ABSTRACT

BACKGROUND: Latin American countries are improving childhood cancer care, showing strong commitment to implement the Global Initiative for Childhood Cancer, but there are scant publications of the situation at a continental level. METHODS: As part of the International Society of Paediatric Oncology Global Mapping project, delegates of each country participating in the Latin American Society of Pediatric Oncology (SLAOP) and chairs of national pediatric oncology societies and cooperative groups were invited to provide information regarding availability of national pediatric cancer control programs (NPCCP), pediatric oncology laws, pediatric oncology tumor registries, and training programs and support to diagnosis and treatment. RESULTS: Nineteen of the 20 countries participating in SLAOP responded. National delegates reported nine countries with NPCCP and four of them were launched in the past 5 years. National pediatric tumor registries are available in eight countries, and three provided published survival results. Fellowship programs for training pediatric oncologists are available in 12 countries. National delegates reported that eight countries provide support to most essential diagnosis and treatments and 11 provide partial or minimal support that is supplemented by civil society organizations. Seven countries have a pediatric oncology law. There are three international cooperative groups and four national societies for pediatric oncology. CONCLUSION: Despite many challenges, there were dramatic advances in survivorship, access to treatment, and availability of NPCCP in Latin America. Countries with highest social development scores in general provide more complete support and are more likely to have NPCCP, training programs, and reported survival results.

BACKGROUND: Hyperandrogenism and supraphysiologic glucocorticoid replacement may lead to subclinical atherosclerosis in people with congenital adrenal hyperplasia (CAH) and predispose the development of cardiovascular diseases from an early age. OBJECTIVES: To determine if cardiometabolic risk factors and subclinical atherosclerosis are more frequent in patients with CAH due to 21-hydroxylase deficiency (21OHD) and if there is an association with clinical, hormonal and treatment of 21OHD. MATERIAL AND METHODS: A descriptive prospective cross-sectional study exploring clinical variables, biochemical, hormonal variables, endothelial dysfunction (flow-mediated dilation < 5%) and carotid intima media thickness (≥ 95 percentile in adolescents and ≥ 75 percentile in adults) and epicardial fat. Adolescents and young patients with 21OHD were compared with controls matched by age, sex, body mass index and Tanner stage. RESULTS: Forty four subjects (22 with CAH), 36 (82%) females, with a mean age of 17.1 ± 5.5 years (range 10-30 years) were included. Family history revealed diabetes, hypertension, and hypercholesterolemia with high frequencies in both groups. The blood pressure was similar in both groups. Blood glucose levels were lower and triglycerides higher in patient (both p < 0.01). Epicardial fat was similar between groups and in patients with CAH it was related to cholesterol levels ​​(r = 0.679, p < 0.01), time since CAH diagnosis (r = 0.462, p = 0.03) and glucocorticoid dose (r = 0.499, p = 0.04). Carotid intima media thickness (CIMT) had a tendency to be increased in patients (p = 0.07) and was directly related to 17-hydroxyprogesterone (r = 0.510, p = 0.018), diastolic blood pressure (r = 0.444, p = 0.04) and the homeostatic model assessment (HOMA) index (r = 0.507, p = 0.01). Endothelial dysfunction was not different between groups. CONCLUSIONS: Some cardiometabolic risk factors were increased in patients with CAH and were associated with clinical, hormonal and treatment parameters of CAH. Cardiometabolic risk should be evaluated regularly in patients with CAH.

ABSTRACT

Desmodus rotundus is one of the wild animal species that has benefitted by habitat alteration and its population has increased due to livestock activities. Common vampire bat population management has been implemented across Mexico due to the economic losses to livestock production, inflicted by vampire bat attacks and rabies transmission. Yucatan is one of the seven most impacted states in Mexico by the number of cattle rabies cases per year. However, there is little research on D. rotundus populations such as the frequency and attack patterns to cattle. This study's objective was to analyze the relationship between D. rotundus abundance and number of bovines attacked in livestock landscapes in Yucatan. The study used data gathered by the State Committee for Protection and Promotion of Livestock in Yucatan through the National Campaign for Common Vampire Bat Population Control. Data collected from January 2014 to December 2017 was analyzed using Pearson correlation. Distribution maps on Desmodus rotundus abundance and number of bovines attacked were also created. Higher abundance of Desmodus rotundus and number of cattle attacks were observed in the central region of Yucatan, particularly in Izamal municipality. Positive correlations were found between (1) abundance of Desmodus rotundus and number of cattle in the region, (2) total number of cattle and number of cattle attacked, and (3) abundance of Desmodus rotundus and number of cattle attacked. We can conclude that there is a relationship between Desmodus rotundus abundance and frequency of cattle attacks in most municipalities across Yucatan. Some outstanding exceptions were observed, which require further detailed investigations.

