ABSTRACT

Sexual awareness is an understudied phenomenon, despite its known effects on mental health. Little is known about the predictors of sexual awareness, including how early sexual debut and early engagement with online dating and hookup apps might impact the development of sexual mindfulness. Given the conceptual overlap between mindfulness and sexual awareness, this study tested a model that hypothesized that general mindfulness and early sexual and online dating debuts may be associated with mental health outcomes and sexual behavior through pathways involving sexual awareness (assertiveness, consciousness, appeal, and monitoring). A sample of 2,379 heterosexually active young adult women completed an online survey. Path models indicated that both early sexual debut (prior to age 15) and early online dating debut (prior to age 18) had significant, positive direct associations with anxiety and depression scores. Early online dating was also associated with condomless sex with casual male partners. However, both early sexual debut and early online dating debut were indirectly linked to greater sexual risk through greater appeal, and to lower sexual risk through sexual assertiveness. Additionally, greater monitoring contributed to more depression for those with an earlier sexual debut. These findings point to potential risks associated with early online dating. They also highlight benefits of studying sexual awareness as a multi-dimensional construct, especially as it helps to clarify divergent findings in the existing literature on the long-term consequences associated with early sexual debut. While some domains were associated with risk (monitoring and appeal), others were indicative of resilience (assertiveness).

ABSTRACT

Background: Physalis peruviana L. fruit contains nutritional and bioactive compounds of immense importance to public health and represents a potential ingredient for the development of functional foods and beverages. Objective: This study aimed to determine the chemical and nutritional composition as well as the antioxidant capacity of the P. peruviana L. fruit grown in Peru in three areas of the Central Andean region. Material and methods: Proximal and physicochemical analyses and estimation of mineral content, vitamin C, total carotenoids, total polyphenols, and antioxidant capacity (2, 2-diphenyl-1-picrylhydrazyl [DPPH] and 2, 2'-azinobis (3-ethylbenzothiazoline-6-sulfonic acid) [ABTS] assays) were performed using standardized methods. Results: The fruits were collected from three regions of the Peruvian Andes (Ancash, Cajamarca, and Cusco). The results showed that the content of potassium (306.54-327.60 mg/100 g) and iron (12.93-14.47 mg/kg) was prominent. The Physalis fruit had high levels of vitamin C (47.20-52.20 mg/100 g), total polyphenols (68.17-83.40 mg equivalents of gallic acid/100 g), and carotenoids (1.12-1.73 mg ß-carotene/100 g). Higher values for antioxidant capacity were obtained with the ABTS method (896-1003.33 µmol Trolox/100 g) than with the DPPH method (290-309 µmol Trolox/100 g). Conclusions: This study confirms that the P. peruviana fruit has properties that could provide important health benefits and that it could be used for the development of functional foods and food supplement.

Los defectos congénitos son alteraciones morfológicas que se originan durante la vida intrauterina que se presentan hasta en un 5% de los recién nacidos vivos. Tienen múltiples etiologías, siendo esta multifactorial en el 90% de los casos. Se realizó un estudio observacional, prospectivo, descriptivo incluyendo a todos los recién nacidos portadores de defectos congénitos en el período 2016-2020. El objetivo de este trabajo es determinar la incidencia de defectos congénitos en recién nacidos del Centro Hospitalario Pereira Rossell en el período mencionado, así como conocer su distribución por aparatos y sistemas, las características demográficas de esta población, la prevalencia de diagnóstico prenatal y la exposición materna a factores de riesgo durante la organogénesis. Se obtuvo una incidencia de 1,7% (423 recién nacidos afectados en 24.870 nacimientos), de los cuales el 34,98% contaba con diagnóstico prenatal. El sistema cardiovascular fue el que presentó una mayor frecuencia de alteraciones, y el defecto congénito más frecuentemente observado individualmente fue la gastrosquisis, con una incidencia de 15,28 cada 10.000 nacidos vivos. La diabetes gestacional se presentó en el 17,25% de las gestantes. Este trabajo nos permitió conocer la incidencia de defectos congénitos, así como su distribución por aparatos y sistemas. Este tipo de sistemas de vigilancia resultan fundamentales para identificar elementos a mejorar, que permitan disminuir la morbilidad y mortalidad de estos pacientes y también identificar precozmente factores de riesgo que aumenten estas patologías de forma significativa.


