ABSTRACT

PURPOSE: Amniotic membrane transplantation (AMT) has shown promising results as an antifibrotic agent in trabeculectomy. We aimed to evaluate the additional effect of AMT in MMC-augmented trabeculectomy. PATIENTS AND METHODS: This retrospective study analyzed the results of the first 12 postoperative months of glaucomatous eyes submitted to Moorfields Safer Surgery Trabeculectomy with MMC alone (non-AMT group) compared to MMC and AMT (AMT group). Both groups were compared in terms of intraocular pressure (IOP), number of antihypertensive medications and need for surgical reinterventions. Absolute and relative success rates 12 months after surgery were defined as IOP <18 mmHg, without and with the use of antihypertensive medications, respectively. RESULTS: The analysis included 51 eyes of 45 glaucoma patients (29 eyes in the non-AMT group and 22 in the AMT group). Mean IOP decreased from 24.72±5.11 mmHg and 26.86±10.62 mmHg preoperatively in non-AMT and AMT groups to 12.86±4.22 mmHg and 12.60±4.43 mmHg, respectively, at 12 months (p = 0.84). Postoperative number of medications decreased significantly in both groups. Absolute success was seen in 71% of non-AMT eyes and 55% of AMT eyes (p = 0.46), whereas relative success was obtained in 14% and 30%, respectively (p = 0.55). Reinterventions were needed in 28% of the eyes (11 bleb injection/needling and 4 Ahmed tube implantation) in the non-AMT group and in 27% of the AMT group (10 bleb injection/needling and 1 Ahmed tube implantation) (p = 0.89). CONCLUSION: Trabeculectomy combined with MMC and AMT did not show better results than trabeculectomy with MMC alone.

Introdução: Um dos domínios de sintomas centrais do Transtorno do Espectro Autista (TEA) é o déficit de comunicação, que pode se apresentar de diversas formas. Objetivo: Analisar os efeitos do ensino do comportamento verbal em pessoas com TEA. Método: Revisão sistemática da literatura por meio de buscas nas bases de dados Scielo, Lilacs e Pubmed, utilizando os seguintes descritores: "transtorno do espectro autista"; "comportamento verbal" e "ensino", bem como seus descritores na língua inglesa. Foram selecionados estudos publicados entre 2014 e 2019, com dados que atendiam à temática estabelecida em português, espanhol e inglês. Resultados: A pesquisa resultou em sete artigos. O operante verbal mais frequente foi o mando, seguido pelo intraverbal e o ecóico. O ensino de comportamento verbal apresentou efeitos positivos. No mando, ocasionou a redução da ocorrência de comportamentos-problema, auxiliando em uma interação social bem-sucedida e contribuindo para um controle social e imediato sobre o ambiente. O ecoico foi apontado como o operante verbal que desempenha papel fundamental no ensino de outros comportamentos verbais. O intraverbal é importante para o desenvolvimento de habilidades sociais e acadêmicas. A limitação dos resultados consiste em não apresentar estudos referentes a todos os operantes verbais, contemplando apenas três, assim como o intervalo de ano de publicação analisado. Conclusão: O ensino dos operantes verbais é importante para um melhor desempenho e desenvolvimento da comunicação funcional de pessoas com autismo, refletindo também em aspectos sociais, acadêmicos e no brincar, além da contribuição para a redução das chances de ocorrência de comportamentos disruptivos.