ABSTRACT

Breast milk from a single mother was collected during a 28-week lactation period. Bacterial diversity was studied by amplicon sequencing analysis of the V3-V4 variable region of the 16S rRNA gene. Firmicutes and Proteobacteria were the main phyla detected in the milk samples, followed by Actinobacteria and Bacteroidetes. The proportion of Firmicutes to Proteobacteria changed considerably depending on the sampling week. A total of 411 genera or higher taxons were detected in the set of samples. Genus Streptococcus was detected during the 28-week sampling period, at relative abundances between 2.0% and 68.8%, and it was the most abundant group in 14 of the samples. Carnobacterium and Lactobacillus had low relative abundances. At the genus level, bacterial diversity changed considerably at certain weeks within the studied period. The weeks or periods with lowest relative abundance of Streptococcus had more diverse bacterial compositions including genera belonging to Proteobacteria that were poorly represented in the rest of the samples.

RESUMEN Los trastornos del desarrollo sexual son estados congénitos en los cuales el desarrollo del sexo cromosómico, gonadal o anatómico es atípico. Por tratarse de un caso sumamente raro consideramos de interés su presentación. Se presenta adolescente masculino de 15 años, con antecedentes de genitales atípicos al nacer, desarrollo de baja talla y estigmas turnerianos, pubertad espontánea y normal. Los estudios genéticos determinaron como sexo cromosómico un mosaico 45,X/46,XY/47XYY, y sexo molecular varón. Se inscribió socialmente como varón, se le realizó cirugía de reconstrucción genital y utilizó tratamiento con hormona de crecimiento biosintética que mantiene actualmente. La evolución clínica ha sido favorable con adecuada integración social. Ante la presencia de genitales atípicos al nacer se necesita de un manejo multidisciplinario. El diagnóstico etiológico de los trastornos de la diferenciación sexual requiere de una alta pericia médica. Un tratamiento integral en estos pacientes les garantiza una buena calidad de vida(AU)


ABSTRACT Sexual development´s disorders are congenital states in which the development of the chromosomal, anatomic or gonadal sex is atypical. Since this is a very rare case, we consider it as of interests for presentation. It is presented a teenager, 15-years-old male, with a history of atypical genitalia at birth, development of short height and Turner's stigmas, and spontaneous and normal puberty. The genetic studies identified as chromosomal sex a mosaic 45,X/46,XY/47XYY and male as molecular sex. He was socially registered as a male, he had a genital reconstruction surgery and he was under treatment with biosynthetic growth hormone that he currently maintains. The clinical evolution has been favourable with adequate social integration. In the presence of atypical genitalia at birth, it is needed a multidisciplinary management. The etiological diagnosis of disorders of sexual differentiation requires a high level of medical expertise. A comprehensive treatment in these patients guarantees them a good quality of life(AU)

RESUMEN Fundamento: La creciente prevalencia de diabetes mellitus tipo 2 (DM2) desde las edades pediátricas, ha convertido a la morbimortalidad relacionada con ella en un problema importante de salud pública. La detección precoz de factores de riesgo de DM2 en adolescentes, permitiría realizar acciones preventivas con enfoque de riesgo, que conducirían a disminuir la incidencia de esta entidad, siendo un pilar fundamental de la atención primaria. Objetivo: Identificar la presencia de factores de riesgo de DM2 en adolescentes. Método: Se realizó un estudio descriptivo transversal en un área de salud (Policlínico "Raúl Gómez García") con una muestra de 96 adolescentes. Se recogieron variables como edad, sexo, antecedentes de diabetes gestacional, bajo peso al nacer, tiempo de lactancia materna, historia familiar de DM2, consumo de frutas/vegetales, actividades físicas y resultantes del examen físico. Resultados: El 62,50% de los adolescentes pertenecía al sexo femenino y la adolescencia temprana. El inadecuado consumo de frutas y vegetales con 81,25% fue el factor de riesgo más común, seguido del sedentarismo con un 45,83%. Todos los adolescentes poseían al menos un factor de riesgo, y el 72,92% poseía 3 o más con predominio en el sexo femenino (p=0,022) Conclusiones: Existe una alta frecuencia de factores de riesgo para el desarrollo de DM2 en los adolescentes, con predominio de factores modificables. La mayoría de los adolescentes poseían un riesgo metabólico incrementado al presentar 3 o más factores de riesgo, en especial las féminas.