Congenital birth defects are morphological disturbances originated during gestation and present in up to 5% of live births. They have multiple etiologies, in 90% of cases of multifactorial origin. A longitudinal, prospective, observational study was carried out and it included all patients with congenital birth defects in 2016-2020. The main objective of this study was to determine the incidence of newborns with congenital birth defects between 2016 and 2020, to determine their distribution by organ, to describe their demographic characteristics, to calculate the prevalence of prenatal diagnosis and to identify maternal risk factors. We obtained an incidence of 1,7% (423 affected newborns in 24870 live births), 34,98% had prenatal diagnoses. The cardiovascular system was the most frequently affected and when classified by individual birth defect, the most frequently observed was gastroschisis with 15.28 cases in 10,000 live births. Gestational diabetes was the maternal risk factor most frequently observed with 17, 25%. This study enabled us to know the incidence of congenital birth defects and their distribution by different organs at our center. These surveillance systems are key to identify areas of potential improvement that might enable us to mitigate morbidity and mortality in this group of patients.


Os defeitos congênitos são alterações morfológicas que se originam durante a vida intrauterina e ocorrem em até 5% dos recém-nascidos vivos. Possuem múltiplas etiologias, sendo multifatoriais em 90% dos casos. Realizou-se um estudo observacional, prospectivo e descritivo incluindo todos os recém-nascidos com defeitos congênitos no período 2016-2020. O objetivo deste trabalho foi determinar a incidência de defeitos congênitos em recém-nascidos do Centro Hospitalar Pereira Rossell no período 2016-2020, bem como conhecer sua distribuição por órgãos e sistemas, as características demográficas dessa população, a prevalência de diagnóstico pré-natal e exposição materna a fatores de risco durante a organogênese. Obteve-se uma incidência de 1,7% (423 recém-nascidos afetados em 24.870 nascimentos), dos quais 34,98% tiveram diagnóstico pré-natal. O sistema cardiovascular foi o que apresentou maior frequência de alterações, e o defeito congênito mais observado individualmente foi a gastrosquise com incidência de 15,28 em cada 10.000 nascidos vivos. O diabetes gestacional ocorreu em 17,25% das gestantes. Este paper permitiu conhecer a incidência de defeitos congênitos, bem como sua distribuição por órgãos e sistemas. Estes tipos de sistemas de vigilância são essenciais para identificar elementos a melhorar, que permitam reduzir a morbilidade e mortalidade desses pacientes e também identificar precocemente fatores de risco que aumentam significativamente essas patologias.

Introducción: En Uruguay el cáncer de mama (CM) ocupa el primer lugar en incidencia y mortalidad por cáncer en mujeres. Objetivo: Evaluar el conocimiento de mujeres uruguayas sobre el tamizaje de cáncer de mama. Material y métodos: Se trata de un estudio observacional descriptivo y transversal. Se aplicó una encuesta dirigida a mujeres, que fue difundida mediante las redes sociales. El consentimiento informado se solicitó al inicio de la encuesta, como requisito excluyente para poder realizar la misma. Se mantuvo el anonimato de las pacientes en el análisis estadístico y se contó con la aprobación del Comité de Ética del Hospital de Clínicas. Resultados: Participaron 1859 mujeres. El 75.1% (1396) de las encuestadas reconoce el CM como el de mayor mortalidad en mujeres. El 52% (967) cree que 3 de cada 10 mujeres tienen riesgo de desarrollar CM, y 18.4% (342) desconoce su prevalencia. El 60.2% (1119) reconoce a la mamografía como prueba que ha logrado disminuir la mortalidad por CM. El 64.2% (1193) cree que se realiza a partir de los 40 años. Sobre la frecuencia, el 60.5% (1125) considera que se realiza de forma anual. Los factores de riesgo para desarrollar CM mayormente considerados fueron tabaquismo (60.9%, 1132), obesidad (57.%, 1060) y sedentarismo (56.8%,1056). Conclusiones: Nuestros resultados evidencian que las encuestadas están informadas de manera adecuada sobre la importancia del CM y de realizar el tamizaje mamográfico para prevenirlo; sin embargo, el 74.3% (1381) cree que el mismo, debe comenzar a hacerse a los 40 años.


