ABSTRACT

OBJECTIVE: The purpose of this investigation was to explore the quality of 38, 48 teeth's hard tissues at different eruption stages and mandibular bone in different postpartum ontogenesis periods. MATERIAL AND METHODS: The research involved 102 male patients divided into groups according to their age: 15-20, 21-30 years old, they were extracted one tooth 38, 48 without inflammatory process signs and a fragment of the mandibular bone alveolar part in the projection of the teeth 38, 48 for orthodontic indications. In the comparison group (49 observations without signs of connective tissue dysplasia), in the study group (53 observations with signs of connective tissue dysplasia) we extracted teeth 38, 48 which were in the bone tissue. We analyzed condition of crown and root systems of extracted 38, 48 teeth, densitometric density of mineral component, size of enamel prisms, size of prismatic shells of organic matrix, spatial organization of collagen fibers in bone tissue, size characteristics of bone plates and mineralization centers of bone tissue. Processing of the obtained data was performed by methods of variation statistics using standard packages Microsoft Excel 2008, Statistica 12.0. RESULTS: The paper reveals one of the surgical dentistry pressing issues related to the tething mechanism of lower human wisdom teeth, considered by the authors from the position of tissue disorders in collagen type 1 observed in connective tissue dysplasia. At age of 15-20 years in connective tissue dysplasia, the enamel prisms hypomineralized areas are generalized; at age of 21-30 years the hypomineralized areas are characterized by local changes. At the age of 15-20 years in connective tissue dysplasia the bone plates splitting is observed at the level of most fibrils, collagen fibers have insufficiently oriented direction in contrast to the age group of 21-30 years where bone plates splitting is characteristic for single fibrils and collagen fibrils are clearly oriented. CONCLUSION: Morphological and histological changes in teeth 38, 48 and in bone tissue prevent correct and timely teething at 15-20 years and create more favorable conditions for teething at 21-30 years with a slower rate in connective tissue dysplasia, but in both groups teeth eruption occurs under unfavorable anatomic conditions.

ABSTRACT

OBJECTIVE: It is necessary to study the human enamel prisms size at different periods of postpartum ontogenesis by atomic force microscopy. MATERIAL AND METHODS: The study involved 94 males which were divided into groups by age: 15-20, 21-30, 31-40, 41-50, 51-60 years old, each examined male had one 3.8 or 4.8 tooth removed (from the fracture line in the region angle of the mandible, according to orthodontic indications) for medical reasons. The shape, roughness, length and width of the enamel prisms were studied in the Image Analysis NT-VDT program according to the method of Omsk State Medical University. RESULTS: The enamel prisms structure at all ages has a regular, ordered shape with 6-sided and 7-sided figures. The most constant structure occurs at the age group of 41-50, 51-60 years. The fastest enamel prisms growth occurs in the direction that increases the prism length at all ages. Growth on a plane which increases the prism width is slow at 41-50 years. It is catching up with the growth rate of enamel prisms in length at 51-60 years old. Enamel prisms at 15-20, 21-30 years old stretch faster in the horizontal direction. CONCLUSION: In maturation process, due to correct and harmonious growth, enamel prisms are timely modified from pyramidal form in the early postpartum period to a prismatic form in the late postpartum period of ontogenesis.

ABSTRACT

OBJECTIVE: To give a quantitative and qualitative characteristic of the structure of the enamel's mineral component structure of impacted teeth with or without connective tissue dysplasia in different periods of early postnatal human ontogenesis using densitometry and atomic force microscopy. MATERIALS AND METHODS: The study involved 120 males with and without connective tissue dysplasia (CTD), which were divided into 3 equal subgroups (60 people with CTD and 60 people without CTD), 20 people in each, according to age: 15-20, 21-30, 31-40 years old. Each of the examined was removed either 3.8 or 4.8 tooth. To study the inorganic component of tooth enamel, a densitometric assessment of enamel's optical density was carried out using computed tomography in the Kodak Dental Systems software (Trophy 2000) and preparation of thin sections of tooth samples 3.8 or 4.8 for atomic force microscopy (AFM) according to the methods of Omsk State Medical University. RESULTS: The structure of tooth enamel in connective tissue dysplasia in the early postpartum period of ontogenesis is characterized by pronounced polymorphisms and an insufficient level of maturity. The ordering and orientation of the enamel prisms are disturbed due to insufficient packing density and a large distance between the enamel prisms at the age of 15-20, 21-30. The established changes indicate the incomplete nature of amelogenesis with connective tissue dysplasia at the indicated ages. CONCLUSIONS: In case of connective tissue dysplasia in the early postnatal period of ontogenesis, an incomplete amelogenesis is observed. This process is manifested by lower values of the mineral component's optical density, low packing of enamel prisms, a large distance between enamel prisms and their irregular shape.

