ABSTRACT

Portable X-ray fluorescence is a new tool in the study of human bone. This research aims to investigate if variations in bone elemental concentrations are related with porous skeletal lesions (PSLs). One hundred well-preserved non-adult skeletons aged 0-11 years were selected from the archaeological site Convent of São Domingos, Lisbon (18th-19th century). Measuring a standard reference material and calculating the technical error of measurement assured elemental data reliability. Moreover, measuring soil samples excluded possible contamination of bones with elements from the soil, except for Pb. Additionally, the Ca/P ratio indicates maintenance of bone integrity. Cribra cranii, orbitalia, humeralis, and femoralis were recorded as present/absent, and the estimated intra-/inter-observer errors were low. The multivariate analysis found higher odds of having cribra orbitalia (OR = 1.76; CI = 0.97-3.20) and cribra femoralis (OR = 1.42; CI = 0.73-2.74) in individuals with lower Fe and higher S. Furthermore, higher levels of P, Ca, and Sr increased the odds of individuals developing cribra femoralis (OR = 2.30; CI = 1.23-4.29). Age also correlated with increased odds of exhibiting cribra orbitalia (OR = 1.86; CI = 0.94-3.68), cribra femoralis (OR = 6.97; CI = 2.78-17.45), and cribra humeralis (OR = 8.32; CI = 2.71-25.60). These findings suggest a shared etiology for these three cribras, contrasting with the higher Fe levels in individuals with cribra cranii. Lower Fe and higher S levels in individuals with cribra suggest a complex etiology, possibly involving conditions like megaloblastic or chronic disease anemia(s). Age-related elemental changes support the hypothesis that age influences cribra frequencies. This study highlights PSL complexity and opens new avenues for research.

ABSTRACT

OBJECTIVES: Search for possible associations between bone elemental concentration and the presence of porous skeletal lesions (PSLs), considering the sex, age, and cause of death (COD) of the individuals. MATERIALS AND METHODS: The sample comprised 107 non-adult individuals (56 females, 51 males) aged 0-20 (x̄ = 13.2, SD = 5.8) from the Coimbra and Lisbon Identified Skeletal Collections. Cribra cranii, orbitalia, humeralis, and femoralis were recorded as present/absent, and elemental concentrations were assessed by portable x-ray fluorescence (pXRF). A multivariate statistical approach was applied. RESULTS: Well-preserved skeletons with minimal diagenesis showed no sex-related elemental variations or PSL associations. In contrast, age-at-death correlated with elevated Ca, P, Sr, and Pb levels. Cribra cranii increased with age while other cribra declined post-adolescence. Higher concentrations of Fe and lower of S were linked to cribra cranii. Respiratory infections as COD increased the odds of expressing cribra femoralis (OR = 5.25, CI = 1.25-15.14), cribra cranii (OR = 2.91, CI = 0.97-8.69), and cribra orbitalia (OR = 2.76, CI = 1.06-7.24). DISCUSSION: Feasible pXRF results and low cribra intraobserver error assure replicability. Elevated Ca, P, and Sr in older individuals may relate to skeletal growth, while increased Pb suggests bioaccumulation. Cribra's increase with age reflects different rates of marrow conversion and bone remodeling. Higher Fe and lower S in individuals with cribra cranii possibly reflects poor nutrition, early alcohol use, and sideroblastic anemia, aligning with 19th-20th-century Portugal's living conditions. Respiratory infections increased cribra expression, revealing intricate interplays among inflammation, anemia(s), marrow expansion, and diet. This research highlights a complex scenario and blazes a new path for cribra interpretation.

ABSTRACT

Referees are crucial elements in football, and they must meet the physical and physiological demands each match poses to them. The aim is to analyse the physical and physiological demands of amateur referees in games at the regional level (4th division), examining the differences between the first and second halves and between assistant (age: 25.10 ± 4.97) and main referees (age: 25.65 ± 5.12). A total of 29 matches were analysed with GPS devices, and internal and external load metrics were analysed. Overall, main referees, due to their central role in game management, showed higher levels of physical and physiological load than assistant referees, with more high-intensity activities, greater distance covered and higher heart rate. The results also revealed that there were no differences between the halves for total distance covered for either the main or assistant referees. However, the main referees covered a greater distance in high-intensity running during the first half (p = 0.05; d = 0.389). These findings emphasise the importance of tailored training protocols to enhance performance and reduce fatigue-related errors, highlighting the significance of endurance, high-intensity running ability, and strategies to manage transient fatigue in referee preparation.

