ABSTRACT

BACKGROUND: There are few prospective studies in the gynecologic surgical literature that compared patient-reported outcomes between open and minimally invasive hysterectomies within enhanced recovery after surgery pathways. OBJECTIVE: This study aimed to compare prospectively collected perioperative patient-reported symptom burden and interference measures in open compared with minimally invasive hysterectomy cohorts within enhanced recovery after surgery pathways. STUDY DESIGN: We compared patient-reported symptom burden and functional interference in 646 patients who underwent a hysterectomy (254 underwent open surgery and 392 underwent minimally invasive surgery) for benign and malignant indications under enhanced recovery after surgery protocols. Outcomes were prospectively measured using the validated MD Anderson Symptom Inventory, which was administered perioperatively up to 8 weeks after surgery. Cohorts were compared using Fisher exact and chi-squared tests, adjusted longitudinal generalized linear mixed modeling, and Kaplan Meier curves to model return to no or mild symptoms. RESULTS: The open cohort had significantly worse preoperative physical functional interference (P=.001). At the time of hospital discharge postoperatively, the open cohort reported significantly higher mean symptom severity scores and more moderate or severe scores for overall (P<.001) and abdominal pain (P<.001), fatigue (P=.001), lack of appetite (P<.001), bloating (P=.041), and constipation (P<.001) when compared with the minimally invasive cohort. The open cohort also had significantly higher interference in physical functioning (score 5.0 vs 2.7; P<.001) than the minimally invasive cohort at the time of discharge with no differences in affective interference between the 2 groups. In mixed modeling analysis of the first 7 postoperative days, both cohorts reported improved symptom burden and functional interference over time with generally slower recovery in the open cohort. From 1 to 8 postoperative weeks, the open cohort had worse mean scores for all evaluated symptoms and interference measures except for pain with urination, although scores indicated mild symptomatic burden and interference in both cohorts. The time to return to no or mild symptoms was significantly longer in the open cohort for overall pain (14 vs 4 days; P<.001), fatigue (8 vs 4 days; P<.001), disturbed sleep (2 vs 2 days; P<.001), and appetite (1.5 vs 1 days; P<.001) but was significantly longer in the minimally invasive cohort for abdominal pain (42 vs 28 days; P<.001) and bloating (42 vs 8 days; P<.001). The median time to return to no or mild functional interference was longer in the open than in the minimally invasive hysterectomy cohort for physical functioning (36 vs 32 days; P<.001) with no difference in compositive affective functioning (5 vs 5 days; P=.07) between the groups. CONCLUSION: Open hysterectomy was associated with increased symptom burden in the immediate postoperative period and longer time to return to no or mild symptom burden and interference with physical functioning. However, all patient-reported measures improved within days to weeks of both open and minimally invasive surgery and differences were not always clinically significant.

BACKGROUND: Opioid over-prescription is wasteful and contributes to the opioid crisis. We implemented a personalized tiered discharge opioid protocol and education on opioid disposal to minimize over-prescription. OBJECTIVE: To evaluate the intervention by investigating opioid use post-discharge for women undergoing abdomino-pelvic surgery, and patient adherence to opioid disposal education. METHODS: We analyzed post-discharge opioid consumption among 558 patients. Eligible patients included those who underwent elective gynecologic surgery, were not taking scheduled opioids pre-operatively, and received discharge opioids according to a tiered prescribing algorithm. A survey assessing discharge opioid consumption and disposal safety knowledge was distributed on post-discharge day 21. Over-prescription was defined as >20% of the original prescription left over. Descriptive statistics were used for analysis. RESULTS: The survey response rate was 61% and 59% in the minimally invasive surgery and open surgery cohorts, respectively. Overall, 42.8% of patients reported using no opioids after hospital discharge, 45.2% in the minimally invasive surgery and 38.6% in the open surgery cohort. Furthermore, 74.9% of respondents were over-prescribed, with median age being statistically significant for this group (p=0.004). Finally, 46.4% of respondents expressed no knowledge regarding safe disposal practices, with no statistically significant difference between groups (p>0.99). CONCLUSION: Despite implementation of the tiered discharge opioid algorithm aimed to personalize opioid prescriptions to estimated need, we still over-prescribed opioids. Additionally, despite targeted education, nearly half of all patients who completed the survey did not know how to dispose of their opioid tablets. Additional efforts are needed to further refine the algorithm to reduce over-prescription of opioids and improve disposal education.

