ABSTRACT

Introducción El queratocono es la ectasia corneal más frecuente y la segunda causa de queratoplastia en el mundo. Las características clínicas y epidemiológicas del queratocono no han sido apenas estudiadas en España y, hasta la fecha, no se ha hecho ningún trabajo en Asturias. Material y métodos Se realizó un estudio transversal analizando todos los casos de queratocono diagnosticado en las primeras consultas de la sección de Córnea y Superficie de los centros Hospital Universitario Central de Asturias (Oviedo) y Hospital Universitario de Cabueñes (Gijón) entre 2017 y 2020. A todos los pacientes se les recogieron datos demográficos, agudeza visual y se les realizó una topografía corneal. Resultados Se incluyeron un total de 112 pacientes (42,0% mujeres y 58,0% hombres). La edad media era de 36,84±15,59, teniendo un 39,3% una edad superior a 40 años. Un 31,8; 28,0; 15,0 y 25,2% se encontraban en los estadios i, ii, iii y iv, respectivamente, en la escala Amsler-Krumeich. En un 7,5% se implantaron segmentos corneales, en un 4,7% se realizó cross-linking y en un 13,2% se realizó queratoplastia. Conclusión Aproximadamente un 40% de los queratoconos se detectan en estadios avanzados y otro 40% a una edad superior a los 40 años. Un 13% de los mismos requieren queratoplastia, por lo que sigue siendo una indicación frecuente en nuestro medio. Es necesario, por tanto, realizar un diagnóstico precoz para actuar antes de que exista una pérdida visual irreversible. (AU)

ABSTRACT

INTRODUCTION: The correction of aphakia when there is no adequate capsular support remains a therapeutic challenge. The use of retroiridial fixation lenses has been extended given their lower complication rate compared to other available options. MATERIAL AND METHODS: Retrospective study including all cases operated with an Artisan® retropupillary aphakia implant. RESULTS: 33 eyes were included out of a total of 28 patients. The follow-up period has been 38.55 months (1-96). 32.3% had a lens dislocation and 67.7% had an intraocular lens dislocation. The previous mean best corrected visual acuity (BCVA) was 1.18 ± 0.79 logMAR and post-intervention 0.36 ± 0.62 (p < 0.01). 93.8% of the patients presented a final BCVA equal to or better and 62.5% an improvement of 3 or more lines. The most frequent complication was corectopia (31.3%) and hypotony in the immediate postoperative period (21.9%). An epiretinal membrane (ERM) developed in 18.8% and cystic macular oedema (CME) in 9.4%. The presence of complications in the postoperative period did not statistically influence the final BCVA. CONCLUSIONS: The retropupillary Artisan® lens allows the correction of aphakia with satisfactory visual results and a low rate of complications.

ABSTRACT

Varón de 51 años con diagnóstico de adenocarcinoma de esófago estadio IV. Es referido desde oncología médica para valoración oftalmológica urgente por llamativo exoftalmos izquierdo de dos semanas de evolución además de diplopía binocular. En la exploración oftalmológica destaca la presencia de un exoftalmos axial no reductible y una limitación de la motilidad ocular a la levoducción. Se realiza una tomografía axial computarizada craneal urgente donde se objetiva una masa orbitaria izquierda polilobulada y heterogénea, intra- y extraconal, que engloba al músculo recto interno, compatible con metástasis, además de otra pequeña masa extraconal en la base de la órbita contralateral. Se indica entonces tratamiento con radioterapia paliativa. Las metástasis en los músculos extraoculares son un hallazgo muy poco frecuente, pero deben sospecharse ante un caso de exoftalmos unilateral y, de ser necesario, remitir al paciente para realizar las pruebas complementarias correspondientes

A computed tomography scan was performed that showed a multiple lobed, intra-and extra-conal, heterogeneous left orbital mass, that surrounded the internal rectus muscle, compatible with metastasis, as well as another small extraconal mass at the base of the contralateral orbit. Palliative radiotherapy was then indicated. Metastases in the extraocular muscles are a very rare finding, but should be suspected in a case of unilateral exophthalmos and, if necessary, refer the patient to have the corresponding complementary tests performed

The patient was referred by the Oncology Department urgently for a same-day assessment by an ophthalmologist due to two weeks of severe unilateral exophthalmos and binocular diplopia.
