ABSTRACT

The field of structural health monitoring (SHM) faces a fundamental challenge related to accessibility. While analytical and empirical models and laboratory tests can provide engineers with an estimate of a structure's expected behavior under various loads, measurements of actual buildings require the installation and maintenance of sensors to collect observations. This is costly in terms of power and resources. MyShake, the free seismology smartphone app, aims to advance SHM by leveraging the presence of accelerometers in all smartphones and the wide usage of smartphones globally. MyShake records acceleration waveforms during earthquakes. Because phones are most typically located in buildings, a waveform recorded by MyShake contains response information from the structure in which the phone is located. This represents a free, potentially ubiquitous method of conducting critical structural measurements. In this work, we present preliminary findings that demonstrate the efficacy of smartphones for extracting the fundamental frequency of buildings, benchmarked against traditional accelerometers in a shake table test. Additionally, we present seven proof-of-concept examples of data collected by anonymous and privately owned smartphones running the MyShake app in real buildings, and assess the fundamental frequencies we measure. In all cases, the measured fundamental frequency is found to be reasonable and within an expected range in comparison with several commonly used empirical equations. For one irregularly shaped building, three separate measurements made over the course of four months fall within 7% of each other, validating the accuracy of MyShake measurements and illustrating how repeat observations can improve the robustness of the structural health catalog we aim to build.

ABSTRACT

The Afar triple junction accustoms the diverging plate dynamics between the Arabian, Nubian, and Somalian plates along the Red Sea, Gulf of Aden, and East African rifts. The average anisotropy obtained from shear-wave splitting measurements agrees with the surface motion recovered by geodetic analyses. However, the vertical layering of anisotropy in this region is yet to be accurately determined. Here, we use earthquake seismic data to map Rayleigh-wave azimuthal anisotropy in the crust and lithospheric mantle beneath the East African Rift System. Our results suggest that a layering of anisotropy is present around the East African Rift System. At shorter periods that sample the crust, rift-parallel anisotropy is present in the vicinity of the rift, but in the central part of the rift, rift-normal anisotropy is found. At longer periods, sampling the lithospheric mantle, the anisotropic pattern is quite different. These observations suggest that the crust and lithospheric mantle are mechanically decoupled beneath the environs of the East African Rift System. Similarly, these results suggest complex dynamics within the crust and lithosphere in the region of the Afar triple junction.

ABSTRACT

Introduction Urothelial carcinomas are the most common types of bladder tumors that have recently shown a changing trend in treatment protocols with the introduction and approval of immune checkpoint inhibitors. The most important immune checkpoint lies with the PD-1-PD-L1 axis. Although multiple drugs have been approved, there is uncertainty about patient selection criteria and diagnostic assays. Recent studies related to the laboratory-developed tests have opened up the horizon of PD-1 and PD-L1 immunohistochemistry even at resource-constrained laboratories. We propose to study these immunohistochemistry markers in our laboratory using newer clones. Materials and Methods We selected 116 consecutive cases of transurethral bladder tumor resection from our laboratory archive and applied PD-1 and PD-L1 immunohistochemistry. The study was approved by the institution's ethics committee. Results We found high expression of PD-1 and PD-L1 in urothelial carcinoma even with different cut-offs of PD-L1. Muscle invasion, lamina invasion, and grade of carcinoma had a statistically significant effect on the expression; however, age and sex did not affect the expression. Conclusion Based on our current study, we can conclude that the clones used in our study show high expression in urothelial carcinoma and can aid in patient selection and treatment protocol, irrespective of age and sex.

Reticulocyte parameters including reticulocyte haemoglobin equivalent (Ret-He) and immature reticulocyte fraction (IRF) are newly recognised hematological parameters that are being used for diagnosis and follow-up of anaemic patients. Reference intervals of these parameters have been established in different populations, however, the data relating to pregnancy are still lacking. One hundred and fifty-five first-trimester pregnant females were screened and the reference interval was calculated after selecting the patient with fixed criteria. R statistical software was used for statistical calculations. We tried to establish a reference interval of Ret-He content and IRF in first-trimester pregnancy in our study.IMPACT STATEMENTWhat is already known on this subject? Ret-He and IRF have been established as the marker of iron deficiency and iron-deficiency anaemia in different age groups and as a marker of response to iron therapy. However, literature is scarce regarding the reference intervals of these parameters, especially in pregnancy.What do the results of this study add? This study establishes the reference interval of newer reticulocyte parameters in first-trimester pregnancy which is not yet established in the literature. Establishing a reference interval is required for any laboratory parameters to be used in the clinical context.What are the implications of these findings for clinical practice and further research? The results of this study may help in making a clinical decision regarding iron deficiency in early pregnancy which is one of the common clinical problems in pregnancy. This study also serves as a baseline study for further studies of reference intervals for newer reticulocyte parameters in pregnancy. A similar study with a larger study population and follow-up with iron therapy may establish these parameters as one of the important markers of iron deficiency in pregnancy and help institute iron therapy on case-to-case basis.