Introduction: One of the central symptom domains of Autistic Spectrum Disorder (ASD) is the communication deficit, which can present itself in different ways. Objective: Analyze the effects of teaching verbal behavior in people with ASD. Method: Systematic literature review by searching the Scielo, Lilacs and Pubmed databases, using the following descriptors: "autism spectrum disorder"; "Verbal behavior" and "teaching", as well as their descriptors in the English language. Studies published between 2014 and 2019 were selected, with data that met the theme established in Portuguese, Spanish and English. Results: The search resulted in seven articles. The most frequent verbal operant was the mand, followed by the intraverbal and the echoic. The teaching of verbal behavior had positive effects. On mand, caused the reduction of the occurrence of problem behaviors and helping in a successful social interaction and contributing to an immediate and social control over the environment. The echoic was pointed out as the verbal operant that plays a fundamental role in the teaching of other verbal behaviors. The intraverbal is important for the development of social and academic skills. The limitation of the results consists in not presenting studies referring to all verbal operants, considering only three and the interval of year of publication analyzed. Conclusion: The teaching of verbal operants is important for a better performance and development of functional communication of people with autism, also reflecting on social, academic and playing aspects, in addition to the contribution to reducing the chances of disruptive behaviors.


Introduccion: Uno de los dominios de síntomas centrales del trastorno del espectro autista (TEA) es el déficit de comunicación, que puede presentarse de diferentes formas. Objetivo: Analizar los efectos de la enseñanza de la conducta verbal en personas con TEA. Metodo: Revisión sistemática de la literatura mediante búsquedas en las bases de datos Scielo, Lilacs y Pubmed, utilizando los siguientes descriptores: "trastorno del espectro autista"; "Conducta verbal" y "enseñanza", así como sus descriptores en el idioma inglés. Se seleccionaron estudios publicados entre 2014 y 2019, con datos que cumplieran con la temática establecida en portugués, español e inglés. Resultados: La búsqueda resultó en siete artículos. La operante verbal más frecuente fue el mand, seguida de la intraverbal y la ecoica. La enseñanza de la conducta verbal mostró efectos positivos. En el mand, provocó la reducción de la ocurrencia de conductas problemáticas y ayudó a una interacción social exitosa y contribuyó a un control social e inmediato sobre el medio ambiente. La ecoica fue señalada como la operante verbal que juega un papel fundamental en la enseñanza de otras conductas verbales. El intraverbal es importante para el desarrollo de habilidades sociales y académicas. La limitación de los resultados consiste en no presentar estudios referentes a todos los operantes verbales, considerando solo tres y el intervalo de año de publicación analizado. Conclusión: La enseñanza de operantes verbales es importante para un mejor desempeño y desarrollo de la comunicación funcional de las personas con autismo, reflexionando también sobre aspectos sociales, académicos y lúdicos, además de contribuir a reducir las posibilidades de conductas disruptivas.

ABSTRACT

The therapeutic effects from Citrus reticulata on painful inflammatory ailments are associated to its flavonoids constituent and phytochemical studies with Citrus genus affirm that the peels have important amounts of it. These bioactive compounds have been a considerable therapeutic source and evaluate potential application of the peel extract is significant. This research aims to investigate the influence of ethanolic crude extract from the peels of Citrus reticulata and its possible mechanism of action in different animal models of pain. The extract reduced hyperalgesia in the second phase of formalin test (vehicle: 501.5 ± 40.0 s; C. reticulata extract 300 mg/kg: 161.8 ± 41.1 s), in the carrageenan model (vehicle at 4th h: 82.5 ± 9.6 %; C. reticulata extract 300 mg/kg at 4th h: 47.5 ± 6.5 %) and in Complete Freund's Adjuvant model (vehicle: 501.5 ± 40.0 s; C. reticulata extract 300 mg/kg: 161.8 ± 41.1 s). The possible contribution of opioidergic and adenosinergic systems in the anti-hyperalgesic effect of C. reticulata extract was observed after treatment, with non-selective antagonists for both systems, which produced reversal effects. In conclusion, these properties of C. reticulata extract suggest a potential therapeutic benefit in treating painful conditions.

