ABSTRACT

Introdução: O envelhecimento facial é um processo gradual, complexo e multifatorial. É o resultado de mudanças na qualidade, volume e posicionamento dos tecidos. Cirurgiões plásticos têm modificado sua abordagem na cirurgia do rejuvenescimento facial optando pelo plano subaponeurótico (SMAS). O objetivo deste estudo é analisar 100 casos de pacientes operados pela técnica de SMAS profundo, avaliando sua aplicabilidade e eficácia. Método: Foram avaliados 100 pacientes, submetidos a cirurgia plástica facial pela técnica de SMAS profundo - "Deep Smas", e acompanhados por 6 meses. Observou-se a satisfação dos pacientes, número de complicações, número de reoperações, riscos e vantagens da técnica. Resultados: Foram operados 100 pacientes, num período de 3 anos. A idade variou de 41 a 79 anos, sendo 95% sexo feminino. As complicações foram 8 casos (8%) de lesões de ramos do nervo facial, sendo: 4 casos lesão do zigomático, 3 casos de lesão do mandibular e 1 caso de lesão do bucal; houve 1 caso (1%) de queloide retroauricular; 1 caso (1%) de hematoma. Em relação às revisões cirúrgicas, houve 8 casos (8%) de complementação cirúrgica por insatisfação das pacientes. Houve 15% de lesões nervosas entre a 1ª e a 40ª cirurgia, 5% entre a 41ª e a 80ª, e nenhuma lesão entre o 81º e o 100º paciente. Conclusão: O lifting facial profundo ou subSMAS mostrou ser efetivo, proporcionando bons resultados estéticos. Apresenta baixa taxa de recidiva e baixa taxa de morbidade, porém, necessita de uma longa curva de aprendizagem.


Introduction: Facial aging is a gradual, complex, and multifactorial process. It is the result of changes in the quality, volume, and positioning of tissues. Plastic surgeons have modified their approach to facial rejuvenation surgery, opting for the subaponeurotic plane (SMAS). The objective of this study is to analyze 100 cases of patients operated on using the deep SMAS technique, evaluating its applicability and effectiveness. Method: 100 patients were evaluated, undergoing facial plastic surgery using the deep SMAS technique - "Deep Smas", and followed up for 6 months. Patient satisfaction, number of complications, number of reoperations, risks, and advantages of the technique were observed. Results: 100 patients were operated on over 3 years. Age ranged from 41 to 79 years, with 95% being female. The complications were 8 cases (8%) of injuries to branches of the facial nerve, of which 4 cases of zygomatic injury, 3 cases of mandibular injury, and 1 case of buccal injury; there was 1 case (1%) of post-auricular keloid; 1 case (1%) of hematoma. Regarding surgical revisions, there were 8 cases (8%) of surgical completion due to patient dissatisfaction. There were 15% of nerve injuries between the 1st and 40th surgery, 5% between the 41st and 80th, and no injuries between the 81st and 100th patient. Conclusion: Deep facial lifting or subSMAS has proven to be effective, providing good aesthetic results. It has a low recurrence rate and low morbidity rate; however, it requires a long learning curve.

Introdução: A demanda por cirurgia para correção de ginecomastia tem aumentado em função da grande quantidade de pacientes submetidos a tratamentos bariátricos. A maioria destes pacientes requerem grande remoção do excesso de pele, tecido gorduroso e reposicionamento do complexo areolopapilar, assim como adequação do volume da aréola. O objetivo deste estudo é descrever a técnica de correção de ginecomastia grau III através de cicatriz horizontal sem cicatriz vertical, com reposicionamento do complexo areolopapilar. Método: Estudo prospectivo de 27 pacientes masculinos com diagnóstico de ginecomastia grau III da classificação de Simon, operados em clínica particular, entre janeiro de 2013 e agosto de 2020, pela técnica de cicatriz horizontal com transposição do complexo areolopapilar e sem cicatriz vertical. Resultados: Foram operados 27 pacientes, num período de 7,5 anos, todos ex-obesos, submetidos previamente a tratamento de perda de peso. A idade variou de 17 a 74 anos, média etária de 52 anos. O peso das peças removidas variou de 175 a 758 gramas, média de 376 gramas. As complicações foram: 1 caso de hematoma tardio (3,7%), 1 caso de seroma (3,7%) e 2 casos de cicatriz hipertrófica (7,4%). Conclusão: O tratamento cirúrgico da ginecomastia grau III através da técnica de cicatriz horizontal, sem cicatriz vertical e com reposicionamento do complexo areolopapilar demonstrou ser efetivo, com baixa morbidade e bons resultados estéticos.