ABSTRACT

UNLABELLED: The aim of the study was to investigate the therapeutic opportunities of S-adenosylmethionine and polyunsaturated lecithin in Child class A patients with alcoholic liver cirrhosis using transient elastography. MATERIALS: The 38 males Child class A alcoholic liver cirrhosis were investigated. RESULTS: The use of S-adenosylmethionine or polyunsaturated lecithin in patients with alcoholic liver cirrhosis is reasonable cause of their possibility to reduce the fibrogenesis in liver tissue and of their possibility to improve the function of remained intact hepatocytes. Thus use of this remedies prevents progression of cirrhotic transformation of the liver and gives a patient an opportunity to get the stable clinical status.

ABSTRACT

THE AIM: development of quality and consequences of medical care evaluation test at chronic calculouse cholecystitis by the expert--gastroenterologist. Defined defects, that statistically significantly determined negative outcomes, had fixed distribution within groups by frequency of these defects. Analysis resulted in calculation of the cumulative information value score of each case separately for diagnosis and treatment processes with assessment of probabilistic limits by binary logistic regression method with determination of limits of appropriate and inadequate medical care provision to chronic calculouse cholecystitis patients provide the algorithm for gastroenterologist.

ABSTRACT

AIM: to develop a technology for assessing the quality and consequences of medical care in chronic noncalculous cholecystitis by an expert--a therapist, a gastroenterologist. MATERIALS AND METHODS: One hundred and ninety-three cases of expert appraisal of medical care quality in chronic noncalculous cholecystitis were analysed. The expert appraisal of the quality of medical care was carried out by comparing with that of the reference - the standard for the diagnosis and treatment of chronic noncalculous cholecystitis. To detect the defects correlated with the negative consequences of medical care, the authors calculated statistical criteria for assessing an association between the indices of accumulated informative value and the consequences of rendered medical care. RESULTS: According to the assessed quality of medical care, the patients were divided into two groups: (1) 163 patients with negative consequences of delivered medical care; (2) 30 patients with its positive consequences (a control group). The identifed drawbacks that significantly determined negative outcomes had a certain distribution in the groups by their frequency. The analysis provided calculations of the accumulated informative value index for each case individually for the processes of diagnosis and treatment, by drawing up the boundaries of proper and improper medical care delivered to patients with chronic noncalculous cholecystitis. CONCLUSION: The formulated criteria of positive consequences of medical care are oriented to the differential diagnosis of biliary tract pathology, to differentiated approaches to treatment analysis, and to the likely outcomes of exposure to medical and diagnostic actions on the pathological process, which allows an expert to predict its further dynamics and consequences of rendered medical care. The algorithmized expert appraisal of cases of medical care delivery makes it possible to objectivize the opinion and conclusions of an expert.

ABSTRACT

The proposed experimental model reproduces the mechanism of isolated contusion of the heart resultant from the chest stroke against the steering wheel prop in a car accident. The conditions of experiment rule out the additional pathogenetic factors occurring in clinical practice (extracardiac injuries, alcohol intoxication, blood loss, pain, etc.) and hence, it is possible to study the mechanisms of developing myocardial dysfunction proper and its contribution to the course and outcome of the posttraumatic period. Clinical, hemodynamic, and pathomorphological equivalents of this kind of blunt injury of the heart were obtained.

ABSTRACT

Experiments on rabbits compared the X-ray, morphological, and magnetic resonance imaging of compression fractures of the vertebral body. Edema of the bone marrow of the vertebral body was ascertained to be a basic morphological substrate that evokes a modified MR signal. The data on 178 children with suspected compression fracture of the vertebral body were used to consider the potentialities of MRI and X-ray study in this pathology. The MRI semiotics of uncomplicated compression fractures of vertebral bodies is presented. The sensitivities of MRI and spondylography in the diagnosis of compression fractures of vertebral bodies were 100 and 62.5%, respectively.

