ABSTRACT

OBJECTIVE: To evaluate whether a structured online supervised group physical and mental health rehabilitation programme can improve health related quality of life compared with usual care in adults with post-covid-19 condition (long covid). DESIGN: Pragmatic, multicentre, parallel group, superiority randomised controlled trial. SETTING: England and Wales, with home based interventions delivered remotely online from a single trial hub. PARTICIPANTS: 585 adults (26-86 years) discharged from NHS hospitals at least three months previously after covid-19 and with ongoing physical and/or mental health sequelae (post-covid-19 condition), randomised (1:1.03) to receive the Rehabilitation Exercise and psycholoGical support After covid-19 InfectioN (REGAIN) intervention (n=298) or usual care (n=287). INTERVENTIONS: Best practice usual care was a single online session of advice and support with a trained practitioner. The REGAIN intervention was delivered online over eight weeks and consisted of weekly home based, live, supervised, group exercise and psychological support sessions. MAIN OUTCOME MEASURES: The primary outcome was health related quality of life using the patient reported outcomes measurement information system (PROMIS) preference (PROPr) score at three months. Secondary outcomes, measured at three, six, and 12 months, included PROMIS subscores (depression, fatigue, sleep disturbance, pain interference, physical function, social roles/activities, and cognitive function), severity of post-traumatic stress disorder, general health, and adverse events. RESULTS: Between January 2021 and July 2022, 39 697 people were invited to take part in the study and 725 were contacted and eligible. 585 participants were randomised. Mean age was 56 (standard deviation (SD) 12) years, 52% were female participants, mean health related quality of life PROMIS-PROPr score was 0.20 (SD 0.17), and mean time from hospital discharge was 323 (SD 144) days. Compared with usual care, the REGAIN intervention led to improvements in health related quality of life (adjusted mean difference in PROPr score 0.03 (95% confidence interval 0.01 to 0.05), P=0.02) at three months, driven predominantly by greater improvements in the PROMIS subscores for depression (1.39 (0.06 to 2.71), P=0.04), fatigue (2.50 (1.19 to 3.81), P<0.001), and pain interference (1.80 (0.50 to 3.11), P=0.01). Effects were sustained at 12 months (0.03 (0.01 to 0.06), P=0.02). Of 21 serious adverse events, only one was possibly related to the REGAIN intervention. In the intervention group, 141 (47%) participants fully adhered to the programme, 117 (39%) partially adhered, and 40 (13%) did not receive the intervention. CONCLUSIONS: In adults with post-covid-19 condition, an online, home based, supervised, group physical and mental health rehabilitation programme was clinically effective at improving health related quality of life at three and 12 months compared with usual care. ABSTRACT

Background: Increased serum interleukin (IL)-33 predicts poor outcomes in patients hospitalised with coronavirus disease 2019 (COVID-19). We examined the efficacy and safety of tozorakimab, a monoclonal antibody that neutralises IL-33, in improving outcomes in ACCORD-2 (EudraCT: 2020-001736-95). Methods: ACCORD-2 was an open-label, phase 2a study in adults hospitalised with COVID-19. Patients were randomised 1:1 to tozorakimab 300 mg plus standard of care (SoC) or SoC alone. The primary end-point was time to clinical response (sustained clinical improvement of ≥2 points on the World Health Organization ordinal scale, discharge from hospital or fit for discharge) by day 29. Other end-points included death or respiratory failure, mortality and intensive care unit admission by day 29, and safety. Serum IL-33/soluble stimulated-2 (sST2) complex levels were measured by high-sensitivity immunoassay. Results: Efficacy analyses included 97 patients (tozorakimab+SoC, n=53; SoC, n=44). Median time to clinical response did not differ between the tozorakimab and SoC arms (8.0 and 9.5 days, respectively; HR 0.96, 80% CI 0.70-1.31; one-sided p=0.33). Tozorakimab was well tolerated and the OR for risk of death or respiratory failure with treatment versus SoC was 0.55 (80% CI 0.27-1.12; p=0.26), while the OR was 0.31 (80% CI 0.09-1.06) in patents with high baseline serum IL-33/sST2 complex levels. Conclusions: Overall, ACCORD-2 results suggest that tozorakimab could be a novel therapy for patients hospitalised with COVID-19, warranting further investigation in confirmatory phase 3 studies.

ABSTRACT

The aim of this study was to determine the association between oral health-related quality of life (OHRQoL) and social marginalization in people aged 60 years and older enrolled in social security in Mexico. A cross-sectional and analytical study was carried out in older adults. To assess the OHRQoL, the OHIP-14 instrument was applied, and the degree of social marginalization and sociodemographic characteristics were analyzed. Measures of central tendency and dispersion, simple frequencies and proportions were estimated. Student's t-test was used for comparison of means, and prevalence ratio (PR) and logistic regression were used to assess associations, all with a significance value of 0.05 and 95% confidence intervals. Perceived OHRQoL in the population measured through the OHIP-14 reached an average value of 9.84 ± 8.91, with the highest value in the dimension of physical pain (2.06 ± 1.91). Perceived treatment need was higher among people with social marginality (p = 0.011). The multivariate analysis shows that marginalized people have a lower OHRQoL. Socially marginalized older adults showed a low a better perception of OHRQoL, independent of demographic and clinical factors.