Introduction: The demand for surgery to correct gynecomastia has increased due to the many patients undergoing bariatric treatments. Most patients require extensive removal of excess skin, and fatty tissue, repositioning the nipple-areolar complex, and adjusting the areola volume. This study aims to describe the correction technique for grade III gynecomastia using a horizontal scar without a vertical scar, with repositioning of the nipple-areolar complex. Method: Prospective study of 27 male patients diagnosed with Simon grade III gynecomastia, operated in a private clinic between January 2013 and August 2020, using the horizontal scar technique with transposition of the nipple-areola complex and without a vertical scar. Results: 27 patients underwent surgery over 7.5 years, all ex-obese, previously submitted to weight loss treatment. Age ranged from 17 to 74 years, mean age of 52 years. The weight of the removed parts ranged from 175 to 758 grams, an average of 376 grams. Complications were 1 case of late hematoma (3.7%), 1 case of seroma (3.7%), and 2 cases of hypertrophic scarring (7.4%). Conclusion: Surgical treatment of grade III gynecomastia using the horizontal scar technique, without a vertical scar and with repositioning of the nippleareolar complex, proved to be effective, with low morbidity and good aesthetic results.

Introduction: The search for an anatomical substitute for the breast, for the purpose of correcting aesthetic problems has a long history of failure until the arrival of silicone. Since the beginning of its use, in the 1960s, many complications have appeared, such as infection, rejection, rupture, in addition to silicone leakage. We did not find in the literature, however, any report on the passage of materials from the human organism to the interior of implants. The objective is to identification by infrared spectrophotometry (FTIR) and clinical analysis, reporting the passage of organic substances into breast implants without any violation of their capsule. Methods: 1500 pairs of breast implants were analyzed, surgically removed from 1998 to 2018. Of which six were included in the study. Results: Three materials were analyzed showing macroscopic changes in their interior, without violating the capsule. A second sample was performed on a similar implant, but without use. The third material was a sample of fatty breast tissue removed from the patient during the surgery. Materials compatible with fat, animal protein and hemoglobin were found inside the implant. Conclusions: The change evidenced in the material inside the two implants indicates the occurrence of the passage of organic materials through an intact capsule.


Introdução: A busca de um substituto anatômico para a mama, para fins de correção de problemas estéticos, tem longa história de insucessos até a chegada do silicone. Desde o início de seu uso, na década de 1960, muitas complicações surgiram, como infecção, rejeição, rotura, além do extravasamento de silicone. Não encontramos na literatura, porém, relato algum sobre a passagem de materiais do organismo humano para o interior de implantes. O objetivo é a identificação por espectrofotometria de infravermelho (FTIR) e análise clínica, relatando a passagem de substâncias orgânicas para o interior de implantes mamários sem que os mesmos apresentem violação qualquer de sua cápsula. Método: Foram analisados 1500 pares de implantes mamários, removidos cirurgicamente no período de 1998 a 2018. Destes, seis foram encaminhados incluídos no estudo. Resultados: Foram analisados três materiais apresentando alterações macroscópicas em seu interior, sem que houvesse violação da cápsula. Uma segunda amostra foi realizada em implante semelhante, porém sem uso. O terceiro material foi uma amostra de tecido gorduroso mamário removido da própria paciente durante o ato cirúrgico. Foram encontrados materiais compatíveis com gordura, proteína animal e hemoglobina no interior do implante. Conclusões: A alteração evidenciada no material do interior dos dois implantes nos indica a ocorrência de passagem de materiais orgânicos através de cápsula intacta.

