos afetam a autonomia dessas pessoas idosas, comprometendo a cognição e desempenho funcional nas atividades da vida diária.


RESUMEN Objetivo: analizar la capacidad cognitiva y el desarrollo funcional de los ancianos con y sin diabetes. Métodos: estudio de casos y controles, comparando personas mayores con y sin diabetes. 218 pacientes entrevistados en consultas externas: geriatría y endocrinología. Instrumentos validados utilizados para la evaluación cognitiva y funcional. Resultados: personas con diabetes con puntuaciones bajas en pruebas cognitivas. La HbA1c y la educación se relacionaron con la pérdida de memoria, comprometiendo las actividades básicas e instrumentales de la vida diaria. Conclusión: evaluó que valores altos de glucemia afectan la autonomía de los ancianos, comprometiendo la cognición y el desempeño funcional en las actividades de la vida diaria.

ABSTRACT

Objetivos: Descrever a construção de tecnologia educacional sobre educação em saúde para crianças com diabetes mellitus tipo I. Método: Estudo metodológico embasado no processo de construção de tecnologia educacional de Teixeira e Mota. Este estudo apresentará a revisão literária e desenvolvimento da tecnologia educacional. Construção textual embasada no Construtivismo Piagetiano, demandas percebidas no diagnóstico situacional e na revisão da literatura. A criação das imagens foi elaborada por designer gráfico. Resultados: A construção textual atentou-se para adequação da linguagem e ilustrações. Evitou-se uso de termos científicos. As informações foram construídas em formato de história em quadrinho, os personagens foram desenhados para serem coloridos e há lacunas para preenchimento dos nomes. Conclusões: As tecnologias educacionais são ferramentas importantes utilizadas no processo de educação em saúde, facilita a atuação dos profissionais, especialmente enfermeiros, para uma assistência integral e envolve o paciente no processo ensino-aprendizagem. Objective: To describe the construction of educational technology on health education for children with type I diabetes mellitus. Methods: Methodological study based on the educational technology construction process of Teixeira and Mota. This study will present the literary review and development of educational technology. Textual construction was based on Piagetian Constructivism, demands perceived in situational diagnosis and literature review. The creation of the images was elaborated by graphic designer. Results: The textual construction focused on language adequacy and illustrations. Use of scientific terms was avoided. The information was built in a comic book format, the characters were designed to be colorful and there are gaps to fill in the names. Conclusion: Educational technologies are important tools used in the health education process, facilitate the performance of professionals, especially nurses, for comprehensive care and involve the patient in the teaching-learning process. Métodos: Estudio metodologico basado en el proceso de construcción de tecnología educativa de Teixeira y Mota. Este estudio presentará la revisión literaria y el desarrollo de TE. La construcción textual se basó en el constructivismo piagetiano, las demandas entendidas en el diagnóstico situacional y la revisión de la literatura. La creación de las imágenes fue elaborada por el diseñador gráfico. Resultados: La construcción textual se centró en la adecuación del lenguaje y las ilustraciones. Se evitó el uso de términos científicos. La información fue construida en un formato de cómic, los personajes fueron diseñados para ser coloridos y hay espacios en blanco para completar los nombres. Conclusión: las tecnologías educativas son herramientas importantes utilizadas en el proceso de educación en salud, facilitan el desempeño de los profesionales, especialmente las enfermeras, para una atención integral e involucran al paciente en el proceso de enseñanzaaprendizaje. ABSTRACT

BACKGROUND: This study aimed to assess the agreement and efficacy of the Ipswich Touch Test compared to the monofilament test in individuals with type 2 diabetes. MATERIALS AND METHODS: A cross-sectional and analytical study was conducted. The inclusion criteria were patients with type II diabetes (n = 250) who did not present ulcers or amputation in either foot. The exclusion criteria were as follows: patients who presented sequelae of cerebrovascular disease or other neurological pathologies, as well as diagnoses of malignancy, alcohol abuse, liver cirrhosis, hepatitis B, AIDS, hypothyroidism, chronic kidney disease or lupus erythaematosus, as these clinical conditions could influence or bias the results (Won and Park in Endocrinol Metab 31:230-238, 2016). Sensitivity, specificity, predictive values, likelihood ratios, and Kappa index were calculated. Other factors assessed were glycated haemoglobin and body mass index. RESULTS: Most of the participants were female (71.2%), and glycated haemoglobin (HbA1c) was greater than 7% in 54.4% of the patients. The mean age was 59.43 years, and the mean time since diagnosis was 12.38 years. The Kappa index was 0.819 (p < 0.001), and the Ipswich Touch Test had a sensitivity of 83.33%, a specificity of 97.66%, a positive predictive value of 85.71%, a negative predictive value of 97.21%, a positive likelihood ratio of 30.19%, and a negative likelihood ratio of 0.17%. The level of significance was 5% in this study. CONCLUSION: The Ipswich Touch Test resented good agreement and efficacy compared to the gold standard-the 10 g monofilament test.