ABSTRACT

The aim of this study was to evaluate the outcomes of trabeculectomy with mitomycin C (MMC) in patients with Pseudoexfoliative Glaucoma (PXG) and compare the results with the outcomes of trabeculectomy without MMC in PXG and with MMC in Primary Open Angle Glaucoma (POAG). Ninety eyes (76 patients) submitted to trabeculectomy were included in a one-year retrospective study. Fifty-eight eyes with PXG were divided into group 1 (28 eyes) and group 2 (30 eyes), with and without MMC application respectively. Then, the group 1 results were compared with 32 eyes with POAG that performed trabeculectomy with MMC (group 3). Main outcome measures were intraocular pressure (IOP), number of IOP lowering medications, rate of bleb failure (encapsulation, flattening and/or vascularization) and the number of eyes submitted to surgical procedures after trabeculectomy (needling, 5-fluorouracil (5FU) or 2nd trabeculectomy). Results revealed that compared to trabeculectomy with MMC in POAG and trabeculectomy with MMC in PXG, trabeculectomy without MMC in PXG leads to higher IOP (preoperative mean ± standard deviation [SD] was 28.6 ± 5.4 mmHg in group 1, 32.2 ± 8.2 mmHg in group 2 and 26.1 ± 6.5 mmHg in group 3; and after one year was 13.9 ± 3.9 mmHg in group 1, 16.1 ± 5.9 mmHg in group 2 and 12.5 ± 4.0 mmHg in group 3); higher number of IOP lowering medications (preoperative mean ± SD was 3.1 ± 0.60 in group 1, 2.8 ± 0.81 in group 2 and 3.4 ± 0.76 in group 3; and after one year was 1.1 ± 1.1 in group 1, 1.1 ± 1.0 in group 2 and 0.33 ± 0.89 in group 3); higher prevalence of bleb failure (47% in group 1, 53% in group 2, and 18% in group 3); and increased participation in surgical procedures following trabeculectomy (47% in group 1, 57% in group 2, and 6% in group 3). We concluded that trabeculectomy without MMC in PXG had the worst surgical outcome. Thus, PXG appears to be a potential risk factor for filtration bleb failure. Therefore, it could be considered in surgical protocols of MMC application.

ABSTRACT

PURPOSE: Evaluate whether there are significant changes in choroidal thickness following trabeculectomy, and how they relate do the decline in intraocular pressure. METHODS: This was a prospective evaluation of 28 eyes who underwent Moorfields modified trabeculectomy. The choroidal thickness was measured via OCT with enhanced depth imaging, before surgery and 1 day, 1 week and 1 month after surgery. Measurements were taken at the fovea, 1000 µm temporal to the fovea and 1000 µm nasal to the fovea. The relationship between choroidal thickness and intraocular pressure was statistically evaluated. RESULTS: The mean intraocular pressure before surgery was 25.07 ± 4.64 mmHg; 8.57 ± 3.62 mmHg after 1 day; 10.36 ± 4.39 mmHg after 1 week and 13.71 ± 5.13 mmHg after 1 month. Mean choroidal thickness increased after trabeculectomy with maximal values at 1 week. The largest increase was found at the fovea, with an average before surgery of 253.54 ± 62.01 µm; 286.75 ± 64.20 µm at 1 day, 286.36 ± 63.14 µm at 1 week and 271.00 ± 60.31 µm at 1 month. Increase in choroidal thickness was significant 1 day and 1 week after surgery in the foveal (p = 0.012, p = 0.007) and temporal (p = 0.040, p = 0.000) locations and 1 week postoperatively on the nasal location (p = 0.016). None of them were significant at 1 month after surgery. Preoperative IOP and choroidal thickness were correlated at all macular locations (ρ = 0.449-0.525, p = 0.004-0.016) yet no correlation was found between increase in choroidal thickness and decline in intraocular pressure in the postoperative period. CONCLUSION: Choroidal thickness appears to increase temporarily after trabeculectomy and these changes were not correlated with the decline in intraocular pressure. Further research is required to fully understand this phenomenon.