ABSTRACT

Cagarras Islands Archipelago, a no-take MPA in Southeast Brazil, was designated as Natural Monument (MONA Cagarras) and, more recently, recognized as Hope Spot for biodiversity conservation. This study aimed to assess plastic contamination by analyzing marine litter and microplastics in MONA Cagarras and surrounding waters. Marine litter (34.12 kg) was caught by artisanal fishermen in MONA Cagarras proximities, and plastics represented ∼79 %. Personal hygiene items and strains of hair were found, suggesting sewage-derived contamination from Ipanema SSO. Microplastics were detected in MONA Cagarras surface waters. Fragments and black particle were the most frequently found microplastic shape and color, respectively. µ-FTIR analysis identified, in descending order of occurrence, polystyrene-PS, polyethylene-PE, polyvinyl chloride-PVC, polypropylene-P, and polyamide-PA. Our integrated results of macro and microplastic contamination highlight an issue of effective conservation and health of marine biodiversity in MONA Cagarras and surrounding waters and a concern for better management of Brazilian MPAs.

ABSTRACT

Sorafenib is a multikinase inhibitor indicated for first-line treatment of unresectable hepatocellular carcinoma. Despite its widespread use in the clinic, the existing knowledge of sorafenib mode-of-action remains incomplete. To build upon the current understanding, we used the Cellular Thermal Shift Assay (CETSA) coupled to Mass Spectrometry (CETSA-MS) to monitor compound binding to its target proteins in the cellular context on a proteome-wide scale. Among the potential sorafenib targets, we identified aldehyde dehydrogenase 2 (ALDH2), an enzyme that plays a major role in alcohol metabolism. We validated the interaction of sorafenib with ALDH2 by orthogonal methods using pure recombinant protein, proving that this interaction is not mediated by other cellular components. Moreover, we showed that sorafenib inhibits ALDH2 activity, supporting a functional role for this interaction. Finally, we were able to demonstrate that both ALDH2 protein expression and activity were reduced in sorafenib-resistant cells compared to the parental cell line. Overall, our study allowed the identification of ALDH2 as a novel sorafenib target and sheds light on its potential role in both hepatocellular carcinoma and sorafenib resistance condition.

ABSTRACT

Host cell proteins (HCPs) are process-related impurities expressed by the host cells during biotherapeutics' manufacturing, such as monoclonal antibodies (mAbs). Some challenging HCPs evade clearance during the downstream processing and can be co-purified with the molecule of interest, which may impact product stability, efficacy, and safety. Therefore, HCP content is a critical quality attribute to monitor and quantify across the bioprocess. Here we explored a mass spectrometry (MS)-based proteomics tool, the sequential window acquisition of all theoretical fragment-ion spectra (SWATH) strategy, as an orthogonal method to traditional ELISA. The SWATH workflow was applied for high-throughput individual HCP identification and quantification, supporting characterization of a mAb purification platform. The design space of HCP clearance of two polishing resins was evaluated through a design of experiment study. Absolute quantification of high-risk HCPs was achieved (reaching 1.8 and 4.2 ppm limits of quantification, for HCP A and B respectively) using HCP-specific synthetic heavy labeled peptide calibration curves. Profiling of other HCPs was also possible using an average calibration curve (using labeled peptides from different HCPs). The SWATH approach is a powerful tool for HCP assessment during bioprocess development enabling simultaneous monitoring and quantification of different individual HCPs and improving process understanding of their clearance.

ABSTRACT

OBJECTIVE: X-ray fluorescence (XRF) is a non-destructive technique that measures the elemental concentration of different materials, including human bone. Recently, it began to be applied to paleopathological studies due to the development of portable devices and their relative ease of use. However, the lack of uniform procedures hampers comparability and reproducibility. This paper aims to provide guidelines for an efficient and standardized evaluation of bone elemental composition with a portable XRF (pXRF) device. MATERIALS: This technical note is based on the application of the Thermo Scientific Niton XL3t 900 GOLDD+. METHODS: This work includes suggestions for the choice and preparation of human bone samples, both from archaeological context and documented collections, and methodological procedures in pXRF setup, such as choice of calibration, assessment of accuracy, and analysis run time. Additionally, recommendations for data validation and statistical analysis are also included. CONCLUSIONS: This technique has great potential in paleopathology since bone chemical variations may be associated with different pathological conditions, environmental contamination (e.g., lead), and/or administered treatments, such as mercury. Following an expected increase in the number of studies, it is essential to establish good practices that allow results from different researchers to be comparable. SIGNIFICANCE: X-ray fluorescence is a non-destructive technique that measures small concentrations (ppm) of elements from magnesium (12Mg) through bismuth (83Bi). LIMITATIONS: pXRF does not detect elements lighter than Mg, and its lower energy excitation penetrates less than other techniques. SUGGESTIONS FOR FURTHER RESEARCH: Other research groups should test these guidelines and comment on their usefulness and replicability.

