ABSTRACT

Background: Humanitarian aid workers (HAWs) are indirectly exposed to atrocities relating to people of concern (POC). This may result in a risk of secondary traumatization demonstrated by post-traumatic stress symptoms (PTSSs). Previous studies have demonstrated that hemispheric lateralization (HL) moderates the relationship between threat exposure and post-traumatic stress symptoms (PTSSs). Aims: We hypothesized that indirect exposure to atrocities (IETA) would be positively correlated with PTSSs among HAWs with right and not left HL. Method: Fifty-four HAWs from several countries that provided humanitarian support in Greece and Colombia participated in this correlational and cross-sectional observation study. They completed scales relating to IETA, PTSSs were assessed using a brief, valid scale, and HL was measured. Results: IETA was positively and significantly related to PTSSs (r = 0.39, p < 0.005). Considering HL, IETA was unrelated to PTSSs among people with right HL (r = 0.29, p = 0.14), while IETA was related to PTSSs among people with left HL (r = 0.52, p = 0.008). Right HL emerged as a protective factor in the relationship between IETA and PTSS. Conclusions: An assessment of dominant HL can serve as one consideration among others when deploying HAWs in specific locations and roles, vis à vis IETA. Moreover, those found to have a higher risk for PTSSs based on their HL could be monitored more closely to prevent adverse reactions to IETA.

ABSTRACT

The targeted or universal decolonization of patients through octenidine for nasal treatment and antiseptic body wash for 3 to 5 days prior elective surgery has been implemented in several surgical disciplines in order to significantly reduce surgical site infections (SSIs) caused by Staphylococcus aureus carriage. However, as most healthcare facilities also screen patients on admission for pilot infection, it is imperative that a prophylactic nasal decolonization procedure not yield a false negative SARS-CoV-2 status in otherwise positive patients. We assessed the effect of a commercially available octenidine-containing nasal gel on two different screening methods-antigen (Ag) detection based on colloidal gold immunochromatography and RT-PCR-in a prospective-type accuracy pilot study in asymptomatic SARS-CoV-2-positive inpatients. All patients still showed a positive test result after using the octenidine-containing nasal gel for about 3 days; therefore, its application did not influence SARS-CoV-2 screening, which is of high clinical relevance. Of note is that Ag detection was less sensitive, regardless of the presence of octenidine. From an infection prevention perspective, these results favor octenidine-based decolonization strategies, even during seasonal SARS-CoV-2 periods. As only asymptomatic patients are considered for elective interventions, screening programs based on RT-PCR technology should be preferred.

ABSTRACT

Fibroblasts are the most abundant stromal constituents of the tumour microenvironment in primary as well as metastatic colorectal cancer (CRC). Their supportive effect on tumour cells is well established. There is growing evidence that stromal fibroblasts also modulate the immune microenvironment in tumours. Here, we demonstrate a difference in fibroblast-mediated immune modulation between primary CRC and peritoneal metastasis. Cancer-associated fibroblasts (CAFs) were isolated from primary cancer and from peritoneal metastases (MAFs) from a total of 17 patients. The ectoenzyme CD38 was consistently expressed on the surface of all MAFs, while it was absent from CAFs. Furthermore, MAFs secreted higher levels of IGFBP2, CXCL2, CXCL6, CXCL12, PDGF-AA, FGFb, and IL-6. This was associated with a decreased activation of macrophages and a suppression of CD25 expression and proliferation of co-cultivated T-cells. Downregulation of IGFBP2 abolished these immunosuppressive effects of MAFs. Taken together, these results show that MAFs contribute to an immunosuppressive tumour microenvironment in CRC metastases by modulating the phenotype of immune cells through an IGFBP2-dependent mechanism.

ABSTRACT: Over the past few years, changes have been observed in the behavior of typical performance curves, after higher technology has been incorporated into the agricultural tractor engines, either to satisfy the ceiling of the new pollutant gas emission limits or to achieve better efficiency. The aim of this study was to assess the typical performance behavior of an agricultural tractor engine, possessing electronic injection management, turbocharger, aftercooler and EGR (Exhaust Gas Recirculation) treatment system. An analysis was done of the information the manufacturers provided, in terms of nominal power, maximum power and maximum power with Booster, maximum torque and maximum torque with Booster. From the results, the percentage difference between the nominal power and maximum power were found to hover anywhere from 1.49 to 11.97%, for the same manufacturer, 0% in another manufacturer and 9% in a third manufacturer, for all the models. Similar results were noted for the data reported on the maximum torque and maximum torque with Booster, giving 0.62% as the minimum value, 7.44% as the average value and 12.65% as the maximum value for the entire series.


RESUMO: O comportamento das curvas típicas de desempenho passou por alterações nos últimos anos, com a adoção de maior tecnologia pelos motores de tratores agrícolas, seja para atender aos novos limites de emissões de gases poluentes ou na busca por maior eficiência. Este trabalho teve como objetivo avaliar o comportamento típico de desempenho de um motor de trator agrícola, equipado com gerenciamento eletrônico da injeção, turbocompressor, aftercooler e sistema de tratamento de gases EGR (Exhaust Gas Recirculation). Também foram analisadas as informações disponibilizadas pelos fabricantes para os dados de potência nominal, potência máxima e potência máxima com Booster, torque máximo e torque máximo com Booster. Os resultados demonstraram que a diferença percentual entre a potência nominal e a potência máxima pode variar de 1,49% a 11,97% para um mesmo fabricante, 0% em outro e em um terceiro fabricante os valores situam-se em aproximadamente 9% para todos os seus modelos. Resultados similares foram encontrados para os dados informados de torque máximo e torque máximo com Booster, com valor mínimo encontrado de 0,62%, valor médio de 7,44% e o valor máximo de 12,65% para toda a série.

