ABSTRACT

A sesquiterpene natural substance called artemisinin was discovered in Artemisia annua. One of its derivatives, artesunate (ART), has the properties of economy, immediate effect, low toxicity, and good tolerance. Since it has a quick and powerful killing effect on plasmodium in the erythrocyte phase and can quickly handle clinical seizure and symptoms, it is currently mostly utilized to treat cerebral malaria and other severe instances of malaria. In addition, it has antitumor, antivirus, anti-hepatic fibrosis, anti-inflammatory, antibacterial, hepatocyte protection, immunological modulation, and other pharmacological properties and can inhibit cell proliferation, induce cell apoptosis, and reduce the incidence of sepsis. In many countries, artemisinin-based combination therapies (ACTs), such as artemether-benflumetol, artesunate-amodiaquine, and artemether-lumefantrine, are the first-line treatments for malaria. Recent research on artesunate by Chinese and international scholars has revealed that compared with monotherapy, artesunate combination therapy offers more benefits in terms of improving pharmacological effects, shortening the duration of medicine, and minimizing adverse effects. Through systematic retrieval of Web of Science Core Collection and integration through CiteSpace (6.2.1) software, this article reviewed the mechanism of artesunate combined with other medications with regard to antimalarial, antitumor, antibacterial, and antiviral features in the previous five years, so as to provide some theoretical basis for rational development and utilization of ART and new drug research and development.

ABSTRACT

One under-studied microelement, manganese (Mn), due to its potential to considerably interact, and limit labile, and moderately-labile soil phosphorus (P) pools, was studied in Nanchang (NC), and Qiyang (QY) under paddy conditions. The Hedley's P sequential fractionation procedure was utilized to extract, and quantify various P fractions at both surface (0-20 cm) and subsurface (20-40 cm) layers. Unfertilized control (CK), nitrogen, phosphorus, and potassium (NPK), and NPK amended with animal manure (NPKM) were used as treatments. From both sites, the manure amended fertilizer (NPKM) compared to chemical NPK formed higher proportions of macro-aggregates (>2 and 2-0.25 mm) in both layers. Total P (TP) values of 842.1 (>2 mm), and 744.4 mg kg-1 (2-0.25 mm) from NC, and QY, respectively were accumulated by NPKM compared to NPK, and CK. Total P values of 806.4, and 350.4 mg kg-1 in the >2 mm aggregate size, respectively for NC, and QY were observed in the subsurface layer. Inorganic moderately labile P (NaOH-Pi) was the dominant fraction under all fertilizer treatments. Concentrations of 232.3 (<0.053 mm), and 202.1 mg kg-1 (0.25-0.053 mm) of NaOH-Pi were accumulated by NPKM, respectively for NC, and QY in the surface layer. In the subsurface layer, concentrations of NaOH-Pi (217.5 mg kg-1; <0.053 mm) from NC, and residual-P (57.3 mg kg-1; >2 mm) from QY were accumulated by NPKM. Similarly, NPKM in contrast to NPK contributed higher Mehlich-3 manganese (M3-Mn) oxide in all aggregate sizes from both sites. Generally, macro-aggregates contributed higher TP, fractions of P, and M3-Mn oxide than micro-aggregates. There was a positive relationship between P pools and M3-Mn oxide at both sites. Additions of animal manure were associated with increased P fractions, and Mn oxides in the paddy soil aggregates, which raises environmental concern.

ABSTRACT

PURPOSE: Dried blood spot (DBS) is one of promising home sampling methods for therapeutic drug monitoring (TDM). However, the associated reliability and feasibility (including yield, adherence, and preference), which are criteria for the promotion of home-based DBS, remain unknown. This systematic review and meta-analysis aimed to evaluate the reliability and feasibility of TDM using DBS sampling. METHODS: In this study, a combination of MeSH and free terms for (dried blood spot*[title/abstract])AND ("Drug Monitoring"[Mesh])AND(home OR venous)was surveyed using EMBASE, PubMed, Cochrane Library, and Web of Science upon gathering published. we registered this study protocol with the International Prospective Registry of Systematic Reviews (CRD42021247559). RESULTS: Approximately half (35/75) of the evaluations reported good agreement between DBS and plasma, and the results for drugs with poor agreement may be improved using a haematocrit-based physiological equation. The yield and adherence to home-based DBS exceeded 87%, and questionnaire-based preference for DBS was 77%. CONCLUSIONS: DBS may be a reliable and feasible home sampling method; however, it requires intricate design and evaluation before implementation.