ABSTRACT

The benefits of Pilates have been extensively researched for their impact on muscular, psychological, and cardiac health, as well as body composition, among other aspects. This study aims to investigate the influence of the Pilates method on the learning process, motor control, and neuromuscular trunk stabilization, specifically in both experienced and inexperienced practitioners. This semi-randomized controlled trial compares the level of experience among 36 Pilates practitioners in terms of motor control and learning of two Pilates-based skills: standing plank and side crisscross. Data will be collected using various assessment methods, including abdominal wall muscle ultrasound (AWMUS), shear wave elastography (SWE), gaze behavior (GA) assessment, electroencephalography (EEG), and video motion. Significant intra- and inter-individual variations are expected, due to the diverse morphological and psychomotor profiles in the sample. The adoption of both linear and non-linear analyses will provide a comprehensive evaluation of how neuromuscular structures evolve over time and space, offering both quantitative and qualitative insights. Non-linear analysis is expected to reveal higher entropy in the expert group compared to non-experts, signifying greater complexity in their motor control. In terms of stability, experts are likely to exhibit higher Lyapunov exponent values, indicating enhanced stability and coordination, along with lower Hurst exponent values. In elastography, experienced practitioners are expected to display higher transversus abdominis (TrA) muscle elasticity, due to their proficiency. Concerning GA, non-experts are expected to demonstrate more saccades, focus on more Areas of Interest (AOIs), and shorter fixation times, as experts are presumed to have more efficient gaze control. In EEG, we anticipate higher theta wave values in the non-expert group compared to the expert group. These expectations draw from similar studies in elastography and correlated research in eye tracking and EEG. They are consistent with the principles of the Pilates Method and other scientific knowledge in related techniques.

ABSTRACT Purpose: This study investigates the protective effect of cilostazol on the development and evolution of diabetic retinopathy in rats. Methods: Sixty male rats were divided into four groups: untreated nondiabetic rats, untreated diabetic rats, cilostazol-treated nondiabetic rats, and cilostazol-treated diabetic rats. The thickness of the internal limiting membrane to the outer limiting membrane, inner plexiform layer, inner nuclear layer, and outer nuclear layer were measured. The number of cell nuclei per 50-μm length in retinal sections was counted to quantify the degree of retinal cell loss. Results: The number of nuclei in the ganglion cell layer was significantly higher in untreated nondiabetic rats (p<0.05). The mean number of nuclei in the cilostazol-treated nondiabetic rats was significantly higher than that in the cilostazol-treated diabetic rats (p<0.05). The cilostazol-treated nondiabetic rats had a significantly higher mean nuclei count in the inner nuclear layer and inner plexiform layer as compared with the other groups (p<0.05). The total mean retinal thickness of the cilostazol-treated nondiabetic rats was significantly higher than that of cilostazol-treated diabetic rats and untreated diabetic rats (p<0.05). Conclusion: By decreasing the loss of ganglion cells and reducing the sensorineural atrophy in the internal retinal layers, cilostazol had a protective effect against changes caused by diabetic retinopathy in diabetic rats.


RESUMO Objetivo: O objetivo deste estudo foi investigar o efeito protetor do cilostazol no desenvolvimento e na evolução da retinopatia diabética em ratos. Métodos: Sessenta ratos machos foram divididos em 4 grupos: ratos não-diabéticos não-tratados, ratos diabéticos não-tratados, ratos não-diabéticos tratados com cilostazol e ratos diabéticos tratados com cilostazol. A espessura da membrana limitante interna à membrana limitante externa, a camada plexiforme interna, a camada nuclear interna e a camada nuclear externa foram medidas. Para quantificar o grau de perda de células da retina, foi contado o número de núcleos de células por 50 μm de comprimento em secções retinianas. Resultados: O número de núcleos no GCL foi significativamente maior em Ratos não-diabéticos não-tratados com cilostazol (p<0,05). O número médio de núcleos em Ratos não-diabéticos tratados com cilostazol foi significativamente maior do que em Ratos diabéticos tratados com cilostazol (p<0,05). A contagem média de núcleos em camada nuclear interna e camada plexiforme interna de ratos não-diabéticos tratados com cilostazol foi significativamente maior do que nos outros grupos (p<0,05). A espessura retiniana média total de Ratos não-diabéticos tratados com cilostazol foi significativamente maior do que em Ratos diabéticos tratados com cilostazol e Ratos diabéticos não-tratados (p<0,05). Conclusão: Os resultados demonstraram que o cilostazol teve um efeito protetor contra as alterações causadas pela retinopatia diabética em ratos diabéticos, diminuindo a perda de células ganglionares e reduzindo a atrofia neurossensorial nas camadas retinianas internas.