Yes, a link has been established but not a cause-effect relationship. Shorter reported sleep duration in childhood is associated with an increased risk of overweight or obesity years later (strength of recommendation [SOR]: B, meta-analyses of prospective cohort trials with high heterogeneity). In toddlers, accelerometer documentation of short sleep duration is associated with elevation of body mass index (BMI) at 1-year follow-up (SOR: B, prospective cohort). Adequate sleep is recommended to help prevent excessive weight gain in children (SOR: C, expert opinion).

ABSTRACT: In a tractor, the transmission system transmits the power generated in the engine to the driving wheels, hydraulic systems, and power take-off (PTO). There are several types of transmission systems available in the Brazilian market ranging from the simplest to the most advanced. Therefore, the objective of this study was to verify the different types of transmission systems present in 228 models of agricultural tractors produced and marketed in Brazil during 2019. The tractor models were grouped based on the engine power and the transmission systems were classified as sliding, partially synchronized, synchronized, semi powershift, full powershift, and continuously variable transmission (CVT). According to this classification, it was found that most tractors belonged to Class III (51.5-73.5 kW) with 66 models in total. However, even with continuous modernization in this field and with tractors becoming increasingly independent from the operator, 66% of transmission systems are still mechanical. However, CVT systems, which are available only in tractors with an engine power above 117.7 kW, are expected to be equipped on less powerful tractors in the coming decades.


RESUMO: Em um trator, o sistema de transmissão tem a função de transmitir a potência gerada no motor para as rodas motrizes, sistemas hidráulicos e tomada de potência (TDP). São diversos os tipos de transmissões disponíveis no mercado brasileiro, desde as mais simples até as de última geração. Neste sentido, o objetivo deste trabalho foi verificar os tipos de transmissões que equipam 228 modelos de tratores agrícolas, produzidos e comercializados no Brasil, durante o ano de 2019. Os modelos de tratores foram agrupados em classes de potência e as transmissões classificadas em: deslizante; parcialmente sincronizada; sincronizada; Semi Powershift; Full Powershift; e CVT. Como resultado, verificou-se que, na Classe III (51,5 a 73,5 kW) são ofertados o maior número de tratores, 66 modelos no total. Mesmo o trator acompanhando as constantes modernizações do campo e estando cada vez mais independente do operador, 66% das transmissões ainda são mecânicas. Porém, há o surgimento da transmissão CVT que, mesmo de modo incipiente, disponível apenas nos tratores com potência acima de 117,7 kW, tende a equipar tratores menos potentes nas próximas décadas.

Recent research proposes that left hemispheric lateralization (HL) may protect against the effects of life events on mental distress. This study extends these findings by examining the protective role of left HL in the relationship between war threat (missile exposure) and PTSD symptoms. A sample of 186 Israelis, exposed to missile attacks, completed brief scales of self-reported missile exposure, a subjective and a neuropsychological HL measure, and of PTSD symptoms. The sample was split into right HL and left HL individuals on both HL measures. Self-reported missile exposure was positively associated with PTSD symptoms in right HL, but not in left HL individuals on both HL measures. These results replicate, extend our previous results and suggest that left HL may even protect against the effects of severe life threatening events. Results are discussed in relation to neuropsychological and neurophysiological differences between the hemispheres.

ABSTRACT

Past studies show that life events (LE) predict mental distress. This research tested whether hemispheric lateralization (HL) moderated the relationship between LE and mental distress. In studies 1 and 2, different instruments for assessing HL were used (questionnaire and neuropsychological test). In both studies, LE or daily hassles were positively correlated with distress (study 1) and with anxiety and depression (study 2), only in people with right but not left HL, controlling for effects of gender. In study 3, experimentally induced stress led to increased perceived stress, again only in participants with right but not left HL. These results show consistently that left HL may protect against adverse effects of LE, hassles or acute stress on well-being. We propose possible mechanisms and future research directions.

ABSTRACT

OBJECTIVE: Endonasal extended frontal sinusotomy is needed in rare cases of therapy-resistant chronic frontal sinusitis. Several types of burs have been invented to ablate strong bone in the frontal sinus access but nevertheless surgical results seem to differ significantly. The benefits and shortcomings of different burs are subjected to a comparative evaluation. STUDY DESIGN: Five cadaver specimens were duplicated by means of dental molding and casting techniques to provide identical microanatomy for comparative studies on frontal sinusotomy. Different burs (conventional straight bur, right-angled bur according to Blokmanis, and curved frontal sinus bur) were applied to create a type IIb frontal sinusotomy according to Draf in microanatomical replicas. The geometry of the frontonasal neoostium was measured and its dimensions were contrasted to the collateral damage. Bone ablation was calculated by repeated weighting and the area of damaged mucosa was quantified using a 3D analyzing system. RESULTS: A frontal sinus neoostium measuring 9 x 10 mm on average was created by type IIb frontal sinusotomy according to Draf. The complementary use of an angled bur showed some specific improvements that applied best to spacious anatomic specimens. Those specimens subjected to an extensive use of the straight drill revealed significant collateral damage forming a contrast to minor enlargement of the neoostium. The curved frontal sinus bur showed considerable technical insufficiencies. CONCLUSIONS: Judicious use of the straight drill is recommended for those rare cases of therapy-resistant frontal sinusitis treated by an extended type of frontal sinusotomy. New technical refinements of the curved frontal sinus burs should be subjected to repeated evaluation.