ABSTRACT

The purpose was to assess the concurrent validity and reliability of two portable powermeters (PowerTap vs. Power2Max) in different types of cycling efforts. Ten cyclists performed two submaximal, one incremental maximal and two supramaximal sprint tests on an ergometer, while pedaling power and cadence were registered by both powermeters and a cadence sensor (GarminGSC10). During the submaximal and incremental maximal tests, significant correlations were found for power and cadence data (r = 0.992-0.997 and 0.996-0.998, respectively, p < 0.001), with a slight power underestimation by PowerTap (0.7-1.8%, p < 0.01) and a high reliability of both powermeters (p < 0.001) for measurement of power (ICC = 0.926 and 0.936, respectively) and cadence (ICC = 0.969 and 0.970, respectively). However, during the supramaximal sprint test, their agreement to measure power and cadence was weak (r = 0.850 and -0.253, p < 0.05) due to the low reliability of the cadence measurements (ICC between 0.496 and 0.736, and 0.574 and 0.664, respectively; p < 0.05) in contrast to the high reliability of the cadence sensor (ICC = 0.987-0.994). In conclusion, both powermeters are valid and reliable for measuring power and cadence during continuous cycling efforts (~100-450 W), but questionable during sprint efforts (>500 W), where they are affected by the gear ratio used (PowerTap) and by their low accuracy in cadence recording (PowerTap and Power2Max).

ABSTRACT

PURPOSE: Monitoring is a fundamental part of the training process to guarantee that the programmed training loads are executed by athletes and result in the intended adaptations and enhanced performance. A number of monitoring tools have emerged during the last century in sport. These tools capture different facets (eg, psychophysiological, physical, biomechanical) of acute training bouts and chronic adaptations while presenting specific advantages and limitations. Therefore, there is a need to identify what tools are more efficient in each sport context for better monitoring of training process. METHODS AND RESULTS: We present and discuss the fine-tuning approach for training monitoring, which consists of identifying and combining the best monitoring tools with experts' knowledge in different sport settings, designed to improve (1) the control of actual training loads and (2) understanding of athletes' training adaptations. Instead of using single-tool approaches or merely subjective decision making, the identification of the best combination of monitoring tools to assist experts' decisions in each specific context (ie, triangulation) is necessary to better understand the link between acute and chronic adaptations and their impact on health and performance. Future studies should elaborate on the identification of the best combination of monitoring tools for each specific sport setting. CONCLUSION: The fine-tuning monitoring approach requires the simultaneous use of several valid and practical tools, instead of a single tool, to improve the effectiveness of monitoring practices when added to experts' knowledge.

ABSTRACT

The current view of hematopoiesis considers leukocytes on a continuum with distinct developmental origins, and which exert non-overlapping functions. However, there is less known about the function and phenotype of ontogenetically distinct neutrophil populations. In this work, using a photoconvertible transgenic zebrafish line; Tg(mpx:Dendra2), we selectively label rostral blood island-derived and caudal hematopoietic tissue-derived neutrophils in vivo during steady state or upon injury. By comparing the migratory properties and single-cell expression profiles of both neutrophil populations at steady state we show that rostral neutrophils show higher csf3b expression and migration capacity than caudal neutrophils. Upon injury, both populations share a core transcriptional profile as well as subset-specific transcriptional signatures. Accordingly, both rostral and caudal neutrophils are recruited to the wound independently of their distance to the injury. While rostral neutrophils respond uniformly, caudal neutrophils respond heterogeneously. Collectively, our results reveal that co-existing neutrophils populations with ontogenically distinct origin display functional differences.

ABSTRACT

PURPOSE: To identify the main training characteristics and competitive demands in women's road cycling. METHODS: A systematic search was conducted on 5 databases according to PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analysis) guidelines. The articles had to be primary studies, written after 1990 with a sample of competitive women between the ages of 15 and 50. The Quality Assessment Tool for Quantitative Studies and the Oxford Levels of Evidence scales were used. RESULTS: The search yielded 1713 articles, of which 20 were included. Studies on training and competitive demands (n = 5) found that both external and internal loads are higher in women than in men. Studies on strength and endurance training (n = 5) showed that both velocity-based and heavy-load strength training programs performed at least 2 days per week and including 3 to 4 lower-body exercises improved performance. Altitude-training studies (n = 3) found that "Live High-Train Low" was effective to increase performance during the first 9 days after the training camp. The 7 remaining studies focused on a range of topics. The methodological quality was strong for 12 studies and moderate for 8. In contrast, the level of evidence was high in 7 and low in the other 13. CONCLUSIONS: Endurance training and competitive demands in women's road cycling are higher than those of men. Strength training is effective in women when the frequency, intensity, and number of exercises are appropriate, while altitude training should be completed a few days before competing. Further studies are warranted to better define the participants' competitive level, using a methodological design with a higher level of evidence.