Introduction: In Uruguay, breast cancer (BC) has the highest incidence and mortality of all cancer in women. Objectives : To assess the knowledge of Uruguayan women about breast cancer screening for the early detection of BC. Material and Methods : This is a descriptive and observational study. A survey was applied to woman, it was disseminated through social networks. Informed consent was requested at the beginning of the survey as an exclusive requirement to be able to carry it out. In the statistical analysis, the anonymity of the patients was maintained and the approval of the Ethics Committee of the Hospital de Clínicas was obtained. Results : 1859 women participated. 75.1% (1396) of those surveyed recognize BC as the one with the highest mortality in women. 52% (967) believe that 3 out of 10 women are at risk of developing BC, and 18.4% (342) do not know its prevalence. 60.2% (1119) recognize mammography as a test that has managed to reduce mortality from BC. 64.2% (1193) believe that it is done after 40 years of age. Regarding the frequency, 60.5% (1125) consider that it is carried out annually. The most considered risk factors for developing BC were smoking (60.9%, 1132), obesity (57%, 1.060) and sedentary lifestyle (56.8%, 1056). Conclusions : Our results show that the respondents are adequately informed about the importance of CM and the performance of screening mammography.

Objetivos: Avaliar o conhecimento de mulheres uruguaias sobre o rastreamento do câncer de mama para a detecção precoce do CM. Material e Métodos : Trata-se de um estudo descritivo e observacional. Foi aplicado um questionário às mulheres, divulgado através das redes sociais. O consentimento informado foi solicitado no início da pesquisa como requisito exclusivo para poder realizá-la. Na análise estatística, foi mantido o anonimato dos pacientes e obtida a aprovação do Comitê de Ética do Hospital de Clínicas. Resultados: participaram 1859 mulheres. 75.1% (1396) dos pesquisados ​​reconhecem o CB como o de maior mortalidade em mulheres. 52% (967) acreditam que 3 em cada 10 mulheres estão em risco de desenvolver CM e 18.4% (342) não conhecem sua prevalência. 60.2% (1119) reconhecem a mamografia como um exame que tem conseguido reduzir a mortalidade por CM. 64.2% (1.193) acreditam que é feito após os 40 anos. Em relação à frequência, 60.5% (1125) consideram que é realizado anualmente. Os fatores de risco mais considerados para desenvolver CM foram tabagismo (60.9%, 1132), obesidade (57%, 1.060) e sedentarismo (56.8%, 1056). Conclusões: Nossos resultados mostram que as entrevistadas estão adequadamente informadas sobre a importância do MC e a realização da mamografia de rastreamento.

ABSTRACT

BACKGROUND: Antibody response against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) after mRNA or adenoviral vector-based vaccines is weak in kidney transplant (KT) patients. However, few studies have focused on humoral response after inactivated virus-based vaccines in KT. Here, we compare antibody response following vaccination with inactivated virus (CoronaVac®) and BNT162b2 mRNA. METHODS: A national multicentre cross-sectional study was conducted. The study group was composed of patients from all KT centres in Uruguay, vaccinated between 1 and 31 May 2021 (CoronaVac®, n = 245 and BNT162b2, n = 39). The control group was constituted of 82 healthy individuals. Participants had no prior confirmed coronavirus disease 2019 (COVID-19) test. Blood samples were collected between 30 and 40 days after the second dose. Serum-specific immunoglobulin G (IgG) antibodies against the receptor-binding domain (RBD) of SARS-CoV-2 Spike protein were determined using the COVID-19 IgG QUANT ELISA Kit. RESULTS: Only 29% of KT recipients showed seroconversion (36.5% BNT162b2, 27.8% inactivated virus, P = 0.248) in comparison with 100% in healthy control with either vaccine. Antibody levels against RBD were higher with BNT162b mRNA than with inactivated virus [median (interquartile range) 173 (73-554) and 29 (11-70) binding antibody units (BAU)/mL, P < 0.034] in KT and 10 times lower than healthy control [inactivated virus: 308 (209-335) and BNT162b2: 2638 (2608-3808) BAU/mL, P < 0.034]. In multivariate analysis, variables associated with negative humoral response were age, triple immunosuppression, estimated glomerular filtration rate and time post-KT. CONCLUSION: Seroconversion was low in KT patients after vaccination with both platforms. Antibody levels against SARS-CoV-2 were lower with inactivated virus than BNT162b mRNA. These findings support the need for strategies to improve immunogenicity in KT recipients after two doses of either vaccine.