ABSTRACT

Diabetic nephropathy (DN) is a common microvascular complication of type 1 diabetes mellitus (T1DM) and type 2 diabetes mellitus (T2DM),which is also the main cause of chronic kidney disease (CKD) and end-stage renal disease (ESRD). However, the treatment methods are limited at present. More and more evidences have indicated that inflammatory response is involved in the pathogenesis and progression of DN. Several anti-inflammatory strategies that target specific inflammatory mediators (transcription factors, pro-inflammatory cytokines, chemokines, adhesion molecules) and intracellular signaling pathways have shown benefits in the DN rodent model. The mechanisms related to inflammation in the development and progression of DN were summarized and new strategies to prevent or treat DN based on inflammation were briefly discussed in this review.

ABSTRACT

ObjectiveTo investigate the structural features of vaginal microbiota in the early threatened abortion patients with the syndrome of kidney deficiency. MethodThirty-one patients with early threatened abortion of kidney deficiency syndrome (DK-TA group) and 116 women with normal early pregnancy (NP group) attending the First Affiliated Hospital of Guangzhou University of Chinese Medicine from May 2018 to December 2020 were selected. The vaginal secretions were collected for 16S rRNA sequencing, which can reveal the vaginal microbiota composition and differential bacteria between the two groups. ResultThe DK-TA group had higher abundance and diversity of vaginal microbiota than the NP group. The Binary jaccard and unweighted_unifrac distance matrix analysis showed that the similarity, dispersion, abundance, and phylogenetic relationship of vaginal microbiota were significantly different between the two groups. At the phylum level, the DK-TA group had lower relative abundance of Bacteroidetes and Fusobacteria and higher relative abundance of Proteobacteria and Gemmatimonadetes than the NP group. At the genus level, the DK-TA group had lower relative abundance of Sneathia and Bifidobacterium and higher relative abundance of Escherichia-Shigella and Shuttleworthia than the NP group. Linear discriminant analysis Effect Size(LEfSe)revealed that Bacteroidetes, Fusobacteria, and Bifidobacteria were dominant in the NP group and Proteobacteria and Firmicutes in the DK-TA group. The function prediction found that DK-TA was closely associated with 38 functional pathways, including cyclic adenosine monophosphate signaling pathway and regulation of tryptophan channels by inflammatory mediators. In addition, the vaginal differential bacteria between the two groups had significant positive or negative association with the differential metabolic pathways. ConclusionProteobacteria, Gemmatimonadetes, and Bacteroidetes in the vaginal microbiota may be biomarkers for threatened abortion of kidney deficiency syndrome.

The dynamic changes of cognitive function has been paid more and more attention by foreign scholars.Dynamic assessment based on ecological momentary assessment(EMA)can capture subtle changes in cognitive function and provide more comprehensive information for early identification and timely intervention of people with cognitive impairment, which is an effective supplement to traditional cognitive assessment.This paper reviewed the concept of ecological momentary assessment, its advantages in cognitive assessment, its feasibility and effectiveness, and its application status in the evaluation of cognitive function in the elderly, so as to provide a reference for making ecological assessment of the cognitive function for older adults that is in line with China's national conditions.

ABSTRACT

Atopic dermatitis (AD) is an inflammatory skin disease characterized by itch. Transient receptor potential vanilloid (TRPV) receptor subtype channel can be activated by different ranges of harmless temperature, and plays an important role in the warm itching in AD. Inhibitors of TRPV channel can alleviate the symptoms of AD, and may be used as a new target for treatment. This paper introduces the role of thermosensitive TRPV channel in the alienation of itching sensitized by warm in AD.