ABSTRACT

The aim of this work was to supplement a diet with chia seeds (Salvia hispanica L.) based on the requirements of finishing lambs for meat, and to analyze biometric parameters and fatty acid profiles in meat. Eighteen male Rambouillet lambs with a bodyweight of 25 kg were used. Animals were kept in individual pens with water and feed provided ad libitum. Three finishing diets were designed with the inclusion of 0, 50 and 100 g dry matter chia seeds and divided among the animals (n = 6). The experimental period lasted 60 days. The weights of the individual lambs were recorded every 14 days. At the end of the experiment, the animals were slaughtered and the weights of the hot carcasses and non-meat components were registered. In addition, an analysis of the fatty acid composition was carried out in the muscles (Longissimus thoracis). The total weight gain and average daily gain displayed significant differences (p < 0.05). Initial and final bodyweights, such as the dry matter intake, did not display differences. The fatty acid profile of the meat tended to decrease the SFA (stearic acid) and increase MUFA (oleic acid) (p < 0.0001) when chia seeds were added to the lamb diets. In conclusion, chia seed supplementation did not increase meat production or other biometric parameters; however, it modified the fatty acid profile in L. thoracis.

ABSTRACT

Previous research indicates that adequate choline nutrition during late gestation improves fetal development. However, there is a lack of studies describing choline's role during early gestation. Thus, the current study hypothesizes that an herbal mixture as a source of choline (Biocholine) positively affects offspring development from ewes supplemented during early gestation. Therefore, the objectives were to evaluate the impact of biocholine on the programming of the offspring early in life through the evaluation of dams and newborn performance. Twenty-eight four-year-old Rambouillet ewes were assigned randomly to two treatments: non-supplementation and 4 gd-1 of biocholine during the early gestation. Compared with the dams without supplementation, the ewes supplemented using biocholine showed no increase in parameters such as birth and weaning weight (p > 0.05). Additionally, the milk yield and quality of colostrum and milk did not present statistical differences (p > 0.05). However, the placental membrane development was reduced in the ewes that received supplementation with biocholine; interestingly, those dams increased the weight of the newborns during the lambing period (p < 0.05). Finally, the current study proposes necessary elucidation of how placental size is programmed and if less placental development has potential benefits in the fetus's development.

ABSTRACT

The current evidence suggests that pacing may not be affected by performance level or sex in trail-running races as may occur in road running races. However, the previous studies included races of >100 km. Therefore, we aimed to verify the influence of performance level and sex on pacing in the last four (2017, 2018, 2019, and 2021) editions of a shorter (56.3 km) ultra-trail running race (i.e., Orsières-Champex-Chamonix; OCC®) that maintained the same race profile. The mean finishing time for the 5656 participants was 10 h 20 min 33 s ± 2 h 01 min 19 s. Pacing variability (CV%) was higher in high-level participants, thus showing a greater ability to adapt their pace to the race profile than low-level runners. Males also had a higher pacing variability than females although the effect sizes were small. Based on the current findings, we may recommend for non-elite OCC® participants to adapt their pace to the race profile with a slower pace during uphills and a faster pace during downhills. Further studies including participants' experience are necessary to confirm the effectiveness of this suggestion in trail-running races of variable distances.