ABSTRACT

Over one quarter of the population in industrialised countries suffers from some type of allergy and inhaled aeroallergens from pollen are the primary cause of allergic ailments. The networks for monitoring biological air quality measure the airborne pollen concentrations that characterize periods of exposure to major airborne aeroallergens but there are certain discrepancies in relation to the allergen-pollen dynamic. In this paper we analyse the airborne allergens Ole e 1, Phl p 1, Phl p 5 and Pla a 1, and interpreted the adjustments and mismatches in their concentrations in relation to airborne pollen. The influence of main environmental patterns was considered. The study was conducted in two urban areas of the centre and southwest of the Iberian Peninsula (Toledo in Spain and Évora in Portugal). Monitoring for pollen followed the standard protocol using Hirst volumetric spore traps and allergenic particles were quantified by ELISA assay. The results indicate that the discrepancies in this relationship were affected by the weather conditions up to 6 days prior. Precipitation and humidity above normal values caused a higher concentration of the allergen Pla a 1. This effect occurred in reverse in the case of humidity for the allergens Ole e 1 and Phl p 1. Humidity and precipitation generated the same pattern in the allergen-pollen relationship in both Phl p 1 and Phl p 5. Our findings show consistent results that allow to interpret the rate of discrepancy between allergen and pollen, and it can be used to improve allergy risk prediction models generated from atmospheric pollen.

ABSTRACT

Objetivo: Analisar os paradigmas enfrentados pela população negra feminina no sistema de saúde na Atenção Primária, identificando os principais fatores que impedem uma melhor acesso e assistência. Método: Trata-se de uma revisão integrativa de literatura, com busca nas bases de dados da Medline, BDENF, LILACS, utilizando como descritores: "Saúde da Mulher", "Atenção Primária a Saúde", "Saúde Pública" e "Política de Saúde" combinados entre operadores boleanos AND e OR, com recorte temporal dos últimos 5 anos. Resultados: Dos 4.541 artigos encontrados, 8 foram selecionados para compor o estudo. Identificaram-se, como desafios à assistência à saúde integral da mulher negra no âmbito da Atenção Básica fatores relacionados ao despreparo dos profissionais mediante as vulnerabilidades e a falta de educação continuada, que se tornam questões limítrofes e desafiadoras enfrentadas pelas mulheres diariamente. A existência de preconceito racial, ainda que fortemente combatida, ainda existe nas instâncias da sociedade e necessita de intervenção nas instituições de saúde visando atender o indivíduo considerado sua integralidade. Fatores socioeconômicos relacionados à moradia da mulher também foram destaque apontados no estudo, revelando um processo de lentidão para realizar exames e acompanhamento dos profissionais. Conclusões: Uma vez que a Atenção Básica é porta de entrada do sistema de saúde, permanece a necessidade de ampliar o arcabouço científico de pesquisas para compreender a temática específica e adequadamente, pois é indubitável que a mulher negra enfrenta várias lacunas para uma assistência sem iniquidades.


Objective: To analyze the paradigms faced by the female black population in the Primary Care health system, identifying the main factors that prevent better access and assistance. Method: This is an integrative literature review, with a search in the databases of Medline, BDENF, LILACS, using as descriptors: "Women's Health", "Primary Health Care", "Public Health" and "Policy of Health" combined between AND and OR Boolean operators, with a time frame of the last 5 years. Results: Of the 4,541 articles found, 8 were selected to compose the study. Factors related to the unpreparedness of professionals through vulnerabilities and lack of continuing education were identified as challenges to comprehensive health care for black women in the scope of Primary Care, which become borderline and challenging issues faced by women on a daily basis. The existence of racial prejudice, although strongly opposed, still exists in society and requires intervention in health institutions in order to care for the individual as a whole. Socioeconomic factors related to the woman's housing were also highlighted in the study, revealing a slow process to carry out tests and follow-up by professionals. Conclusions: Since Primary Care is the gateway to the health system, there is still a need to expand the scientific framework of research to understand the topic specifically and properly, as it is undoubted that black women face several gaps for care without inequities.


Objetivo: Analizar los paradigmas a los que se enfrenta la población negra femenina en el sistema de atención primaria de la salud, identificando los principales factores que impiden un mejor acceso y atención. Método: Se trata de una revisión integral de la literatura, en la que se analizan las bases de datos de Medline, BDENF, LILACS, utilizando como descriptores: "Salud de la mujer", "Atención primaria de la salud", "Salud pública" y "Política de salud" combinados entre operadores de la UE y OR, con un recorte de tiempo de los últimos 5 años. Resultados: De los 4.541 artículos encontrados, 8 fueron seleccionados para conformar el estudio. Se han determinado los factores relacionados con la falta de preparación de los profesionales a través de la vulnerabilidad y la falta de educación continua como problemas para la plena atención de la salud de las mujeres negras bajo la atención básica, que se convierten en problemas contiguos y difíciles a los que se enfrentan diariamente las mujeres. La existencia de prejuicios raciales, a pesar de ser fuertemente combatidos, sigue existiendo en los niveles de la sociedad y necesita una intervención en las instituciones de salud, con el objetivo de atender a la persona considerada su integralidad. En el estudio también se destacaron los factores socioeconómicos relacionados con la vivienda de las mujeres, lo que reveló un proceso de lentitud en la realización de los exámenes y los profesionales que los acompañaban. Conclusión: Dado que la atención básica es el punto de partida del sistema de salud, sigue siendo necesario ampliar el marco de investigación científica para comprender el tema específico y apropiado, ya que es evidente que las mujeres negras se enfrentan a varias lagunas en materia de asistencia sin injusticias.

