ABSTRACT

Low body weight and advanced age are among the best predictors of osteoporosis. Osteoporosis Self-Assessment Tool (OST) values are calculated by a simple formula [(body weight in kilograms - age in years) × 0.2] to identify patients at increased risk of osteoporosis. In our recent single-center study, we demonstrated an association between OST and poor outcomes in postmenopausal women after transcatheter aortic valve replacement (TAVR). We aimed to investigate the impact of osteoporotic risk in men with aortic stenosis who underwent TAVR in a large cohort. In this multi-center study, 1,339 men who underwent TAVR between April 2010 and July 2023 were retrospectively analyzed. Women were excluded from the present study. All patients were deemed appropriate for TAVR after a review by a multidisciplinary team. Baseline characteristics of patients were compared by dividing patients into three tertiles, based on the OST value: ≤ - 6.16, - 6.16 to - 4.14, and - 4.14 < for tertiles 1, 2, and 3, respectively. Primary endpoint was all-cause mortality after TAVR. Tertile 1 (patients with the lowest OST values) included older patients with smaller body mass index, lower hemoglobin and albumin levels. In addition, they had greater clinical frailty scale, slower 5-meter walk test, weaker hand grip strength, and more cognitive impairment, indicating increased frailty. They were more severely symptomatic, with lower ejection fractions, smaller aortic valve areas, and more tricuspid regurgitation than were patients in the other two groups. Multivariate analysis revealed that OST tertiles 3 was associated with decreased risk of all-cause mortality (hazard ratio, 0.66; 95% confidence interval, 0.48-0.90), compared with OST tertile 1 as a reference. For OST tertiles 1, 2, and 3, the estimated 1-year survival rates of all-cause mortality post-TAVR were 83.6% ± 1.9%, 91.1% ± 1.4%, and 93.1% ± 1.3%, respectively, (log-rank, p < 0.001). In conclusions, in men as same as women, osteoporotic risk assessed by OST values was overlapped with increased frailty. The simple OST formula was useful for predicting all-cause mortality in patients undergoing TAVR in large registry datasets.

ABSTRACT

Patients with hypertrophic cardiomyopathy (HCM) often have atrial fibrillation, and empiric anticoagulation is recommended in these patients, regardless of other risk factors. However, anticoagulation is not recommended for patients who require hemodialysis (HD) because of the high bleeding risk. We herein report a case of left atrial appendage closure (LAAC) using the Watchman FLX system for a dilated phase HCM patient complicated by persistent atrial fibrillation and requiring HD. LAAC with the Watchman FLX system may be an alternative to antithrombotic medications in patients with dilated HCM complicated by atrial fibrillation and requiring HD.

ABSTRACT

We performed X-ray absorption studies for the electrolytes of a Ti-Mn redox flow battery (RFB) to understand the redox reaction of the Ti/Mn ions and formation of precipitates in charged catholyte, because suppression of the disproportionation reaction is a key to improve the cyclability of Ti-Mn RFB and enhance the energy density. Hard X-ray absorption spectroscopy with a high transmittance and soft X-ray absorption spectroscopy to directly observe the 3d orbitals were complementarily employed. Moreover, the Ti/Mn 3d electronic structure for each precipitate and solution in the charged catholyte was investigated by using scanning transmission X-ray microscopy: the valence of Mn in the precipitate is mostly attributed to 4+, and the solution includes only Mn2+ . This charge disproportionation reaction should occur after the Mn ions in the catholyte should be oxidized from Mn2+ to Mn3+ by charge.

ABSTRACT

A 60-year-old woman with a history of hypothyroidism was referred to our hospital for shortness of breath and a left ventricular ejection fraction (LVEF) of 13%, which required continuous dobutamine injection with intra-aortic balloon pump support. An endomyocardial biopsy obtained from the right ventricle revealed an infiltration of giant cells and eosinophils, indicating giant cell myocarditis. In addition to heart failure treatment, combined immunotherapy with steroids, tacrolimus, and intravenous immunoglobulin was administered. Transthoracic echocardiography demonstrated a dramatic improvement in the LVEF after this therapy, and the patient was discharged home without symptoms on day 72.

ABSTRACT

Although cardiovascular involvement is a well-known complication correlated with a poor prognosis in patients with systemic sclerosis, there are few reports on valvular heart disease. Forty patients with systemic sclerosis were retrospectively analyzed. Valvular heart disease was found in six patients, five of whom had severe tri-leaflet aortic stenosis. Three of these 5 patients were ≤71 years old. Two frail elderly patients who underwent transcatheter aortic valve replacement died within two years. Premature-onset aortic stenosis is not uncommon in patients with systemic sclerosis. When considering mechanical intervention, the evaluation of frailty is important.

