ABSTRACT

A late adolescent girl presented with medically uncontrolled glaucoma and aphakia post-fire-cracker injury to the right eye. She underwent single-loop fixation of the posterior chamber intraocular lens (IOL) and Ahmed glaucoma valve (AGV) implantation with reduction of intraocular pressure (IOP) in the immediate postoperative period. She sustained second trauma 6 days later resulting in tube retraction and an IOP of 38 mm Hg. An anterior repositioning of the tube-plate complex was performed and IOP remained under control for 5 months. She then developed a tenon cyst and the IOP rose to 24 mm Hg, for which topical timolol and dorzolamide and digital massage were administered. The IOP was in the lower teens, sans medication and aided vision of 0.50 LogMAR at the 1 year follow-up. This case highlights the outcomes of single-loop fixation of IOL with AGV in a post-traumatic setting and subsequent management of complications.

ABSTRACT

Purpose: To study the endoscopic ostium characteristics and outcome of 8 × 8 mm osteotomy in external dacryocystorhinostomy (DCR) using the microdrill system. Methods: This prospective interventional pilot study was performed on 40 eyes of 40 patients with primary acquired nasolacrimal duct obstruction (NLDO) from June 2021 to September 2021 in patients undergoing external DCR. An 8 × 8 mm osteotomy was performed using round, cutting burr attached to a microdrill system. Success was defined as patent ostium on lacrimal syringing (anatomical) and a Munk score <3 (functional) at 12 months. Postoperative endoscopic ostium evaluation was done using a modified DCR ostium (DOS) scoring system at 12 months. Results: The mean age of the study participants was 42.41 ± 11.77 years and the male-to-female ratio was 1:4. The mean duration of surgery was 34.15 ± 1.66 minutes and that for osteotomy creation was 2.5 ± 0.69 minutes. The mean intraoperative blood loss was 83.37 ± 11.89 ml. Anatomical and functional success rates were 95% and 85%, respectively. The mean modified DOS score was "excellent" in 34 patients (85%), "good" in 1 patient (2.5%), "fair" in 4 patients (10%), and "poor" in 1 patient (2.5%). Complications included nasal mucosal injury in 10% (4/40) of patients, complete cicatricial closure of ostium in 2.5% (1/40), incomplete cicatricial closure in 10% (4/40), nasal synechiae in 5% (2/40), and canalicular stenosis in 2.5% (1/40). Conclusion: An 8 × 8 mm-sized osteotomy created by powered drill and covered by lacrimal sac-nasal mucosal flap anastomosis in external DCR is an effective technique that has minimal complications and shorter surgical time.

ABSTRACT

INTRODUCTION: Rosai Dorfman disease (RDD) is a rare benign histiocytic proliferative disorder of lymph node sinuses. Langerhans cell histiocytosis (LCH) is a solitary or multisystem clonal proliferation of abnormal dendritic cells (Langerhans cells) with varied presentations. The co-occurrence of these two entities is quite rare. CASE DESCRIPTION: A six-year-old boy presented with multiple mass lesions in the neck since two years and a nodular lesion in right upper eyelid for the past 4 months. He was diagnosed with tubercular lymphadenitis 2 years back, and was given a course of anti-tubercular therapy (ATT) elsewhere. No improvement was seen. Fine needle aspiration cytology (FNAC) of the cervical lymph nodes revealed reactive lymphadenitis while lymph node biopsy showed features of RDD. Excision biopsy of the orbital mass showed features of both RDD and LCH. The patient was started on tablet prednisolone. Six months later, complete resolution of lymph node enlargement and remaining orbital mass was noted. Post operative contrast enhanced Magnetic Resonance Imaging of head and neck was normal. CONCLUSION: The coexistence of RDD and LCH may be a result of divergent differentiation from a common lineage or a de novo phenotypic evolution.

ABSTRACT

The manual small-incision cataract surgery (MSICS) is instrumental in tackling cataract-induced blindness in developing countries, especially with a sizeable proportion being hard brunescent cataracts. MSICS has a unique set of complications related to wound construction, the creation of the capsular opening, and the technique of nuclear delivery. A poorly constructed sclero-corneal tunnel or a small capsulorhexis hampers the nuclear extraction, and the extensive intracameral maneuvers increase the chances of postoperative corneal edema and iritis. Though MSICS has been shown to have universal applicability, producing replicable visual outcomes requires a significant learning curve. This article reviews the relevant published literature on complications of MSICS utilizing the databases of PubMed, Medline, Cochrane, and Google Scholar.