Objetivo: Identificar os desafios da equipe de Enfermagem frente ao paciente com câncer, bem como dificuldades enfrentadas no que se diz respeito a terminalidade e cuidados paliativos utilizados Metodologia: Foi realizada a revisão integrativa com base na literatura recente. A pesquisa foi realizada entre abril e maio de 2023, tendo como base de dados a MEDLINE, BDENF e LILACS, onde foram selecionados artigos completos nos idiomas português, inglês e espanhol publicados nos últimos cinco anos. Resultados: Foram encontrados 122.880 artigos validados nas bases de dados, onde após utilizar critérios de inclusão e exclusão, e leitura de título e resumo foram selecionados 12 artigos para o estudo, onde foram encontradas evidências que retratam as dificuldades vivenciadas pela equipe de enfermagem no que se diz respeito a vivência na área da oncologia terminal e a importância dos cuidados paliativos. Conclusões: Pode-se concluir a importância da presença da família no âmbito hospitalar e/ou domiciliar, bem como a necessidade de um aprofundamento na terapêutica paliativa ainda na graduação, tendo em vista os desafios vivenciados pela equipe de enfermagem frente a finitude da vida dos pacientes oncológicos terminais.


Objective: To identify the challenges of the nursing team facing the patient with cancer in its terminal phase, as well as the difficulties faced with regard to terminality and palliative care used Methodology: Na integrative review was carried out based on recent literature. The research was carried out bet ween april and may 2023, using MEDLINE, BDENF and LILACS as a database, where full articles were selected in Portuguese, English and Spanish, published in the last five years. Results: 122,880 validated articles were found in the databases, where after using inclusion and exclusion criteria, and reading the title and abstract, 12 were selected for the study where evidence was found that portrays the difficulties experienced by the nursing team regarding the experience in the area of terminal oncology and the importance of palliative care. Conclusions: It can be concluded the importance of the presence of the family in the hospital and/or home environment, as well as the need for a deepening of palliative therapy during graduation, in view of the challenges experienced by the nursing team in the face of the terminality of life of terminal cancer patients.

La investigación se realizó entre abril y junio de 2023, utilizando como base de datos MEDLINE, BDENF y LILACS, donde se seleccionaron artículos completos en portugués, inglés y español, publicados en los últimos cinco anos. Resultados: se encontraron 122.880 artículos validados en las bases de datos, donde luego de aplicar criterios de inclusión y exclusión, lectura del título y resumen, se seleccionaron 12 para el estudio. Donde se encontró evidencia que retrata las dificultades vividas por el equipo de enfermería en cuanto a la experiencia en el área de oncología terminal y la importancia de los cuidados paliativos. Conclusiones: Se puede concluir la importancia de la presencia de la familia en el hospital y/o ambiente domiciliario, así como la necesidad de una profundización de la terapia paliativa durante la graduación, frente a los desafíos vividos por el equipo de enfermería frente a la terminalidad de la vida de los pacientes oncológicos terminales

ABSTRACT

Objetivo: Examinar e mapear as evidências científicas sobre o compartilhamento de desinformações relacionadas a vacinação contra a COVID-19 entre usuários das redes sociais. Metodologia: Scoping Review, baseado nos procedimentos recomendados pelo Instituto Joanna Briggs. Estabeleceu-se a pergunta norteadora: "Qual o comportamento dos usuários de redes sociais quanto ao compartilhamento de informações e desinformações em saúde relacionados à vacinação contra COVID-19?". A coleta dos dados foi realizada em abril de 2023 nas bases de dados PubMed, Biblioteca Virtual em Saúde, Scopus, Web of Science e EMBASE. Foram excluídos textos publicados antes de 2020, protocolos de revisão sistemática ou meta análise e estudos fora do recorte temático. Resultados: Os 9 estudos tiveram delineamento de pesquisas experimentais do tipo análise netnográfica. Quanto a plataforma de disseminação, é possível observar que o Facebook é a mídia social que mais veicula fake news relacionadas à vacinação de COVID-19 seguido do Twitter (33,3%) e Instagram (22,2%). Evidencia-se a forte propensão de engajamento a publicações de cunho antivacina e disseminação de eventos adversos e/ou efeitos colaterais dos imunizantes com ênfase na Pfizer-BioNTech. O perfil dos disseminadores está associado a figuras públicas e jovens de 18 a 44 anos, que também possuem maior propensão de crença na fidedignidade das informações encontradas. Os estudos associam a queda nas taxas de imunização pelo medo dos efeitos colaterais, incluindo hospitalização, miocardites, coágulos sanguíneos e óbito, bem como a desconfiança governamental. Conclusão: o compartilhamento de fake news é um forte fator de hesitação vacinal gerando medo, insegurança e preocupação.


Objective: To examine and map scientific evidence on the sharing of misinformation related to COVID-19 vaccination among social media users. Methodology: Scoping Review, based on procedures recommended by the Joanna Briggs Institute. The guiding question was established: "What is the behavior of users of social networks regarding the sharing of health information and misinformation related to vaccination against COVID-19?". Data collection was carried out in April 2023 in the PubMed, Virtual Health Library, Scopus, Web of Science and EMBASE databases. Texts published before 2020, systematic review or meta-analysis protocols and studies outside the thematic scope were excluded. Results: The 9 studies had the design of experimental researches of the netnographic analysis type. As for the dissemination platform, it is possible to observe that Facebook is the social media that most conveys fake news related to the COVID-19 vaccination followed by Twitter (33.3%) and Instagram (22.2%). There is evidence of a strong tendency to engage with anti-vaccine publications and the dissemination of adverse events and/or side effects of immunizations, with an emphasis on Pfizer-BioNTech. The profile of disseminators is associated with public figures and young people aged 18 to 44, who are also more likely to believe in the reliability of the information found. Studies associate the drop in immunization rates with fear of side effects, including hospitalization, myocarditis, blood clots and death, as well as government distrust. Conclusion: The sharing fake news is a strong factor in vaccine hesitancy, generating fear, insecurity and concern.

