ABSTRACT

CASO CLÍNICO: Presentamos a un paciente de 77 años, con un cuadro de necrosis retiniana aguda con afectación bilateral; la reacción en cadena de la polimerasa (PCR) de la muestra de humor acuoso fue positiva al virus de varicela zóster. En su evolución desarrolla vasculitis de Kyrieleis al mes de inicio del tratamiento y con el análisis por PCR negativo. DISCUSIÓN: La PCR es un método rápido, con una sensibilidad y especificidad del 97%. La actuación debe ser urgente por la rapidez de la progresión. El famciclovir oral es buena alternativa por su mejor biodisponibilidad


CASE REPORT: The case is reported of acute retinal necrosis with bilateral involvement due to Varicella Zoster virus in a 77 year-old man. Polymerase chain reaction (PCR) of aqueous humor was positive for Varicella Zoster virus (VZV). He developed a Kyrieleis’ vasculitis a month after the starting treatment, when the PCR analysis was negative. DISCUSSION: PCR is a quick and safe technique, with a high sensitivity and specificity of 97%, useful to diagnose and monitor the viral activity. The intervention must be urgent, due to the dramatically rapid evolution. Oral famciclovir oral is good alternative owing to its bioavailability

ABSTRACT

Objetivo/Método: Destacamos la posibilidad de descubrir macroaneurismas en el momento que debutan y debatir sus formas de tratamiento. Resultados/Conclusiones: Presentamos el caso de una paciente diabética tipo II de 6 años de evolución que acudió a revisión rutinaria. La exploración del ojo izquierdo evidenció una lesión de aspecto microquístico con exudación asociada. La agudeza visual del ojo izquierdo era 0,4; con estenopeico mejoró a 0,7. Mediante AFG del ojo izquierdo se diagnosticó Telangiectasias yuxtafoveolares. Meses después se realizó una nueva AFG en el ojo izquierdo, descubriéndose la aparición de 'novo' de un macroaneurisma arterial en el sector nasal superior. Dado su emplazamiento, no fue tratado (AU)

ABSTRACT

OBJECTIVES/METHODS: With this case we try to emphasize the possibility to detect aneurysms and discuss their treatment. RESULTS/CONCLUSIONS: We report the case of a patient with Type II Diabetes of six years duration who came for a routine review. At examination, a macular lesion in the left eye was found (microcystic-like appearance with associated exudation). The left eye acuity was 0,4 which improved with glasses to 0,7. After performing a fluorescein angiography, we diagnosed it as yuxtafoveolar teleangiectasias and it was studied in order to evaluate laser treatment. Some months later, we performed another fluorescein angiography and an arterial macroaneurysm <> located in the nasal superior quadrant was noticed. Owing to its location, it was not treated.