ABSTRACT

Objective:To explore the mediating role of self-perceptions of aging between frailty and cognitive function in community-dwelling older adults.Methods:From February to July 2021, a total of 528 elderly people in Xinxiang community were investigated with the frailty phenotype, the brief self-perceptions ageing questionnaire and the Mini-mental state examination(MMSE) scale.According to the MMSE total score and education level, the subjects were divided into cognitive impairment group (illiteracy≤17, primary school≤20, junior high school and above≤24, n=74) and cognitive normal group( n=454). SPSS 25.0 software was used for common method deviation test, descriptive statistics and correlation analysis, while AMOS 24.0 software was used to build structural equation model and Bootstrap method was used for intermediary effect test. Results:(1)The prevalence of cognitive impairment among the elderly in the community was 14.1%. The differences between the cognitively normal group and cognitively impaired group were statistically significant in terms of age, education, number of chronic diseases suffered and depression ( χ2=59.21, 6.53, 9.84, 25.47, all P<0.05). The differences were statistically significant in terms of frailty( χ2=75.65, P<0.001) and self-perceptions of aging ( t=77.67, P<0.001). (2)Self-perceptions of aging in the cognitively impaired group (47.39±8.66) was higher than that in the cognitively normal group (38.22±8.24) ( t=77.67, P<0.001) .Frailty score in cognitively impaired group (2.00 (1.00, 3.00)) was higher than that in the cognitively normal group (0.00 (0.00, 1.00))( Z=-8.63, P<0.001) . (3)Frailty was negatively correlated with cognitive function ( r=-0.492, P<0.01), and positively correlated with self-perceptions of aging ( r=0.540, P<0.01). Self-perceptions of aging was negatively correlated with cognitive function ( r=-0.541, P<0.01) . After controlling the influencing factors such as age, education level, chronic diseases and depression, the correlation was still significant (all P<0.01) . (4) Self-perceptions of aging played a partially mediating role in the relationship between frailty and cognitive function, the mediating effect accounted for 58.5% of the total effect. Conclusion:Frailty and self-perceptions of aging have a significant impact on the cognitive function of the elderly in the community, and self-perceptions of aging plays a partial intermediary role between the frailty and cognitive function of the elderly in the community.

ABSTRACT

To summarize the experience of 5 children with relapsed and refractory acute B lymphocyte leukemia treated with Blinatumomab, during the whole nursing processes, careful nursing was carried out before, during and after infusion. The key points of nursing include: reasonable cooperation with nursing staff and specialized training, design a condition observation form, timely grasp changes in the condition, early identify adverse reactions such as cytokine storms, neurotoxicity, and allergies, and ensure the safety of the children. Through personnel, articles and personalized liquid preparation table, ensure accurate dose, smooth infusion and save hospital expenses ¥3, 846~201, 910 for children. Payattention to the psychological burden of children and their families and attach importance to psychological care. Follow the psychological burden of children and caregivers, and pay attention to psychological nursing. As the results, 3 chidlren successfully completed the first cycle of Blinatumomab. But 2 chidlren interrupted due to the tumor burden. Follow up for 5-12 months showed that 2 cases survived, 1 case died of progressive disease after discharge, and 2 cases died of transplant complications.

ABSTRACT

Inflammation, the basic pathological process of many diseases, can occur in various tissues and organs of the body and cause many diseases including cancer. So far, there are thousands of anti-inflammatory drugs on the market, but most of these drugs have adverse reactions of gastrointestinal injury, and can even cause greater damage to the body. In recent years, the research on the repurpose of Chinese medicine is in the ascendant, and the innovative research on the specific antimalarial drug artemisinin has attracted extensive attention from scholars in China and abroad. Artesunate is a water-soluble derivative of artemisinin, which has the characteristics of quick effect and low toxicity. In addition to its significant therapeutic effect on malaria, artesunate also has a potential anti-inflammatory effect. In this review, the anti-inflammatory effect and mechanism of artesunate were elaborated in detail by consulting the relevant literature. It was found that artesunate had good anti-inflammatory effects in the respiratory system, liver injury, osteoarthritis, dermatitis, kidney inflammation, colitis, neuroinflammation, and even in novel coronavirus disease 2019 (COVID-19). It was concluded that artesunate mainly participated in apoptotic signal transduction, mediated immune regulation, and improved oxidative stress to play an anti-inflammatory role by acting on nuclear factor-κB (NF-κB), nuclear factor E2-related factor 2 (Nrf2), phosphatidylinositol-3 kinase (PI3K)/protein kinase B (Akt)/mammalian target of rapamycin (mTOR), Toll-like receptor 4 (TLR4)/myeloid differentiation factor 88 (MyD88)/tumor necrosis factor receptor-associated factor 6 (TRAF6), high mobility group box 1 (HMGB1)/receptor for advanced glycation endproduct (RAGE), and other pathways. Through the review of the anti-inflammatory effect and mechanism of artesunate, it is expected to provide a reference for the application of artesunate in inflammation resistance and further development and utilization of artesunate in the future.