Metodología: Scoping Review, basado en los procedimientos recomendados por el Instituto Joanna Briggs. Se estableció la pregunta guía "¿Cuál es el comportamiento de los usuarios de las redes sociales con respecto al intercambio de información sanitaria y desinformación relacionada con la vacunación contra la COVID-19?". La recogida de datos se realizó en abril de 2023 en las bases de datos PubMed, Virtual Health Library, Scopus, Web of Science y EMBASE. Se excluyeron textos publicados antes de 2020, protocolos de revisión sistemática o metaanálisis y estudios fuera del ámbito temático. Resultados: Los 9 estudios tenían el diseño de investigaciones experimentales del tipo análisis netnográfico. En cuanto a la plataforma de difusión, se puede observar que Facebook es el medio social que más transmite noticias falsas relacionadas con la vacunación COVID-19 seguido de Twitter (33,3%) e Instagram (22,2%). Se evidencia una fuerte tendencia a las publicaciones antivacunas y a la difusión de eventos adversos y/o efectos secundarios de las vacunas, destacando Pfizer-BioNTech. El perfil de los divulgadores se asocia a personajes públicos y jóvenes de 18 a 44 años, que además son más propensos a creer en la fiabilidad de la información encontrada. Los estudios asocian la caída de las tasas de inmunización con el miedo a los efectos secundarios, incluyendo hospitalización, miocarditis, coágulos de sangre y muerte, así como la desconfianza del gobierno. Conclusiones: El intercambio de noticias falsas es un factor importante en la indecisión sobre las vacunas, ya que genera miedo, inseguridad y preocupación.

ABSTRACT

Objetivo: Examinar e mapear as evidências científicas sobre o impacto da sindemia de COVID-19 e tuberculose pulmonar durante período pandêmico no Brasil. Metodologia: Scoping Review, baseado nos procedimentos recomendados pelo Instituto Joanna Briggs. Estabeleceu-se a pergunta norteadora: "Qual o impacto da pandemia de COVID-19 e fatores associados ao aumento de casos de Tuberculose Pulmonar no Brasil durante período pandêmico?". Foram realizadas buscas em três bases de dados nacionais e internacionais, sobre trabalhos publicados até dezembro de 2022. Dos 819 estudos encontrados, 20 foram selecionados para leitura na íntegra, resultando em uma amostra final de 11 estudos analisados. Resultados: As 11 publicações analisadas foram publicadas de 2020 a 2022 durante período pandêmico, de âmbito nacional e internacional com delineamento de estudos experimentais, do tipo ensaio clínico com randomização. A maioria dos estudos incluídos, 7 (70%) foram publicados no ano de 2022. Quanto ao tipo de estudo, 40% eram estudos transversais, 03 estudos (30%) eram do tipo estudo retrospectivo, observacional. Apenas 3 eram estudos ecológico de série temporal no qual estes evidenciaram que o Brasil e suas macrorregiões apresentaram tendência temporal crescente para a notificação de TB no período pré-pandêmico. Segundo Berra et al. (2022) apenas a Região Nordeste apresentou tendência temporal decrescente de casos curados. Para o abandono do tratamento, todas as regiões, exceto o Nordeste, apresentaram um aumento nos números de casos, e em relação ao óbito, o Brasil e a Região Nordeste apresentaram tendência temporal crescente.


Objective: To examine and map the scientific evidence on the impact of the COVID-19 syndemic and pulmonary tuberculosis during the pandemic period in Brazil. Methodology: Scoping Review, based on procedures recommended by the Joanna Briggs Institute. The guiding question was established: "What is the impact of the COVID-19 pandemic and factors associated with the increase in cases of Pulmonary Tuberculosis in Brazil during the pandemic period?". Searches were carried out in three national and international databases, on works published until December 2022. Of the 819 studies found, 20 were selected for full reading, resulting in a final sample of 11 analyzed studies. Results: The 11 analyzed publications were published from 2020 to 2022 during the pandemic period, nationally and internationally with experimental study design, of the clinical trial type with randomization. Most of the studies included, 7 (70%) were published in the year 2022. As for the type of study, 40% were cross-sectional studies, 03 studies (30%) were retrospective, observational studies. Only 3 were ecological time series studies in which they showed that Brazil and its macro-regions showed an increasing temporal trend for TB notification in the pre-pandemic period. According to Berra et al. (2022) only the Northeast Region showed a decreasing temporal trend of cured cases. For treatment abandonment, all regions, except the Northeast, showed an increase in the number of cases, and in relation to death, Brazil and the Northeast Region showed an increasing temporal trend. Objetivo: Examinar y mapear la evidencia científica sobre el impacto de la epidemia de COVID-19 y de la tuberculosis pulmonar durante el período pandémico en Brasil. Metodología: Scoping Review, basado en los procedimientos recomendados por el Instituto Joanna Briggs. Se estableció la pregunta guía "¿Cuál es el impacto de la pandemia de COVID-19 y los factores asociados al aumento de casos de Tuberculosis Pulmonar en Brasil durante el período pandémico?". Se realizaron búsquedas en tres bases de datos nacionales e internacionales, sobre trabajos publicados hasta diciembre de 2022. De los 819 estudios encontrados, 20 fueron seleccionados para lectura completa, resultando en una muestra final de 11 estudios analizados. Resultados: Las 11 publicaciones analizadas fueron publicadas entre 2020 y 2022 durante el periodo pandémico, a nivel nacional e internacional con diseño de estudio experimental, del tipo ensayo clínico con aleatorización. La mayoría de los estudios incluidos, 7 (70%) fueron publicados en el año 2022. En cuanto al tipo de estudio, 40% fueron estudios transversales, 03 estudios (30%) fueron retrospectivos, observacionales. Solamente 3 fueron estudios de series temporales ecológicas en los cuales mostraron que Brasil y sus macrorregiones presentaron una tendencia temporal creciente para la notificación de TB en el período prepandémico. Según Berra et al. (2022) sólo la Región Nordeste mostró una tendencia temporal decreciente de casos curados. Para el abandono del tratamiento, todas las regiones, excepto la Nordeste, mostraron un aumento en el número de casos, y en relación a la muerte, Brasil y la Región Nordeste mostraron una tendencia temporal creciente. ABSTRACT

