ABSTRACT

Among 131 patients with angioreticulomas of different parts of the central nervous system 16 and systemic angioreticulomatosis, which in half of the patients was in the form of multifocal lesion within the boundaries of the central nervous system, and in the remaining patients, in the form of the Hippel-Lindau syndrome. The authors point out frequent recurrence of this pathological condition, relatively poor results of surgical treatment, and the difficulty of intravital diagnosis, particularly in the case of asymptomatic visceral pathology in the Hippel-Lindau disease. The complex of diagnostic measures included total computer tomography which allowed systemic angioreticulomatosis to be verified in different stages of its clinical development. The clinical, computer-tomographic, and post-mortem findings in systemic angioreticulomatosis are compared.

ABSTRACT

The authors suggest a method of highly sensitive electrothermometry tested on 56 patients with extra- and intracerebral tumours of the brain matter of supratentorial localization. This method makes it possible to determine the localization, boundaries, dimensions and character of the tumour during the operation, which is essential for its subsequent removal with the least possible injury to surrounding tissues.

ABSTRACT

The most expedient surgical tactics in hour-glass tumors of intravertebral-mediastinal localization are discussed. In 10 of 27 cases total removal of the tumor through a vertebral approach proved possible. Total removal of a rather large intrathoracic node by Guleke's method was conducted in 2 cases. In 10 cases with large intrathoracic nodes, total removal of the tumor was carried out by means of a combined intravertebral-thoracic (transpleural) approach suggested by the authors. It is emphasized that multi-stage operations and partial resection of the tumor are inexpedient and that attempts to remove it totally through a thoracic approach only are inadmissible.

ABSTRACT

Results of clinical and psychological examinations of 10 patients with various manifestations of speech akinesia are presented. Four of these patients have developed the syndrome of akinetic mutism. An analysis of the time course of the akinetic mutism formation and a comparison of the symptoms of its initial stage with the manifestations of the speech akinesia observed in the course of stereotaxic operations performed for therapeutic purpose in 6 patients with extrapyramid system diseases allow a conclusion that there exists a common mechanism of various manifestations of speech akinesia consisting in insufficiency of motivation stimulation. Data of anatomo-histopathological examination of the brain of 2 patients who had the akinetic mutism point to a great rle of the frontal divisions of the cortex and its associative connections in the complicated, multi-level structural and functional basis of speech akinesia.

ABSTRACT

A new method of operative intervention for syringomyelia is described in which after the syringomyelia cyst is opened its permanent drainage is accomplished by introducing into its cavity the free end of an adjacent posterior root transected before-hand directly at the site of the funnel-shaped transition of the dura mater from the spinal cord onto the cuff. As a result, all movements of the spinal cord are made together with this root, which prevents the draining root from falling out of the cystic cavity and does not create conditions for it to inflict damage to the cystic walls and the surrounding cerebral tissue. The method was used in operations on 9 patients.

ABSTRACT

Operations on the brain require an optimum coagulation effect to be produced by forceps for bipolar coagulation. New forceps models are described in which the elimination of heat from the operating ends is considerably improved by making the whole forceps of copper and providing intermittent supply of high-frequency current to it. As a result the coagulation effect is distinguished by slight necrosis of the tissues adjoining the ends of the forceps and no sticking of these tissues to these ends.

ABSTRACT

Among 449 patients with intravertebral tumours, 35 patients (7.8%) had sand-glass shaped tumours, 19 of them being localized in the neck, which comprised 19.6% of the 97 cervical intravertebral tumours, and 45.9% of neurinomas. An analysis of the material permits to conclude that when the symptoms of spinal cord compression are combined with an extension of the intervertebral foramen, the indications for myelography for precising the diagnosis are relative. A one-stage total excision of the tumour is indicated, either via a vertebral approach, or via a bilateral approach by way of laminectomy and from the neck. Whenever the intervention has to be divided into two stages, the intravertebral node should be removed first, and the extra-vertebral one afterwards from the cervical incision, always striving to cut the interval between the two interventions down to 3-4 weeks.

ABSTRACT

In cases of a pre-operatively diagnosed mediastinal tumour, but with no symptoms of its penetrating the vertebral canal via the intervertebral foramen, and with the surgeon suddenly discovering during anterio-lateral or lateral transpleural thoracotomy the tumor's spur going deep into the intervertebral foramen then it is a modification of the operation involving an effective removal of the mediastinal-intravertebral newgrowth, as described in this report, is most appropriate. This operation is performed in a single-stage fashion both on the level of the thoracic cavity and on that of the vertebral canal by undertaking a through examination of the intervertebral foramen from both sides. The authors believe this modification of the operative intervention to present advantages over the classical Guleke procedure, even in cases with an exactly established diagnosis of neurinoma, when one of its major node lies in the posterior mediastinum and the second--in the vertebral canal. Three cases in which this modification of the operation was applied with success are reported.