ABSTRACT

Objetivo: Compreender o cotidiano de crianças e adolescentes com diagnóstico de diabetes mellitus tipo 1.. Métodos: Estudo exploratório com abordagem qualitativa. Para a coleta dos dados, desenvolveu-se entrevista semiestruturada realizada com 16 crianças e adolescentes atendidas, novembro de 2017 a fevereiro de 2018. As informações coletadas foram analisadas por meio do método de Análise de Conteúdo de Bardin. Resultados: Categorizados em quatro temáticas: significância do diabetes mellitus para as crianças e os adolescentes, sentimento relacionado às mudanças ocasionadas pelo diabetes mellitus, aspectos relacionados aos hábitos de vida e alterações significativas para a vida. Conclusões: Diabetes mellitus tipo 1 gera mudanças no contexto familiar, social e no estilo de vida e, com isso, há grande impacto psicológico à criança e ao adolescente. Por isso, é importante que o profissional de enfermagem possa identificar os desdobramentos desta patologia em crianças e adolescentes e se mostre parte da rede de apoio para eles. (AU)


Objective: To understand the daily life of children and adolescents diagnosed with type 1 diabetes mellitus. Methods: Exploratory study with a qualitative approach. For data collection, a semi-structured interview was conducted with 16 children and adolescents. The information collected was analyzed using the Bardin Content Analysis Method. Results: There were four themes: significance of diabetes mellitus for children and adolescents, feeling related to changes caused by diabetes mellitus, aspects related to life habits and significant changes in life. Conclusions: That type 1 diabetes mellitus generates changes in the family, social and lifestyle context and, with this, there is a considerable psychological impact on the child and the adolescent. Therefore, it is important that the nursing professional can identify the consequences of this pathology in children and adolescents and be part of the support network for them. (AU)


Objetivo: Comprender la vida diaria de niños y adolescentes diagnosticados con diabetes mellitus tipo 1. Métodos: Estudio exploratorio con abordaje cualitativo. Para la recolección de los datos, se desarrolló una entrevista semiestructurada realizada con 16 niños y adolescentes. La información recopilada se analizó mediante el Método de Análisis de Contenido de Bardin. Resultados: Clasificaron en cuatro temáticas: significancia de la diabetes mellitus para los niños y adolescentes, sentimiento relacionado con los cambios ocasionados por la diabetes mellitus, aspectos relacionados con los hábitos de vida y cambios significativos para la vida. Conclusiones: La diabetes mellitus tipo 1 genera cambios en el contexto familiar, social y en el estilo de vida y, con ello, hay gran impacto psicológico al niño y al adolescente. Por eso, es importante que el profesional de enfermería pueda identificar los desdoblamientos de esta patología en niños y adolescentes y se muestre parte de la red de apoyo para ellos. (AU)

ABSTRACT

Background: Approximately 2-4% of individuals worldwide with diabetes mellitus have foot ulcers. This study aims to assess the factors affecting the outcomes of severe foot ulcers in diabetic individuals. Methods: An analytical prospective cohort study was conducted from March 1st, 2015, to March 1st, 2017. A total of 34 individuals was selected. The study included patients with foot ulcers below the ankle who were at risk of amputation. All tests used a <5% level of significance and confidence interval of 95%. A Pearson's chi-squared test and binary multiple regression were performed to assess the factors related to healing. Results: Only 11.7% of the individuals required amputation; ulcers classified as 2/B according to the University of Texas Diabetic Foot Ulcer Classification System healed before the 1/B ulcers. Neuropathic ulcers were the most prevalent (58.8%); 61.8% healed after 1 year. Most of the individuals were overweight, 47.1% had reduced glomerular filtration rates, and 78.8% had glycated hemoglobin >7%. Body mass index and osteomyelitis were the two significant variables in logistic regression. Conclusions: In this study, osteomyelitis was the main complication related to the risk of amputation, and elevated body mass index and osteomyelitis were the significant factors that induced a slower healing time.

RESULTS: painful diabetic peripheral neuropathy was present in 37 (75.5%) of the individuals with neuropathy. Deformities and loss of protective plant sensibility were related to neuropathy (p=0.014 and p=0.001, respectively). Of the 40 (34.2%) individuals in the sample who presented peripheral arterial disease, 26 (65%) presented calcification risk. CONCLUSION: signs of painful peripheral polyneuropathy, peripheral arterial disease, deformities, loss of protective plantar sensibility, and dry skin were identified as risk factors for ulceration.