ABSTRACT

Pericarditis is a relatively common diagnosis worldwide; however, there are few data published on the frequency, diagnosis, and management of pericardial diseases in pregnant women. Ultrasonography has established its utmost importance and is worldwide recognized in pregnancy and fetal evaluation. Moreover, point-of-care ultrasonography of pregnant women, guided by clinical examination and history, can play an equally fundamental role. We present a case of a 37-year-old pregnant woman who presented at the emergency department with pleuritic chest pain, fever, and cough. Bedside point-of-care ultrasonography confirmed pericarditis revealing an organized pericardial effusion, leading to patient hospitalization and initiation of therapy. The importance and acknowledgment of portable and hand-held ultrasonography devices are growing since it enables physicians not only to make a fast and accurate diagnosis but also to access evolution in inpatient and outpatient settings.

Esse estudo seccional, realizado no período 2014-2015, com aplicação de questionários, teve por objetivo conhecer a percepção sobre vertebrados em condição de sinantropia e a opinião sobre controle populacional para a população do município de Niterói, RJ. Dentre os 474 entrevistados, 76,8% aceitavam o controle populacional para ratos, 63,3% para pombos, 40,5% para cães, 33,8% para gatos, 25,5% para morcegos, 16,5% para serpentes e 14,1% para gambás. As serpentes foram os animais mais associados ao medo (59,1%) e risco de agressão (47,7%), pombos (89,2%) e morcegos ao risco de doenças (57,4%), ratos à nojo/repulsa (56,6%), gambás à natureza (73,8%), cães à companhia (79,8%), e gatos à amizade (57,4%). O estudo demonstrou que as relações dos humanos com os animais podem ser complexas, ambíguas e paradoxais, e fornece dados que poderão ser utilizados para políticas públicas de manejo dessas espécies.(AU)


This study (sectional investigation), in the 2014-2015, using questionnaires, aimed to understand the perception of vertebrates in synanthropic conditions and the opinion about their population control for the population of the municipality the Niterói, RJ. Among the 474 people interviewed, 76.8% accepted population control for rats, 63.3% for pigeons, 40.5% for dogs, 33.8% for cats, 25.5 % for bats, 16.5% for snakes and 14.1% for possums. Snakes were the animals most associated with the representation of fear (59.1%) and risk of aggression (47.7%), pigeons (89.2%) and bats with risk of disease (57.4%), rats with disgust and repulsion (56.6%), possums with nature (73.8%), dogs with company (79.8%), and cats with friendship (57.4%). In addition to demonstrating how complex human relations with animals can be ambiguous and paradoxical, this study provides data that can be used for public policies for the management of these species.(AU)


El objetivo de este estudio fue conocer la percepción de los vertebrados en condiciones de sinantropía y la opinión sobre el control poblacional de la población del municipio de Niterói, RJ. Entre los 474 entrevistados, el 76,8% aceptó el control de la población de ratas, el 63,3% de palomas, el 40,5% de perros, el 33,8% de gatos, el 25,5% de murciélagos, el 16,5% de serpientes y el 14,1% de zarigüeyas. Las serpientes fueron los animales más asociados al miedo (59,1%) y al riesgo de agresión (47,7%), las palomas (89,2%) y los murciélagos al riesgo de enfermedad (57,4%), las ratas al asco/repulsión (56,6%), las zarigüeyas a la naturaleza (73,8%), los perros a la compañía (79,8%) y los gatos a la amistad (57,4%). Abstract Objective To analyze the influence of selenium in female fertility. Data sourceA search was performed in the following databases: MEDLINE, Web of Science, Scopus, SciELO, LILACS, MDPI, ScienceDirect, and Europe PMC. The descriptors selected were "selenium" AND "female" AND "fertility". The search interval was from 1996 to 2021. Study selectionThe evaluation was performed independently by two reviewers, and a third reviewer confirmed the inclusion of papers in case of divergence between the first two reviewers. Papers were selected after the title and abstract were read, and those that met the eligibility criteria had the full text read. Data collectionThe following data was extracted: author, year of publication, country, type of study, objective, method, sample size, follow-up period, patients' mean age, inclusion and exclusion criteria, and concentration of serum and capillary selenium. The data was organized in chronological order of paper publication. Data synthesisThe number of papers identified totaled 3,800, out of which 7 were included in the systematic review. The studies indicated a positive correlation between serum selenium and antioxidant concentration in the follicular fluid, reduction in antithyroid antibodies, oocyte production and follicle number. Conclusion Selenium supplementation is promising in women with this micronutrient deficiency to promote improvement of the reproductive efficiency and prevent damage to the pregnancy. Further studies on this theme are still required.

