ABSTRACT

Anti-tumor necrosis factor (T N F) - α monoclonal antibody is a first-line treatment for inflammatory bowel disease, and its therapeutic response has attracted more and more attention. Although therapeutic drug monitoring and antidrug antibody monitoring have been widely used in clinic, however, there are no laboratory indicators that can predict the therapeutic response to anti-TNF-α before the drug treatment, so as to provide a guidance of selecting therapeutic drug. Studies have shown that some baseline biomarkers can predict the therapeutic response to anti-TNF-α. This article reviewed the progress of research on biomarkers in predicting efficacy of anti-TNF-α monoclonal antibody in treatment of inflammatory bowel disease.

ABSTRACT

Objective:To investigate the correlation of centromere protein K (CENPK) expression in triple-negative breast cancer (TNBC) with epithelial-mesenchymal transformation (EMT) and its clinical significance. Methods:Immunohisochemical SABC method was used to detect the expressions of CENPK and EMT-related proteins (E-cadherin, Vimentin and N-cadherin) in 69 specimens of TNBC and cancer-adjacent tissues collected from patients in Binzhou People's Hospital Affiliated to Shandong First Medical University from January 2015 to December 2018. The correlations of CENPK expression with clinicopathological characteristics of patients and expressions of EMT-related proteins were analyzed. Results:The positive expression rates of CENPK in TNBC and cancer-adjacent tissues were 72.46% (50/69) and 14.49% (10/69), respectively, and the difference was statistically significant ( χ2 = 47.18, P<0.001). The CENPK expression in TNBC was related to pathological grade, clinical stage and lymphatic metastasis (all P<0.05). The CENPK expression in TNBC was negatively correlated with E-cadherin expression ( r = -0.447, P<0.01), but positively correlated with Vimentin and N-cadherin expressions ( r values were 0.503 and 0.415, both P<0.01). The median overall survival time of CENPK high-expression group was 24 months (95% CI 10-39 months), while that of CENPK low-expression group was 42 months (95% CI 19-50 months). The difference in overall survival between the two groups was statistically significant ( χ2 = 7.36, P = 0.016). Conclusions:CENPK may be involved in the occurrence and development of TNBC through EMT, and the expression of CENPK may be related to the prognosis of patients.

ABSTRACT

Cancer is accounted as the second-highest cause of morbidity and mortality throughout the world. Numerous preclinical and clinical investigations have consistently highlighted the role of natural polyphenolic compounds against various cancers. A plethora of potential bioactive polyphenolic molecules, primarily flavonoids, phenolic acids, lignans and stilbenes, have been explored from the natural sources for their chemopreventive and chemoprotective activities. Moreover, combinations of these polyphenols with current chemotherapeutic agents have also demonstrated their strong role against both progression and resistance of malignancies. Signal transducer and activator of transcription 3 (STAT3) is a ubiquitously-expressed signaling molecule in almost all body cells. Thousands of literatures have revealed that STAT3 plays significant roles in promoting the cellular proliferation, differentiation, cell cycle progression, metastasis, angiogenesis and immunosuppression as well as chemoresistance through the regulation of its downstream target genes such as Bcl-2, Bcl-xL, cyclin D1, c-Myc and survivin. For its key role in cancer development, researchers considered STAT3 as a major target for cancer therapy that mainly focuses on abrogating the expression (activation or phosphorylation) of STAT3 in tumor cells both directly and indirectly. Polyphenolic molecules have explicated their protective actions in malignant cells via targeting STAT3 both in vitro and in vivo. In this article, we reviewed how polyphenolic compounds as well as their combinations with other chemotherapeutic drugs inhibit cancer cells by targeting STAT3 signaling pathway.