Estimates of Within-Subject and between subject biological variation for the white blood cell differential count (DC) have not been reported in South Asia. Therefore, we attempted to measure the short-term biological variation estimates for DC. The study was conducted on 28 healthy volunteers (15 males and 13 females). Blood from the volunteers was collected in the morning in K3-EDTA vials and analyzed in triplicate on the Sysmex XN-1000 analyzer, for six consecutive days. The Within subject, between subject and analytical coefficient of variation of the DC was calculated from the results by nested repeated measures ANOVA after outlier exclusion. The Reference change values (RCV) were also calculated. The within-subject variation for eosinophil Count and between subject variation for basophils in our study from South Asia was greater than the published European and American studies. Males and females showed similar biological variation for DC. The within-subject variation of other parameters (Neutrophils, Lymphocytes, Monocytes and Basophils) were similar or showed only mild differences to the published studies. The markedly different within-subject variation for Eosinophil counts suggest that the RCV for DC in South Asians need to be different from the published data in order to have clinical relevance. The Within-subject variation values of the other parameters seem transportable from the published European and American studies, but the small differences found mean that further regional estimates need to be reported for robust evidence of the same.

BACKGROUND: Data on adjunctive use of magnesium with ibutilide for conversion of persistent rheumatic atrial fibrillation and flutter to sinus rhythm is lacking. AIM: We aimed to study the efficacy of adjunctive supplementation of intravenous magnesium with ibutilide for conversion of persistent rheumatic atrial fibrillation and flutter to sinus rhythm and to define a definite level of serum magnesium which leads to significant increase in rates of such conversion. METHODS AND RESULTS: This was a prospective study including 33 Rheumatic heart disease patients (13 males and 20 females) with mean age of 49.27 ± 11.4 years and persistent AF or AFl. All patients received intravenous magnesium to raise serum magnesium level in range of 4 mg/dl to 4.5 mg/dl prior to administration of Ibutilide. 25 out of 33 (76%) patients converted to sinus rhythm. Upon univariate analysis, presence of background beta blocker therapy, serum potassium and magnesium at time of Ibutilide injection were found to have significant relation with conversion to sinus rhythm. Upon multivariate analysis serum magnesium level at the time of Ibutilide injection was found to have significant contribution on post injection rhythm reversal (p-value = 0.006). The level of magnesium at 3.8 mg/dl was found to have maximum sensitivity of 96% and specificity of 62.5% for conversion to sinus rhythm by ibutilide with magnesium (p-value< 0.05). CONCLUSIONS: Ibutilide is highly effective in cardioversion of persistent rheumatic atrial fibrillation/flutter patients. Raising Serum Magnesium levels above 3.8 mg/dl significantly improves efficacy of ibutilide.

Introduction The diagnosis of anemia in pregnancy should be early to prevent complications to mother and baby. It should be simple and safe, and therefore feasibility of reticulocyte parameters may prove beneficial in this regard. Newer reticulocyte parameters have not been studied extensively especially in pregnancy. The present study was conducted to evaluate reticulocyte hemoglobin content (Ret-He), immature reticulocyte fraction, and reticulocyte count and to compare them with serum ferritin, mean corpuscular volume (MCV), and mean corpuscular hemoglobin (MCH) in assessment anemia in pregnancy. Material and Methods Prospective study included first trimester pregnant females where hemoglobin, MCV, and MCH were categorized and statistically analyzed with serum ferritin and reticulocyte parameters. Results The study included 155 first trimester pregnant females showing statistical difference in Ret-He between all categories of hemoglobin and MCV ( p < 0.05). ROC curve analysis using MCV < 83 fl and MCH <27 pg showed that area under curve for Ret-He at cutoff of 27.8 ng/mL was 0.93 (95% confidence interval 0.90-0.98, sensitivity 93%, specificity 83%). The cutoff of 27.2 ng/mL Ret-He for the diagnosis of iron deficiency anemia has specificity of 86%, sensitivity 71%, positive predictive value 0.39, and likelihood ratio of 5.1. Conclusion Ret-He is a feasible and easily available parameter which may be helpful in differentiating between microcytic hypochromic and normocytic normochromic anemia along with frank and borderline anemia in first trimester pregnant females. This in turn is essential to initiate an early therapy so as to prevent the maternal and fetal complications.