OBJECTIVES:: Brazilians who are free of traditional atherogenic risk factors frequently suffer myocardial infarctions (MI). Calcified carotid artery plaque (CCAP) on panoramic images (PIs) has been shown to be a validated "risk indicator" of future MIs. The diagonal earlobe crease (DELC) is likewise a validated "risk indicator" of future MI. We sought to determine the prevalence of DELC+ among patients having CCAP+ on their PIs. METHODS:: In a masked cross-sectional study, three dentists evaluated PIs of patients (N = 481) over age 55 for presence of CCAP+. Among the CCAP+ cohort, the presence of DELC+ was determined by two additional dentists masked to the presence of CCAP+ on PIs. A control group (CCAP-) comprised of individuals with PIs devoid of plaque was similarly evaluated for evidence of DELC+. RESULTS:: A mixed-gender group (8.6%) of patients (N = 41; mean age 67.5 ± 6.8) evidenced CCAP+ on their PIs and approximately 88% of these individuals (N = 36) had concomitant DELC+. Among members of the control group (N = 41), 17 patients were DELC+. Our results demonstrated that among the full mixed-gender cohort (N = 82), the association between CCAP+ and DELC+ was statistically significant (p = 0.0001). However, when adjusting for gender, only the CCAP+ men were significantly (p = 0.00011) more likely to evidence DELC+ on clinical examination. CONCLUSIONS:: Males with atherosclerosis of their carotid arteries as evidenced by PIs displaying calcified atheromas are significantly more likely to evidence a DELC+ than those without an imaged atheroma.

ABSTRACT

Citrus species are widely related to antihyperalgesic and anti-inflammatory effects. The aim of this study was to investigate if treatment with ethanolic extract from peels of mature Citrus reticulata Blanco causes antihyperalgesic effects on the referred mechanical hyperalgesia in a model of dextran sulphate of sodium (DSS)-induced colitis in mice, as well as the possible oxidative damage in different regions of the brain induced by its inflammatory reaction. Antihyperalgesia (30 to 300 mg/kg) was investigated by behavioral response (frequency of response to von Frey filament stimulation) in Swiss mice, while damage to central nervous system was investigated through techniques that evaluated oxidative stress using male black C57 BL6 mice (n=8). Treatment of the animals with the extract (100 mg/kg) from days 3 to 5 after colitis induction reduced referred the mechanical hyperalgesia (32.6 ± 5.1) in relation to the control group (57.4 ± 2.0). Levels of lipid peroxidation or carbonyl proteins were augmented in colitis-induced animals in relation to the disease group. These results indicated an antihyperalgesic effect of the studied extract and a potential impairment of the central nervous system functioning caused by inflammation during colitis, which could be related to mental disorders observed in patients suffering of this pathology.

ABSTRACT

INTRODUCTION: Vogt-Koyanagi-Harada syndrome (VKHS) is an inflammatory systemic autoimmune disease principally affecting pigmented tissues in the ocular, auditory, integumentary and central nervous systems. Patients are generally women in the fourth decade of life. The prognosis is correlated mainly with the time between diagnosis and the start of treatment and number of recurrent episodes of inflammation. Most complications are mainly ocular. The purpose of this paper is to describe a clinical case of VKHS. MATERIAL AND METHODS: A child with a challenging clinical presentation in which the dermatological symptoms occurred before ocular manifestations. DISCUSSION AND CONCLUSION: VKHS is rare in children and can be a diagnostic challenge. It seemed interesting to share this case as an opportunity to expand our knowledge of the clinical spectrum of diseases and reflect about current diagnostic criteria.

