ABSTRACT

In this study, we described and discussed the late onset spontaneous posterior capsule rupture with intraocular lens (IOL) dislocation years after uncomplicated cataract surgery and implantation of hydrophobic acrylic IOLs. Eight patients presented with spontaneous posterior capsule rupture and IOL dislocation 5-20 years after uncomplicated phacoemulsification and IOL (AcrySof, Alcon, US) implantation. None of the patients had undergone posterior capsulotomy in the past. Four of the patients admitted habitual eye rubbing. An intact and well-centered continuous curvilinear capsulotomy edge was observed in all cases. IOLs were dislocated or displaced behind the anterior capsulotomy with a significant decrease in vision. A large rupture with a curled edge of the broken posterior capsule was visible. Dislocated IOLs were removed, and a three-piece IOL was inserted in the sulcus in six cases and suture fixated to the sclera in two cases. Improved vision was achieved in all cases. Although the mechanism underlying this late complication is unclear, habitual eye rubbing or IOL design may play a role. Further investigation is needed to prevent this complication in the future.

ABSTRACT

The effective management and regulation of fine particulate matter (PM2.5) is essential in the Republic of Korea, where PM2.5 concentrations are very high. To do this, however, it is necessary to identify sources of PM2.5 pollution and determine the contribution of each source using an acceptance model that includes variability in the chemical composition and physicochemical properties of PM2.5, which change according to its spatiotemporal characteristics. In this study, PM2.5 was measured using PMS-104 instruments at two monitoring stations in Bucheon City, Gyeonggi Province, from 22 April to 3 July 2020; the PM2.5 chemical composition was also analyzed. Sources of PM2.5 pollution were then identified and the quantitative contribution of each source to the pollutant mix was estimated using a positive matrix factorization (PMF) model. From the PMF analysis, secondary aerosols, coal-fired boilers, metal-processing facilities, motor vehicle exhaust, oil combustion residues, and soil-derived pollutants had average contribution rates of 5.73 µg/m3, 3.11 µg/m3, 2.14 µg/m3, 1.94 µg/m3, 1.87 µg/m3, and 1.47 µg/m3, respectively. The coefficient of determination (R2) was 0.87, indicating the reliability of the PMF model. Conditional probability function plots showed that most of the air pollutants came from areas where PM2.5-emitting facilities are concentrated and highways are present. Pollution sources with high contribution rates should be actively regulated and their management prioritized. Additionally, because automobiles are the leading source of artificially-derived PM2.5, their effective control and management is necessary.

ABSTRACT

This study was conducted to investigate the general properties of an enzymatic extract of Sargassum muticum (SM) produced using a crude enzyme from Shewanella oneidensis PKA 1008 and their anti-inflammatory activities. The SM was mixed with crude enzymes from S. oneidensis PKA 1008 (1:1 (v/v)) and incubated at 30 °C for 0, 3, 6, 12, 24, 48, and 60 h. S. oneidensis PKA 1008 crude enzyme showed the highest SM enzymatic extracts degradation ability when reacted with SM for 48 h. These evaluations demonstrated a 134.25% increase in reducing sugar content and a 14.90% reduction in viscosity at 48 h. The pH, lightness (L*) and yellowness (b*) of the SM enzymatic extracts decreased significantly with increasing reaction time. Moreover, the SM enzymatic extracts demonstrated significant anti-inflammatory activity. These results indicate that the crude enzyme from S. oneidensis PKA 1008 can be used to enhance the polysaccharide degradation of SM, and the resultant oligosaccharides may have an anti-inflammatory effect.