ABSTRACT

Purpose: To compare the functional outcome of retropupillary iris claw lenses (RPIC-IOL) and scleral fixated intraocular lenses (SFIOL) in children with large lens subluxations. Methods: Sixty eyes of patients between 6 and 18 years of age having >7 clock hour lens subluxation were included and equally divided into group A (RPIC-IOL implantation) and group B (Gore-Tex sutured SFIOL implantation). Cases with anterior and posterior segment abnormalities, trauma and glaucoma were excluded. Primary outcome was improvement in best-corrected visual acuity (BCVA) at 1.5 years. Secondary outcomes were assessment of intraocular lens (IOL) tilt, mean change in astigmatism at 1.5 years, and median operating time. All surgeries were performed by the same surgeon. Results: The mean improvement in BCVA in group A was 0.28 ± 0.41 logMAR and group B was 0.44 ± 0.45 logMAR (P = 0.3). Significant IOL tilt was seen in 4 eyes in group A (13.33%) and 5 eyes in group B (16.66%) (P = 0.120). Mean change in astigmatism was 4.38 ± 5.9D in group A and 4.91 ± 4.4D in group B (P = 0.299). The median operating time was 40 min in group A and 90 min in group B (P < 0.001). No significant posterior segment complications were seen in either technique. Conclusion: Both procedures had comparable visual outcomes. RPIC-IOL implantation was relatively quick and comparatively easier; it may be preferred in cases with high risk of retinal detachment.

ABSTRACT

Introduction With the advent of newer microsurgical techniques, the results for cataract surgery have become quite promising. An accurate intraocular lens (IOL) power calculation is one of the most important factors in optimizing the results. The aim of this study was to evaluate the accuracy of four IOL power calculation formulas, namely, Barrett Universal II, Holladay 1, SRK/T and Hoffer Q, using optical biometry in children undergoing cataract surgery with primary IOL implantation. Methods This was a cross-sectional study. A total of 60 eyes of 42 children aged 5-16 years with congenital cataract and having undergone uneventful cataract surgery with IOL implantation were included in the study. Eyes were divided into three groups based on the axial length (AL): short (AL <22.00 mm), medium (AL 22-24.50 mm) and medium long (AL 24.50-26.00 mm). Optical biometry was used and the IOL power was calculated using the Barrett Universal II formula. The predicted postoperative refraction with the other three formulas, namely, SRK/T, Holladay 1 and Hoffer Q, using the same IOL power was estimated. This was compared with the actual postoperative refraction (spherical equivalent at 12 weeks) to give the absolute prediction error. The mean of all absolute prediction errors gave the mean absolute prediction error (MAE) values for each formula that were then compared. Results The MAE was 0.64 ± 0.73 for Barrett Universal II, 0.7 ± 0.72 for Holladay 1, 0.71 ± 0.65 for Hoffer Q and 0.8 ± 0.75 for SRK/T. Thus, Barrett Universal II had the lowest MAE across the whole group. The difference in the MAEs was not statistically significant. Conclusion Barrett Universal II had the lowest MAE and thus was predictable for the highest number of eyes in our study, although this was not statistically significant (p=0.176).

ABSTRACT

We report a case who presented with decreased vision, significant hypotropia, proptosis and gross limitation of extraocular motility for one year. Suspecting an orbital tumour, we asked for a computed tomography of the orbit which revealed a mass lesion in the inferior orbit. However, incisional biopsy reported inflammatory infiltration. Diagnosing it as orbital inflammatory disease, a course of oral steroids was given for four weeks. It was only after the reduction in inflammation that a foreign body was palpable in the inferior fornix. Surgical exploration revealed a large wooden foreign body measuring 3.3 × 1 × 0.3 cm. Though intraorbital foreign bodies are not rare, ambiguous history, delayed presentation and nonspecific CT findings made this case diagnostically challenging.