ABSTRACT

Organoboron complexes are of interest as chromophores for dye sensitizers owing to their light-harvesting and carrier-transporting properties. In this study, compounds containing boron ß-ketoiminate (BKI) as a chromophore were synthesized and used as dye sensitizers in dye-sensitized solar cells. The new dyes were orange or red crystals and showed maximum absorptions in the 410-450 nm wavelength region on titanium dioxide substrates. These electrodes exhibited maximum efficiencies of over 80% in incident photon-to-current conversion efficiency spectra, suggesting that the continuous process of light absorption-excitation-electron injection was effectively performed. Open-circuit photovoltages were relatively high owing to the large dipole moments of the BKI dyes with a linear molecular structure. Thus, a maximum power conversion efficiency of 5.3% was successfully observed. Comparison of BKI dyes with boron ß-diketonate dyes revealed certain differences in solution stability, spectral properties, and photovoltaic characteristics.

Perovskite solar cells (PSCs) without a mesoporous TiO2 layer, that is, planar-type PSCs exhibit poorer cell performance as compared to PSCs with a porous TiO2 layer, owing to inefficient electron transfer from the perovskite layer to the compact TiO2 layer in the former case. The matching of the conduction band levels of perovskite and the compact TiO2 layer is thus essential for enhancing PSC performance. In this study, we demonstrate the shifting of the conduction band edge (CBE) of the compact TiO2 layer through a TiCl4 treatment, with the aim of improving PSC performance. The CBE of the compact TiO2 layer was shifted to a higher level through the TiCl4 treatment and then shifted in the opposite direction, that is, to a lower level, through a subsequent heat treatment. These shifts in the CBE were reflected in the PSC performance. The TiCl4-treated PSC showed an increase in the open-circuit voltage of more than 150 mV, as well as a decrease of 100 mV after being heated at 450 °C. On the other hand, the short-circuit current decreased after the treatment but increased after heating at temperatures higher than 300 °C. The treated PSC subjected to subsequent heating at 300 °C exhibited the best performance, with the power conversion efficiency of the PSC being 17% under optimized conditions.

ABSTRACT

The photoelectrochemical production and degradation properties of hydrogen peroxide (H2 O2 ) were investigated on a WO3 /BiVO4 photoanode in an aqueous electrolyte of hydrogen carbonate (HCO3- ). High concentrations of HCO3- species rather than CO32- species inhibited the oxidative degradation of H2 O2 on the WO3 /BiVO4 photoanode, resulting in effective oxidative H2 O2 generation and accumulation from water (H2 O). Moreover, the Au cathode facilitated two-electron reduction of oxygen (O2 ), resulting in reductive H2 O2 production with high current efficiency. Combining the WO3 /BiVO4 photoanode with a HCO3- electrolyte and an Au cathode also produced a clean and promising design for a photoelectrode system specializing in H2 O2 production (ηanode (H2 O2 )≈50 %, ηcathode (H2 O2 )≈90 %) even without applied voltage between the photoanode and cathode under simulated solar light through a two-photon process; this achieved effective H2 O2 production when using an Au-supported porous BiVO4 photocatalyst sheet.

ABSTRACT

Recombination reactions in dye-sensitized solar cells (DSSCs) may substantially decrease the open-circuit voltage (Voc) with cobalt complex redox electrolyte. Managing steric hindrance in the dye structure is necessary to inhibit recombination reactions and thereby increase the Voc and achieve high power-conversion efficiency (PCE). New dyes with large-sized donors based on triphenylamine and modified with 4-(hexyloxy)phenyl groups were developed to identify an effective inhibitor for the recombination reaction in DSSCs with a cobalt complex redox electrolyte. The 4-(hexyloxy)phenyl tetra-adducts dye MK-123 effectively inhibited the recombination reaction, and the DSSC fabricated using this dye exhibited the highest Voc (greater than 900 mV) among the cells with the investigated dyes. However, the short-circuit current (Jsc) of the MK-123 cell was lower than that of the cell with the simple triphenylamine donor dye, MK-89. In contrast, the cell with bis-adducts dye MK-136 also exhibited an increase in its Voc without a decrease in its Jsc. Among the investigated dyes, MK-136 exhibited the highest PCE of 8.9%. The effects of the steric hindrance of the 4-(hexyloxy)phenyl substituent are discussed.