ABSTRACT

Various power meters are used to assess road-cycling performance in training and competition, but no previous study has analyzed their interchangeability in these conditions. Therefore, the purpose was to compare the data obtained from two different power meters (PowerTap vs. Power2Max) during cycling road races. A national-level under-23 male competitive cyclist completed six road-cycling official competitions (five road races and one individual time trial), in which power output was simultaneously registered with the two power meters. After this, the main power output variables were analyzed with the same software. The average and critical power obtained from the PowerTap power meter were slightly lower than from the Power2Max power meter (3.56 ± 0.68 and 3.62 ± 0.74 W·kg-1, 5.06 and 5.11 W·kg-1, respectively), and the correlations between both devices were very high (r ≥ 0.996 and p < 0.001). In contrast, the PowerTap power meter registered a significantly higher (p < 0.05) percentage of time at <0.75 and >7.50 W·kg-1 and power profile at 1, 5 and 10 s. In conclusion, the data obtained in competitions by the two power meters were interchangeable. Nevertheless, the Power2Max power meter underestimated the pedaling power during short and high-intensity intervals (≤10.0 s and >7.50 W·kg-1) compared to the PowerTap power meter. Therefore, the analysis of these efforts should be treated with caution.

The aim of this research was to assess the reliability of a specific change of direction test (i.e., "On-Court COD test") in youth badminton players, evaluate the effect of age on On-Court COD performance, and examine its correlations with linear speed, change of direction speed, and vertical jump tests. Forty-two young badminton players (27 males and 15 females; age: 17.3±1.6 years, body height: 170.3±7.5 cm, body mass: 59.0±9.7 kg) were divided into two age groups (Under 17 years and under 19 years). Tests included: linear sprints (5, 10, and 20-m), bilateral/unilateral countermovement jumps, a hexagon test, traditional and modified 505 change of direction tests; and On-court COD. Results showed an excellent intraclass correlation coefficient score (0.90) and a very low coefficient of variation values (1.6%) for the On-Court COD test. Comparing age groups, under 19 players were significantly faster in linear sprints (i.e., 5, 15 and 20-m; small to moderate effect sizes) and in all change of direction tests (moderate to large effect sizes). Moreover, the On-Court COD test showed moderate to large (r=.513-.779) relationships with both acceleration and COD abilities in under 17 players, and with linear sprints, COD, and jump performances in under 19 players. These data indicate that the On-Court COD test is a useful and reliable means to assess COD performance in youth badminton players and it is associated with acceleration, sprint and jump performance.

ABSTRACT

OBJECTIVES: To evaluate compliance with an Enhanced Recovery After Surgery (ERAS) protocol for open gynecologic surgery at a tertiary center and the relationship between levels of compliance and peri-operative outcomes. METHODS: This retrospective cohort study was conducted between November 2014 and December 2020. Two groups were defined based on compliance level (<80% vs ≥80%). The primary outcome was to analyze overall compliance since implementation of the ERAS protocol. The secondary endpoint was to assess the relationship between compliance and 30-day re-admission, length of stay, re-operation, opioid-free rates, and post-operative complications. We also assessed compliance with each ERAS element over three time periods (P1: 2014-2016, P2: 2017-2018, P3: 2019-2020), categorizing patients according to the date of surgery. Values were compared between P1 and P3. RESULTS: A total of 1879 patients were included. Overall compliance over the period of 6 years was 74% (95% CI 71.9% to 78.2%). Mean overall compliance increased from 69.7% to 75.8% between P1 and P3. Compliance with ERAS ≥80% was associated with lower Clavien-Dindo complication rates (grades III (OR 0.55; 95% CI 0.33 to 0.93) and V (OR 0.08, 95% CI 0.01 to 0.60)), 30-day re-admission rates (OR 0.61; 95% CI 0.43 to 0.88), and length of stay (OR 0.59; 95% CI 0.47 to 0.75). No difference in opioid consumption was seen. Pre-operatively, there was increased adherence to counseling by 50% (p=0.01), optimization by 21% (p=0.02), and carbohydrate loading by 74% (p=0.02). Intra-operatively, compliance with use of short-acting anesthetics increased by 37% (p=0.01) and avoidance of abdominal drainage increased by 7% (p=0.04). Use of goal-directed fluid therapy decreased by 16% (p=0.04). Post-operatively, there was increased compliance with avoiding salt and water overload (8%, p=0.02) and multimodal analgesia (5%, p=0.02). CONCLUSIONS: Over the time period of the study, overall compliance increased from 69.7% to 75.8%. Compliance (≥80%) with ERAS is associated with lower complication rates, fewer 30-day re-admissions, and shorter length of stay without impacting re-operation rates and post-operative opioid use.

