ABSTRACT

Volcanic plumes pose a hazard to health and society and a particular risk for aviation. Hazard mitigation relies on forecasting plume dispersion within the atmosphere over time. The accuracy of forecasts depends on our understanding of particle dispersion and sedimentation processes, as well as on the accuracy of model input parameters, such as the initial particle size distribution and concentrations of volcanic particles (i.e., volcanic ash) in the atmosphere. However, our understating of these processes and the accurate quantification of input parameters remain the main sources of uncertainty in plume dispersion modeling. It is usually impractical to sample volcanic plumes directly, but particle sedimentation can be constrained in the laboratory. Here, we describe the design of a new experimental apparatus for investigating the dynamics of free-falling volcanic particles. The apparatus can produce a sustained column of falling particles with variable particle concentrations appropriate to a volcanic plume. Controllable experimental parameters include particle size distributions, types, and release rates. A laser-illuminated macrophotography system allows imaging of in-flight particles and their interactions. The mass of landing particles is logged to inform deposition rates. Quantitative measurements include particle morphology characterization, settling velocities, flow rates, and estimation of concentrations. Simultaneous observations of particle interaction processes and settling dynamics through direct control over a wide range of parameters will improve our parameterization of volcanic plume dynamics. Although the apparatus has been specifically designed for volcanological investigations, it can also be used to explore the characteristics of free-falling particle columns occurring in both environmental and industrial settings.

ABSTRACT

Supraglacial lake drainage events influence Greenland Ice Sheet dynamics on hourly to interannual timescales. However, direct observations are rare, and, to date, no in situ studies exist from fast-flowing sectors of the ice sheet. Here, we present observations of a rapid lake drainage event at Store Glacier, west Greenland, in 2018. The drainage event transported 4.8 × 106 m3 of meltwater to the glacier bed in ∼5 h, reducing the lake to a third of its original volume. During drainage, the local ice surface rose by 0.55 m, and surface velocity increased from 2.0 m⋅d-1 to 5.3 m⋅d-1 Dynamic responses were greatest ∼4 km downstream from the lake, which we interpret as an area of transient water storage constrained by basal topography. Drainage initiated, without any precursory trigger, when the lake expanded and reactivated a preexisting fracture that had been responsible for a drainage event 1 y earlier. Since formation, this fracture had advected ∼500 m from the lake's deepest point, meaning the lake did not fully drain. Partial drainage events have previously been assumed to occur slowly via lake overtopping, with a comparatively small dynamic influence. In contrast, our findings show that partial drainage events can be caused by hydrofracture, producing new hydrological connections that continue to concentrate the supply of surface meltwater to the bed of the ice sheet throughout the melt season. Our findings therefore indicate that the quantity and resultant dynamic influence of rapid lake drainages are likely being underestimated.

ABSTRACT

Hemopure (hemoglobin glutamer-250 [bovine]; HBOC-201) is a hemoglobin (Hb)-based oxygen carrier registered with the Medicines Control Council of South Africa. It is indicated for the treatment of adult patients who are acutely anemic, for the purpose of maintaining tissue oxygen delivery thus eliminating, delaying, or reducing the need for allogeneic red blood cells (RBCs). Hemopure is a volume expander, and circulatory volume must be carefully monitored for signs of fluid overload. Hemopure is not as effective as RBCs for restoring Hb content and concentration, but in cases of severe anemia where allogeneic blood is not an option or is unavailable, it may offer an immediate alternative for improving oxygen transport. This document provides clinical recommendations on the safe and effective use of Hemopure based on the postmarketing experience in South Africa as well as a better understanding of Hemopure properties reflected in recent publications.

ABSTRACT

Acetaminophen (APAP) hepatotoxicity is protected by S-adenosyl-l-methionine (SAMe) treatment 1hour (h) after APAP in C57/Bl6 mice. This study examined protein carbonylation as well as mitochondrial and cytosolic protein adduction by 4-hydroxynonenal (4-HNE) using mass spectrometry (MS) analysis. Additional studies investigated the leakage of mitochondrial proteins and 4-HNE adduction of these proteins. Male C57/Bl6 mice (n=5/group) were divided into the following groups and treated as indicated: Veh (15ml/kg water, ip), SAMe (1.25mmol/kg, ip), APAP (250mg/kg), and SAMe given 1h after APAP (S+A). APAP toxicity was confirmed by an increase (p<0.05) in plasma ALT (U/l) and liver weight/10g body weight relative to the Veh, SAMe and S+A groups 4h following APAP treatment. SAMe administered 1h post-APAP partially corrected APAP hepatotoxicity as ALT and liver weight/10g body weights were lower in the S+A group compared the APAP group. APAP induced leakage of the mitochondrial protein, carbamoyl phosphate synthase-1 (CPS-1) into the cytosol and which was reduced in the S+A group. SAMe further reduced the extent of APAP mediated 4-HNE adduction of CPS-1. MS analysis of hepatic and mitochondrial subcellular fractions identified proteins from APAP treated mice. Site specific 4-HNE adducts were identified on mitochondrial proteins sarcosine dehydrogenase and carbamoyl phosphate synthase-1 (CPS-1). In summary, APAP is associated with 4-HNE adduction of proteins as identified by MS analysis and that CPS-1 leakage was greater in APAP treated mice. SAMe reduced the extent of 4-HNE adduction of proteins as well as leakage of CPS-1.