ABSTRACT

Objective:To understand the infection status of key human parasites in Yantai City, Shandong Province, and to provide scientific basis for establishing strategy for prevention and control of the disease. Methods:According to the "National Investigation Plan of Human Parasitic Infection Status" and implementation rules, stratified cluster random sampling method was adopted to select 39 survey sites in 10 counties (cities, districts) of Yantai City from 2015 to 2019. The respondents were residents in each survey site, with no less than 200 people in each survey site. The modified garten thick smear method (one fecal two test) and the direct smear method were respectively used to detect the eggs of intestinal worms and the trophozoites or cysts of intestinal protozoa. Besides, the transparent adhesive paper anal swab was used to detect pinworms in children aged 3 - 9 years. SPSS 18.0 software was used for statistical analysis. Comparison between infection rates was analyzed by χ 2 test or Fisher's exact probability test with 0.05 of test level. Results:A total of 8 507 people were investigated from 2015 to 2019. The total infection rate of intestinal parasites was 1.75% (149/8 507), and no protozoa was detected. A total of 4 species of intestinal worms were detected, including 1.41% (120/8 507) of whipworm, 0.16% (14/8 507) of ascaris, 0.14% (12/8 507) of pinworm and 0.07% (6/8 507) of hookworm. Among 149 cases of worm infection, 3 cases were ascaris and whipworm mixed infection, accounting for 2.01%. The infection rate of pinworm was 1.90% (11/578) in 578 children aged 3 - 9 years detected by transparent adhesive paper anal swab. From 2015 to 2019, the incidence of intestinal parasites infection first increased and then decreased, the difference was statistically significant (χ 2 = 469.38, P < 0.05). The infection rates of male and female were 1.72% (70/4 071) and 1.78% (79/4 436), respectively, with no significant difference between them (χ 2 = 0.05, P > 0.05). There was a significant difference of intestinal parasites infection rate in different age groups (χ 2 = 23.34, P < 0.05). The infection rate of intestinal parasites in ≥80 years old group was the highest with 2.84% (8/282). There was a significant difference of intestinal parasites infection rate among different professionals (χ 2 = 41.71, P < 0.05). Intestinal parasites infection rate of farmers was the highest with 2.58% (113/4 388). There was a significant difference of intestinal parasites infection rate among people with different cultural degree(χ 2 = 51.91, P < 0.05). Infection rate of illiterate people was the highest with 4.98% (16/321). Parasitic infection was detected in 10 counties (cities, districts), except Laishan District. The highest infection rate was Haiyang City (10.18%, 102/1 002), and the results of other counties (cities, districts) were lower than 1.20%. There was significant difference in infection rate in different regions (χ 2 = 433.87, P < 0.05). The infection rate in urban area was 0.51% (22/4 281), and the infection rate in rural area was 3.01% (127/4 226). The difference between urban and rural areas was statistically significant (χ 2 = 76.70, P < 0.05). Conclusion:The infection rate of intestinal parasites has been reduced to a lower level in rural area of Yantai City, and farmers are the key prevention and control population of intestinal parasites infection, so attention should be paid to strengthen the publicity of parasitic disease prevention and control.

ABSTRACT

To analyze the correlation between lipid profile and disease activity in patients with inflammatory bowel disease (IBD).A total of 307 Crohn′s disease (CD) patients, 232 ulcerative colitis (UC) patients and 165 healthy subjects from the same geographic region were included. Total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C) and lipoprotein(a)[Lp(a)] were retrieved from their medical records. Crohn disease activity index (CDAI) and Mayo scores were calculated as measurement of disease severity for CD and UC separately. Patients with CD and UC had lower TC, TG, HDL-C and LDL-C levels than those in control group ( P<0.05). Additionally, CDAI was negatively associated with TC, HDL-C and LDL-C levels ( r=-0.218, -0.210, -0.176, P<0.05), while TG level was not associated with CDAI. Mayo scores was not significantly associated with TC, HDL-C, LDL-C and TG. Patients with CD had higher Lp(a) levels than those in UC and control group ( P<0.05). Furthermore, patients with active CD had higher Lp (a) levels than those with inactive disease ( P<0.05).The Lp(a) levels in CD patients were positively associated with CDAI ( r=0.151, P<0.05), while Lp(a) level in UC group was nor assocriated with Mayo score. Patients with IBD have dyslipidemia and lipid profile is associated with disease activity in CD patients.