Modifications in saddle height affect the range of movement of the lower limb's joints during pedalling. Although its effect on movement patterns is poorly understood. The purpose of this study was to analyse the acute effects of small changes in bicycle saddle height on pedalling coordination and its variability. Lower extremity kinematic data were collected in random order for ten well-trained cyclists while pedalling at three different saddle heights: preferred, 2% higher and 2% lower than preferred position. A dynamical systems approach was used to quantify the coordination and its variability for selected joint couplings. Modifications in saddle height produced large changes in the frequency of movement patterns, although they were not enough to alter the coordination classification. Lowering the saddle height increased the frequency of the proximal coordinative hip-ankle pattern (F = 11.77, p < .01) and knee-ankle couplings (F = 14.39, p < .01), while decreasing inphase coordination (F > 11.03, p < .01) during the propulsive phase. Pedalling coordination variability was not affected, being greatest during the movement transitions and when the ankle joint was included in the coupling. This study demonstrated that pedalling pattern coordination and coordination variability were generally stable to acute small changes in saddle height in well-trained cyclists.

INTRODUCTION: Kidney transplantation is the best therapeutical option for CKD patients. Graft loss risk factors are usually estimated with the cox method. Competing risk analysis could be useful to determine the impact of different events affecting graft survival, the occurrence of an outcome of interest can be precluded by another. We aimed to determine the risk factors for graft loss in the presence of mortality as a competing event. METHODS: A retrospective cohort of 1454 kidney transplant recipients who were transplanted between July 1, 2008, to May 31, 2019, in Colombiana de Trasplantes, were analyzed to determine risk factors of graft loss and mortality at 5 years post-transplantation. Kidney and patient survival probabilities were estimated by the competing risk analysis. The Fine and Gray method was used to fit a multivariable model for each outcome. Three variable selection methods were compared, and the bootstrapping technique was used for internal validation as split method for resample. The performance of the final model was assessed calculating the prediction error, brier score, c-index and calibration plot. RESULTS: Graft loss occurred in 169 patients (11.6%) and death in 137 (9.4%). Cumulative incidence for graft loss and death was 15.8% and 13.8% respectively. In a multivariable analysis, we found that BKV nephropathy, serum creatinine and increased number of renal biopsies were significant risk factors for graft loss. On the other hand, recipient age, acute cellular rejection, CMV disease were risk factors for death, and recipients with living donor had better survival compared to deceased-donor transplant and coronary stent. The c-index were 0.6 and 0.72 for graft loss and death model respectively. CONCLUSION: We developed two prediction models for graft loss and death 5 years post-transplantation by a unique transplant program in Colombia. Using a competing risk multivariable analysis, we were able to identify 3 significant risk factors for graft loss and 5 significant risk factors for death. This contributes to have a better understanding of risk factors for graft loss in a Latin-American population. The predictive performance of the models was mild.

ABSTRACT

BACKGROUND: The aim of this study was to analyze the shoulder functional profile (rotation range of motion [ROM] and strength), upper and lower body performance, and throwing speed of U13 versus U15 male handball players, and to establish the relationship between these measures of physical fitness and throwing speed. METHODS: One-hundred and nineteen young male handball players (under (U)-13 (U13) [n = 85]) and U15 [n = 34]) volunteered to participate in this study. The participating athletes had a mean background of sytematic handball training of 5.5 ± 2.8 years and they exercised on average 540 ± 10.1 min per week including sport-specific team handball training and strength and conditioning programs. Players were tested for passive shoulder range-of-motion (ROM) for both internal (IR) and external rotation (ER) and isometric strength (i.e., IR and ER) of the dominant/non-dominant shoulders, overhead medicine ball throw (OMB), hip isometric abductor (ABD) and adductor (ADD) strength, hip ROM, jumps (countermovement jump [CMJ] and triple leg-hop [3H] for distance), linear sprint test, modified 505 change-of-direction (COD) test and handball throwing speed (7 m [HT7] and 9 m [HT9]). RESULTS: U15 players outperformed U13 in upper (i.e., HT7 and HT9 speed, OMB, absolute IR and ER strength of the dominant and non-dominant sides; Cohen's d: 0.76-2.13) and lower body (i.e., CMJ, 3H, 20-m sprint and COD, hip ABD and ADD; d: 0.70-2.33) performance measures. Regarding shoulder ROM outcomes, a lower IR ROM was found of the dominant side in the U15 group compared to the U13 and a higher ER ROM on both sides in U15 (d: 0.76-1.04). It seems that primarily anthropometric characteristics (i.e., body height, body mass) and upper body strength/power (OMB distance) are the most important factors that explain the throw speed variance in male handball players, particularly in U13. CONCLUSIONS: Findings from this study imply that regular performance monitoring is important for performance development and for minimizing injury risk of the shoulder in both age categories of young male handball players. Besides measures of physical fitness, anthropometric data should be recorded because handball throwing performance is related to these measures.