ABSTRACT

INTRODUCTION: Cataract surgery is becoming more common due to an aging world population. Intraocular lenses and surgical technique have developed remarkably recently, but the development of postoperative medication to prevent postsurgery complications has been relatively delayed. We still largely depend on eye drops for the management of post-cataract-surgery patients. Mental and physical problems that often occur in elderly cataract patients make it difficult for patients to apply eye drops by themselves. It is necessary to develop new effective drug delivery methods. AREAS COVERED: This updated review article provides a brief review of why drug management is needed following cataract surgery and an overview of current developments in new drug delivery methods for ophthalmic treatment. In particular, various novel drug delivery methods that can be used for post-cataract-surgery management and their current development stages are extensively reviewed. EXPERT OPINION: Rapidly developing technologies, such as intraocular and external ophthalmic implants, polymers, and nanotechnology, are being actively applied to develop novel drug delivery systems for safe and effective management after cataract surgery. Their goal is to achieve sufficient drug release for the desired duration with a single application. These will largely replace the inconvenience of eye drops for elderly patients in the future.

ABSTRACT

In this study, gamma-irradiated mackerel (Scomber japonicus) meat was stored in a refrigerator for 20 days to examine the physicochemical changes related to fishy smell. The effect of gamma irradiation on the inhibition of the activity of crude urease extracted from Vibrio parahaemolyticus was also evaluated. Increased levels of trimethylamine (TMA) and volatile basic nitrogen (VBN) content, which are the main components causing fishy smell, were significantly reduced by day 20 of storage after gamma irradiation, indicating that freshness was maintained during storage. The ammonia nitrogen contents of 3, 7, 10, and 20 kGy gamma-irradiated groups were significantly decreased by 6.5, 15.2, 17.4, and 23.9%, respectively, compared to non-irradiated groups on day 20 of storage. In addition, urease activity decreased in a gamma irradiation intensity-dependent manner. Volatile organic compounds (VOCs) were measured during the storage of gamma-irradiated mackerel meat. The contents of ethanol, 2-butanone, 3-methylbutanal, and trans-2-pentenal, which are known to cause off-flavors due to spoilage of fish, were significantly reduced by day 20 of storage. Therefore, gamma irradiation can be considered useful for inhibiting urease activity and reducing fishy smell during fish storage.

In this study, the physicochemical changes related to fishy smell were determined by storing high hydrostatic pressure (HHP)-treated mackerel (Scomber japonicus) meat in a refrigerator for 20 days. The inhibition of crude urease activity from Vibrio parahaemolyticus using HHP treatment was also investigated. The mackerel meat storage experiment demonstrated that production of trimethylamine (TMA) and volatile basic nitrogen (VBN), the main components of fishy smell, was significantly reduced on the 20th day of storage after the HHP treatment compared to the untreated mackerels. The results demonstrated that the increased ammonia nitrogen rates in the 2000, 3000, and 4000 bar, HHP-treated groups decreased by 23.8%, 23.8%, and 31.0%, respectively, compared to the untreated groups. The enzyme activity of crude urease was significantly reduced in the HHP-treated group compared to that in the untreated group. Measurement of the volatile organic compounds (VOCs) in mackerel meat during storage indicated that the content of ethanol, 2-butanone, 3-methylbutanal, and trans-2-pentenal, which are known to cause off-flavor due to spoilage, were significantly reduced by HHP treatment. Collectively, our results suggested that HHP treatment would be useful for inhibiting the activity of urease, thereby reducing the fishy smells from fish and shellfish.

ABSTRACT

Resuspension of particulate matter (PM) in classrooms, which increases the risk of negative impact on student health from exposure to PM, is influenced by humidity level in the indoor environment. The goal of this study is to investigate the properties of PM resuspension in accordance with relative humidity through classroom test chamber experiments. In actual classrooms, it is challenging to control factors influencing resuspension. Therefore, the classroom chamber that reflects the environment of elementary school classroom (e.g., structure, floor material) is used in this study. The humidity of the classroom chamber is adjusted to 35%, 55%, 75%, and 85% by placing it inside a real-size environmental chamber, which allows artificial control of climatic conditions. At the respective humidity conditions, PM resuspension concentration and resuspension factor caused by occupant walking across the classroom chamber are analyzed. The results show that both of the resuspension concentration and resuspension factor reveal a linear negative correlation to humidity increase. Furthermore, coefficient of determination (R2) indicating goodness-of-fit of the linear regression model between the resuspension concentration and humidity is 0.88 for PM10 and 0.93 for PM2.5. It implies that accuracy of the regression model for estimating PM10 and PM2.5 resuspension concentrations is 88% and 93%, respectively.

