ABSTRACT

INTRODUCTION: This article explores the experiences of peer recovery support specialists (PRSS) and their colleagues working in residential adult services (RAS) facilities in a rural state to better understand how the role has been implemented in this setting. In West Virginia, PRSS and RAS services have been covered by Medicaid since 2018 for the treatment of substance use disorder (SUD). Thus, the aim of this study is to uncover what has and has not worked well in the early years of service implementation from the perspectives of both PRSS and their colleagues working in the SUD field. METHODS: The study interviewed forty-eight clinical and administrative staff at RAS facilities, including nine PRSS, across fourteen focus groups between 2020 and 2021. The study asked participants about general knowledge of West Virginia's 1115 Medicaid Waiver supporting the funding of PRSS, communication among providers, transitioning patients to other providers, scope of practice, barriers and facilitators to providing services, ethical challenges, and COVID-19. Data analysis utilized a phenomenological approach to describe individuals' unique experiences. RESULTS: Three core themes emerged from the analysis: (i) Lived Experience as Added Value, (ii) Inadequate Funding for PRSS Position, and (iii) Role Difficulties. Participants described the lived experience of PRSS as both beneficial to the recovery journey of patients with SUD and unique in that only PRSS can provide the specific skillset that benefits these patients; these characteristics also helped facilities to connect patients to supportive resources. Two funding limitations emerged as barriers to PRSS employment: funding available through Medicaid billing was insufficient to hire staff and the corresponding hourly compensation rate was not competitive against those offered via grants. Finally, participants identified an underdeveloped hiring system and an unclear scope of practice that led to staff feeling unsupported. CONCLUSION: PRSS's experiential expertise complements clinical expertise by producing effective health care and support for patients. Despite their high value and expertise in SUD treatment, PRSS face major challenges in the workplace, including inadequate pay, underdeveloped support structures, and narrow job eligibility requirements. Future research should further quantify PRSS's levels of expertise and identify "value-added" benefits of this position.

Introduction: Success of opioid overdose interventions involving first responders is dependent on the comfort level that bystanders have with first responders and their willingness to call for assistance. Positive or negative experiences with first responders following witnessing an overdose may influence a person's willingness to call a first responder for assistance in the future. Purpose: The objective of this study was to examine changes in bystanders' perceptions of first responders following witnessing an overdose attended by emergency medical services or a law enforcement official. It specifically explored perception changes among a sample of individuals residing in Appalachia who use prescription opioids nonmedically. Methods: Individuals from West Virginia who used prescription opioids nonmedically were interviewed to examine changes in perceptions of first responders following witnessing an overdose. The analytic sample (N = 50) consisted of participants who witnessed an overdose for which 911 was called and stayed until a first responder arrived. Chi-square contingency tables and ANOVA were conducted to assess relationships between individual and contextual characteristics with changes in perceptions. Results: Findings indicate that the majority (63%) had improved perceptions of first responders, 6% had diminished perceptions, and 24% were unchanged. Changes in perceptions varied by income, presence during substance use, and prior concerns about first responders. Implications: Individuals who reported experiencing a positive interaction with first a responder after witnessing an overdose may be more likely to call 911 during an overdose and support other interventions by first responders (e.g., referral to syringe service programs or treatment with medications for opioid use disorder).

ABSTRACT

Introduction: Central Appalachia has been disproportionately affected by the opioid epidemic and overdose fatalities. We developed West Virginia Peers Enhancing Education, Recovery, and Survival (WV PEERS), a program based on peer recovery support, to engage individuals using opioids and link them with a range of services. Methods: Community partners providing services to individuals with opioid use disorder (OUD) were identified and collaborations were formalized using a standardized memorandum of understanding. The program was structured to offer ongoing peer recovery support specialist (PRSS) services, not just a one-time referral. A website and cards describing the WV PEERS program were developed and disseminated via community partners and community education sessions. Results: Overall, 1456 encounters with individuals with OUD (mean= 2 encounters per individual) occurred in a variety of community settings over 8 months. The majority of referrals were from harm reduction programs. Overall, 63.9% (n=931) of individuals served by WV PEERS accessed services for substance use disorders and/or mental health problems. Over half (52.3%; n = 487) of individuals entered substance use and/or mental health treatment, and nearly a third (30.4%; n = 283) remained in treatment over six months. Implications: Using the WV PEERS model, PRSSs effectively engaged and linked individuals with OUD to mental health and substance use treatment in rural central Appalachia. Future research is needed to determine whether these services reduce the risk of overdose mortality.

BACKGROUND: The wholesale and retail trade (WRT) sector employs over 21 million workers, or nearly 19% of the annual average employment in private industry. The perception is that workers in this sector are generally at low risk of occupational injury and death. These workers, however, are engaged in a wide range of demanding job activities and are exposed to a variety of hazards. Prior to this report, a comprehensive appraisal of the occupational fatal and nonfatal burdens affecting the retail and wholesale sectors was lacking. The focus of this review is to assess the overall occupational safety and health burden in WRT and to identify various subsectors that have high rates of burden from occupational causes. Ultimately, these findings should be useful for targeted intervention efforts. METHODS: We reviewed Bureau of Labor Statistics (BLS), 2006 fatality, injury, and illness data for the WRT sector and provide comparisons between the WRT sector, its' subsectors, and private industry, which serves as a baseline. The BLS data provide both counts and standardized incidence rates for various exposures, events, and injury types for fatalities, injuries, and illnesses. In an effort to estimate the economic burden of these fatalities, injuries, and illnesses, a focused review of the literature was conducted. RESULTS AND CONCLUSION: In 2006, WRT workers experienced 820,500 injuries/illnesses and 581 fatalities. The total case injury/illness rate for the retail sector was 4.9/100 FTE and for the wholesale sector 4.1/100 FTE. The WRT sector represents 15.5% of the private sector work population in 2006, yet accounts for 20.1% of nonfatal injuries and illnesses of the private sector. In 2003, the disparity was only 2% but increased to 3% in 2004 and 2005. Three WRT subsectors had injury/illness rates well above the national average: beer/wine/liquor (8.4/100); building materials/supplies (7.6/100); and grocery-related products (7.0/100). Occupational deaths with the highest rates were found in gasoline stations (9.8/100,000), convenience stores (6.1/100,000), and used car dealers (5.5/100,000). In terms of actual numbers, the category of food and beverage stores had 82 fatalities in 2006. Based on 1993 data, costs, both direct and indirect, in the WRT sector for fatal injuries were estimated to exceed $8.6 billion. The full economic loss to society and the family has not been adequately measured. Overexertion and contact with objects/equipment represent the top two events or exposures leading to injury or illness. Together they account for 57% of the events or exposures for nonfatal WRT injuries and illnesses. This sector is important because it is large and pervasive as a result, even a relatively small increase in injury rates and accompanying days away from work will have significant impact on working families and society.