Resumo Objetivo: A doença de Behçet é uma vasculite inflamatória sistémica, de etiologia desconhecida. Pode atingir virtualmente todos os sistemas, sendo as manifestações oculares comuns. O objectivo deste trabalho foi analisar doentes com doença de Behçet ocular quanto aos parâmetros demográficos, manifestações clinicas, terapêutica e principais complicações. Métodos: Estudo descritivo e retrospetivo, que incluiu 11 doentes com o diagnóstico de doença de Behçet ocular, segundo os critérios do ISG, observados na consulta de Inflamação Ocular do Hospital Prof. Doutor Fernando da Fonseca nos últimos 3 anos. Resultados: Identificaram-se 11 doentes, 5 homens e 6 mulheres, todos caucasianos. A idade média ao diagnóstico foi de 33,45±6,49 anos. A manifestação ocular foi o primeiro sinal da doença em 2 doentes. Em 72,7% dos casos as manifestações oculares foram bilaterais. Identificaram-se 4 casos de panuveíte, 3 de uveíte posterior, 2 de uveíte anterior, 1 de queratite e 1 caso de episclerite. O glaucoma e a catarata foram a complicação ocular mais frequente. O tratamento sistémico incluiu a corticoterapia oral em associação com terapêutica adjuvante imunossupressora, sendo os mais utilizados a azatioprina e a ciclosporina. Em 3 doentes houve necessidade de terapêutica biológica com infliximab para controlo da doença. Conclusão: A manifestação ocular mais frequente foi a panuveíte. Esta doença pode condicionar complicações oculares com diminuição irreversível da acuidade visual. A orientação destes doentes exige uma abordagem global e interdisciplinar.


Abstract Objective: Behçet's disease is a systemic inflammatory vasculitis of unknown etiology. It can virtually reach all systems, being common ocular manifestations. The aim of this study was to analyze patients with ocular Behcet's disease regarding demographic parameters, clinical manifestations, therapeutic approach and main complications. Methods: Descriptive and retrospective study, including 11 patients diagnosed with ocular Behcet's disease, according to the ISG criteria, observed in the Ocular Inflammation appointment of the Hospital Prof. Doutor Fernando da Fonseca in the last 3 years. Results: We identified 11 patients, 5 males and 6 females, all caucasian. The mean age at diagnosis was 33.45 ± 6.49 years. Ocular manifestation was the first sign of the disease in 2 patients. In 72.7% of the cases, ocular manifestations were bilateral. There were four cases of panuveitis, 3 of posterior uveitis, 2 of anterior uveitis, 1 of keratitis and 1 case of episcleritis. Glaucoma and cataract were the most frequent ocular complications. Systemic treatment included oral corticosteroid therapy in combination with adjuvant immunosuppressive therapy, and the most commonly used was azathioprine and cyclosporine. In 3 patients there was a need for biological treatment with infliximab to control the disease. Conclusion: The most frequent ocular manifestation was panuveitis. This disease can promote eye complications with an irreversible decrease in visual acuity. Orientation of these patients requires a global and interdisciplinary approach.

Aim: To present a challenging case of hypotony after trabeculectomy and its treatment. Case description: A 22-year-old woman with juvenile glaucoma underwent a conventional trabeculectomy with mitomycin C on the right eye (OD). In the immediate postoperative period, we observed a hyperfiltration bleb with hypotony refractory to conservative measures leading to hypotony maculopathy. A surgical revision with scleral flap resuture and conjunctival graft was performed with a satisfactory result and resolution of hypotony maculopathy. After two years, the patient complained of low visual acuity (VA) of the OD. During examination, we observed a fine and avascular bleb with Seidel and visualization of the underlying uveal tissue, an intraocular pressure (IOP) of 5 mmHg, and chorioretinal folds. A new revision of the trabeculectomy was performed. During the procedure, it was not possible to identify the scleral flap, so the fistula was closed with a patch of collagenous membrane derived from bovine pericardium (Tutopatch® graft). A good clinical evolution occurred. After 2 months, IOP was 15 mmHg without Seidel or changes in the fundus and VA was 20/20. After 8 months of follow-up, the IOP remains stable without further complaints. Conclusion: This case illustrates the difficulties faced in the management of a common complication of trabeculectomy and highlights some of the options available for its treatment. There are few reports of scleral melting after trabeculectomy. However, trauma and scleral necrosis associated with mitomycin are listed as the main causes. The use of a scleral patch derived from bovine pericardium allows effective suturing and closure of the aqueous leak.

