ABSTRACT

The root fractures occur relatively rarely and account for 0.5-2.5% of all dental injuries. The fracture type, severity of dislocation, mobility of fragments and diastasis have negative influence on the healing process. The aim of this study was to describe the treatment and the 42-month follow-up period of three trauma histories in a 12-year-old patient in 1 year and to emphasize the negative effects of recurrent traumas on the healing pattern and prognosis of root fractures.

ABSTRACT

An increased wish by patients to maintain their dentition and recent advances in dentistry have led to the treatment and preservation of teeth that once would have been removed. Consequently, hemisections, root amputations, and bisections are procedures frequently used. The prognosis of endodontically treated teeth depends on the type of reconstruction as well as the success of the endodontic treatment. Along with technological improvements in adhesive resins, cements, and composite restoratives, the evolution of fiber-reinforced posts enables the rehabilitation of endodontically treated teeth with greater esthetics and virtually no predisposition to root fracture. The purpose of this report was to describe the endodontic, surgical, and restorative treatments of a mandibular first molar with an endodontically and periodontally diseased mesial root, for functional, orthodontic, and aesthetic reasons. This case report shows that the clinical application of fiber-reinforced composite resins in hemisectioned teeth may be a treatment choice for highly damaged teeth with endodontic and/or periodontal lesions. It also emphasizes the advantages of the elimination of orthodontic and prosthetic treatment needs by different treatment strategies, especially in adolescent patients.

ABSTRACT

AIM: To investigate the incidence, aetiological factors and results of dental trauma on primary dentition of the children referred to the Süleyman Demirel University, Department of Paedodontics in a two year period. DESIGN: A retrospective study. PARTICIPANTS: Eighty four children (50 boys and 34 girls) with 159 affected deciduous teeth. RESULTS: The incidence of trauma was slightly higher in boys (59.5%). The age of the children ranged from 9 months to 6 years and the peak ages for trauma were 3 years and 5 years of age in respective genders. Falls were the most frequent cause of traumatic dental injuries (29.7%) and luxations were the most common type of trauma with 135 cases (84.9%). The most frequently performed treatment options were examination and follow up only (37.7%). Half of the cases (51.1%) were referred to our clinic in the first week after the trauma occurred. CONCLUSION: The results of this study suggest that a preventive educational programme should be instituted, directed at parents and teachers at nursery schools, in order to inform them about what to do in dental trauma cases and the importance of immediate attendance for dental care.

ABSTRACT

PURPOSE: Various space maintainers are used in pediatric dentistry. However, their construction requires time-consuming laboratory procedures. Recently, fiber-reinforced composites (FRC) have been introduced and used in different branches of dentistry. The objective of this study was to assess long-term results for a newly developed space maintainer as an alternative procedure for practitioners. METHODS: This study used Splint-it (Jeneric/Pentron, Wallingford, Conn), a FRC, to prepare a newly developed space maintainer chairside in 1 appointment. A total of 40 space maintainers were applied to 29 children (14 girls, 15 boys) between 7 to 14 years old (mean +/- SD = 10 years, 1 month +/- 1 year, 11 months) to protect the space of their early extracted first and second primary molar teeth. For 4 children, space maintainers were prepared with artificial teeth to restore the anterior teeth loss, which occurred due to trauma. To protect the space until the fixed partial dentures were constructed, 5 space maintainers were applied to 3 children who had 2 permanent first molar teeth extracted. Appliances were observed for up to 2 years. RESULTS: Twenty-nine (73%) space maintainers were dislodged at the end of the sixth month. The space maintainers placed on primary teeth (1 or both abutments) showed the highest failure rate (94%). CONCLUSIONS: This study suggested that Splint-it space maintainers can be accepted as successful appliances only for short periods. Prolonged use of this material for space maintenance in children must be further evaluated.