ABSTRACT

The emplacement mechanisms of rhyolitic lava flows are enigmatic and, despite high lava viscosities and low inferred effusion rates, can result in remarkably, laterally extensive (>30 km) flow fields. Here we present the first observations of an active, extensive rhyolitic lava flow field from the 2011-2012 eruption at Cordón Caulle, Chile. We combine high-resolution four-dimensional flow front models, created using automated photo reconstruction techniques, with sequential satellite imagery. Late-stage evolution greatly extended the compound lava flow field, with localized extrusion from stalled, ~35 m-thick flow margins creating >80 breakout lobes. In January 2013, flow front advance continued ~3.6 km from the vent, despite detectable lava supply ceasing 6-8 months earlier. This illustrates how efficient thermal insulation by the lava carapace promotes prolonged within-flow horizontal lava transport, boosting the extent of the flow. The unexpected similarities with compound basaltic lava flow fields point towards a unifying model of lava emplacement.

INTRODUCTION: Prostatic metastasis from a primary bowel adenocarcinoma has been only rarely reported in the medical literature. The case reported here is rare in the fact that the primary tumor was from a right-sided bowel adenocarcinoma. It is unusual because initial immunostaining was not fully conclusive, and so a relatively new method of immunostaining, CDX2, was used to ascertain its histopathology. CASE PRESENTATION: We describe the case of a 54-year-old Caucasian man who had a right hemicolectomy for a primary cecal adenocarcinoma, which was completely excised. Following the procedure, he received adjuvant chemotherapy. Computed tomography scans showed no evidence of local recurrence or metastatic disease. Then, five years later, he presented to his general practitioner with urinary symptoms. An abnormal prostate was palpated on digital rectal examination. Trans-rectal prostatic biopsies were performed, which showed colorectal metastases within the prostate gland. This was confirmed with CDX2 immunohistochemistry. There was no further evidence of distant metastases on positron emission tomography-computed tomography scans. CONCLUSIONS: This case demonstrates a rare isolated hematogenous spread to the prostate from a primary cecal adenocarcinoma, several years after definitive treatment and excision. This highlights the importance of accurate immunohistochemistry and imaging in planning further management and treatment.

ABSTRACT

The present review of fluid therapy studies using balanced solutions versus isotonic saline fluids (both crystalloids and colloids) aims to address recent controversy in this topic. The change to the acid-base equilibrium based on fluid selection is described. Key terms such as dilutional-hyperchloraemic acidosis (correctly used instead of dilutional acidosis or hyperchloraemic metabolic acidosis to account for both the Henderson-Hasselbalch and Stewart equations), isotonic saline and balanced solutions are defined. The review concludes that dilutional-hyperchloraemic acidosis is a side effect, mainly observed after the administration of large volumes of isotonic saline as a crystalloid. Its effect is moderate and relatively transient, and is minimised by limiting crystalloid administration through the use of colloids (in any carrier). Convincing evidence for clinically relevant adverse effects of dilutional-hyperchloraemic acidosis on renal function, coagulation, blood loss, the need for transfusion, gastrointestinal function or mortality cannot be found. In view of the long-term use of isotonic saline either as a crystalloid or as a colloid carrier, the paucity of data documenting detrimental effects of dilutional-hyperchloraemic acidosis and the limited published information on the effects of balanced solutions on outcome, we cannot currently recommend changing fluid therapy to the use of a balanced colloid preparation.

ABSTRACT

A total of 52 volunteers were recruited to take part in a dual-centered, randomized, blinded study so investigators could determine whether the level of airborne infection could be significantly reduced in patients randomly assigned to treatment with either Nasaleze cellulose extract alone or a combination of Nasaleze cellulose and powdered garlic extract (PGE). One puff into each nostril was recommended, and volunteers who developed an infection while traveling were told to use at least 3 puffs per nostril until symptoms were reduced. This study took place over an 8-wk period across Finland and the United Kingdom between November 2006 and March 2007. Volunteers were instructed to use a 5-point scale to assess their health and to record infectious episodes and symptoms in a daily diary. The activetreatment group (Nasaleze cellulose with PGE) experienced significantly fewer infections than the control group (20 vs 57; P<.001) and far fewer days on which an infection was obviously present (126 d in the active group vs 240 d in the control group; P<.05). Consequently, volunteers in the active group were less likely to pick up an airborne infection when PGE was added to this novel cellulose extract. Volunteers in the control group were much more likely to report more than 1 infectious episode over the treatment period or to endure longer periods of infection. The investigators concluded that the combination Nasaleze Travel formulation significantly reduced the number of airborne infections to which volunteers were exposed while traveling.

ABSTRACT

The distribution of the narrowly endemic butterfly Pseudophilotes sinaicus (Lycaenidae) was studied. Potential habitat within its range was first located and then the quality of that habitat assessed. Degree of shelter, diversity of plant species, and resource area of an individual food plant (Thymus decussatus) all affected habitat quality and together were used to develop an index of habitat suitability applicable to each site. The butterfly's distribution was then studied within the identified network of suitable habitat patches: isolated patches with a small resource area were least likely to contain butterflies. Population size in a patch (as opposed merely to patch occupancy) was affected by resource area and the quality of habitat within that patch. Metapopulation processes and variation in habitat quality therefore appear to combine to describe the distribution of patches occupied by P. sinaicus and their population sizes. This finding provides insights into some of the processes operating on an endemic species throughout its geographical range and has important implications for the conservation of this rare butterfly.