Objetivo: examinar as evidências científicas disponíveis sobre o preenchimento das cadernetas da criança e do idoso, bem como realizar uma comparação dos principais desafios encontrados para o não preenchimento desses instrumentos de assistência. Métodos: revisão integrativa da literatura através de busca na base de dados SCIELO, GOOGLE SCHOLAR e PUBMED utilizando os descritores "Assistência de Enfermagem", "Caderneta da criança" e "Caderneta do idoso". Resultados: Quanto ao preenchimento das cadernetas, os resultados mostraram que a Caderneta da Criança apesar de ter um preenchimento insuficiente possui maior adesão de preenchimento pela equipe multiprofissional quando comparada ao preenchimento da caderneta de Saúde da Pessoa. Tais dificuldades, estão associadas às limitações no acesso aos serviços, carência de infraestrutura e baixa qualificação de profissionais. Por conseguinte, o enfermeiro deve estar atento ao adequado preenchimento, sendo que este possui a atribuição de atualizar e estimular a equipe a utilização. Considerações finais: Há poucas evidências sobre estudos acerca dos registros em Caderneta de Saúde da Pessoa Idosa, em contrapartida, há mais evidências sobre registros em Caderneta da Criança realizado pela equipe multiprofissional, como Agente Comunitário de Saúde, Nutricionista, Terapeuta Ocupacional, Médico, Enfermeiro, entre outras.


Objective: to examine the available scientific evidence on the completion of child and elderly child and elderly booklets, as well as to compare the main challenges encountered in not completing these care instruments. Methods: integrative literature review through search in the SCIELO, GOOGLE SCHOLAR and PUBMED databases using the descriptors "Nursing Notes", "Child's Notebook" and "Elderly Notebook". Results: regarding the completion of the booklets, the results showed that the Child's Booklet, despite having insufficient completion, has greater adherence to completion by the multidisciplinary team when compared to the completion of the Person's Health booklet. Such difficulties are associated with limitations in access to services, lack of infrastructure and low qualification of professionals. Final considerations: there is little evidence about studies about the records in the Health Booklet of the Elderly, on the other hand, there is more evidence about records in the Children's Booklet carried out by the multidisciplinary team, such as Community Health Agent, Nutritionist, Occupational Therapist, Physician, Nurse, among others.


Objetivo: examinar la evidencia científica disponible sobre la cumplimentación de folletos de niños y ancianos, así como comparar los principales desafíos encontrados al no completar estos instrumentos de atención. Métodos: revisión integradora de la literatura a través de la búsqueda en las bases de datos SCIELO, GOOGLE SCHOLAR y PUBMED utilizando los descriptores "Nursing Notes", "Child's Notebook" y "Elderly Notebook". Resultados: con respecto a la finalización de los folletos, los resultados mostraron que el Cuadernillo del Niño, a pesar de tener una cumplimentación insuficiente, tiene mayor adhesión a la finalización por parte del equipo multidisciplinario en comparación con la finalización del folleto de Salud de la Persona. Tales dificultades están asociadas con limitaciones en el acceso a los servicios, falta de infraestructura y baja calificación de los profesionales. Reflexiones finales: hay poca evidencia sobre estudios sobre los registros en el Cuader\nillo de Salud del Adulto Mayor, por otro lado, hay más evidencia sobre registros en el Cuadernillo Infantil realizados por el equipo multidisciplinario, como Agente Comunitario de Salud, Nutricionista, Terapeuta Ocupacional, Médico, Enfermera, entre otros.

ABSTRACT

Objetivo: Identificar as evidências disponíveis na literatura acerca das intervenções de enfermagem frente aos cuidados diante das complicações inerentes ao tratamento hemodialítico. Metodologia: Trata-se de uma revisão integrativa da literatura através de buscas nas bases de dados PubMed; BVS e Scopus, em abril de 2023. Estabeleceu-se a pergunta norteadora: "Quais são as evidências disponíveis na literatura acerca das intervenções de enfermagem frente a complicações apresentadas por pacientes submetidos à hemodiálise?". Foram excluídos protocolos de revisão sistemática ou meta análise e estudos incompletos, que não fazem referências ao objetivo da pesquisa. Resultados: Na busca foram selecionados 09 estudos para essa revisão, que atenderam aos critérios de elegibilidade. Foi evidenciado na literatura que a implementação de medidas preventivas específicas durante o processo de HD pode reduzir significativamente a incidência de complicações em pacientes submetidos a esse procedimento. Quanto as complicações mais frequentes em pacientes em tratamento, são elas: náuseas, cefaleia, hipotensão, cãibras, coagulação e infecções. Evidencia-se que a identificação e gerenciamento precoce dessas complicações podem melhorar os resultados clínicos e a qualidade de vida dos pacientes. Os cuidados de enfermagem como monitorização frequente dos sinais vitais, higienização adequada, administração correta de fármacos, orientações voltadas para o autocuidado, além de medidas inovadoras são importantes para garantir a segurança do paciente durante tratamento. É necessário o conhecimento dos profissionais sobre as complicações associadas à HD para que assim estejam aptos a intervir da maneira correta. Considerações finais: Os estudos mostraram que é imprescindível que os profissionais de saúde envolvidos na assistência, especialmente enfermeiros, estejam aptos para intervir em possíveis complicações durante a terapia, a fim de minimizar os impactos à saúde do paciente.