Introdução: O envelhecimento do terço superior da face se caracteriza por ptose da sobrancelha, rugas glabelares e rugas transversais frontais. O tratamento pode ser realizado através da frontoplastia coronal, da frontoplastia temporal com incisões limitadas e da frontoplastia endoscópica. O objetivo deste estudo é descrever a técnica de frontoplastia endoscópica subperiostal com miotomia dos músculos da região glabelar e fixação do retalho na fáscia temporal profunda, avaliando sua aplicabilidade e eficácia. Métodos: Foram avaliadas 24 pacientes, do sexo feminino, submetidas a frontoplastia endoscópica associada a blefaroplastia superior, com idade variando entre 37 e 72 anos, em um período de 10 anos. Medidas entre a distância da linha interpupilar e a porção superior da sobrancelha foram realizadas através de análise fotográfica com uso do sistema digital de imagem Mirror 6,0, na região medial, central e lateral de cada lado, no pré-operatório, e no pós-operatório de 6 meses. Resultados: A média de idade foi de 52 anos. Houve significância estatística (p<0,05) em todas as áreas da sobrancelha avaliadas, a média de foi de 0,3 cm mais alta. As complicações foram: 1 caso de extrusão fio, 1 caso de assimetria, 2 casos de correção insuficiente da sobrancelha e 2 casos de recidiva de rugas glabelares. Conclusão: A frontoplastia endoscópica subperiostal com miotomia dos músculos da região glabelar e flxação do retalho na fáscia temporal profunda com pontos demonstrou ser efetiva no tratamento do envelhecimento do terço superior da face, com resultados estatisticamente comprovados, baixa morbidade e bons resultados estéticos.


Introduction: The aging of the upper third of the face is characterized by ptosis of the eyebrow, glabellar wrinkles and frontal transverse wrinkles. Treatment can be performed through coronal frontoplasty, temporal frontoplasty with limited incisions and endoscopic frontoplasty. The aim of this study is to describe the technique of subperiosteal endoscopic frontoplasty with myotomy of the muscles of the glabellar region and fixation of the flap in the deep temporal fascia, evaluating its applicability and effectiveness. Methods: Twenty-four female patients who underwent endoscopic frontoplasty associated with upper blepharoplasty, aged between 37 and 72 years old, over a 10-year period were evaluated. Measurements between the distance from the inter-pupillary line and the upper portion of the eyebrow were performed through photographic analysis using the Mirror 6.0 digital image system, in the medial, central and lateral regions on each side, in the preoperative period, and in the 6 month postoperative period. Results: The average age was 52 years. There was statistical significance (p<0.05) in all evaluated eyebrow areas, the mean was 0.3 cm higher. Complications were: 1 case of wire extrusion, 1 case of asymmetry, 2 cases of insufficient correction of the eyebrow and 2 cases of recurrence of glabellar wrinkles. Conclusion: Subperiosteal endoscopic frontoplasty with myotomy of the muscles of the glabellar region and fixation of the flap in the deep temporal fascia with stitches, proved to be effective in the treatment of aging of the upper third of the face, with statistically proven results, low morbidity and good aesthetic results.