OBJETIVOS: Determinar la prevalencia puntual de lesiones de piel asociadas a la dependencia física, realizar caracterización sociodemográfica de las personas afectadas, caracterizar las lesiones con base en disposiciones del Grupo Nacional para el Estudio y Asesoramiento en Úlceras por Presión y Heridas Crónicas (GNEAUPP), el National Pressure Ulcer Advisory Panel (NPUAP) y el European Pressure Ulcer Advisory Panel (EPUAP) y establecer la habilidad del personal de salud para el reconocimiento y clasificación de las lesiones de piel asociadas a dependencia. METODOLOGÍA: Estudio multicéntrico descriptivo de prevalencia puntual en instituciones de servicios de salud de tercer nivel y centros gerontológicos de Tunja, Colombia. RESULTADOS: Participaron cuatro instituciones. La muestra fue de 409 personas, 24 de las cuales presentaron lesiones de piel asociadas a la dependencia para una prevalencia del 5,86%. De los afectados, el 41,6% eran hombres y el 58,4% mujeres; el 79,1% con 50 años o más; en cuanto a la procedencia, el 53,8% pertenecía al área urbana y el 46,2% a la rural; un 56,6% de los pacientes pertenecían al régimen subsidiado, el 30% al contributivo y 13,4% al especial. La etiología de las lesiones estuvo discriminada de la siguiente manera: por presión, 67,3%; LESCAH, 13,4%; MARSI, 11,5% y desgarros cutáneos, 7,7%. CONCLUSIONES: la prevalencia encontrada se relaciona con resultados de estudios colombianos previos, la afectación es mayor en mujeres y la mayor frecuencia de ocurrencia de lesiones corresponde a las ocasionadas por la presión y cizalla; las lesiones de piel asociadas a la dependencia se presentaron con mayor frecuencia en personas de procedencia urbana. El 75% de las personas que diligenciaron la encuesta reportaron no haber recibido capacitación institucional para identificación y clasificación de lesiones de piel asociadas a la dependencia


OBJECTIVES: To determine the exact prevalence of skin lesions associated with dependence physical, conduct socio-demographic characterization of those affected, characterizing lesions based on provisions of the National Group for the Study and advice on Pressure Ulcers and Chronic Wounds (GNEAUPP), the National Pressure Ulcer Advisory Panel (NPUAP) and the European Pressure Ulcer Advisory Panel (EPUAP), establish the ability of health personnel to recognize and classify lesions associated with dependence. METHODOLOGY: descriptive multicenter point prevalence in third level health care institutions and gerontology centers from Tunja, Colombia. RESULTS: four institutions participated, the sample was 409 people, 24 had lesions associated with dependence for a prevalence of 5.86%; of those affected 41.6% were men and 58.4% women, 79.1% with ≥50 years regarding the origin 53.8% belonged to urban zones and 46.2% rural zones; 56.6% of patients belonged to the subsidized health scheme, 30% to contributory health scheme 13.4% to special. The etiology of lesions was discriminated as follows: pressure 67.3%; LESCAH 13.4%; MARSI 11.5% and 7.7% skin tears. CONCLUSIONS: The prevalence found is related to results of Colombian previous studies, the effect is greater in women and increased frequency of occurrence of injuries corresponds to those caused by pressure and mechanical friction, LPAD occurred more often in people of origin urban. 75% of people who completed the survey reported not having received institutional training for identification and classification (LPAD)

ABSTRACT

The management of infections caused by carbapenem-resistant organisms has been a challenge. We report a rare emergence of resistance to the novel beta-lactam/ beta-lactamase combination ceftolozane/tazobactam by Klebsiella pneumoniae, causing urinary tract infection. The K. pneumoniae, in this case, was reported to be sensitive to the other novel beta-lactam/ beta-lactamase combination of ceftazidime/avibactam. The timely administration of ceftazidime/avibactam resulted in prompt clinical resolution of the urinary tract infection caused by an extensively drug-resistant K. pneumoniae.

