ABSTRACT

Clinico-epidemiological and clinico-nosological indices of the hip injury (HI) in a structure of polysystemic and polyorgan damage, depending on gender, age of the injured patients, as well as the main mechanisms and conditions of trauma, were investigated. Four hundreds of charts of stationary patients, the injured persons, suffering polytrauma, who were treated in The Ternopyl University clinic in 2008 - 2012 yrs, were analyzed. The HI, as a component of polysystemic damage, was revealed in 54 patients. In accordance to the ranking analysis data, the HI coexistence in polytrauma probably depends on gender and age of the injured persons (mainly the men of employable age). Mostly such injures have occurred while the traffic accident in 61.1% of observations and while domestic conditions - in 29.6%. Severity of coexistent HI in a polytrauma structure probably depends on their mechanisms. So, in falling down in most cases combination with other skeletal trauma and damage of head was noted; in a direct punch - with cranio-cerebral trauma; in coexistence of various mechanisms the most severe injuries have occurred with damage of several anatomo-functional regions, not the hip only, but head, thorax and abdomen.

ABSTRACT

The results of a medical help delivery on prehospital stage to 200 injured persons, suffering penetrating abdominal wounding with hepatic damage, were analyzed. The main cause of death in 87.5% of the injured persons with open hepatic damage is a non-compensated blood loss, in 12.5%--traumatic shock and a prehospital stage duration. In accordance to data of the expert estimation, the anesthesiologic adequacy have constituted in the group of survivors--80.76%, and in those, who died--68.75%, confirming a very high level of the anesthesy adequacy. Level of adequacy of the blood loss compensation have constituted in group of survivors--57.69%, and in the group of patients, who died--37.50%, what could not be considered satisfactory. In patients--survivors a conditional adequacy of application of hemostatics have constituted 76%, and in a group of the injured persons, who have died--0.

ABSTRACT

Results of investigation of collagen metabolism in Dupuitren's contracture (DC) were summarized. The patients were operated for calculous cholecystitis and DC stages II - III. The changes revealed witnessed about more expressed degradation of collagen and affection of the elastin components of connective tissue. On background of the pathological process progress in palmar aponeurosis in patients, suffering DC, a content of oxyproline have enhanced trustworthy in urine and reduced in tissue of a changed palmar aponeurosis.

ABSTRACT

The results of morphological investigation of hepatic tissues in 98 patients, suffering contracture of Dupuytren, chronic hepatitis and hepatic fibrosis stages I, II i III, were adduced. The patients were operated on for calculous cholecystitis. The attempt to organize the groups with certain morphological changes, depending on the hepatic pathological process persistence (up to 5 yrs, from 5 to 10 yrs, over 10 yrs) have appeared nonfair. The changes revealed are too different and characteristic not only for the examined patients.

ABSTRACT

Dupuytren's contracture--a pathology that should not be seen as an isolated lesion tendon-aponeurotic structures of the palmar surface of the hand, but as a disease that requires careful research and a comprehensive, differentiated approach to treatment. The aim of the study was to investigate the functional activity of the phagocytic system of Dupuytren's contracture patients with chronic hepatitis and liver fibrosis in liquidators of the CHNPP accident consequences. The resulting study 188 patients aged 45-65 years showed the correlation of the data of the functional activity of phagocytic system with degree of liver fibrosis, thus objectively assess the patient's condition and make the appropriate correction in the diagnostic criteria as well as in medical and rehabilitative programs.

ABSTRACT

The results of morphological investigations of the palms tissues in the patients, suffering Dupuytren's contracture (DC) and in whom chronic hepatitis was diagnosed, are adduced. The patients were divided on three groups, depending on the hepatic fibrosis grade present. Determination of morphological changes in various stages in patients, suffering hepatic fibrosis stage one and lasting up to 5 years, witnesses the possibility of the DC recurrence and its severity enhancement. The authors stress the necessity of morphological investigations conduction with the objective to determine the hepatic fibrosis grade while surgical intervention planning.

ABSTRACT

The results of Dupuytren's contracture treatment in patients, suffering chronic hepatitis with the hand deformity grade II and III, in the injured persons and liquidators of the Chernobyl disaster consequences, conducted in 2000-2010 yrs., were adduced. Beginning from the third week, when an active movements had emerged, the patients were treated locally, including lidase and solution, which consisted of dimexid, isotonic saline and furazolidon.

There were examined 131 injured persons, suffering penetrating abdominal wounding and hepatic injury. Correlation analysis was done, basing on studying of the results of the injured persons state estimation, using prognostic scales, aiming to prognosticate the traumatic process course.

ABSTRACT

Efficacy of treatment of "high-energy trauma" depends on timely establishing of diagnosis. Modern radiological diagnosis permits to reveal the damage of the abdominal parenchyma organs and kidneys, as well as retroperitoneal space hematoma. The ultrasonograhic investigation method (UIM) is considered the leading one in estimation of traumatic damage of parenchymatous organs of abdominal cavity and retroperitoneal space because of contraindications absence, as well as radiological load, noninvasiveness and accessibility. The issues of ultrasonographic diagnosis of retroperitoneal hematoms in closed abdominal trauma and lumbar region were enlighted, their echosemiotics is adduced. There were examined 65 injured persons, suffering polysystemic and polyorgan damages, in whom retroperitoneal hematoma was revealed. The UIM data were compared with intraoperative diagnosis results. There were established the method possibilities not only in the hematoms diagnosis but in dynamical control conduction for the structure changes in the damaged region in conditions of favorable course or in complications occurrence, retroperitoneal abscess in part.

Orthopedic examination of 1100 school-children was conducted in Rakhiv Region, Zakarpats'ka Oblast for detection of inbreeding. Significant correlation between the coefficient of inbreeding and the number of abnormalities in locomotor apparatus was detected.