INTRODUCTION: The treatment of breast ptosis using mastopexy associated with the inclusion of a silicone prosthesis in a single surgical procedure is a challenge for surgeons. The aim of this study is to describe the 15-year experience with the placement of silicone breast implants in double, subfascial and submuscular space, in the treatment of patients with breast ptosis, and to analyze the aesthetic results of patients who underwent such surgeries. METHOD: During the 15-year period, between 2005 and 2020, 640 mastopexies were performed with the inclusion of silicone breast implants in double space, with high-profile round polyurethane prostheses whose volumes ranged from 135 to 435 ml, in patients with grade 2 and 3 breast ptosis. RESULTS: The age of the patients ranged from 18 to 55 years and the average age was 34 years. The postoperative follow-up time was 18 months. 400 patients (62.5%) had grade II ptosis, whereas 240 of them (37.5%) had grade III ptosis. The main complications were: 19 cases (3%) of residual sagging skin, 19 cases (3%) of unsightly scars, 12 cases (2%) of partial areola necrosis. There was no case of infection or seroma. There were 330 primary surgeries (52%) and 310 secondary surgeries (48%). CONCLUSION: After 15 years of performing mastopexy with prosthesis in double space, both in primary and secondary surgeries, it can be concluded that the technique reached its maturity providing good long-term results with the maintenance of the projection of the upper pole of the breast and low recurrence of ptosis in the lower pole and low rate of complications. Introdução: O tratamento da ptose mamária utilizando a mastopexia associada à inclusão de prótese de silicone em tempo cirúrgico único é um desafio para os cirurgiões. Existem várias técnicas descritas na literatura. O objetivo deste estudo é descrever a colocação de implantes mamários de silicone em plano subfascial, seguido de ampla dissecção anterior da fáscia do músculo peitoral maior separando-a totalmente do restante do parênquima mamário no tratamento de pacientes com ptose mamária; e analisar os resultados estéticos dos pacientes operados. Métodos: Durante o período de setembro de 2017 a fevereiro de 2019 foram realizadas 64 mastopexias com cicatriz em "T" invertido associadas à inclusão de implantes mamários de silicone em plano subfascial, bilateralmente, próteses redondas texturizadas de perfil alto cujos volumes variaram de 180ml a 380ml, em pacientes com ptose mamária. Resultados: A média de idade foi de 34 anos, sendo que variou de 19 a 55 anos. O tempo se seguimento pós-operatório foi de 1 a 18 meses. As principais complicações foram: 3 casos (4,6%) de flacidez residual de pele no seguimento de 8 meses; dois casos (3,1%) de cicatrizes inestéticas; um caso (1,5%) de necrose parcial de aréola. Não houve nenhum caso de infecção ou seroma. Conclusão: A técnica de colocação de implantes mamários de silicone em plano subfascial, seguido de ampla dissecção anterior da fáscia do músculo peitoral maior separando-a totalmente do restante do parênquima mamário foi efetiva no tratamento de pacientes com ptose mamária.

There are several techniques described in the literature. This study aims to describe the placement of silicone breast implants in the subfascial plane, followed by an extensive anterior dissection of the pectoralis major muscle fascia, totally separating it from the rest of the breast parenchyma in the treatment of patients with breast ptosis. Moreover, analyze the aesthetic results of operated patients. Methods: During the period from September 2017 to February 2019, 64 mastopexies with an inverted "T" scar were performed associated with silicone breast implants placed in the subfascial plane, bilaterally, textured high-profile round prostheses whose volumes ranged from 180ml to 380ml, in patients with breast ptosis. Results: The average age was 34 years, ranging from 19 to 55 years. The postoperative follow-up time was 1 to 18 months. The main complications were: 3 cases (4.6%) of residual skin flaccidity in the 8-month follow-up; two cases (3.1%) of unsightly scars; one case (1.5%) of partial areola necrosis. There was no case of infection or seroma. Conclusion: The technique of placing silicone breast implants in the subfascial plane, followed by an extensive anterior dissection of the pectoralis major muscle fascia, totally separating it from the rest of the breast parenchyma, was effective in the treatment of patients with breast ptosis.

ABSTRACT

Abdominal compartment syndrome is directly related to an increase in intraabdominal pressure (IAP), which can lead in severe cases to serious clinical consequences. Routine measurement of IAP in specific cases has been advocated by some surgical specialties. However, few studies in plastic surgery have focused on the use of IAP. The authors review the literature and describe a method of IAP analysis used for 12 patients who underwent abdominoplasty.

INTRODUCTION: Healing is a highly complex process with different phases. Because some medicinal plants have antiinflammatory properties, they could influence the wound healing process. There are many species of medicinal plants used for research, among them the Orbignya phalerata (Babassu). PURPOSE: To analyze, comparatively, the tensiometric and histologic alterations in the healing process of colon anastomosis with the use of aqueous extract of the babaçu mesocarp. METHODS: Forty Wistar male rats were used, divided in two groups of 20 animals named control and experimental group. Each group was further divided in two sub-groups of 10 animals and evaluated on the third and seventh post-operative days. Colotomy and one suture line coloraphy was performed in all animals. The control group received an intraperitoneal single dose of isotonic saline solution at the time of the surgery. The animals from the experimental group received babassu aqueous extract dose with 50mg/kg/weight, concentration of 25 mg/ml dose, with the same administration method. The following parameters were evaluated: a) macroscopic appearance of abdominal wall and cavity; b) tensiometric evaluation of anastomosis (bursting pressure); c) histologic characteristics of anastomosis. RESULTS: The macroscopic examination showed the presence of adherences in all animals with statistical significance in the ones from the experimental group at third day when compared to the control group. No difference was observed in the seventh-day group. No fistulae, abscesses and/or hemorrhage were observed. In regard to the tensiometric variation, the bursting pressure for the control and experimental groups, both for the third day (25.4 mmHg - 14.8 mmHg) and at seventh (183 mmHg - 175 mmHg), were similar with no significant difference. Histological analysis for the third day showed significant difference in the experimental group in relation to the controls, in regard the presence ofto mononuclear ones whereas at seventh day there was significant statistical difference in all variables from the experimental group compared to the control one. CONCLUSION: A favorable effect with administration of the aqueous extract of the babaçu mesocarp was seen in histological parameters of the healing process of colon anastomosis however there was no difference in the tensiometric evaluation between the control and experimental group.