ABSTRACT

Objective To investigate the prevalence of hepatitis D virus (HDV) infection among patients with chronic hepatitis B virus (HBV) infection in some regions of China. Methods Serum samples were collected from 3 131 patients with chronic HBV infection in 10 provinces, cities, and autonomous regions of China from March 2021 to June 2022, and anti-HDV IgG ELISA was used for the detection of all serum samples. Nested reverse transcription-polymerase chain reaction (nRT-PCR) was used to detect HDV RNA in anti-HDV IgG-positive samples, and the nRT-PCR amplification products of HDV RNA-positive samples were sequenced and analyzed to determine HDV genotype. The clinical features of anti-HDV IgG-positive patients were analyzed. The Mann-Whitney U rank sum test was used for comparison of continuous data between two groups, and the chi-square test or the Fisher's exact test was used for comparison of categorical data between two groups. Results The positive rate of anti-HDV IgG in the 3 131 patients with chronic HBV infection was 0.70% (22/3 131), and that in the patients with chronic HBV infection in Inner Mongolia Autonomous Region, Xinjiang Uygur Autonomous Region, Beijing, and Hunan Province was 1.81% (16/886), 0.88% (2/226), 0.28% (2/708), and 1.00% (2/200), respectively; the patients with chronic HBV infection in Inner Mongolia Autonomous Region had a significantly higher positive rate of anti-HDV IgG than those in Beijing ( P =0.004), and there was no significant difference between the other regions ( P > 0.05). Clinical features of the patients with chronic HBV infection in Inner Mongolia Autonomous Region showed that compared with the anti-HDV IgG-negative group, the anti-HDV IgG-positive group had a significantly higher proportion of patients with Mongol nationality ( P =0.001), abnormal alanine aminotransferase ( P =0.007), or antiviral treatment ( P =0.029), as well as a significantly lower median HBV DNA level ( P =0.030). A total of 19 HDV RNA-positive samples were identified, all of which had HDV genotype 1. Conclusion The prevalence rate of HDV varies greatly across different regions of China, with a higher prevalence rate of HDV in patients with chronic HBV infection from Inner Mongolia Autonomous Region. HDV genotype 1 is the predominant genotype in some provinces and cities of northern China.

ABSTRACT

Introduction: Therapeutic drug monitoring (TDM) has been shown to be clinically beneficial for critically ill patients. However, this is a burden for neonates or children with small circulating blood volumes. Here, we aimed to develop and validate a microsampling TDM platform (including dried blood spots (DBS) and capillary microsamples (CMS)) for the simultaneous quantification of vancomycin, meropenem, and linezolid. Methods: Paired DBS and CMS samples were obtained from an intensive care unit (ICU) to evaluate its clinical application. Estimated plasma concentrations (EPC) were calculated from DBS concentrations. Agreement between methods was evaluated using Deming regression and Bland-Altman difference plots. Results: The microsampling methods validation showed excellent reliability and compatibility with the analysis of the sample matrix and hematocrit range of the studied population. The DBS and CMS accuracy and precision results were within accepted ranges and samples were stable at room temperature for at least 2 days and 8 h, respectively. Hematocrit had no impact on CMS, but sightly impacted DBS measurements. The CMS and DBS antibiotic concentrations correlated well (r > 0.98). The drug concentration ratio in DBS samples to that in CMS was 1.39 for vancomycin, 1.34 for meropenem, and 0.94 for linezolid. The EPC calculated from the DBS using individual hematocrit ranges presented comparable absolute values for vancomycin (slope: 1.06) and meropenem (slope: 1.04), with a mean of 98% and 99% of the measured CMS concentrations, respectively. Discussion: This study provides a microsampling TDM platform validated for clinical use for a rapid quantification of three antibiotics and is suitable for real-time TDM-guided personalization of antimicrobial treatment in critically ill children.