ABSTRACT Foundation: The increasing prevalence of diabetes mellitus type 2 (DM2) from the pediatric ages, has made its morbidity and mortality a major public health problem. Early detection of DM2 risk factors in adolescents, would allow preventive actions with a risk approach, which would lead to decrease the incidence of this condition, being a fundamental pillar of primary care . Objective: To identify the presence of DM2 risk factors in adolescents. Method: A cross-sectional descriptive study was conducted in a health area ("Raúl Gómez García" Polyclinic) with a sample of 96 adolescents. Variables such as age, sex, history of gestational diabetes, low birth weight, breastfeeding time, family history of DM2, fruit / vegetable intake, physical activity and physical examination results were collected. Results: The 62.50% of the adolescents belonged to the feminine sex and the early adolescence. The inadequate consumption of fruits and vegetables with 81.25% was the most common risk factor, followed by a sedentary lifestyle with 45.83%. All adolescents had at least one risk factor, and 72.92% had 3 or more with a predominance in females (p = 0.022). Conclusion: There is a high frequency of risk factors for developing of DM2 in adolescents, with a predominance of modifiable factors. Most adolescents had an increased metabolic risk when presenting 3 or more risk factors, especially females.

Antecedentes: Uno de los mecanismos para garantizar la calidad de la atención en salud es el análisis de eventos adversos en los tratamientos. El área de la rehabilitación oral es una de las más propensas, dada la complejidad de sus procedimientos. Objetivo: Analizar los eventos adversos que se presentaron en la clínica del posgrado de rehabilitación oral de la Facultad de Odontología de la Pontificia Universidad Javcriana de Bogotá durante 2013. Métodos: De 595 historias clínicas correspondientes a todos los pacientes que finalizaron su tratamiento en 2013. 590 cumplieron con el requisito de estar firmadas por el paciente y por el profesor. A partir de las historias que contenían algún reporte de evento adverso, se registraron los datos demográficos de los pacientes, los tipos de eventos adversos reportados, las posibles causas que su ocurrencia y si eran o no prevenibles. Resultados: En 36 (6,1 %) de las historias clínicas analizadas se encontró algún reporte de evento adverso. El evento más frecuente fue la perdida de la restauración (42 %). La causa más frecuente fue la fractura completa de la restauración (19 %). El 58 % se consideró evento adverso prevenible. En el 61,1 % de los casos fueron prótesis fijas dcntorrctcnidas. Conclusiones: La frecuencia de eventos adversos reportada en las historias clínicas de pacientes atendidos en el área de rehabilitación es baja. Teniendo en cuenta de condición de prevenible de estos casos, es importante registrarlos y analizarlos para asegurar la calidad en la atención de los pacientes.


Background: One of the mechanisms that ensures the health care quality is the analysis of adverse events in the treatment. The oral rehabilitation area is among the more likely because of the complex procedures involved. Objective: To analyze the adverse events that occurred in the clinic of the oral rehabilitation graduate program at the Pontificia Universidad Javeriana Dentistry School in Bogotá during 201 "i.Methods: Out of 595 medical records including all the patients who completed their treatment in 2013. 590 fulfilled the requirement of being signed both by the patient and the teacher. Based on the medical records that reported any kind of adverse event, the patient information regarding demographic data, type of reported adverse event, potential causes for their occurrence, and whether they were preventable was gathered. Results: In 36 (6.1%) of the examined medical records, at least one report of adverse event was found. The most frequently found event was the restoration loss (42%). The most frequently found cause was the full fracture of the restoration (19%). Fifty-eight (58 %) of the adverse events were preventable. Sixty-one (61.1%) of the eases were retained fixed dental prosthcscs. Conclusions: Ihc frequency of adverse events reported in the medical records of patients treated in the rehabilitation area is low. Considering that the eases arc preventable, it is important to record and examine these eases in order to ensure the health care quality for the patients.

BACKGROUND: Although anaplastic large cell lymphoma (ALCL) is curable in high-income countries (HIC), data from low- and middle-income countries (LMIC) are lacking. We therefore conducted a retrospective study of the Central American Association of Pediatric Hematology Oncology (AHOPCA) experience in treating ALCL. PROCEDURE: We included all patients age <18 years newly diagnosed with ALCL treated between 2000 and 2013 in seven AHOPCA institutions. Retrospective data were extracted from the Pediatric Oncology Network Database. RESULTS: Thirty-one patients met inclusion criteria. Twenty-five (81%) had advanced disease (stages III and IV), six (19%) were treated on the APO (doxorubicin, prednisone, vincristine) regimen, 15 (49%) on multi-agent chemotherapy designed for T-cell lineage malignancies (GuatALCL protocol), and 10 (32%) on BFM-based treatment regimens. Five-year overall event-free survival and overall survival were, respectively, 67.1 ± 8.6% and 66.7 ± 8.7%. All 10 events occurred in patients treated on BFM-based treatment regimens or the GuatALCL protocol, none on APO treatment: two patients experienced relapse, six treatment related mortality (TRM), and two abandonment. CONCLUSIONS: Treatment of ALCL in countries with limited resources is feasible with similar outcomes as in HIC, though the causes of treatment failure differ. Less intensive regimens may be preferable in order to decrease TRM and improve outcomes. Prospective clinical trials determining the ideal treatment for LMIC children with ALCL are necessary.