RESUMEN Objetivo: Identificar los factores de riesgo para la ulceración del pie de acuerdo con el rastreo de neuropatía diabética periférica y la enfermedad arterial periférica en los individuos diabéticos tipo I y II asistidos en los centros de referencia del Distrito Federal, Brasil. Método: Estudio transversal y analítico, con la evaluación de 117 individuos en ambulatorios del Distrito Federal. Las variables continuas fueron comparadas por medio de la prueba de Mann-Whitney, y las variables categorizadas, de las pruebas de chi cuadrado para los análisis univariados y la regresión logística para los análisis multivariados. Resultados: La neuropatía diabética periférica dolorosa estuvo presente en 37 (el 75,5%) de los individuos con neuropatía. Las deformidades y la pérdida de sensibilidad protectora plantar tuvieron relación con la neuropatía (p=0,014 y p=0,001, respectivamente). De los 40 (el 34,2%) individuos de la muestra con enfermedad arterial periférica, 26 (el 65%) presentaron riesgo de calcificación. Conclusión: Identificadas las señales de polineuropatía dolorosa periférica, la enfermedad arterial periférica, las deformidades, la pérdida de sensibilidad protectora plantar y la piel seca como los factores de riesgo para ulceración.

As variáveis contínuas foram comparadas por meio do teste de Mann-Whitney, e as variáveis categorizadas, dos testes de qui-quadrado para análises univariadas e regressão logística para análises multivariadas. Resultados: a neuropatia diabética periférica dolorosa esteve presente em 37 (75,5%) dos indivíduos com neuropatia. Deformidades e perda de sensibilidade protetora plantar tiveram relação com neuropatia (p=0,014 e p=0,001, respectivamente). Dos 40 (34,2%) indivíduos da amostra com doença arterial periférica, 26 (65%) apresentaram risco de calcificação. Conclusão: identificados sinais de polineuropatia dolorosa periférica, doença arterial periférica, deformidades, perda de sensibilidade protetora plantar e pele seca como fatores de risco para ulceração.

ABSTRACT

Este estudo procurou compreender a vivência de professores no processo de inclusão de crianças com estomia, tendo como referencial a Fenomenologia Sociológica. Foram realizadas entrevistas, com cinco professores, no período entre dezembro de 2015 e janeiro de 2016. A coleta de dados foi efetuada por narração do professor, posterior transcrição e análise dos dados. Emergiram quatro categorias temáticas: sentimentos vividos pelos professores; fatores que interferem nos cuidados; estratégias eficazes para o cuidado; estratégias para melhoria dos cuidados. Os professores, valorizaram a integração da criança com estomia na escola; consideram que esta ocorre com a modificação da formação pedagógica; e apontaram a necessidade da presença do enfermeiro nas escolas. Os resultados mostram a importância do desenvolvimento de ações e estratégias de apoio aos professores e aos alunos com estomia no contexto de uma escola inclusiva, considerando que o enfermeiro escolar é determinante para a promoção em saúde destas crianças.


This study has sought to understand the experience of teachers in the process of inclusion of children with stoma, being Sociological Phenomenology the reference. Interviews were conducted with five teachers between December 2015 and January 2016. Data collection was carried out from the narration of teachers, with later transcription and data analysis. Four thematic categories emerged: feelings experienced by teachers, factors that interfere with care, effective strategies for care, and strategies to improve care. The teachers valued the integration of the child with stoma in the school; they consider that this happens with the modification of the pedagogical formation, and they also pointed out the need for the presence of nurses in schools. The results show the importance of the development of actions and strategies to support teachers and students with stoma in the context of an inclusive school, considering that the school nurse is a determining factor for the health promotion of these children.

ABSTRACT

Objetivou-se compreender o atendimento ao binômio mãe-bebê em um Centro de Parto Normal da rede pública, com a especificidade do parto anterior na rede hospitalar. Trata-se de estudo descritivo com abordagem qualitativa, realizado com oito puérperas de Brasília, por meio de entrevista com roteiro semiestruturado. Os dados foram colhidos entre fevereiro e julho de 2015. Adotou-se a Análise do Conteúdo, Modalidade Temática, para o tratamento dos dados. Delinearam-se três categorias: "As Boas Práticas da Assistência e a Autonomia das Parturientes", "Cuidado versus Descaso" e "Insensibilidade com a Dor Alheia". Constatou-se que, no Centro de Parto Normal, houve o respeito às boas práticas na assistência ao parto e contentamento com o atendimento diferenciado. O estudo evidenciou o empoderamento das mulheres pela busca de informações sobre o processo de parir e seus direitos, e reforça a construção de mais Centros de Parto Normal, valorizando as boas práticas no processo de parturição (AU).


