ABSTRACT

BACKGROUND: With increasing life expectancy, the prevalence of nonagenarians with cardiovascular disease is steadily growing. However, this population is underrepresented in randomized trials and thus poorly defined, with little quality evidence to support and guide optimal management. The aim of the present study was to evaluate the clinical management, therapeutic approach, and outcomes of nonagenarians admitted to a tertiary care center intensive coronary care unit (ICCU). METHODS: We prospectively collected all patients admitted to a tertiary care center ICCU between July 2019 - July 2022 and compared nonagenarians to all other patients. The primary outcome was in-hospital mortality. RESULTS: A total of 3807 patients were included in the study. Of them 178 (4.7%) were nonagenarians and 93 (52%) females. Each year the prevalence of nonagenarians has increased from 4.0% to 2019, to 4.2% in 2020, 4.6% in 2021 and 5.3% in 2022. Admission causes differed between groups, including a lower rate of acute coronary syndromes (27% vs. 48.6%, p < 0.001) and a higher rate of septic shock (4.5% vs. 1.2%, p < 0.001) in nonagenarians. Nonagenarians had more comorbidities, such as hypertension, renal failure, and atrial fibrillation (82% vs. 59.6%, 23% vs. 12.9%, 30.3% vs. 14.4% p < 0.001, respectively). Coronary intervention was the main treatment approach, although an invasive strategy was less frequent in nonagenarians in comparison to younger subjects. In-hospital mortality rate was 2-fold higher in the nonagenarians (5.6% vs. 2.5%, p = 0.025). CONCLUSION: With increasing life expectancy, the prevalence of nonagenarians in ICCU's is expected to increase. Although nonagenarian patients had more comorbidities and higher in-hospital mortality, they generally have good outcomes after admission to the ICCU. Hence, further studies to create evidence-based practices and to support and guide optimal management in these patients are warranted.

ABSTRACT

BACKGROUND: Sleep disorders are experienced by up to 40% of the population but their diagnosis is often delayed by the availability of specialists. OBJECTIVE: We propose the use of search engine activity in conjunction with a validated web-based sleep questionnaire to facilitate wide-scale screening of prevalent sleep disorders. METHODS: Search advertisements offering a web-based sleep disorder screening questionnaire were shown on the Bing search engine to individuals who indicated an interest in sleep disorders. People who clicked on the advertisements and completed the sleep questionnaire were identified as being at risk for 1 of 4 common sleep disorders. A machine learning algorithm was applied to previous search engine queries to predict their suspected sleep disorder, as identified by the questionnaire. RESULTS: A total of 397 users consented to participate in the study and completed the questionnaire. Of them, 132 had sufficient past query data for analysis. Our findings show that diurnal patterns of people with sleep disorders were shifted by 2-3 hours compared to those of the controls. Past query activity was predictive of sleep disorders, approaching an area under the receiver operating characteristic curve of 0.62-0.69, depending on the sleep disorder. CONCLUSIONS: Targeted advertisements can be used as an initial screening tool for people with sleep disorders. However, search engine data are seemingly insufficient as a sole method for screening. Nevertheless, we believe that evaluable web-based information, easily collected and processed with little effort on part of the physician and with low burden on the individual, can assist in the diagnostic process and possibly drive people to seek sleep assessment and diagnosis earlier than they currently do.

ABSTRACT

Introduction Patients suffering from cardiogenic shock (CS) and mitral regurgitation (MR) demonstrate worse prognosis, with higher mortality rates. We sought to evaluate the effectiveness of urgent valve intervention of the mitral valve, using transcatheter edge-to-edge repair (TEER) procedures in patients presenting with CS in a tertiary Intensive Coronary Care Unit (ICCU). Methods and Results Patients with unremitting CS and severe MR were selected for urgent TEER. Baseline clinical and echocardiographic characteristics were recorded, as well as procedural success (MR severity and hemodynamics), and 30-days and 6-month mortality. Urgent TEER was done in 13 patients, whose average age was 70 years; 12 (92%) of the patients were male. All 13 patients had suffered previous ischemic heart disease-12 (92%) with either acute severe MR or worsening of previously known MR by an acute ischemic event. Using the SCAI criteria, 8 patients (61%) were classified as 'E' (Extreme) category; 4 (31%) were classified as 'C'. At 30 days, 12 out of the 13 patients survived (corresponding to an 8% mortality rate); all of those 12 patients remained alive at 6 months post-admission/procedure. Conclusions The use of TEER was associated with greater 30-day and 6-month survival rates, compared to the worldwide mortality rates of patients admitted with CS. This finding may change the previous paradigm that CS and MR are associated with the worst outcome, and we might be able to offer these patients a safe and effective therapeutic option.

ABSTRACT

Empirical studies show that epidemiological models based on an epidemic's initial spread rate often fail to predict the true scale of that epidemic. Most epidemics with a rapid early rise die out before affecting a significant fraction of the population, whereas the early pace of some pandemics is rather modest. Recent models suggest that this could be due to the heterogeneity of the target population's susceptibility. We study a computer malware ecosystem exhibiting spread mechanisms resembling those of biological systems while offering details unavailable for human epidemics. Rather than comparing models, we directly estimate reach from a new and vastly more complete data from a parallel domain, that offers superior details and insight as concerns biological outbreaks. We find a highly heterogeneous distribution of computer susceptibilities, with nearly all outbreaks initially over-affecting the tail of the distribution, then collapsing quickly once this tail is depleted. This mechanism restricts the correlation between an epidemic's initial growth rate and its total reach, thus preventing the majority of epidemics, including initially fast-growing outbreaks, from reaching a macroscopic fraction of the population. The few pervasive malwares distinguish themselves early on via the following key trait: they avoid infecting the tail, while preferentially targeting computers unaffected by typical malware.

ABSTRACT

The modal view in the cognitive and neural sciences holds that consciousness is necessary for abstract, symbolic, and rule-following computations. Hence, semantic processing of multiple-word expressions, and performing of abstract mathematical computations, are widely believed to require consciousness. We report a series of experiments in which we show that multiple-word verbal expressions can be processed outside conscious awareness and that multistep, effortful arithmetic equations can be solved unconsciously. All experiments used Continuous Flash Suppression to render stimuli invisible for relatively long durations (up to 2,000 ms). Where appropriate, unawareness was verified using both objective and subjective measures. The results show that novel word combinations, in the form of expressions that contain semantic violations, become conscious before expressions that do not contain semantic violations, that the more negative a verbal expression is, the more quickly it becomes conscious, and that subliminal arithmetic equations prime their results. These findings call for a significant update of our view of conscious and unconscious processes.