BACKGROUND: Incidence and mortality rates of childhood cancer represent a global public health issue, however, the worldwide prevalence of head and neck cancer in pediatric patients (HNCPP) is still unknown. Therefore, this study aimed to describe the frequency and distribution of HNCPP worldwide. METHODS: A specific search strategy was performed using MEDLINE, Scopus, and EMBASE to include studies based on hospital records, national cancer registries, and pathology files. Studies quality was assessed using the risk of bias checklist of the Joanna Briggs Institute Critical Appraisal. RESULTS: Nineteen publications (15,970 cases) were included. Global frequency ranged from 0.25 % to 15 %. Male patients older than 10 years of age were most affected by lymphomas, followed by carcinomas and sarcomas. Non-Hodgkin lymphoma, Hodgkin lymphoma, rhabdomyosarcoma, thyroid carcinoma, and nasopharyngeal carcinoma were the main histopathological subtypes. Neck/lymph nodes were anatomical hotspots. CONCLUSIONS: This HNCPP global overview may guide secondary prevention strategies and future etiological studies.

ABSTRACT

INTRODUCTION: Impulse oscillometry (IOS) is used to measure airway impedance. It is an effective tool for diagnosing and treating respiratory diseases, and it has the advantage that it does not require forced respiratory maneuvers. IOS reference values are required for each population group. OBJECTIVE: This study aimed to determine the IOS reference values and bronchodilator response in healthy preschool children living in Bogotá, Colombia. METHODS: We performed a cross-sectional study in preschool children who had no history of respiratory disease; 96 children fit the parameters for testing to determine normal values according to the American Thoracic Society and European Respiratory Society criteria. RESULTS: Values for respiratory resistance (Rrs) and reactance (Xrs) at 5, 10, and 20 Hz, respiratory impedance (Zrs, and resonance frequency (Fres) were established. Height was the most influential independent variable for IOS values; an increase in height led to a reduction in Rrs5 and Rrs20 and an increase in Xrs5. After the administration of 400 mcg of salbutamol the values for Rrs5(-17.48%), Rrs20(-8.63%), Fres (-10.68%), and area of reactance (-35.44%) were reduced, meanwhile Xrs5 (15.35%) was increased. CONCLUSIONS: Normal IOS values before and after the administration of 400 mcg of salbutamol were determined for a population of children aged 3-5 years at 2,640 m. Reference IOS equations for these children are presented. A relative change of up to -28% and 36% after the use of salbutamol for respiratory resistance and reactance, respectively, should be considered as an upper limit of the normal range, and possible appropriate cut-off values for defining significant response for evaluating therapeutic interventions.

ABSTRACT

Objetivo: Determinar el efecto del extracto etanólico de Chuquiraga spinosa (huamanpinta) sobre el síndrome metabólico (SM) e hipercolesterolemia inducidas en ratas con fructosa y colesterol. Materiales y Métodos: Se indujo el SM con la administración vía oral de fructosa al 60% P/V durante 90 días y la inducción de hipercolesterolemia fue mediante la administración oral de solución de colesterol a una dosis diaria de 120 mg/kg suspendida en goma de tragacanto al 2%. Se formaron 06 grupos de 15 animales cada grupo, dividido en grupo blanco negativo (solución salina fisiológica) y positivo (fructosa más colesterol), extracto etanólico a dosis de 50, 250 y 500 mg/kg y un grupo atorvastatina más enalapril a 20 mg/kg (se administró fructosa más colesterol a todos los grupos). Los tratamientos respectivos se iniciaron en el día 31 posinducción y se continuó con la inducción coadministrando los tratamientos hasta los 90 días. Se determinó el perfil lipídico, glicemia y niveles de presión arterial. En la determinación de la toxicidad crónica se realizó la evaluación a nivel hematológico y bioquímico de todos los grupos que fueron inducidos y recibieron tratamiento. Resultados: Se obtuvo una disminución de los niveles de colesterol total, triglicéridos, glucosa, presión arterial y del peso corporal, así como una elevación de los niveles de HDL-colesterol, que fueron estadísticamente significativos (p<0,05). En la determinación de la toxicidad crónica se observo que no hubo toxicidad y no se evidenció alteraciones anatomopatológicas. Conclusiones: El extracto etanólico de Chuquiraga spinosa presenta efecto positivo sobre los niveles de perfil lipídico, glucosa y presión arterial inducido en ratas, sin efectos tóxicos.