The present study aimed to gain insight on care to the mother-baby binomial in a Center for Normal Delivery of Brazil's public health network, for women who had previous deliveries in the hospital network. Descriptive qualitative study with eight women resident in Brasilia who had just given birth, through semi-structured interviews. Data was collected between February and July 2015. For data treatment, content analysis in thematic modality was used. Three categories emerged: "Good Delivery and Birth Care Practices and Parturients' Autonomy in the Decision-making Process", "Care versus Neglect" and "Indifference to the Suffering of Others". The Center for Normal Delivery implemented good delivery and birth care practices, and the participants were satisfied with the care provided to them. The study showed the women's empowerment to become more involved and informed on the process of giving birth and on their rights and stresses the need for the establishment of more Normal Birth Centers in the country to ensure good birth care practices (AU).


Estudio cuya finalidad fue comprender el atendimiento al binomio madre-bebé en un Centro de Parto Natural de la red pública, con especificidad del parto anterior en la red hospitalar. Es un estudio descriptivo de abordaje cualitativo, realizado con ocho puérperas de Brasília, por medio de entrevista con guion semiestructurado. Los datos fueron obtenidos entre febrero y julio de 2015. Se utilizó el Análisis de Contenido, Modalidad Temática, para el tratamiento de las informaciones. Resultaron tres categorías: "Buenas Prácticas de la Asistencia y Autonomía de las Parturientas", "Cuidado versus Descaso" y "Insensibilidad con el Dolor Ajeno". Se concluyó que, en el Centro de Parto Natural, hubo respeto a las buenas prácticas en la asistencia al parto y satisfacción con el atendimiento diferenciado. El estudio evidenció el empoderamiento de las mujeres por la búsqueda de informaciones acerca del proceso de parir y sus derechos, y destaca la importancia de la construcción de más Centros de Parto Natural, valorando las buenas prácticas en el proceso de parto (AU).

Objetivo: Avaliar os aspectos relativos à qualidade de vida dos pacientessubmetidos a transplante renal no Hospital Regional da Asa Norte. Métodos: Trata-se de um estudo transversal, descritivo, com abordagemquantitativa. A amostra constituiu-se por pacientes que realizaramtransplante renal no Hospital Regional da Asa Norte, no período de março de 2008 a setembro de 2012 (n=21). Utilizaram-se dois instrumentos: a. ficha de avaliação sociodemográfica, econômica e de identificação das dificuldades evidenciadas pelos pacientes no período0 pós-transplante; b. questionário WHOQOL-bref, para avaliação da qualidade de vida.Resultados: Observou-se que 81% eram homens; faixa etária entre 26 e45 anos; 62% com ensino médio; 52,4% casados; 66,7% desempenhavam alguma atividade; 42,9% eram naturais do Distrito Federal; 66,7% residiam no Distrito Federal; 52,4% com renda familiar menor ou igual a dois salários mínimos; tempo de diálise anterior ao transplante de 3 a 120 meses; tempo de transplante de 24 a 54 meses; 90,5%, 85,7%, 71,4% não se opuseram quanto ao uso das medicações, controle da dietae controle ambulatorial, respectivamente, no pós-transplante. Os escores obtidos na escala WHOQOL-bref foram: domínio psicológico, 76,0; relações sociais, 74,0; meio ambiente, 60,75; e físico, 60,5. O valor médio de Q1 e Q2 foram, respectivamente, 81,0 e 89,25.Conclusão: Todos os domínios do WHOQOL-bref obtiveram escoremédio acima de 60,0, sendo os melhores resultados obtidos nos domínios psicológico e relações sociais. O domínio que mais influenciana percepção da qualidade de vida é o domínio físico.Ao relacionar asvariáveis com o WHOQOL-bref, evidenciou-se p>0,05.

Results: It was observed that 81% were men, age group between 26 and 45 years; 62% with secondary education; 52.4% married, 66.7% played some activity; 42.9% were born in the Distrito Federal, 66 , 7%lived in Distrito Federal; 52.4% with household income less or equal to two minimum wages; dialysis` time before transplantation around3-120 months; transplant time around 24-54 months, 90.5%, 85.7%, 71.4% were not opposed to the use of medications, diet and outpatient control, respectively, in the post-transplant. The obtained scores on thescale WHOQOL-bref were: the psychological domain. 76.0; social relations,74.0; environment, 60.75, and physical, 60.5. The average value of Q1 and Q2 were, respectively, 81.0 and 89.25. Conclusion: All the domains of WHOQOL-bref achieved an average score above 60.0, being the best results in the psychological and socialrelations domains. The area that most influences the perception of life`s quality is the physical domain. By linking the variables with the WHOQOL-bref, there was p> 0.05.