ABSTRACT

Objective:To understand the iodine nutrition status of children aged 8 to 10 years in Yantai City, Shandong Province, and to provide evidence for making prevention and control strategies in the future. Methods:From 2016 to 2018, a multi-stage stratified simple random sampling method was used in Yantai City, 5 to 7 counties (cities, districts) were selected each year in the east, west, south, north, and middle directions. Each county (city, district) was divided into 5 locations of east, west, south, north, and middle, and one township (town, street) was selected in each location. One village primary school was selected from each township (town, street), if there was no village primary school, the township central primary school was selected. For each primary school, 40 children aged 8 to 10 years (half male half female) were selected. Random urine samples and edible salt samples were collected, and urinary iodine and salt iodine contents were measured, while thyroid volume was examined by B-ultrasound.Results:From 2016 to 2018, the medians iodine contents of edible salt (1 000, 1 400, 1 200 samples) in the households of children aged 8 to 10 years were 21.02, 21.50 and 21.15 mg/kg, respectively; the qualified iodized salt consumption rates were all < 90%; the qualified rate of iodized salt had decreased year by year, and the difference was statistically significant (χ 2=85.92, P < 0.01). From 2016 to 2018, 1 000, 1 400, and 1 200 urine samples of children aged 8 to 10 years were tested, and the medians urinary iodine were 190.05, 148.20, and 145.00 μg/L, and there was no correlation between children's salt iodine contents and their own urinary iodine levels ( r=- 0.036, P > 0.05). From 2016 to 2018, the overall goiter rate for children aged 8 to 10 years was 3.61% (130/3 600), and there was no correlation between urinary iodine levels and thyroid volume in children ( r=0.108, P > 0.05). From 2016 to 2018, the number of children who consumed non-iodine salt accounted for 19.50% (702/3 600), the median urinary iodine was 155.00 μg/L, and the goiter rate was 3.70% (26/702). Conclusion:Although the qualified iodized salt consumption rate in children in Yantai City is less than 90%, the urinary iodine level and goiter rate of children have reached the standards for elimination of iodine deficiency disorders, indicating that children's dietary nutrition level and salt iodine content can meet the children iodine requirement.

ABSTRACT

Objective:Mesenteric fat hypertrophy is present in about a quarter of Crohn′s disease (CD) patients and it can be easily detected by bowel ultrasound (US). The purpose of this research was to assess the correlation between mesenteric fat hypertrophy and behavior and activity of CD.Methods:A total of 89 CD patients who admitted to the First Affiliated Hospital of Nanjing Medical University from August 2018 to November 2019 were recruited in this study. The total CD patients were divided into two groups depending on with or without mesenteric fat hypertrophy by US tests. Crohn′s disease activity index (CDAI), simplified endoscopic score for Crohn′s disease (SES-CD), serum inflammatory indicators and fecal calprotectin were assessed. Results:Mesenteric fat hypertrophy was significantly associated with stricturing behavior (B2, P<0.01). CDAI ( P=0.002) , blood platelet ( P=0.001) , C-reactive protein ( P=0.024) , fecal calprotectin ( P=0.004) and bowel wall thickness ( P<0.01) in patients with mesenteric fat hypertrophy were significantly higher than those without, but not the erythrocyte sedimentation rate ( P=0.110) and SES-CD ( P=0.115) . Serum albumin ( P=0.001) in patients with mesenteric fat hypertrophy was lower than that in patients without mesenteric fat hypertrophy. Conclusion:Mesenteric fat hypertrophy is correlated with intestinal stenosis and disease activity in patients with Crohn′s disease.

ABSTRACT

OBJECTIVE: To investigate the relationship of polycyclic aromatic hydrocarbons( PAH) metabolites,DNA oxidative damage and ring finger protein 2( RING2) expression in coke oven workers. METHODS: A judgment sampling method was used to select 497 coke oven workers in a steel plant as exposure group and 175 water treatment workers in the same plant as control group. The levels of urinary 1-hydroxypyrene, 2-hydroxynathalene, 2-hydroxyfluorene,9-hydroxyphenanthrene and 8-hydroxy deoxyguanosine(8-OHd G) were detected by high performance liquid chromatography. The RING2 expression in whole blood was measured by reverse transcription-polymerase chain reaction. RESULTS: The relative expression of urinary 1-hydroxypyrene,2-hydroxynathalene,2-hydroxyfluorene,9-hydroxyphenanthrene and RING2 in exposure group were higher than that in control group( P < 0. 01). The logistic regression analysis indicated that the higher the level of 1-hydroxypyrene,the higher the risk of high-RING2 expression( P < 0. 05) after adjusting for factors such as sex, age, smoking status, alcohol drinking,2-hydroxynathalene,2-hydroxyfluorene and 9-hydroxyphenanthrene.In 1-hydroxypyrene middle and high level groups, the 8-OHd G concentration of high-RING2 expression workers was significantly higher than those of low-RING2 expression workers( P < 0. 05). CONCLUSION: With the increase of urinary1-hydroxypyrene,the risk of high-RING2 expression was elevated, the degree of DNA oxidative damage was gradually increased.