There is no rigorous publication on the biological variation of common hematological parameters in South Asians to date. Also, there are few publications worldwide dealing with a variation of Reticulocyte parameters. Therefore, an attempt was made to estimate the short term within-subject and between-subject biological variation of common hematological and reticulocyte parameters. Twenty-eight healthy individuals (fifteen males and thirteen females) were selected after clinical and laboratory examination. Blood was collected in K3-EDTA vials in the morning for six consecutive days and analysed in triplicate on the Sysmex XN-1000 analyzer. Outliers were excluded and the within-subject, between-subject and analytical coefficient of variation calculated after statistical analysis by nested repeated measures ANOVA. The Reference change values (RCV), and estimates for desirable imprecision, bias, total error and index of individuality calculated. The within-subject biological variation for the studied subset belonging to South Asia closely followed published European and American studies and were similar for males and females. The between-subject variation showed differences from the published studies for white blood cells, platelets, red blood cells, hemoglobin, platelet indices and reticulocyte hemoglobin as well as between males and females for hemoglobin, red blood cell count and hematocrit. All the indices of individuality were low. This study supports the contention that the conclusions from within-subject biological variation for common hematological parameters are important and transportable to a South Asian population for short-term serial measurements. For quality specifications dependent on between-subject variation, the lower estimates from European and American studies should be used.

BACKGROUND: Pancytopenia, an important hematological presentation is associated with different causes, which may vary in different regions. Uttarakhand, a north Himalayan state of India lacks studies of pancytopenia and the prevalent causes present in this region. Therefore, the present study was conducted to study the clinico-hematological profile of pancytopenia in a tertiary care center in the Uttarakhand, a north Himalayan state of India. It was also intended to study if these causes showed any variation from other studies done in different regions of India. MATERIALS AND METHODS: The present observational study was conducted in a tertiary care institute situated in Uttarakhand state of India over a period of 1 year and 8 month from June 2017 to Feb 2019 including all the cases of pancytopenia. RESULTS: The most common cause of pancytopenia was megaloblastic anemia (25%), followed by aleukemic leukemia and hypoplastic/aplastic anemia (19.1% each). Visceral leishmaniasis also constituted an important cause of pancytopenia in this study (11.7%). CONCLUSION: The study concludes that megaloblastic anemia and aleukemic leukemia are the most common cause of pancytopenia. Although leishmaniasis is considered to be non-endemic in this region, it constitutes an important cause of pancytopenia here. The clinicians, especially the physicians practicing the primary care and pathologists, should be aware of the different causes of pancytopenia present in this Himalayan region of India; therefore, delay in diagnosis can be prevented along with unnecessary investigations.

Rainfall erosivity (or water erosion) has severe implications on agriculture, water, and land use management. Though, there were Rainfall erosivity studies on regional and global scale, tropical cyclones' Rainfall erosivity is poorly assessed and have not been documented for one of the most cyclones affecting regions of the world like Taiwan. Here, using 15-years of raindrop size distributions (RSD) and 60-years of hourly rain gauges data, we estimated cyclones (also called typhoons) rainfall erosivity over Taiwan, and establish that typhoons' mean rainfall erosivity is higher than the global mean rainfall erosivity. Moreover, regional variability of typhoons rainfall erosivity showed an increasing pattern from north to south (Taipei to Pingtung), with relatively higher values over eastern and southern parts of Taiwan. The annual mean erosivity of typhoons rainfall showed raising trends over eastern and southern Taiwan during 1958-2017. Our results provide an insight in assessing the land use and agricultural management for Taiwan.