Objective: To determine the effect of ethanolic extract of Chuquiraga spinosa (huamanpinta) on the metabolic syndrome and hypercholesterolemia in rats induced with fructose and cholesterol. Material and methods: SM was induced in rats with oral administration of 60% fructose for 90 days and the induction of hypercholesterolemia was by oral administration of cholesterol solution at a daily dose of 120 mg/kg suspended in gum tragacanth 2 %. Six groups of 15 animals were formed in each group, divided into negative white group (physiological saline solution) and positive (fructose plus cholesterol), ethanolic extract at doses of 50, 250 and 500 mg/kg and one atorvastatin plus enalapril group at 20 mg/Kg. The respective treatments were initiated on day 31 postinduction and continued induction along with treatments up to 90 days. The lipid profile, glycemia and blood pressure levels were determined. In the determination of chronic toxicity, hematological and biochemical evaluation of all the groups that were induced and treated were performed. Results: There was a decrease in total cholesterol, triglycerides, glucose, blood pressure and body weight, as well as a rise in HDL-cholesterol levels, which were statistically significant (p <0.05). In the determination of chronic toxicity, it was observed that there was no toxicity and no anatomopathological alterations were evidenced. Conclusions: The ethanolic extract of Chuquiraga spinosa has a positive effect on the metabolic syndrome and induced hypercholesterolemia in rats, without toxic effects.

ABSTRACT

Fatigue is a common symptom in patients with Lyme disease. The purpose of this study was to characterize fatigue in untreated adult patients presenting with erythema migrans. Selected variables were assessed to determine if any correlated with the presence or severity of fatigue. Fatigue was assessed on the day of the evaluation by a Visual Analogue Scale (VAS), over the past 14days by the 11-item Fatigue Severity Scale (FSS-11) and over the past 28days based on a question from the 36-item Short Form General Health Survey version 2. Fifty-one patients with erythema migrans whose mean age was 49.8years, and 33 (64.7%) of whom were male, were evaluated in this study. The 3 measures of fatigue were positively correlated with one another (P≤0.01). Twenty-six (51%) had fatigue based on a VAS score above 0. Ten (19.6%) had severe fatigue based on an FSS-11 score of ≥4. The strongest correlate for higher fatigue scores was having a greater total number of symptoms. Based on the FSS-11 assessment tool, approximately 20% of early Lyme patients have severe fatigue. Having a high total number of symptoms was associated with both the presence and severity of fatigue. Because prior studies have demonstrated the presence of elevated levels of proinflammatory cytokines and other molecules in the serum of highly symptomatic patients with erythema migrans, the symptom of fatigue in early Lyme disease may be a component of what has been referred to as the acute sickness response.

ABSTRACT

OBJECTIVE: Characterising the oral health of patients undergoing different types of dialysis or kidney transplant. MATERIALS AND METHODS: This was a descriptive study which involved multivariate analysis of information taken from an initial database regarding 336 patients; 49 % were receiving haemodialysis, 34 % peritoneal dialysis, 7 % pre-dialysis and 10 % kidney transplant. Illustrative variables were age, gender, marital status, occupation, education, oral hygiene and gingival indexes, flossing, decayed, missing, and filled teeth (DMFT) index, renal disease aetiology and type of dialysis being received. A hierarchical clustering method was used. RESULTS: Four groups of patients were identified. Class 1 (37.8 %) consisted of unmarried men having had secondary education and having good oral hygiene, very high DMFT, with haemodialysis but no stomatological pathology. Class 2 (20.24 %) included haemodialysis patients who had received elementary education, were unemployed, had inadequate oral hygiene, severe gingivitis, very high DMF rate and high Candida frequency. Class 3 (31.2 %) included women undergoing peritoneal dialysis who were over 70 years old, had received elementary education, were housewives, edentulous and who had loss of vertical dimension. Class 4 (10.7 %) included men who had received renal transplant, secondary education and were employees; one third of them were edentulous and had soft tissue alterations. CONCLUSION: Multivariate analysis indicated a possible relationship between the type of dialysis received and patients' socioeconomic characteristics regarding oral health status.