Fonte dos dadosUma busca foi realizada nas seguintes bases de dados: MEDLINE, Web of Science, Scopus, SciELO, LILACS, MDPI, ScienceDirect e Europe PMC. Os descritores selecionados foram "selenium" AND "female" AND "fertility". O intervalo de busca foi de 1996 a 2021. Seleção dos estudosA avaliação ocorreu de maneira independente por dois revisores, sendo que um terceiro corroborou a eleição dos artigos em casos de divergência. Os estudos foram selecionados através da leitura do título e resumo, e aqueles que contemplaram os critérios de elegibilidade foram lidos na íntegra. Coleta dos dadosOs seguintes dados foram extraídos: autor, ano de publicação, país, tipo de estudo, objetivo, método, tamanho da amostra, tempo de acompanhamento, média de idade das pacientes, critérios de inclusão e exclusão, concentração de selênio sérico e capilar. Os dados foram organizados em ordem cronológica de publicação do estudo. Síntese dos dadosForam identificados 3.800 artigos e incluídos 7 estudos na revisão sistemática. Os resultados indicaram correlação positiva entre o nível de selênio sérico e a concentração de antioxidantes no fluido folicular; diminuição dos níveis de anticorpos antitireoidianos; produção de oócitos, e número de folículos. Conclusão A suplementação de selênio é promissora em mulheres com deficiência do micronutriente, a fim de promover melhora na eficiência reprodutiva e prevenir danos na gravidez. Salientou-se a necessidade de realização de mais estudos sobre o tema.

ABSTRACT

O objetivo deste estudo foi compreender a inserção e a atuação do profissional de Educação Física no NASF em Santos-SP. Trata-se de uma pesquisa de abordagem qualitativa, na qual participaram 3 profissionais de Educação Física. A coleta de dados se deu por entrevista semiestruturada, e os dados foram interpretados à luz da análise de conteúdo. Como resultado, compreende-se que a inserção dos profissionais no NASF é inerente ao cargo ocupado. A atuação se dá num cenário limitante em número de profissionais e legitimação, trabalho anterior isolado e uma prática pedagógica instrumental, entretanto, aberta aos ideários críticos. Assim, embora haja um potencial alinhamento da atuação em relação às diretrizes do NASF, conclui-se que ainda existem limites a superar (AU)


The aim was to comprehend the insertion and performance of the Physical Education professional in the Family Health Support Centers (NASF) in the city of Santos-SP. It is a qualitative research in which three Physical Education professionals participated as volunteers. For this, semi-structured interviews were carried out and the data were analyzed through content analysis. As a result, the insertion of these professionals in the NASF is inherent to the position held. The performance is developed in a limiting context in terms of number of professionals and legitimacy, previous separate work, and an instrumental pedagogical practice; however, it opens to critical framework. Although the performance is potentially aligned with the NASF, we concluded there are limits to surpass yet (AU).


El objetivo de este estudio fue conocer la inserción y desempeño del profesional de Educación Física en el NASF en Santos-SP. Se trata de una investigación cualitativa, en la que participaron 3 profesionales de la educación física. La recolección de datos se realizó a través de entrevistas semiestructuradas y los datos se interpretaron a la luz del análisis de contenido. Como resultado, se entiende que la inserción de profesionales em el NASF es inherente al cargo que ocupan. La actuación se desarrolla en un escenario que limita el número de profesionales y la legitimidad, un trabajo previo aislado y una práctica pedagógica instrumental, sin embargo, abierta a ideas críticas. ABSTRACT

OBJETIVO: Avaliar a influência das palmilhas customizadas na pressão plantar de diabéticos com neuropatia em comparação ao grupo sham. MÉTODOS: O método do estudo, devidamente registrado no Registro Brasileiro de Ensaios Clínicos ­ REBEC (Clinical Trial Brazilian Register) (http://www.ensaiosclinicos.gov.br/) RBR-5NQK4K, incluiu um ensaio clínico randomizado, controlado, prospectivo, duplo-cego, com uma amostra de 46 voluntários que serão randomizados aleatoriamente numa razão de 1: 1 para serem direcionados aos grupos intervenção e controle. O grupo intervenção receberá palmilhas customizadas, com barra retrocapital e placa de etil vinil acetato (EVA) no mesmo formato da barra retrocapital, com a finalidade de reduzir a pressão no antepé. No grupo controle, as palmilhas planas serão confeccionadas sem nenhum objetivo terapêutico. Este projeto foi conduzido de acordo com o padrão de protocolo para ensaios clínicos randomizados (SPIRIT). Junto com a avaliação clínica, serão coletados dados demográficos da amostra para identificar e confirmar a presença de neuropatia periférica, em seguida, será avaliada a podobarografia e, por fim, os pacientes responderão ao questionário FAAM para avaliação da funcionalidade do pé. O desfecho primário será a análise dos pontos de pressão em KiloPascal (kPa) no pé dos pacientes com neuropatia diabética por meio da podobarografia nos pacientes do grupo intervenção e controle. O desfecho secundário será a funcionalidade do pé nas atividades de vida diária através do questionário FAAM (Foot and Ankle Ability Measure), considerando os voluntários na avaliação inicial, 3º e 6º meses. CONSIDERAÇÕES FINAIS: os futuros resultados deste estudo nos mostrarão principalmente se há ou não uma alteração estrutural na análise da pressão plantar decorrente do uso contínuo da palmilha, além de avaliar se o uso da palmilha terapêutica é eficaz na funcionalidade do pé nos mesmos portadores quando comparado à palmilha sham.