ABSTRACT

The authors showed new immune and morphologic phenomenon--toxic lymphoinhibition--mixed wooden dust (wood laminate, plywood) induces decrease of circulating pool of lymphatic elements and increases mitotic activity in T-dependent zones of central and peripheral lymphatic organs. Pine dust used in experiments and having antigen effects of highly molecular substances activates macrophages and moderate functional stress of T-immunity. With this, leukocyte and lymphocyte counts grow, qualitative and quantitative parameters of phagocytosis increase as well.

ABSTRACT

The analysis of the RNA virus spread of tick encephalitis in the course of infection has been performed. The virus RNA is detectable first, (as early as one day after the infection) in the lymph nodes. Later it is retained in the cells of T-dependent areas. Viral RNA is also the first to be detected in the vascular structures and ventricular system of the brain. The correlation between the time of the viral RNA replication and specific initial damage in the lymphoid tissue and central nervous system is established. The question is raised on the role of the lymphocyte recirculation in the spread of the virus in the body.

ABSTRACT

Lymph nodes of humans and experimental animals with tick-borne encephalitis (TBE) were studied using electron microscopy. TBE virus reproduction occurred in macrophages, reticular and plasma cells. Severe organelle destruction was found in the above cells and lymphocytes. The results suggest that macrophage, reticular and plasmatic cell death is due to the virus reproduction, while lymphocyte death is due to the toxic effect. It is evident that the outcome is determined by pathological process in lymphoid organs.

ABSTRACT

Radioisotopes were used to study the water spaces of the bone and bone marrow in 47 dogs with a created model of acute osteomyelitis. By the 3rd-5th day of experimental osteomyelitis sharp hyperhydration of the bone marrow and bone proper was revealed which was caused by a 2.5-2.8-fold increase of the volume of intercellular fluid in the zone of the affection, leading to circulatory disorders and necrosis. Dehydration of the tissues is accomplished by inflicting numerous percutaneous osteoperforations (NPO) with Kirschner's wire. The osteoperforations are made not at random but in a regular manner through both walls of the bone in two interperpendicular planes. Circulation in the zone of the affection is restored in this case and osteonecrosis in prevented. Conservative methods of treatment of acute hematogenous osteomyelitis were developed according to the stage of its development and were applied in 168 patients whose ages ranged from 20 days to 14 years; in 75% of patients who were admitted in the early stages of AHO the disease took and abrupt course. In patients with neglected forms of AHO three-fourfold infliction of NPO in the subacute stage produced a stimulating effect on reparative regeneration in the zones of osteonecrosis and the forming sequestra.

ABSTRACT

Morphological and immunological assays were employed to examine 18 epileptic patients and 40 Krushinskii-Molodkina rats undergoing subconvulsive and convulsive acoustic stimulations. After multiple audiogenic epileptiform convulsive fits the epileptogenic areas of the brain of epileptic patients (biopsy material) and the neocortex of the sensorimotor area, amygdaloid complex and hippocampus of rats manifested vascular alterations in the form of chronic or subacute vasculitis. The changes revealed in the ratio of B and T lymphocytes in the peripheral blood of the epileptic patients in combination with pronounced stimulation of cellular and humoral immunity responses to antigens of homologous and autologous brain extracts, more well-defined in marked inflammatory process, may attest to the immune genesis of endo- and perivasculitis.

ABSTRACT

Examinations of 70 patients with keeled deformation of the chest (KDC) have revealed signs of connective tissue dysplasia in all the examinees, as well as heart rhythm and conduction disorders. Echography has shown ventricular and atrial septal defects, mitral valve prolapse, dilatation of the aortic root, tricuspidal valve prolapse, abnormally located chorda. The development of KDC in the presence of external connective tissue stigmata, involvement of the cardiovascular system, changed dermatoglyphic pattern of the palms and fingers (as evidenced by dermatoglyphic analysis), as well as findings of histologic examinations of the cartilage removed in the course of thoracoplasty may indicate a generalized abnormality of the connective tissue in this patient population and KDC may be regarded as one of its manifestations.

ABSTRACT

The authors discuss their experience in the use of a biopolymeric composition of a biological glue KL-3 and an antiseptic for preoperative filling of bronchiectatic cavities in 28 patients with bronchiectases. The filling is performed before the operation during bronchoscopy conducted under anaesthesia. As a result endobronchitis is relieved, the time needed for preoperative management is shortened, and the number of possible postoperative complications is reduced to minimum.