INTRODUÇÃO: A cicatrização é processo complexo, com diferentes fases. Embora exista grande variedade de plantas medicinais poucas têm sido investigadas com intenção de melhorar a cicatrização de órgãos e tecidos, dentre elas, a Orbignya phalerata (babaçu). OBJETIVO: Analisar comparativamente as alterações tensiométricas e histológicas na cicatrização de anastomoses colônicas proporcionadas pelo uso do extrato aquoso do mesocarpo do babaçu (Orbignya phalerata). METODOS: Utilizaram-se 40 ratos Wistar, machos, separados em dois grupos de vinte, denominados grupos controle e experimento, Os ratos de cada grupo foram divididos em dois sub-grupos de 10 animais cada e avaliados no terceiro e sétimo dia de pós-operatório. Realizaram-se colotomia e colorrafia em plano único. Os animais do grupo controle receberam no ato operatório dose única de solução salina isotônica em dose e via de administração semelhantes ao grupo experimento e os do grupo experimento receberam extrato aquoso de babaçu em dose de 50 mg/kg/peso, concentração de 25 mg/ml por via intraperitoneal. Foram analisados os seguintes parâmetros: a) avaliação macroscópica da parede e cavidade abdominal; b) avaliação tensiométrica da anastomose (pressão e ruptura); c) características histológicas da anastomose. RESULTADOS: O exame macroscópico evidenciou a presença de aderências em todos os animais, sendo estatisticamente maior no grupo experimento de três dias quando comparados com o grupo controle. Nos animais do grupo de sete dias não houve diferença. Não houve ocorrência de deiscência de anastomose, fístulas, abscessos e/ou hemorragia. Com relação à tensiometria, constatou-se que a média de ruptura dos grupos controle e experimento tanto para os animais do grupo de três dias (25,4 mmHg - 14,8 mmHg) quanto para os de sete dias (183 mmHg - 175 mmHg), foram iguais, não ocorrendo diferença estatisticamente significante. ABSTRACT