ABSTRACT

Particulate matter with aerodynamic diameter less than 2.5 µm (PM2.5) has become a major public concern in closed indoor environments, such as subway stations. Forecasting platform PM2.5 concentrations is significant in developing early warning systems, and regulating ventilation systems to ensure commuter health. However, the performance of existing forecasting approaches relies on a considerable amount of historical sensor data, which is usually not available in practical situations due to hostile monitoring environments or newly installed equipment. Transfer learning (TL) provides a solution to the scant data problem, as it leverages the knowledge learned from well-measured subway stations to facilitate predictions on others. This paper presents a TL-based residual neural network framework for sequential forecast of health risk levels traced by subway platform PM2.5 levels. Experiments are conducted to investigate the potential of the proposed methodology under different data availability scenarios. The TL-framework outperforms the RNN structures with a determination coefficient (R2) improvement of 42.84%, and in comparison, to stand-alone models the prediction errors (RMSE) are reduced up to 40%. Additionally, the forecasted data by TL-framework under limited data scenario allowed the ventilation system to maintain IAQ at healthy levels, and reduced PM2.5 concentrations by 29.21% as compared to stand-alone network.

BACKGROUND: Osteoarthritis (OA) is a chronic disease that commonly afflicts the elderly. This disease reduces the health-related quality of life (HRQoL) and causes a significant social burden. Whether the effect of coexisting chronic conditions on HRQoL varies according to the presence of OA remains unclear. Therefore, this study aimed to investigate this notion. METHODS: A total of 13,395 participants were identified from the 2009-2013 Korean National Health and Nutrition Examination Survey for analysis. HRQoL was assessed using the European quality of life-5 dimensions (EQ-5D) index. Patients with OA were defined as those diagnosed by a physician or those who displayed both, symptoms and radiological findings consistent with OA at the time of the survey. Associations between OA and 8 chronic conditions were tested using regression analysis. RESULTS: The EQ-5D index was lower in patients with OA than in those without (mean difference, -0.145; 95% confidence interval [CI], -0.138 to -0.151; P<0.001). Most patients with OA and chronic conditions showed a lower score than those without. EQ-5D was particularly lower in OA patients with hypertension, dyslipidemia, stroke, and renal failure. The estimated ß coefficient for the interaction term was significant in renal failure (-0.034; 95% CI, -0.055 to -0.012), after adjusting for demographic and socio-economic variables. CONCLUSION: OA significantly affects HRQoL of Korean elderly individuals alone or when combined with other conditions. OA combined with renal failure is particularly detrimental. These results indicate the importance of managing OA, which is an underestimated disease in public health surveys.

BACKGROUND: The efficacy of two artificial tears, carboxymethylcellulose (CMC) and hyaluronate (HA), was compared in the treatment of patients with dry eye disease. METHODS: We conducted a systematic review and meta-analysis on randomized controlled trials in the PubMed, Embase, Cochrane Library, and ClinicalTrials.gov databases. The efficacy was compared in terms of the mean change from baseline in tear break-up time. The meta-analysis was conducted using both random and fixed effect models. The quality of the selected studies was assessed for risk of bias. RESULTS: Five studies were included involving 251 participants. Random effect model meta-analysis showed no significant difference between CMC and HA in treating dry eye disease (pooled standardized mean difference [SMD]=-0.452; 95% confidence interval [CI], -0.911 to 0.007; P=0.053). In contrast, fixed effect model meta-analysis revealed significant improvements in the CMC group when compared to the HA group (pooled SMD=-0.334; 95% CI, -0.588 to -0.081; P=0.010). CONCLUSION: The efficacy of CMC appeared to be better than that of HA in treating dry eye disease, although meta-analysis results were not statistically significant. Further research is needed to better elucidate the difference in efficacy between CMC and HA in treating dry eye disease.