ABSTRACT

Metabolic homeostasis is a basic function necessary for the survival of the organism. α1-acid glycoprotein (AGP) is an acute phase protein with a glycosylation degree of up to 45%. It has high affinity and low capacity. Although the biological role of AGP is not fully understood, it has been proven that it can regulate immunity and metabolism, and play an important role in transporting drugs and maintaining capillary barrier function. In this review, the structural characteristics, biochemical characteristics and the regulation of AGP expression were reviewed, with emphasis on the regulatory role of AGP in metabolism, suggesting that AGP may be a potential key factor in metabolic pathways, which provides a new research direction for metabolic diseases.

ABSTRACT

OBJECTIVE To study the absorbed components of Xiebai powder in blood. METHODS UPLC-Q-TOF-MS/MS method was adopted. SD rats were randomly divided into blank group and administration group ,with 10 rats in each group. Blank group was given water intragastrically ,and administration groups were given 2 g/mL(by the amount of crude drug )Xiebai powder solution intragastrically. Administration volume was 11.3 mL/kg,twice a day for 3 days. One point five hours after last administration,blood was taken from the abdominal aorta of each rat ,the serum was processed to obtain the supernatant for analysis;the relevant data in positive and negative ion mode were collected ,and the absorbed components of Xiebai powder in blood were analyzed and identified by using self-built secondary mass spectrometry database and consulting the relevant literature. RESULTS Totally 17 components from Xiebai powder were identified ,among which 6 components came from sovereign Moru salba,7 from minister Cortex Lycii ,12 from assistant Glycyrrhiza uralensis ,i.e. kukoamine A ,chlorogenic acid ,tachiogroside B,astringin,neoglycyrrhizin,glycyrrhizin,azelaic acid ,isoglycyrrhizin,glycyroside,anthocyanin,sebacic acid ,parthenolide, anthocyanin,18β-glycyrrhetinic acid ,6-gingerol,palmitoamide,erucamide. These compounds were mainly flavonoids ,alkaloids and organic acids. CONCLUSIONS In this study ,17 absorbed components of Xiebai powder in blood are preliminarily determined,which are consistent with the effect of Xiebai powder. They may be the pharmacodynamic substances of Xiebai powder.

ABSTRACT

Malaria is a serio us and life-threatening infectious disease that has a profound impact on human life. Artemisinin is still the first-line drug for clinical antimalarial treatment recommended by the World Health Organization. The antimalarial activity of artemisinin is mainly reflected in the peroxide bridge structure. Artemisinin-based combination therapy (ACT)is the first-line treatment for malaria in many countries. ACT mainly include artemether-lumefantrine ,artesunate-amodiaquine and dihydroartemisinin- piperaquine,etc. Compared with artemisinin monotherapy ,ACT has the advantages of shortening the length of hospital stay , speeding up parasite clearance ,and saving economic costs ,etc. However ,there are still problems such as drug resistance. This article reviews the application status ,advantages and disadvantages of ACT at home and abroad in recent years ,in order to provide ideas for the subsequent screening of long-acting adjuvant antimalarial drugs in ACT and to solve the problem of drug resistance.

Objective:To summarize the clinical experience in treating acute superior mesenteric artery embolization with intestinal necrosis.Methods:Clinical data, surgical methods, complications and 30-day follow-up of patients with acute superior mesenteric artery embolization and intestinal necrosis admitted at Xijing Hospital of Air Force Military Medical University from Jan 2008 to Dec 2020 were retrospectively analyzed.Results:A total of 46 patients were included. The 30-day overall mortality rate was 19.6%, and that of those treated by enterostomy after enterectomy was 12.1%, and that was 38.5% in those undergoing primary entero-anastomosis after enterectomy. Univariate analysis showed that rebound pain was positive ( P=0.025), MAP was higher than 105mmHg ( P=0.04), heart rate was less than 120 beats/min ( P=0.04), and Fullen intestinal ischemia grade ( P=0.03) was significantly related to survival rates. Conclusions:Rebound pain, MAP, heart rate, Fullen grade, and surgical methods significantly affect the prognosis of patients. More patients survive with intestinal resection and enterostomy than those with intestinal resection and immediate anastomosis.