Objetivos: caracterizar demográficamente las adolescentes en estudio; evaluarlas nutricionalmente utilizando el índice de masa corporal y la circunferencia de cintura, así como identificar la relación entre la menarquia, el color de la piel, el bajo peso al nacer, el exceso de peso corporal, la obesidad abdominal y la presencia de acantosis nigricans. Métodos: se realizó un estudio observacional, descriptivo transversal en la Secundaria Básica Urbana José María Heredia, desde junio de 2010 a enero de 2011. La población estuvo constituida por 85 adolescentes, a las cuales se les preguntó acerca de la menarquia, se les realizaron las mediciones antropométricas, y se buscó la presencia de acantosis nigricans. Resultados: la edad media de las adolescentes fue 12,05 años, de las cuales el 63,5 porciento había experimentado la menarquia, y su edad media de presentación fue de 11,00±0,801 años. El 17,6 por ciento presentó exceso corporal, el 34,1 por ciento adiposidad abdominal y el 12,9 porciento acantosis nigricans, asociadas estadísticamente con la aparición de la menarquia. Conclusiones: la mayoría de las adolescentes que habían experimentado la menarquia eran maduradoras tempranas, y de color de piel blanca; mostraron un riesgo incrementado de desarrollar exceso de peso corporal, obesidad abdominal y signos clínicos de resistencia a la insulina. La aparición de la menarquia no estuvo influida por el bajo peso al nacer(AU)

Methods: a cross-sectional, observational and descriptive study was conducted in Jose Maria Heredia junior high school from June 2010 to January 2011. The population was made up of 85 adolescents, who were asked about menarche, were anthropometrically measured and searched for acanthosis nigricans. Results: the average age of female adolescents was 12,05 years. Menarche was experienced by 63,5 percent of them and the average age of onset was 11,00±0,801 years. In this group, 17,6 percent presented with overweight, 34,1 percent with abdominal adiposity and 12,9 percent had acanthosis nigricans, all of these factors statistically associated with the onset of menarche. Conclusions: most of the female adolescents experienced menarche at early ages; they were Caucasians and showed a high risk of developing overweight, abdominal obesity and clinical signs of insulin-resistance. The onset of menarche was not affected by low birthweight(AU)

Introducción: la nutrición juega un papel crítico en la salud del adolescente, y el consumo de una dieta inadecuada puede influir desfavorablemente sobre el crecimiento somático y la maduración sexual, por lo que constituye una etapa crucial del desarrollo, en la que prevalecen con alta frecuencia trastornos nutricionales. Métodos: se desarrolló un estudio descriptivo transversal, con el objetivo principal de identificar y describir los gustos, preferencias y frecuencia de consumo de alimentos de los adolescentes de séptimo grado que asisten a la Secundaria Básica José María Heredia, en el curso 2009-2010. La población estuvo constituida por 192 adolescentes, a los cuales con previo consentimiento de los padres, se les aplicó una encuesta alimentaria de gustos, preferencias y frecuencia semanal de consumo de alimentos. Resultados: los grupos de alimentos que más gustaron fueron mantequilla, mayonesa y queso crema, dulces, confituras y refrescos, mayormente consumidos a diario, a diferencia de las frutas y vegetales. El 14,1 por ciento de los adolescentes gustaban de bebidas alcohólicas, y el 15,6 por ciento las consumía al menos una vez a la semana. Conclusiones: existen deficiencias en la educación alimentaria e inadecuados hábitos alimentarios en los adolescentes estudiados(AU)


Introduction: the nutrition plays a critical role in the adolescent health and the consumption of a inappropriate diet may to influence in an unfavorable way on the somatic growth and the sexual maturation being a crucial stage of development where frequently prevail the nutritional disorders. Methods: a cross-sectional and descriptive study was conducted to identify and to describe the tastes, preferences and frequency of food consumption of adolescent of 7th degree studying in the "José María Heredia" Secondary School in the 2009-2010 course. Sample included 192 adolescents and with the parents consent underwent to alimentary survey on tastes, preferences and weekly frequency of food consumption. Results: the more preferred foods were the butter, mayonnaise and cream-cheese, sweets, preserves and soft drinks, mainly consumed daily; unlike the fruits and vegetables. The 14,1 percent of adolescents preferred the alcohol and the 15,6 percent consumed them at least once a week. Conclusions: there are deficiencies in the alimentary education as well as inappropriate alimentary habits in the study adolescents(AU)