Objective: To identify the evidence available in the literature about nursing interventions in the face of complications inherent to hemodialysis treatment. Methodology: This is an integrative review of the literature through searches in the PubMed databases; VHL and Scopus in April 2023. The guiding question was established: "What is the evidence available in the literature about nursing interventions in the face of complications presented by patients submitted to hemodialysis?" Systematic review protocols or meta-analysis and incomplete studies were excluded, which do not make reference to the purpose of the research. Results: In the search, 09 studies were selected for this review, which met the eligibility criteria. It was evidenced in the literature that the implementation of specific preventive measures during the HD process can significantly reduce the incidence of complications in patients undergoing this procedure. The most common complications in patients undergoing treatment are: nausea, headache, hypotension, cramps, clotting, and infections. Evidence shows that early identification and management of these complications can improve clinical outcomes and patients' quality of life. Nursing care such as frequent monitoring of vital signs, proper hygiene, correct drug administration, self-care guidelines, and innovative measures are important to ensure patient safety during treatment. Professionals need to be aware of the complications associated with HD so that they are able to intervene in the right way. Final considerations: Studies have shown that it is essential that healthcare professionals involved in care, especially nurses, are able to intervene in possible complications during therapy in order to minimize the impacts on patient health.


Propósito: identificar la evidencia disponible en la literatura sobre intervenciones de enfermería frente a la atención ante complicaciones inherentes al tratamiento hemodiáltico. Metodología: Se trata de una revisión integral de la literatura a través de búsquedas en bases de datos de PubMed; BVS y Scopus en abril de 2023. Se planteó la siguiente pregunta: "¿Cuál es la evidencia disponible en la literatura sobre intervenciones de enfermería frente a complicaciones presentadas por pacientes sometidos a hemodiálisis?". Se excluyeron los protocolos de revisión sistemática o metaanálisis y los estudios incompletos, que no hacen referencia al objetivo de la investigación. Resultados: En la búsqueda se seleccionaron 09 estudios para esta revisión, que cumplieron con los criterios de elegibilidad. Se ha demostrado en la literatura que la aplicación de medidas preventivas específicas durante el proceso de HD puede reducir significativamente la incidencia de complicaciones en los pacientes sometidos a este procedimiento. Las complicaciones más frecuentes en los pacientes tratados son náuseas, cefalea, hipotensión, calambres, coagulación e infecciones. Es evidente que la identificación y manejo tempranos de estas complicaciones pueden mejorar los resultados clínicos y la calidad de vida de los pacientes. La atención de enfermería, como la vigilancia frecuente de los signos vitales, la higiene adecuada, la administración adecuada de medicamentos, la orientación sobre el autocuidado y las medidas innovadoras son importantes para garantizar la seguridad de los pacientes durante el tratamiento. Los profesionales deben ser conscientes de las complicaciones asociadas a la HD para que puedan intervenir de la manera correcta. Consideraciones finales: los estudios han demostrado que es esencial que los profesionales de la salud involucrados en la atención, especialmente las enfermeras, puedan intervenir en posibles complicaciones durante el tratamiento para minimizar el impacto en la salud del paciente.

ABSTRACT

A hanseníase é uma doença crônica, infectocontagiosa, de notificação compulsória e investigação obrigatória no Brasil, de evolução lenta, causada pelo bacilo Mycobacterium leprae que acomete predominantemente pele e nervos periféricos causando incapacidades funcionais permanentes de diversos graus, além de ser uma patologia ainda cercada por estigma e discriminação social. O objetivo do artigo consiste em avaliar o perfil epidemiológico da hanseníase no Estado do Rio Grande do Norte entre 2018 a 2022, estado que ocupa, atualmente, o 3° lugar a nível nacional de registro geral de casos de hanseníase. Trata-se de um estudo epidemiológico, descritivo, retrospectivo e de abordagem quantitativa, com base em dados obtidos no Departamento de Informática do Sistema Único de Saúde (DATASUS) e analisados com auxílio do software livre Jamovi 2.3.21 solid. O perfil epidemiológico da hanseníase no período de 2018 a 2022, no Estado do Rio Grande do Norte, apresentou predomínio pelo sexo feminino, acometendo adultos entre 50 a 69 anos de idade, com registro ignorado ou em branco no grau de escolaridade, predominando a forma multibacilar do tipo Virchowiana, causando grau 0 de incapacidade e a grande parte dos casos evoluindo para a cura. Portanto, faz-se relevante conhecer melhor o perfil epidemiológico e a realidade de cada município, visto que é uma patologia fortemente relacionada a condições econômicas, sociais e ambientais desfavoráveis passíveis de intervenção.


Leprosy is a chronic, contagious infectious disease, compulsorily notifiable and compulsory research in Brazil, of slow evolution, caused by the bacillus Mycobacterium leprae that predominantly affects skin and peripheral nerves causing permanent functional impairments of various degrees, besides being a pathology still surrounded by stigma and social discrimination. The objective of the article is to evaluate the epidemiological profile of leprosy in the State of Rio Grande do Norte between 2018 and 2022, which currently occupies the 3rd place at the national level of general registration of cases of leprosy. This is an epidemiological, descriptive, retrospective and quantitative approach study, based on data obtained in the Department of Information Technology of the Unified Health System (DATASUS) and analyzed with the help of free software Jamovi 2.3.21 solid. The epidemiological profile of leprosy in the period from 2018 to 2022, in the state of Rio Grande do Norte, showed predominance by the female sex, affecting adults between 50 and 69 years of age, with ignored or blank registration in the level of schooling, predominating the multibacillary form of the Virchowiana type, causing grade 0 disability and the great majority of cases evolving to cure. Therefore, it is important to know better the epidemiological profile and reality of each municipality, since it is a pathology strongly related to unfavorable economic, social and environmental conditions that can intervene.