ABSTRACT

Objective To test the cardiac toxicity of new compound HMS-01 and evaluate the safety profile for clinical trials. Methods Manualpatch clamp method was used to measure human Ether-a-go-go-Related Gene (hERG) potassium channel currents with different concentrations of HMS-01. Cisapride was selected as the positive control drug. HMS-01 was diluted to the concentration of 0.3, 1, 3, 10 and 30 µmol/L and applied to the cells. The changes in electrical currents were recorded and the inhibition rate was calculated. Results At the highest concentration of 30µmol/L, the inhibitory rate of HMS-01 on hERG channel was less than 30%. There was no obvious inhibitory effect compared with cisapride. Conclusion Compared with the cisapride, HMS-01 has no obvious inhibitory effect on hERG channel and has no cardiotoxicity.

Objective:To detect the incidence and analyze the clinical significance of regions of homozygosity (ROH) through the single nucleotide polymorphism array (SNP array).Methods:The SNP array detection results of 5 116 pregnant women in the Third Affiliated Hospital of Guangzhou Medical University from January 2016 to December 2020 were retrospectively analyzed. The pregnant women with ROH (5 Mb as the threshold) were followed up to analyze the relationship between ROH and abnormal fetal phenotype. Whole exon sequencing was performed in 4 cases of consanguineous marriage to detect potential recessive causative genes in the ROH region.Results:(1) A total of 39 cases of ROH were detected, with a positive rate of 0.76% (39/5 116). Among them, 25 cases (64%, 25/39) were detected only on single chromosome, and chromosome 11 had the highest detection rate, suggesting the risk of uniparental disomy; fourteen cases (36%,14/39) were detected on multiple chromosomes, most commonly on chromosomes 11, 1, 3, 4 and 8. (2) The number of cases and detection rate of ROH detected by different prenatal diagnosis indicators were as follows: 12 cases (1.78%, 12/676) in pregnant women with abnormal non-invasive prenatal testing result, 12 cases (0.37%, 12/3 284) in pregnant women with ultrasound abnormality, 4 cases (4/4) in pregnant women with consanguineous marriage, 3 cases (0.92%, 3/326) in pregnant women with previous adverse pregnancy, 2 cases (1.15%, 2/174) in pregnant women with high risk of serology in screening, 2 cases (4.00%, 2/50) in pregnant women with abnormal fetal chromosomal karyotype, 2 cases (0.79%, 2/253) in pregnant women with advanced maternal age, 1 case (0.56%, 1/178) in pregnant women with related parental genetic factors and 1 case (0.58%, 1/171) in pregnant women with the other factors. (3) The follow-up results of 39 cases of prenatal ROH showed that there were 16 cases of term birth, 15 cases of termination of pregnancy, 2 cases of preterm births, 1 case of fetal death and 5 cases lost to follow-up.Conclusions:Chromosomal ROH phenomenon is not rare. By analyzing the detection rate of ROH in prenatal diagnosis, combined with the results of fetal phenotype and postpartum follow-up, the clinical characteristics of ROH are discussed, so as to better understand the relationship between ROH and its phenotype.

ABSTRACT

Unnatural amino acids are widely used in medicine, pesticide, material, and other industries and the green and efficient synthesis has attracted a lot of attention. In recent years, with the rapid development of synthetic biology, microbial cell factories have become a promising means for biosynthesis of unnatural amino acids. This study reviewed the construction and application of microbial cell factories for unnatural amino acid, including the synthetic pathway reconstruction, design/modification of key enzymes and their coordinated regulation with precursors, blocking of competitive alternative pathways, and construction of cofactor circulation systems. Meanwhile, on the basis of the new principles for designing the microbial cell factories, new biosynthetic pathways adapted to cells and the production environment, as well as new biomanufacturing system established based on cell adaptive evolution and intelligent fermentation regulation, we looked forward to the further construction and application of microbial cell factories for industrial bio-production.

ABSTRACT

Cases in which the duplication of chromosome 8p (dup 8p) is observed are characterized by facial dysmorphism, agenesis/hypoplasia of the corpus callosum, heart defects and severe mental retardation. The frequency of dup 8p cases is higher compared to other chromosomes because of the Non-allelic homologous recombination (NAHR) between two segmental duplication regions (SDs) containing olfactory receptor gene clusters, REPD (repeat-distal) and REPP (repeat-proximal), located in chromosome 8p23.1. Here we generated a human iPSC line from a patient's amniotic fluid cells with a 18 Mb duplication in 8p23.3p22, which will serve as useful tools for studying dup 8p syndrome.