Objective: To evaluate the plasma H2S levels and H2S synthesis activity in streptozotocin induced type 2 diabetes rats compared to the healthy controls and also to observe the effect of the aqueous extract of Swietenia macrophylla (S. macrophylla) seeds on the experimental groups. Methods: Seeds of S. macrophylla were separated, washed, shed-dried and finally extract was prepared. Thirty two wistar rats were selected for the experimental study. Streptozotocin was used for the induction of diabetes. H2S concentration in plasma was measured. H2S synthesizing activity in plasma was measured. Statistical analysis have done using Microsoft excel, Office 2003. Values were expressed by mean±SD. P<0.05 were considered statistically significant.Results:rats. The glucose levels are significantly lowered in the rats treated with metformin (5.48±0.03) mmol/L as well as with aqueous extract of S. macrophylla seeds (3.72±0.04) mmol/L. The HbA1c percentages in different groups of study subjects also indicate similar trends. Our study shows both the plasma H2S levels (22.07±0.73) mmol/L and plasma H2S synthesis activity (0.411±0.005 mmol/100 g) are significantly reduced in the streptozotocin induced diabetic rats.Conclusions:Although considering a small sample size, it can conclude that the fasting blood Fasting blood glucose level (7.74±0.02) mmol/L was significantly increased in diabetic glucose levels are inversely related to plasma H2S levels as well as H2S synthesis activity in plasma and the extract of S. macrophylla is associated with increased plasma H2S levels with effective lowering of blood glucose in streptozotocin induced diabetic rats.

ABSTRACT

Hypertriglyceridemia is defined as an abnormal concentration of triglyceride in the blood and has been associated with atherosclerosis, even in the absence of hypercholesterolemia. This case report is of 40-year-old man diagnosed to have hypertriglyceridemia who attended for routine screening in our diagnostic laboratory at Barasat. He was nonsmoker, non-alcoholic, had a reasonable diet with abundant fruits and vegetables, and was on regular exercise. He was not taking any lipid lowering medications. He hailed from Barasat, and was employed in Government sector in Barasat and policeman by profession. His father died at the age of 57 years in a heart attack; but his mother is healthy and now almost 62 years of age, and he has two brothers one elder and another younger to him, both are healthy. His blood pressure was normal, his body-mass index was 27, and his waist circumference was 96 cm and hip circumference was 103. His waist/hip ratio was 0.932. The Biochemical analyses were as follows- Fasting Glucose: 186 mg/dL, Total Cholesterol: 90 mg/dL, Triglycerides: 372 mg/dL, High-density cholesterol: 3.80 mg/dL, Low-density cholesterol: 2.90 mg/dL, VLDL: 83.20 mg/dL, Cholesterol/HDL-C ratio: 23.6:1, LDL-C/HDL-C: 0.07:1. This study revealed the increased prevalence of dyslipidemia to be more prevalent in 31-40 year males, suggesting that this group is at an increased risk of developing CAD leading to young infarcts. Combination lifestyle therapies i.e., enhanced physical activity and dietary modification and therapeutic intervention would help us in the treatment and management of dyslipidemia.

In an attempt to differentiate between three important but clinically similar conditions of exudative ascites like tuberculous peritonitis, spontaneous bacterial peritonitis (SBP) and malignant ascites, we evaluated the biochemical parameters of ascitic fluid as a diagnostic aid. The serum ascitic albumin gradient (SAAG), lactate dehydrogenase (LDH), pH, adenosine deaminase(ADA), carcino-embryonic antigen (CEA) and carbohydrate antigen (CA-125) levels were measured in 36 patients with tuberculous peritonitis, 30 patients with SBP and 30 patients with ascites due to malignant disorders. The LDH level was significantly lower in tuberculous peritonitis patients than in malignant and SBP groups. A value of < 110 U/l gave the assay a sensitivity of 94% and a specificity of 93%, positive predictive value of 89% and negative predictive value of 96% for tuberculous peritonitis. The ADA activity was significantly higher in tuberculous peritonitis group than in the other two groups. A cut off value > 33 U/l gave the ADA test a sensitivity of 89%, specificity of 100%, positive predictive value of 100 % and a negative predictive value of 94% for tuberculosis. A pH value of <7.26 with high SAAG (>11 g/l) predicted SBP with reasonable accuracy. Elevated ascitic fluid CEA (>2 ng/ml) and CA - 125(> 35 U/l) was found exclusively in cases of malignant ascites with a single case of tuberculous peritonitis showing CA-125 value > 35 U/l. All these tests are rapid, non-invasive, and easily reproducible and offer good predictive accuracy which is comparable to that of more invasive procedures like peritoneoscopy and biopsy.
