ABSTRACT

Objective: To use a self-developed, prefabricated cap for making interocclusal records in implant-supported fixed prosthetic treatment, and to evaluate its effect of clinical application and accuracy of transferring intra-oral intercuspal position. Methods: Series of prefabricated caps for occlusal recording of implant-supported fixed prostheses were designed based on the healing abutments, and fabricated with three-dimensional (3D) printing. According to the inclusion and exclusion criteria, 12 partially edentulous patients who visited the Department of Prosthodontics, Peking University School and Hospital of Stomatology between November 2020 and September 2021, had lost no fewer than 2 contiguous teeth in distal extension (Kennedy Class Ⅰ or Ⅱ), and had received implant placement 3 months ago were enrolled. Self-control study design was used. Two occlusal records of each included case were obtained by the following 2 methods: for the test group, appropriate prefabricated caps were used; and for the control group, polyvinyl siloxane occlusal record was directly set on the healing abutments. The working casts were mounted on the mechanical articulator using the 2 groups of occlusal records successively. Accuracy of occlusal relationship of the mounted casts was evaluated. Diagnostics test was performed to obtain the sensitivity and positive predictive value, which were determined in photographs by comparing the intra-oral occlusal contact points with those in the mounted casts, point-by-point. Virtual casts were taken by intraoral and extraoral scans in intercuspal position and imported to Geomagic Studio 2014. Then the root mean square values of occlusal clearance space between the upper and lower occlusal surfaces of remaining posterior teeth, and the deviations of the interocclusal position of the occlusal surfaces of the remaining teeth in the mandibular arches when superimposing the maxillary arches of the intraoral and extraoral scans were calculated. As a self-control design, paired t test was used. Results: Twelve participants were enrolled, including 3 men and 9 women aged (52.6±12.1) years, and 36 missing teeth. The prefabricated caps could fit on the healing abutments with good retention and stability. The sensitivity of the test group (0.73±0.14) was significantly better than that of control group (0.63±0.12,P<0.01), with no significant difference in positive predictive value between the 2 groups (P>0.05). The deviations of the interocclusal position of the occlusal surfaces of the remaining teeth were (357.0±140.2) µm for the test group, and (399.4±206.3) µm for the control group, with no significant difference between them (P>0.05). Conclusions: Interocclusal position record based on prefabricated cap in this study for implant-supported fixed prosthetic treatment can improve the consistency between the intra-oral occlusion and the occlusion in dental casts. This technique has good accuracy, clinical convenience and usability.

ABSTRACT

OBJECTIVE: To evaluate the effectiveness of the integrated schistosomiasis control measures in Changzhou City from 2015 to 2020, so as to provide insights into the formulation of the strategy used to consolidate the schistosomiasis elimination achievements. METHODS: The annual schistosomiasis control working report and integrated schistosomiasis control data were collected in Changzhou City from 2015 to 2020, and the prevalence of Schistosoma japonicum infections in humans and livestock and snail status were analyzed to evaluate the effectiveness of the integrated schistosomiasis control measures. RESULTS: During the period from 2015 to 2020, a total of 112 061 person-time individuals received serological tests for S. japonicum infections in Changzhou City, and the sero-prevalence was 0.15% to 1.09% during the 6-year period, with a significant difference seen among years (χ2 = 288.11, P < 0.05). From 2015 to 2020, a total of 13 435 person-time individuals received stool examinations, with no egg-positives identified; among 5 840 herd-time livestock receiving schistosomiasis examinations, no positives were detected, while a 100% coverage of fencing livestock was seen each year. During the 6-year period, a total of 38.40 hm2 snail habitats were found, including 8.97 hm2 emerging snail habitats, and among the 2 344 snails dissected, no S. japonicum infection was found. Chemical treatment covered an area of 385.71 hm2, and environmental improvements covered an area of 200.39 hm2. The mean density of living snails was less than 0.1 snails/0.1 m2 in snail habitats found in Changzhou City each year from 2015 to 2020, and the coverage of harmless toilets was 100% in 2020. During the 6-year period, a total of 3.740 6 million person-time individuals were given schistosomiasis health education in Changzhou City. CONCLUSIONS: Changzhou City is now at the post-elimination surveillance stage; however, there are still factors affecting schistosomiasis transmission. The schistosomiasis surveillance system remains to be improved to consolidate the schistosomiasis elimination achievements in Changzhou City.