La Hanseniasis es una enfermedad crónica, una enfermedad contagiosa, una notificación obligatoria e investigación obligatoria en Brasil, de evolución lenta, causada por el bacilo Mycobacterium leprae, que ataca predominantemente la piel y los nervios periféricos, causando discapacidades funcionales permanentes de diversos grados, además de ser una enfermedad aún rodeada de estigma y discriminación social. El propósito del artículo es evaluar el perfil epidemiológico de la lepra en el estado de Río Grande del Norte entre 2018 y 2022, un estado que actualmente ocupa el tercer lugar en el registro general nacional de casos de lepra. Se trata de un estudio de enfoque epidemiológico, descriptivo, retrospectivo y cuantitativo basado en datos obtenidos del Departamento de Datos del Sistema de Salud Pública (DATASUS) y analizado con la ayuda del software sólido Jamovi 2.3.21 libre. El perfil epidemiológico de la lepra en el período 2018-2022, en el estado de Rio Grande do Norte, mostró predominio del sexo femenino, atacando a adultos de 50 a 69 años, con un registro desconocido o en blanco en el grado de escolaridad, predominando en la forma multibacible del tipo Virchowiana, causando un grado de discapacidad y el gran número de casos. evolucionando hacia la curación. Por lo tanto, es importante conocer mejor el perfil epidemiológico y la realidad de cada municipio, ya que se trata de una enfermedad que está fuertemente relacionada con condiciones económicas, sociales y ambientales desfavorables que pueden ser intervenidas.

Objetivo: Mapear e examinar as evidências científicas sobre a aplicação da estratégia de atenção às doenças prevalentes na infância no contexto da Atenção Primária. Método: Scoping Review, baseado nos procedimentos recomendados pelo instituto Joanna Briggs. Definiu-se a pergunta norteadora: "Quais evidências científicas disponíveis acerca da aplicação da estratégia de atenção às doenças prevalentes na infância (AIDPI) no contexto da atenção básica em saúde?". Realizou-se buscas em cinco bases de dados nacionais e internacionais, sobre trabalhos publicados entre 2018 até dezembro de 2022. Dos 2227 estudos encontrados, 329 foram selecionados para leitura na íntegra, resultando em uma amostragem final de 08 estudos analisados. Resultados: Os artigos foram publicados de 2018 a 2022, com abordagem quantitativa dos dados, sendo 04 estudos (50%) com delineamento transversal exploratório. No que tange a aplicação do AIDPI, 06 (75%) apontaram que a aplicabilidade da estratégia AIDPI ainda é deficitária no atendimento e manejo das doenças prevalentes na infância no contexto do primeiro nível de atenção à saúde, devido à falta de treinamento dos profissionais, escassez de recurso e problemas estruturais. Conclusão: As evidências mostram que apesar do longínquo tempo de criação da AIDPI e os estudo apontam baixo índice de adesão e a inaplicabilidade da estratégia, sua aplicação ainda é negligenciada, o que afeta diretamente nos índices de morbimortalidade infantil por causas evitáveis, passíveis de resolução e atendimento no primeiro nível de assistência à saúde.

The guiding question was defined as: "What scientific evidence is available on the application of the strategy of care for diseases prevalent in childhood (IMCI) in the context of primary health care? A search was conducted in five national and international databases, on papers published between 2018 and December 2022. Of the 2227 studies found, 329 were selected for reading in full, resulting in a final sample of 08 studies analyzed. Results: The articles were published from 2018 to 2022, with a quantitative approach to data, being 04 studies (50%) with exploratory cross-sectional design. Regarding the application of AIDPI, 06 (75%) pointed out that the applicability of the AIDPI strategy is still deficient in the care and management of prevalent childhood diseases in the context of the first level of health care, due to lack of training of professionals, scarcity of resources, and structural problems. Conclusion: The evidence shows that despite the long time since the creation of the IMCI and the studies show low rates of adherence and inapplicability of the strategy, its application is still neglected, which directly affects the rates of infant morbidity and mortality from preventable causes, amenable to resolution and care at the first level of health care.

Conclusiones: La evidencia muestra que a pesar del largo tiempo transcurrido desde la creación de la AIEPI y de que los estudios muestran bajos índices de adherencia e inaplicabilidad de la estrategia, su aplicación sigue siendo descuidada, lo que incide directamente en los índices de morbimortalidad infantil por causas prevenibles, susceptibles de resolución y atención en el primer nivel de atención de salud.

ABSTRACT

Resumo Hemocromatose hereditária (HH) é a doença genética mais comum em descendentes de europeus e sua epidemiologia em nosso país é incerta. Considerando o cenário das políticas públicas em HH no mundo contemporâneo, este artigo propõe uma reflexão sobre o tema, com objetivo de fazer uma revisão bibliográfica narrativa sobre a abordagem adotada para essa doença em países desenvolvidos e a nível nacional. Além disso, discute sobre o custo-benefício da incorporação do índice de saturação da transferrina (ST) e ferritina sérica (FS) no nosso sistema de saúde, com a finalidade de identificar a HH antes que surjam suas complicações, bem como seu rastreio em campanhas nacionais de prevenção. O valor gasto para o screening da HH com dosagem de ST e FS pelo Sistema Único de Saúde (SUS) é muito menor do que os custos gerados quando o dano por excesso de ferro já está estabelecido. Nos casos suspeitos de HH, deveria ser viabilizada pelo SUS a pesquisa da mutação genética para o gene HFE, que atualmente só está disponível de forma privada. Com essas medidas, modifica-se a história natural da doença, reduzindo a morbimortalidade dos portadores e custos ao sistema público de saúde.