ABSTRACT

Objective To investigate the correlation between the subjective well-being,optimism and coping mode of patients with inflammatory bowel disease(IBD). Methods A total of 159 IBD patients were surveyed and analyzed by the general information questionnaire, General Well-Being Schedule, Revised The Life Orientation Test, and Simplified Coping Style Questionnaire. Results The subjective well-being scores of IBD patients were (75.04 ± 14.32). And the Pearson analysis showed that subjective well-being scores were positively correlated with optimism scores and positive coping scores(r=0.408, P<0.01; r=0.429, P<0.01). Negative coping was negatively correlated with subjective well-being scores(r=-0.174, P<0.05). Optimism had a direct positive predictive effect on subjective well-being with a path coefficient of 0.22, and coping mode was an intermediary variable of optimism and subjective well-being. Conclusion The subjective well-being of patients with IBD needs to be further improved, which is both related to optimism and coping mode. Coping mode takes effects as a mediator in optimism and subjective well-being.

ABSTRACT

Objective To explore the relationship between health?related quality of life and resili?ence of patients with inflammatory bowel disease( IBD) ,and provide a new entry point to improve the quality of life. Methods The general information questionnaire, the Chinese version of inflammatory bowel disease questionnaire ( IBDQ) and the Chinese version of Connor?Davidson mental resilience scale ( CD?RISC) were used to investigate 102 patients with IBD. Results The average scores of IBDQ and CD?RISC of IBD pa?tients were (157.6±31.7) and (62.5±15.0). Except for the correlation between the score of optimistic di?mension and the social function dimension, the total score and the scores of other dimensions of IBDQ were significantly correlated with the total score and the scores of dimensions of CD?RISC( r=0.200?0.490, P<0.05). Multiple regression analysis showed that tenacity, residence, moderate and severe disease severity, and disease duration with more than 10 years could significantly predict the level of health?related quality of life (β=0.319,0.268,-0.218,-0.373,-0.260, P<0.05) . Conclusion The health?related quality of life of IBD patients has a strong relationship with resilience. Health care workers can help improve health?related quality of life of the patients through fostering resilience.

ABSTRACT

Objective@#To explore the influence of smoking and polycyclic aromatic hydrocarbons (PAHs) on urinary 8-hydroxydeoxyguanosine (8-OHdG) in coke oven workers and investigate their dose-dependent relationships.@*Methods@#A total of 436 workers exposed to coke oven emissions (COEs) and 132 controls were recruited in this study. Questionnaires were completed in a personal interview. Then their urine samples were also collected and the concentrations of urinary four OH-PAHs and 8-OHdG were determined by high performance liquid chromatography (HPLC) which was used to evaluate the levels of occupational PAHs internal exposure among workers and the DNA damage.@*Results@#The differences of concentrations of urinary 2-NAP (2-hydroxynathalene) , 2-FLU (2-hydroxyfluorene) , 9-PHE (9-hydroxyphenanthrene) , 1-OHP (1-hydroxypyrene) between exposure group and control group were statistically significant (P<0.05) . In exposure group and control group, the level of 8-OHdG in heavy smoking workers were significantly higher than that in other groups (P<0.05) . Multivariate logistic regression analysis revealed high levels of urinary 8-OHdG were associated with a significantly increased risk of having higher urinary1-hydroxypyrene levels[OR=1.43 (95%CI: 1.06-1.94) , P<0.01] and heavy smoking [OR=1.44 (95%CI: 1.08-1.91) , P<0.01], respectively. Trend test showed that linear dose response relationship between smoking, 1-OHP in urine and higher concentrations of 8-OHdG (P<0.05) . Smoking could significant modify the effects of urinary 1-hydroxypyrene, while co-exposure to both heavy smoking and high urinary 1-hydroxypyrene levels[OR=5.64 (95%CI: 2.15-14.80) , P<0.05].@*Conclusion@#Urinary 1-hydroxypyrene is a useful biomarker for evaluating total PAHs exposure, coke oven workers with heavy smoking present more serious DNA oxditive damage.