SARS-CoV-2 es un coronavirus de ARN que causa infecciones respiratorias como la actual pandemia de COVID-19. Los sistemas de salud combaten esta infección con cuidados paliativos; sin embargo, existen pocos tratamientos específicos para este patógeno. Este contexto representa la posibilidad de buscar tratamientos alternativos, como el uso de moléculas naturales. El objetivo de este estudio fue determinar in silico la interacción de péptidos de plantas aromáticas con proteínas específicas de SARS-CoV-2 que no comprometan la respuesta inmune. Se procesaron quinientos ochenta y tres péptidos con menos de 30 aminoácidos de Thymus vulgaris L., Cymbopogon citratus, Salvia officinalis, Ocimum basilicum L y Zingiber officinale. La metodología aplicó filtros de acuerdo a los más altos puntajes de docking molecular para encontrar 20 péptidos por cada planta. Los péptidos registraron interacción molecular fuerte de los sitios activos de las proteínas Spike RBD, S2 y Nsp4, empleando una energía de menos de ­150 kcal/mol. La proteína Nsp4 mostró la mayor interacción con todas las especies. El 35% y el 65% de estos péptidos se registraron con baja activación de la respuesta inmune a través de la antigenicidad, puntuación inferior a 0,5 y ausencia de alergenicidad. Estos resultados indican el uso de moléculas de origen vegetal que pueden implementarse en el consumo para combatir la replicación viral del SARS-CoV-2.


SARS-CoV-2 is an RNA coronavirus that causes respiratory infections as the current COVID-19 pandemic. The health systems combat this infection with palliative care; however, there are few specific treatments for this pathogen. This context represents the possibility of searching for alternative treatments, such as using molecules from natural products. Our main objective was the in silico study of aromatic plant peptides and their interaction with specific proteins of SARS-CoV-2 that do not compromise the immune response. Five hundred eighty-three peptides with less than 30 amino acids from Thymus vulgaris L., Cymbopogon citratus, Salvia officinalis, Ocimum basilicum L, and Zingiber officinale were processed. The methodology applied filters according to the highest molecular docking scores to find 20 peptides for each plant species. The peptides show solid molecular interaction of the Spike RBD, S2, and Nsp4 proteins' active sites, using less than ­150 kcal/mol energy. Nsp4 protein exposes the most interaction with all species. 35 and 65% of these peptides were recorded with low activation of the immune response through antigenicity, score below 0.5, and absence of allergenicity. These results indicate the use of plant-derived molecules that can be implemented in consumption to combat the viral replication of SARS-CoV-2.

Introducción. La viruela del mono es una infección zoonótica con una tasa de transmisión global aumentada durante 2022. Actualmente, la enfermedad no tiene tratamientos específicos disponibles; por lo tanto, se puede lograr un enfoque preventivo a través de la inmunización. Objetivo. Diseño in sílico de una vacuna aplicando técnicas computacionales avanzadas utilizando una construcción de múltiples epítopos del M. virus. Materiales y métodos. Los antígenos se seleccionaron en base a informes sobre proteínas que provocan la activación de linfocitos T y B citotóxicos. Los ensayos inmunoinformáticos fueron antigenicidad, alergenicidad, toxicidad, afinidad de unión al complejo mayor de histocompatibilidad (CMH) y estimulación de IFN-γ. Resultados y discusión. Ocho epítopos de las proteínas M1R, ADN polimerasa, B6R y A35R de M. virus mostraron una respuesta significativa para las células inmunitarias. Se eligieron once epítopos con antigenicidad >0,3, no alergénicos y no tóxicos, de los cuales 4 presentaron alta afinidad por los linfocitos T, 4 generaron alta activación de linfocitos B y 3 se asociaron con resultados de activación de IFN-γ. La construcción in sílico del candidato vacunal de 509 aminoácidos con alta similitud topológica registró principalmente carga negativa, además de ser soluble con índice alifático >80%, estable y particular con activación CMH y alta afinidad molecular con TLR-3, y además presentó multiantigenicidad, similar a las vacunas generadas por esta metodología con M. tuberculosis e Influenza. La simulación de inyección de una dosis de la construcción molecular mostró la activación de las células plasmáticas auxiliares T durante aproximadamente 15 a 25 días y una alta expresión de IFN-γ e IL-2 durante ocho días. Conclusión. Estos resultados indican un excelente proceso de inmunización que podría potenciarse con dosis múltiples.