Abstract Hereditary hemochromatosis (HH) is the most common genetic disease among European descendants and its epidemiology in Brazil is unclear. Considering the contemporary public policy scenario aimed at HH, this narrative bibliographic review reflects on the approach adopted for this disease at the national level in developed countries. It also discusses the cost-effectiveness of incorporating transferrin saturation (TS) and serum ferritin (SF) indexes in the Brazilian healthcare system for early HH identification, as well as its screening in national prevention campaigns. The amount spent on ST- and FS-based HH screening by the Brazilian National Health System (SUS) is much lower than the costs generated by the already established iron overload. In suspected cases, genetic mutation research of the HFE gene, which is currently only performed privately, should be made available by the SUS. These measures can modify the natural history, reducing HH morbidity and mortality and its costs to the public health system.

BACKGROUND: Alpha-1 antitrypsin deficiency (AATD) is a rare disease that is associated with an increased risk of pulmonary emphysema. The European AATD Research Collaboration (EARCO) international registry was founded with the objective of characterising the individuals with AATD and investigating their natural history. METHODS: The EARCO registry is an international, observational and prospective study of individuals with AATD, defined as AAT serum levels < 11 µM and/or proteinase inhibitor genotypes PI*ZZ, PI*SZ and compound heterozygotes or homozygotes of other rare deficient variants. We describe the characteristics of the individuals included from February 2020 to May 2022. RESULTS: A total of 1044 individuals from 15 countries were analysed. The most frequent genotype was PI*ZZ (60.2%), followed by PI*SZ (29.2%). Among PI*ZZ patients, emphysema was the most frequent lung disease (57.2%) followed by COPD (57.2%) and bronchiectasis (22%). Up to 76.4% had concordant values of FEV1(%) and KCO(%). Those with impairment in FEV1(%) alone had more frequently bronchiectasis and asthma and those with impairment in KCO(%) alone had more frequent emphysema and liver disease. Multivariate analysis showed that advanced age, male sex, exacerbations, increased blood platelets and neutrophils, augmentation and lower AAT serum levels were associated with worse FEV1(%). CONCLUSIONS: EARCO has recruited > 1000 individuals with AATD from 15 countries in its first 2 years. Baseline cross sectional data provide relevant information about the clinical phenotypes of the disease, the patterns of functional impairment and factors associated with poor lung function. ABSTRACT

BACKGROUND: Globally, there is a scarcity of effective treatments for SARS-CoV-2 infections (causing COVID-19). Repurposing existing medications may offer the best hope for treating patients with COVID-19 to curb the pandemic. IMU-838 is a dihydroorotate dehydrogenase inhibitor, which is an effective mechanism for antiviral effects against respiratory viruses. When used synergistically with oseltamivir, therapeutic effects have been observed against influenza and SARS-CoV-2 in rodents. The IMU-838 and Oseltamivir in the Treatment of COVID-19 (IONIC) trial is a randomised controlled trial that will investigate whether time to clinical improvement in patients with COVID-19 is improved following a 14-day course of IMU-838+oseltamivir versus oseltamivir alone. METHODS: IONIC trial is an open-label study in which participants will be randomised 1:1 in two parallel arms: the intervention arm (IMU-838+oseltamivir) and the control arm (oseltamivir only). The primary outcome is time to clinical improvement; defined as the time from randomisation to a two-point improvement on WHO ordinal scale; discharge from hospital, or death (whichever occurs first). The study is sponsored by the University Hospitals Coventry and Warwickshire NHS Trust and funded by LifeArc. DISCUSSION: The IONIC protocol describes an overarching trial design to provide reliable evidence on the effectiveness of IMU-838 (vidofludimus calcium) when delivered in combination with an antiviral therapy (oseltamivir) (IONIC intervention) for confirmed or suspected COVID-19 infection in adult patients receiving usual standard of care. ETHICS AND DISSEMINATION: This study has been independently reviewed and approved by Wales Research Ethics Committee. ABSTRACT

In Spain, students pursuing a career in athletic training, physical education, or scientific evaluation of sports enroll in a bachelor's degree in sports sciences. This degree provides knowledge and skills in a broad array of sports settings and promotes research-based interdisciplinary knowledge. However, the student's syllabus rarely includes specific academic training on anti-doping regulations or doping prevention. The purpose of this study was to assess the anti-doping knowledge of the students undertaking a bachelor's degree in sports sciences in Spanish universities. One thousand two hundred and thirty-three bachelor students in sport science (907 males, 322 females, and 4 participants with non-binary sex) from 26 Spanish universities completed a validated questionnaire about general anti-doping knowledge. The questionnaire is an adapted version of the Play True Quiz of the World Anti-Doping Agency and contains 37 multiple-choice questions. The score obtained in the questionnaire was transformed into a 0−100-point scale. The questionnaire was distributed among students within each university by a faculty member and it was filled out online. Students obtained a score of 65.8 ± 10.10 points (range = 32−92 points). There was an effect of the course in the score obtained (p < 0.001). Students of the first course (63.6 ± 9.5 points) had lower scores than the remaining courses (p < 0.037) while the students of the fourth course obtained the highest scores (68.7 ± 9.5 points; p < 0.019). The students with an itinerary on sports performance were the respondents with the highest anti-doping knowledge (67.2 ± 10.2) points, followed by the students with an itinerary on health (66.7 ± 9.5 points). The knowledge of basic anti-doping rules and doping prevention strategies of the bachelor students in sports sciences in Spain was suboptimal. Increasing doping prevention information in the syllabus of the bachelor's degree in sports sciences is essential as these future professionals will directly work with populations at risk of doping.