ABSTRACT

A porous and thick photoelectrode of WO3 in the monoclinic phase was prepared to realize the recovery of H2 and high-value-added oxidation reagents with efficient solar energy conversion. The WO3 photoelectrode enabled the efficient production and accumulation of O2 , S2 O8 (2-) , Ce(4+) , and IO4 (-) as oxidation products. Most notably, S2 O8 (2-) , which possesses the highest oxidizability among all the peroxides, was generated with high applied bias photon-to-current efficiency (2.2 %) and faraday efficiency (≈100 %) upon irradiation from the back side of the photoelectrode. The design of a tandem photoelectrode system combining a dye-sensitized solar cell (DSSC) was also challenged for the realization of this photoelectrode system without external bias. A high solar energy conversion efficiency (5.2 %) was achieved in the tandem system comprising the WO3 photoelectrode connected to two DSSCs with a near-IR-utilizing dye in series for the production of H2 and S2 O8 (2-) .

ABSTRACT

(Dibenzoylmethanato)boron difluoride derivatives containing triphenylamine moieties were synthesized as a new type of electron-donor/π-acceptor system. These new compounds exhibited long-wavelength absorptions in the UV/Vis spectra, and reversible oxidation and reduction waves in cyclic voltammetry experiments. Their amphoteric redox properties are based on their resonance hybrid forms, in which a positive charge is delocalized on the triphenylamine moieties and a negative charge is localized on the boron atoms. Molecular orbital (MO) calculations indicate that their HOMO and LUMO energies vary with the number of phenylene rings connected to the difluoroboron-chelating ring. This is useful for optimizing the HOMO and LUMO levels to an iodine redox (I(-)/I3(-)) potential and a titanium dioxide conduction band, respectively. Dye-sensitized solar cells fabricated by using these compounds as dye sensitizers exhibited solar-to-electric power conversion efficiencies of 2.7-4.4 % under AM 1.5 solar light.

Intermolecular interactions in cyclometalated Ru complex dye (FT89) dimers, carbazole organic dye (MK-45 and MK-111) dimers, FT89-MK-45 complexes, and FT89-MK-111 complexes were investigated using density functional theory (DFT) and time-dependent DFT (TD-DFT) to elucidate the improvement mechanism of dye-sensitized solar cell (DSSC) performance due to cosensitization with FT89 and MK dyes. All of the dimers and complexes form intermolecular cyclic hydrogen bonds via the carboxyl groups. The FT89 dimer and complexes with the TiO2Na model system promote intermolecular interactions with I2via the NCS ligand of the FT89 monomer. The computational results verify that MK-111 behaves not only as a sensitizer but also inhibits FT89 aggregation by effectively serving as a coadsorbent similar to deoxycholic acid (DCA) in the dye solution, suppressing recombination of the injected electrons in TiO2 with I2, improving DSSC performance.

ABSTRACT

The formation of 1:1 adducts of Pt(bpy)2(2+) (bpy = 2,2'-bipyridine) with various N bases (B) has been ascertained in water at ambient temperature by spectrophotometric titration and electrospray ionization mass spectroscopy. A pseudo-five-coordinated complex, [Pt(bpy)2(B)](2+) with a monodentating bpy, is proposed based on density functional theory calculation. The formation constants (Kc) increase with the nucleophilicity of B except for sterically hindered N-bases, indicating an associative ligand-substitution mechanism.

ABSTRACT

[2.n](2,6)Pyridinocrownophanes (3a-e) were efficiently prepared by intramolecular [2 + 2] photocycloaddition of vinylpyridine derivatives under irradiation using a 400-W high-pressure mercury lamp through a Pyrex filter. They were of cis-configuration with respect to the cyclobutane ring, which was proven by the specific methine proton NMR resonances at delta 3.98-4.08. From ESI-MS analysis 3a-e were found to form 1:1 complexes with Ag+ cation. In a liquid-liquid extraction, 3 showed the highest affinity toward Ag+ cation among several heavy metal nitrates. In this series, 3c possessing four ethereal oxygen atoms was found to show the highest Ag+ affinity, according to the liquid-liquid extraction and determination of stability constant with the cation. The photoreactivity of precursor vinylpyridines is discussed by the predictor gamma(r(c)) proposed by Caldwell.

ABSTRACT

Novel, potent inhibitors of dipeptidyl peptidase IV (DPP IV, EC 3.4.14.5, CD26), containing the fluoroolefin peptide isostere psi [CFz.dbnd6;C], have been prepared via the intermediacy of the Peterson fluoroolefination reaction. The nitrile containing inhibitors were found to inhibit dipeptidyl peptidase IV competitively with K(i) values for the l-3 and u-3 inhibitors of 7.69 and 6.03 microM, respectively. In contrast to earlier reported fluoroolefin containing inhibitors, the nitriles underwent no detectable degradation at pH 7.6 under buffered conditions.