Introducción: la nutrición juega un papel crítico en la salud del adolescente, y el consumo de una dieta inadecuada puede influir desfavorablemente sobre el crecimiento somático y la maduración sexual, por lo que constituye una etapa crucial del desarrollo, en la que prevalecen con alta frecuencia trastornos nutricionales. Métodos: se desarrolló un estudio descriptivo transversal, con el objetivo principal de identificar y describir los gustos, preferencias y frecuencia de consumo de alimentos de los adolescentes de séptimo grado que asisten a la Secundaria Básica "José María Heredia", en el curso 2009-2010. La población estuvo constituida por 192 adolescentes, a los cuales con previo consentimiento de los padres, se les aplicó una encuesta alimentaria de gustos, preferencias y frecuencia semanal de consumo de alimentos. Resultados: los grupos de alimentos que más gustaron fueron mantequilla, mayonesa y queso crema, dulces, confituras y refrescos, mayormente consumidos a diario, a diferencia de las frutas y vegetales. El 14,1 por ciento de los adolescentes gustaban de bebidas alcohólicas, y el 15,6 por ciento las consumía al menos una vez a la semana. Conclusiones: existen deficiencias en la educación alimentaria e inadecuados hábitos alimentarios en los adolescentes estudiados(AU)

Introducción: la adolescencia es una etapa crucial, con alta prevalencia de trastornos nutricionales, que luego continúan durante la vida adulta y se asocian con complicaciones a corto y a largo plazo. Objetivos: identificar la asociación entre el exceso de peso corporal y los antecedentes patológicos familiares de interés y algunos factores perinatales, la presencia de la hipertensión arterial y su relación con la historia familiar de hipertensión, el exceso de peso corporal y el incremento de adiposidad abdominal; y por último, identificar la presencia de acantosis nigricans y su relación con iguales factores. Métodos: se desarrolló un estudio descriptivo transversal para caracterizar el estado nutricional de los adolescentes de séptimo grado que asisten a la Secundaria Básica José María Heredia, en el curso 2009-2010. La población estuvo constituida por 192 adolescentes, a los cuales, con previo consentimiento de los padres, se les realizaron las mediciones antropométricas (peso, talla, circunferencia de cintura y se calculó el índice de masa corporal), y además se midió la tensión arterial y se buscaron signos clínicos de resistencia insulínica. También se aplicó una encuesta a los padres acerca de los antecedentes patológicos familiares y algunos datos de sus hijos. Resultados: se encontró que el 20,31 por ciento de los adolescentes presentaba sobrepeso u obesidad, lo cual se relacionó de forma significativa con la presencia de hipertensión arterial y signos clínicos de resistencia insulínica. La hipertensión arterial se halló en el 9,9 por ciento de los adolescentes, sin relación con factores hereditarios. Conclusiones: la obesidad y el sobrepeso corporal constituyen un problema de salud en la población adolescente estudiada, lo cual se asocia con la presencia de hipertensión arterial y resistencia insulínica, no existió relación en su comportamiento con factores no modificables (antecedentes patológicos familiares y personales)(AU)

Objectives: to identify the association between excess of body weight and the interesting family pathological backgrounds and some perinatal risks, the presence of high blood pressure and its relation to the family history of high blood pressure and the increase of abdominal adiposity and finally, to identify the presence of acanthosis nigricans and its relation with the similar factors. Methods: a cross-sectional and descriptive study was conducted to characterize the nutritional status of the adolescents of the seventh grade from the José María Heredia secondary school during 2009-2020. Group included 192 adolescents who with a previous consent of parents underwent anthropometric measurements (weight, height, waist circumference and estimation of the body mass index) and also the blood pressure and search of clinical signs of insulin resistance. A survey to parents on the family pathological backgrounds and on some data of their children was applied. Results: there was that the 20,31 percent of adolescents had excess weight or obesity, which was related in a significant way to the presence of a high blood pressure and clinical signs of insulin resistance. The high blood pressure was present in the 9,9 percent of adolescents without any relation to hereditary factors. Conclusions: obesity and body excess weight are a health problem in study adolescent population, which is associated with the presence of a high blood pressure and insulin resistance; there was not relation in its behavior to non-modifiable factors (family and personal pathological backgrounds)(AU)