INTRODUÇÃO: A cicatrização é processo complexo, com diferentes fases. Embora exista grande variedade de plantas medicinais poucas têm sido investigadas com intenção de melhorar a cicatrização de órgãos e tecidos, dentre elas, a Orbignya phalerata (babaçu). OBJETIVO: Analisar comparativamente as alterações tensiométricas e histológicas na cicatrização de anastomoses colônicas proporcionadas pelo uso do extrato aquoso do mesocarpo do babaçu (Orbignya phalerata). METODOS: Utilizaram-se 40 ratos Wistar, machos, separados em dois grupos de vinte, denominados grupos controle e experimento, Os ratos de cada grupo foram divididos em dois sub-grupos de 10 animais cada e avaliados no terceiro e sétimo dia de pós-operatório. Realizaram-se colotomia e colorrafia em plano único. Os animais do grupo controle receberam no ato operatório dose única de solução salina isotônica em dose e via de administração semelhantes ao grupo experimento e os do grupo experimento receberam extrato aquoso de babaçu em dose de 50 mg/kg/peso, concentração de 25 mg/ml por via intraperitoneal. Foram analisados os seguintes parâmetros: a) avaliação macroscópica da parede e cavidade abdominal; b) avaliação tensiométrica da anastomose (pressão e ruptura); c) características histológicas da anastomose. RESULTADOS: O exame macroscópico evidenciou a presença de aderências em todos os animais, sendo estatisticamente maior no grupo experimento de três dias quando comparados com o grupo controle. Nos animais do grupo de sete dias não houve diferença. Não houve ocorrência de deiscência de anastomose, fístulas, abscessos e/ou hemorragia. Com relação à tensiometria, constatou-se que a média de ruptura dos grupos controle e experimento tanto para os animais do grupo de três dias (25,4 mmHg - 14,8 mmHg) quanto para os de sete dias (183 mmHg - 175 mmHg), foram iguais, não ocorrendo diferença estatisticamente significante. INTRODUCTION: Healing is a highly complex process with different phases. Because some medicinal plants have antiinflamatory properties, they could influence the wound healing process. There are many species of medicinal plants used for research, among them the Orbignya phalerata (Babassu). PURPOSE: To analyze, comparatively, the tensiometric and histologic alterations in the healing process of colon anastomosis with the use of aqueous extract of the babaçu mesocarp. METHODS: Forty Wistar male rats were used, divided in two groups of 20 animals named control and experimental group. Each group was further divided in two sub-groups of 10 animals and evaluated on the third and seventh post-operative days. Colotomy and one suture line coloraphy was performed in all animals. The control group received an intraperitoneal single dose of isotonic saline solution at the time of the surgery. The animals from the experimental group received babassu aqueous extract dose with 50mg/kg/weight, concentration of 25 mg/ml dose, with the same administration method. The following parameters were evaluated: a) macroscopic appearance of abdominal wall and cavity; b) tensiometric evaluation of anastomosis (bursting pressure); c) histologic characteristics of anastomosis. RESULTS: The macroscopic examination showed the presence of adherences in all animals with statistical significance in the ones from the experimental group at third day when compared to the control group. No difference was observed in the seventh-day group. No fistulae, abscesses and/or hemorrhage were observed. ABSTRACT

A cirurgia mamária sofreu grandes modificações nos últimos anos, visando a diminuição das cicatrizes e a melhora e manutenção da sua forma, no pós ope- ratório tardio. Empregando-se a filosofia da técnica periareolar à técnica de cicatriz vertical, podemos reduzir o comprimento da cicatriz, evitando o seu prolongamento abaixo do sulco submamário e compensando o excesso de pele em torno da aréola. No período de março de 2001 a julho de 2002, 53 pacientes foram submetidas à mamaplastia redutora e/ou mastopexia utilizando-se a técnica de cicatriz vertical associado ao retalho de parede torácica mantido sob uma alça de músculo peitoral maior. O objetivo da utilização da combinação destas duas técnicas foi o de proporcionar um bom resultado sob o ponto de vista estético: cicatriz reduzida, báscula mínima e preenchimento do pólo superior mamário.


Breast surgery has been greatly modified in the past few years looking for shortening of scars and improvement and maintenance of breast shape in the late post-operative period. Working with the philosophy of periareolar technique and vertical scar technique altogether, its possible to reduce scar length, avoiding the area below the inframammary crease by compensating skin excess around the areola. From March 2001 to July 2002, 53 patients were submitted to reduction mammaplasty and/or mastopexy with vertical scar technique associated to the chest wall based flap kept under a bipedicled flap of pectoralis major muscle. The goal of this combination is to achieve a good result under the aesthetic view: reduced scar, minimal breast descent and good upper pole fullness.

ABSTRACT

The authors report their experience in using ultrasound-assited liposuction in 348 patients in the period between October 1998 and July 2001. The application of ultrasound brought additional benefits to results, such as improved skin accomodation and the treatment of difficult cases, such as, gynecomasty, dorsum and secondary liposuction. The technique was perfomed in three stages- super-wet infiltration (1.35-1), followed by ultrasound-assisted liposuction and conventional liposuction. The equipment used was a Lisonix 2000generator at the setting of 5, that is, 50 (per cent) of the device's power, which proved to be the setting that causes the least secondary effects, such as tissue fibrosis and seroma. This study also considers the history of ultrasound-assisted liposuction and focuses on the physical aspects of ultrasound, with a review of the literature on the use of the ultrasound technique.