| OBJECTIVE: To evaluate the influence of customized insoles in the plantar pressure of diabetes patients with neuropathy in comparison to the sham group. METHODS: The work method, duly registered at the Registro Brasileiro de Ensaios Clínicos ­ REBEC (Clinical Trial Brazilian Register) (http://www.ensaiosclinicos.gov.br/) RBR-5NQK4K, includes a randomized, controlled, prospective, doubleblinded clinical trial, with a sample of 46 volunteers that will be randomly randomized in a 1: 1 ratio to be referred to intervention and control groups. The intervention group will receive customized insoles, with a retrocapital bar and an ethyl vinyl acetate plaque (EVA) in the same shape as the retrocapital bar, in order to reduce the pressure on the forefoot. In the control group, flat insoles will be prepared without any therapeutic objective. This project was developed according to the standard protocol for randomized clinical trials (SPIRIT). Along with the clinical evaluation, demographic data of the sample will be collected to identify and confirm the presence of peripheral neuropathy, next, the pedobarographic will be evaluated, and finally, the patients will answer the FAAM questionnaire to assess foot functionality. The primary outcome will be analyzing pressure points in KiloPascal (kPa) in the patients' feet through pedobarographic of the patients in the intervention and control groups. The secondary outcome will be the foot functionality in activities of daily living through the FAAM (Foot and Ankle Ability Measure), considering the volunteers in the initial evaluation, third and sixth months. FINAL CONSIDERATIONS: Mainly, results of this study will show whether there is a structural alteration in the analysis of the plantar pressure due to the continuous use of insoles and present the evaluation of whether the use of therapeutic insoles improves the foot functionality of the same users when compared to sham insoles.

ABSTRACT

O objetivo deste estudo foi compreender a inserção e a atua-ção do profissional de Educação Física no NASF em Santos-SP. Trata-se de uma pesquisa de abordagem qualitativa, na qual participaram 3 pro-fissionais de Educação Física. A coleta de dados se deu por entrevista semiestruturada, e os dados foram interpretados à luz da análise de conteúdo. Como resultado, compreende-se que a inserção dos profis-sionais no NASF é inerente ao cargo ocupado. A atuação se dá num ce-nário limitante em número de profissionais e legitimação, trabalho an-terior isolado e uma prática pedagógica instrumental, entretanto, aberta aos ideários críticos. Assim, embora haja um potencial alinhamento da atuação em relação às diretrizes do NASF, conclui-se que ainda existem limites a superar.

Resumo Contexto A insuficiência venosa crônica (IVC) é uma doença comum, uma disfunção da bomba muscular da panturrilha que repercute na hemodinâmica das estruturas. Objetivos Analisar os efeitos dos exercícios de fortalecimento muscular da panturrilha na hemodinâmica venosa em pacientes com IVC. Métodos O estudo contou com a participação de 13 pacientes e 25 membros inferiores com IVC, classificados entre C1 e C5 da classificação Clínica, Etiologia, Anatomia e Fisiopatologia (CEAP), oriundos do ambulatório de cirurgia vascular do Hospital Universitário Regional dos Campos Gerais. As variáveis analisadas foram dinamometria isométrica, goniometria, perimetria, adipometria no início, primeiro mês, segundo mês e final da aplicação do protocolo de exercícios. Resultados Os valores da dorsiflexão e da plantiflexão aumentaram 5º (p < 0,001). Na adipometria notou-se uma redução de 5 mm (p < 0,001). Quando separados em grupos pela CEAP, na C2 houve aumento de 5º na dorsiflexão (p = 0,02) e na plantiflexão (p < 0,001); na C3 houve aumento de 5º na dorsiflexão (p = 0,003) e redução de 1 mm na adipometria (p < 0,004); na C1 houve aumento da perimetria de 1,2 cm (p = 0,04). Conclusões A aplicação de protocolos de exercícios deveria ser considerada como uma forma de tratamento para IVC, pois impacta positivamente em fatores de risco e nas funções que estão deterioradas pela patologia.