ABSTRACT

OBJECTIVE: To understand the prevalence and risk factors of Toxoplasma gondii infections among high-risk populations in Changzhou City, so as to provide the scientific basis for formulating effective control measures of toxoplasmosis. METHODS: Four types of high-risk populations living in Changzhou City were recruited from 2016 to 2018 as the study subjects, including HIV/AIDS patients, cancer patients, pregnant women and livestock and poultry breeding or processing workers. Each subject was investigated for the knowledge on knowledge, attitude and practice of toxoplasmosis prevention and control. In addition, serum specific IgG and IgM antibodies against T. gondii were detected, and the risk factors of T. gondii infections were identified. RESULTS: A total of 900 participants at high risk of T. gondii infections were investigated in Changzhou City from 2016 to 2018, including 150 HIV/AIDS patients, 250 cancer patients, 250 pregnant women and 250 livestock and poultry breeding or processing workers. The overall awareness rate of toxoplasmosis prevention and control knowledge was 16.8% (151/900), and the awareness rate was significantly greater in women than in men (χ2 = 41.34, P < 0.05). The awareness rate of toxoplasmosis prevention and control reduced with ages (χ2 = 147.78, P < 0.05), and increased with the education level (χ2 = 166.42, P < 0.05). In ad dition, there was an occupation-specific awareness rate of toxoplasmosis prevention and control (χ2 = 92.26, P < 0.05), and the highest awareness rate was seen in cadres and staff (47.2%, 34/72). Among all high-risk populations, 34.6% (311/900) had fre- quent contacts with cats/dogs, 40.4% (364/900) raised cats/dogs at home, 0.9% (8/900) ate raw meat, and 15.8% (142/900) sepa- rated chopping boards for raw and cooked food. Among the participants aware of toxoplasmosis prevention and control knowledge, 24.5% (37/151) contacted cats/dogs frequently, which was significantly lower than those not aware of toxoplasmosis prevention and control knowledge (36.6%, 274/749) (χ2 = 8.11, P < 0.05), and 35.1% (53/151) separated chopping boards for raw and cooked food, which was significantly higher than those not aware of toxoplasmosis prevention and control knowledge (11.9%, 89/749) (χ2 = 50.97, P < 0.05). The overall seroprevalence of T. gondii infections was 11.0% (99/900), and the positive rates of IgG antibodies against T. gondii were 6.0%, 13.9%, 4.8% and 17.3% in HIV/AIDS patients, livestock and poultry breeding or processing workers, pregnant women and cancer patients, respectively (χ2 = 25.87, P < 0.05). A higher seroprevalence of T. gon- dii infection was seen in men than in women (χ2 = 8.88, P < 0.05), and the seroprevalence increased with ages (χ2 = 37.03, P < 0.05) and reduced with education levels (χ2 = 25.07, P < 0.05). There was an occupation-specific seroprevalence of T. gondii in- fection (χ2 = 22.09, P < 0.05), and the highest seroprevalence was detected in peasants (57/330, 17.3%). CONCLUSIONS: The awareness of toxoplasmosis prevention and control knowledge is low among high-risk populations in Changzhou City. Health edu- cation pertaining to toxoplasmosis prevention and control requires to be strengthened in cancer patients and livestock and poultry breeding or processing workers who have a high seroprevalence of T. gondii infections, so as to change the poor behavior styles.

ABSTRACT

BACKGROUND: Although numerous multicentre studies have estimated the association between ozone exposure and mortality, there are currently no nationally representative multicentre studies of the ozone-mortality relationship in China. OBJECTIVE: To investigate the effect on total (nonaccidental) and cause-specific mortality of short-term exposure to ambient ozone, and examine different exposure metrics. METHODS: The effects of short-term exposure to ozone were analysed using various metrics (daily 1-h maximum, daily 8-h maximum and daily average) on total (nonaccidental) and cause-specific (circulatory and respiratory) mortality from 2013 to 2015 in 34 counties in 10 cities across China. We used distributed lag nonlinear models for estimating county-specific relative risk of mortality and combined the county-specific relative rates by conducting a random-effects meta-analysis. RESULTS: In all-year analyses, a 10 µg m-3 increase in daily average, daily 1-h maximum and daily 8-h maximum ozone at lag02 corresponded to an increase of 0.6% (95% CI: 0.33, 0.88), 0.26% (95% CI: 0.12, 0.39) and 0.37% (95% CI: 0.2, 0.55) in total (nonaccidental) mortality, 0.66% (95% CI: 0.28, 1.04), 0.31% (95% CI: 0.11, 0.51) and 0.39% (95% CI: 0.16, 0.62) in circulatory mortality, and 0.57% (95% CI: -0.09, 1.23), 0.11% (95% CI: -0.22, 0.44) and 0.22% (95% CI: -0.28, 0.72) in respiratory mortality, respectively. These estimates had a different seasonal pattern by cause of death. In general, the seasonal patterns were consistent with the times of year when ozone concentrations are highest. CONCLUSIONS: Our findings suggest that in China, the acute effects of ozone are more closely related to daily average exposure than any other metric.