Introduction. Monkey pox is a zoonotic infection with an increased global transmission rate during 2022, denoted epidemiological trouble in public health. Currently, the disease has no specific treatments available; thus, a preventive approach can be achieved through immunization. Objective. was to design in silico a vaccine applying advanced computational techniques using a multi-epitope construct of the Monkeypox virus. Materials and methods. Antigens were selected based on reports about proteins that cause the activation of cytotoxic T and B lymphocytes. The immunoinformatics assays were antigenicity, allergenicity, toxicity, MHC binding affinity, and IFN-γ stimulation. Results and discussion. Eight epitopes of the M1R, DNA polymerase, B6R, and A35R proteins of the M. virus showed a significant response for immune cells. Eleven epitopes with antigenicity >0.3, non-allergenic and non-toxic were chosen, of which 4 presented high affinity to T lymphocytes, 4 generated high activation of B lymphocytes, and 3 were associated with IFN-γ activation results. The in silico construction of the 509-amino acid vaccine candidate with high topological similarity registered mainly a negative charge, in addition to being soluble with an aliphatic index >80%, stable and particular with MHC activation and high molecular affinity with TLR-3, and also presented multi-antigenicity, similar to vaccines generated by this methodology with M. tuberculosis and Influenza. One-dose injection simulation of the molecular construct showed activation of T helper plasma cells for about 15 to 25 days and high expression of IFN-γ and IL-2 for eight days. Conclusion. These results indicate an excellent immunization process that could be potentiated with multi-dosing.

PURPOSE: To analyze the influence of playing 2 consecutive prolonged badminton matches on the shoulder strength and range of motion (ROM) of young players. METHODS: Sixteen elite junior badminton players (12 males and 4 females; mean (SD): age 16.2 [0.8] years, body mass 63.5 [6.6] kg, height 173.2 [6.3] cm) participated in a cross-sectional study. Shoulder internal (IR)/external rotation (ER) ROM and IR/ER strength measures were conducted before and after 2 consecutive prolonged (ie, 35 min) matches and 12 hours after the second match. RESULTS: After consecutive matches, IR strength of the dominant side and ER strength of the dominant and nondominant sides (effect size [ES] = 0.20-0.57) were reduced. Shoulder total ROM of the dominant side was decreased (ES = 0.80), while on the nondominant side, IR (ES = 0.66) was also decreased. After 12 hours, results showed decreased values in the IR/ER strength of the dominant side (ES = 0.36-1.00), as well as ER of both dominant and nondominant sides (ES = 0.30-0.59). IR ROM of the nondominant side (ES = 0.69) was also decreased. CONCLUSION: Present results showed that 2 consecutive matches on the same day with brief rest periods led to significant impairments in shoulder strength and ROM levels. These data can potentially elucidate the need for shoulder-specific training and recovery strategies prior to or during competitions.

ABSTRACT

We tested whether maternal energy supplementation during the last third of gestation improves birth weight, neonatal wellbeing, and mother-young bonding. Thirty-six pregnant French Alpine goats were randomly allocated among three nutritional treatments for the last third of pregnancy: (i) Control, fed alfalfa (T-0; n = 12); (ii) alfalfa + 150 g/head daily energy concentrate (T-150; n = 12); (iii) alfalfa + 300 g/head daily energy concentrate (T-300; n = 12). At birth, we collected progeny data on birth weight, birth type, sex, rectal temperature, heart rate, respiratory rate, time to standing, time to udder connection, and time to first feeding. For the dams, we collected data on the duration of labor, time to clean the progeny, and time to allow first suckling. At birth, body weight, rectal temperature, heart rate, and the respiratory rate did not differ among treatments (p > 0.05). In the dams, labor duration was not affected by the treatments (p > 0.05). The T-150 dams were faster to clean the newborn and allow first suckling (p < 0.05). The T-150 progeny were faster to stand and the T-300 progeny were faster to connect to the udder (p < 0.05). We conclude that energy supplementation of the dam during the last third of gestation does not affect the birth weight of the progeny, but enhances the mother-young bonding.