Abstract Background Chronic venous insufficiency (CVI) is a common disease that causes calf muscle pump dysfunction and has repercussions for the hemodynamics of the structures involved. Objectives To analyze the effects on venous hemodynamics of exercises to strengthen the calf muscles in patients with CVI. Methods The study analyzed 25 lower limbs with CVI, classified from C1 to C5 according to the Clinical, Etiology, Anatomy, and Pathophysiology (CEAP) classification, in 13 patients recruited from a Lymphedema and Angiodysplasia Clinic at the Hospital Universitário Regional dos Campos Gerais (Brazil). The variables analyzed were collected by isometric dynamometry, goniometry, leg circumference measurement, and adipometry at baseline, after 1 month and 2 months and at the end of the exercise protocol. Results Dorsiflexion and plantar flexion measurements increased by 5º (p < 0.001). Adipometry detected a reduction in 5 mm (p < 0.001). When grouped by CEAP class, C2 exhibited 5º increases in dorsiflexion (p = 0.02) and plantar flexion (p < 0.001); C3 exhibited a 5ºincrease in dorsiflexion (p = 0.003) and a 1mm reduction in adipometry (p < 0.004); and C1 exhibited a 1.2cm increase in leg circumference (p = 0.04). Conclusions Administration of exercise protocols should be considered as a treatment option for CVI, since it has a positive impact on risk factors and on the functions that are impaired by this pathology.

ABSTRACT Objective: to produce a predictive model for the incidence of COVID-19 cases, severity and deaths in Ponta Grossa, state of Paraná. Methods: this is an ecological study with data from confirmed cases of COVID-19 reported between March 21, 2020 and May 3, 2020 in Ponta Grossa and proportion of severity, hospitalization and lethality in the literature. A susceptible-infected-recovered (SIR) epidemic model was developed, and reproduction rate (R0), duration of epidemic, peak period, number of cases, hospitalized patients and deaths were estimated. Deaths were calculated by age group and in three scenarios: at day 24, at day 34, and at day 44 of the epidemic. Results: in the three scenarios assessed in this study, the variation in the number of cases was explained by an exponential curve (r2=0.74, 0.79 and 0.89, respectively, p<0.0001 in all scenarios). The SIR model estimated that, in the best scenario, the peak period will be around 120 days after the first case (between July 11, 2020 and July 25, 2020), estimated R0 will be 1.07 and will infect 0.23% of the population. In the worst scenario, peak period will involve 4,375 (95% CI; 4156-4594) cases and 825 (95% CI; 700-950) cases in the best scenario. Most cases and hospital admissions will involve patients aged 20 to 39 years, the number of deaths will be higher among the elderly and more pronounced among patients aged ≥80 years. Conclusion: this is the first study that provides COVID-19 projections for a municipality that is not a large capital. It shows a peak period at a later moment; therefore, the municipality will have more time to prepare and adopt protective measures to reduce the number of simultaneous cases.


RESUMEN Objetivo: obtener un modelo predictivo para la ocurrencia de casos, severidad y muertes por COVID-19 en Ponta Grossa-Paraná. Métodos: estudio ecológico con datos de casos confirmados de COVID-19 notificados del 21/03/2020 al 3/3/2020 en Ponta Grossa y proporción de severidad, hospitalización y letalidad en la literatura. Se construyó un modelo epidemiológico (SIR) infectado-recuperado susceptible y tasa de reproducción estimada (R0), duración de la epidemia, fecha pico, número de casos, hospitalizaciones y muertes. Este último por grupo de edad y en tres escenarios: a los 24 días, a los 34 días y a los 44 días de epidemia. Resultados: en los tres escenarios evaluados, la variación en el número de casos se explicó por una curva exponencial (r2 = 0.74, 0.79 y 0.89, respectivamente y p <0.0001 en total). El modelo SIR estimó que, en el mejor escenario, el pico ocurrirá alrededor de 120 días después del primer caso (entre el 7/11/2020 y el 25/7/2020), el R0 estimado será de 1.07 y alcanzará 0.23 % de habitantes infectados. En el peor de los casos, el pico estimado será de 4375 (IC del 95%: 4156-4594) y 825 (IC del 95%: 700-950) en el mejor de los casos. El mayor número estimado de casos y hospitalizaciones estará en el rango entre 20 y 39 años, el número de muertes será mayor entre los ancianos y más pronunciado entre ≥ 80 años. Conclusión: este es el primer estudio con proyecciones para COVID-19 en un municipio fuera de las grandes capitales y demostró que el pico llegará tarde, por lo tanto, el municipio tendrá más tiempo de preparación y que las medidas de protección pueden reducir el número simultáneo de casos.