ABSTRACT

OBJECTIVE: To analyze the mammographic, MRI, and clinicopathological characteristics of primary breast lymphoma (PBL). METHODS: Clinicopathological characteristics were retrospectively reviewed and analyzed in 32 patients with histopathologically proven PBL. All cases were female (mean age, 50 years; median age 46 years; range, 30-68 years). RESULTS: In the 32 cases, there were bilateral tumors in 5 cases and 27 cases of unilateral involvement. The patients presented with a painless mass in 25 cases and with pain in 7 cases. Rapid enlargement was observed in 6 cases. Mammograms of 17 tumors showed a round, circumscribed mass with high density or iso-density in 9, asymmetry in 7 and no abnormality was found in one. 37 lesions were detected on dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) in 29 patients, among which 25 (67.6%) lesions appeared as enhancing masses and 12 (32.4%) lesions as non-mass enhancements. All lesions showed iso- or hypo-intense signal on the T1WI and hyper-intense signal on the T2W1. Analysis of the enhancing masses based on the 2013 ACR BI-RADS lexicon revealed that among the mass descriptors in 25 lesions, the most common features were circumscribed or irregular margin in 24 (96.0%) and homogeneous internal pattern in 19 (76.0%). Of the 12 non-mass enhancing lesions, the most common descriptors included regional or multiple regional distribution in 6 (50.0%), and heterogeneous internal pattern in 8 (66.7%). The tumors showed early enhancement with penetrating vessels on DCE-MRI in 16 lesions, associated with skin thickening in 14, and axillary lymphadenopathy in 19. The time-signal intensity curve (TIC) in 14 patients was mainly of type Ⅲ (7/14, 50.0%). CONCLUSIONS: Some imaging features may alert the diagnosis of primary breast lymphoma, but final diagnosis depends on histopathology. Early diagnosis is helpful for selecting appropriate treatment regimens.

ABSTRACT

We hypothesised that hypocretin (orexin) plays a role in the determination of ventilatory chemosensitivity. 130 patients with narcolepsy-cataplexy (mean ± SD age 20 ± 10 yrs, 69% male) and 117 controls (22 ± 6.9 yrs, 62% male) were recruited and tested for human leukocyte antigen (HLA)-DQB1*0602 status, hyperoxia hypercapnic (change in minute ventilation (δV'(E))/carbon dioxide tension (δP(CO(2))) L·min(-1)·mmHg(-1)) and hypoxic (δV'(E) /change in arterial oxygen saturation measured by probe oximetry (δS(p,O(2))) L·min(-1) per %S(p,O(2))) responsiveness, and by spirometry. Hypocretin deficiency was determined either by measures of cerebrospinal fluid hypocretin-1 (37 patients) or by positive HLA-DQB1*0602 status. All patients and 49% of controls underwent polysomnography and multiple sleep latency testing. Despite similar spirometric values, patients had a higher apnoea/hypopnoea index (AHI) (2.8 ± 5.4 versus 0.8 ± 1.6 h(-1); p = 0.03) and lower minimal oxygen saturation during sleep (87% ± 7 versus 91 ± 4%; p = 0.0002), independent of age, sex and body mass index. Patients had depressed hypoxic responsiveness (0.13 ± 0.09 versus 0.19 ± 0.13 L·min(-1) per %S(p,O(2)); p<0.0001), independent of AHI, but hypercapnic responsiveness did not differ. Examined by HLA status, positive (26 out of 117) controls had lower hypoxic but similar hypercapnic responsiveness than those marker-negative (0.13 ± 0.08 versus 0.20 ± 0.14 L·min(-1) per %S(p,O(2)); p<0.0001). Thus, a lower hypoxic responsiveness in the narcolepsy-cataplexy group is a result of DQB1*0602 status rather than the clinical features of disease.

ABSTRACT

In holographic fabrication of photonic crystals the shape and size of the dielectric columns or particles ("atoms") are determined by the isointensity surfaces of the interference field. Therefore their photonic band gap (PBG) properties are closely related to their fabrication design. As an example, we have investigated the PBGs of a kind of holographically formed two-dimensional (2-D) square lattice with pincushion columns rotated by 45 degrees , and shown that this structure has complete PBGs in a wide range of dielectric contrast comparable to or even larger than those of the same lattice with square columns reported before. The optical design for making this structure is also given. This work may demonstrate the unique feature and advantages of photonic crystals made by holographic method and provide a guideline for their design and experimental fabrication.