Um modelo epidemiológico suscetível-infectado-recuperado (SIR) foi construído e estimadas taxa de reprodução (R0), duração da epidemia, data do pico, número de casos, hospitalizações e óbitos. Estas últimas por faixa etária e em três cenários: aos 24 dias, aos 34 dias e aos 44 dias de epidemia. Resultados: nos três cenários avaliados, a variação no número de casos foi explicada por uma curva exponencial (r2=0,74, 0,79 e 0,89, respectivamente e p<0,0001 em todos). O modelo SIR estimou que, no melhor cenário, o pico ocorrerá em torno de 120 dias após o primeiro caso (entre 11/07/2020 e 25/07/2020), o R0 estimado será 1,07 e chegará a 0,23% dos habitantes infectados. No pior cenário, o pico estimado será de 4375 (IC 95% 4156-4594) casos e 825 (IC 95% 700-950) no melhor cenário. O maior número estimado de casos e hospitalizações será na faixa entre 20 e 39 anos, o número de óbitos será maior entre idosos e mais acentuado entre ≥ 80 anos. Conclusão: este é o primeiro estudo com projeções para a COVID-19 em um município fora das grandes capitais e mostrou que o pico será tardio, portanto, o município terá mais tempo de preparo e que medidas protetivas podem reduzir o número simultâneo de casos.

ABSTRACT

Abstract Although stakeholder theory has been on the academic agenda for more than 30 years, empirical evidence is still scant about stakeholders' influence as determinant factors of local governments' performance. Evidence from 82 empirical studies on stakeholder influence as a source of local government effectiveness is assessed in this article. The choice of studies was based in a systematic literature review, encompassing articles published in the period from 2007 to 2017 in which stakeholder influences are considered determinant factors of local government performance. This research contributes to this field of knowledge by synthesizing the pertinent publications and pointing out an agenda for future studies.


Resumen Si bien la teoría de los stakeholders ha estado en la agenda académica desde hace más de 30 años, las evidencias empíricas son escasas acerca de la influencia de los stakeholders como factores determinantes del desempeño de los gobiernos locales. Las evidencias de 82 estudios empíricos sobre las influencias de los stakeholders como fuentes de la efectividad del gobierno local se evalúan aquí. La elección de los documentos se basó en una revisión sistemática de la literatura, que abarca artículos publicados en el período de 2007 a 2017 en los que las influencias de los stakeholders se consideran factores determinantes del desempeño del gobierno local. Este trabajo contribuye a este campo de conocimiento sintetizando las publicaciones pertinentes y señalando una agenda para futuros estudios.


Resumo Embora a teoria dos stakeholders esteja na agenda acadêmica nos últimos 30 anos, até o momento, existe pouca evidência sobre influência de stakeholders como fator determinante do desempenho de governos locais. A investigação examina os fatores determinantes do desempenho de governos locais através de artigos publicados em periódicos ranqueados no Journal of Citation Report. A escolha dos artigos foi embasada em revisão sistemática de literatura em publicações do período de 2007 a 2017 nas quais influências de stakeholders tenha sido usada como variável explanatória de desempenho de governos locais. Este artigo contribui para o campo de conhecimento sintetizando as publicações mais relevantes, apontando lacunas na literatura e propondo uma agenda para estudos futuros.

Abstract Objective: To compare two groups of patients - the coronary endarterectomy group, with patients undergoing coronary artery bypass grafting (CABG) with coronary endarterectomy (CE), and the control group, with patients undergoing CABG without CE. We analyzed the rate of major outcomes (perioperative acute myocardial infarction [AMI], stroke, and mortality) and minor outcomes (time of cardiopulmonary bypass [CPB], time of aortic clamp, and postoperative length of hospital stay). We also determined the rates of early graft patency in patients undergoing CE. Methods: We reviewed a database of patients submitted to CABG, with or without associated CE, between January 2011 and June 2017. Twenty-five patients submitted to CE were compared with 201 patients submitted only to conventional surgery; the two groups presented similar preoperative characteristics and all the European System for Cardiac Operative Risk Evaluation (EuroSCORE) II variables did not presented statistically significant difference. We considered statistically significant values of P< 0.05. Results: There was no statistically significant difference in relation to time of post-surgical hospitalization (P=0.8139), incidence of perioperative AMI (P=0.2976), stroke (P=0,2976), and mortality rate (P=1.0000), but endarterectomy was associated with longer aortic clamping time (P=0.0004) and CPB time (P=0.0030). The rate of patency evaluated in patients submitted to endarterectomy (78,95%) was compatible with that described in the literature. Conclusion: In this sample, coronary endarterectomy was associated with the rate of early graft patency similar to that of the literature, with morbidity and mortality rates similar to those of conventional surgery.