ABSTRACT

Pneumatose intestinal (PI) consiste em um achado de imagem correspondente à presença de ar na parede intestinal. É um sinal infrequente, mais comum em idosos e sem predileção por sexo. Existem várias causas relatadas na literatura, sendo alguns casos reportados como idiopáticos. A modalidade diagnóstica de maior sensibilidade é a tomografia computadorizada de abdome. A maioria dos casos é assintomática, porém, uma pequena parcela pode evoluir com complicações graves, necessitando de intervenções imediatas. A inexistência de um consenso a respeito do manejo dos pacientes com PI, somada ao pouco conhecimento dos médicos acerca dessa enfermidade, resulta em um cenário clínico delicado. O objetivo deste artigo é relatar o caso de um paciente com PI, destacando o manejo terapêutico adotado.


Pneumatosis intestinalis (PI) consists in an image finding corresponding to the presence of air in the intestinal wall. It is an infrequent sign, more common in the elderly and without gender predilection. There are numerous causes described in the literature, and some cases are referred as idiopathic. The diagnostic modality with the highest sensibility is the computed tomography of the abdomen. The majority of cases are asymptomatic, but a small portion can evolve with severe complications, needing immediate interventions. The lack of a consensus regarding PI patients' management, along with the low knowledge level of physicians about the existence of this disorder, results in a delicate clinical scenario. The objective of this article is to report the case of a patient with PI, highlighting the adopted therapeutic management.

ABSTRACT

O envelhecimento é um processo universal e multifatorial que acontece em um contexto de grandes mudanças, principalmente na configuração dos arranjos familiares, com repercussão na busca por Instituições de Longa Permanência para Idosos (ILPIs). O sedentarismo é um fator marcante nas ILPIs, levando idosos rapidamente ao estado de dependência e incapacidade funcional. O objetivo deste estudo foi identificar a produção científica a respeito de fatores relacionados à capacidade funcional de idosos residentes de ILPIs, por meio de artigos indexados nas bases de dados eletrônicas LILACS e SciELO, no período compreendido entre 2010 e 2014. Foram selecionados 11 artigos. Foi observado que os estudos tendem a ser descritivos e transversais, sendo que apenas um artigo teve intervenção com a prática de atividade física. Outro fator notório foi a presença de doenças crônicas não transmissíveis, bem como a predominância do sexo feminino. Os estudos relatam também um menor desempenho cognitivo e uma aptidão funcional baixa, alta incidência de quedas, dependência nas atividades de vida diária e aumento de peso. A intervenção com equipe multidisciplinar é indispensável para a melhoria e manutenção da capacidade funcional e independência de idosos institucionalizados. Destaca-se a necessidade de desenvolvimento de novos estudos e produções científicas com essa temática, uma vez que a população idosa no Brasil deve continuar aumentando e, consequentemente, tende a crescer também o número de institucionalizações.


Aging is a universal and multifactorial process that takes place in a context of great change, especially in the setting of family arrangements, consequently with repercussions in the search for Geriatric Long-Term Care Facilities (GLTCFs). A sedentary lifestyle is a defining factor in GLTCFs, leading elderly quickly to a dependency status and disability. The aim of this study was to identify the scientific literature about factors related to functional capacity of elderly residents of GLTCFs through articles indexed in electronic databases LILACS and SciELO, in the period between 2010 and 2014. Eleven articles were selected. It was observed that the studies tend to be descriptive and cross-sectional, and only one article had intervention with physical activity. Another notable factor was the presence of chronic diseases, as well as the predominance of women. The studies also report a lower cognitive performance and low functional capacity, high incidence of falls, dependence in activities of daily living and weight gain. The intervention of a multidisciplinary team is essential for the improvement and maintenance of functional capacity and independence of institutionalized elderly. It is necessary to produce new studies on this theme, since the elderly population in Brazil should continue to increase and consequently the number of institutions also tends to grow.

AIM: To determine the relationship between macular thickness (MT) and visual field (VF) parameters, as well as with changes in the retinal nerve fiber layer (RNFL) thickness in patients with glaucoma and ocular hypertension (OH). MATERIALS AND METHODS: Cross-sectional statistical analysis of spectral domain optical coherence tomography (SD-OCT) compared with several VF parameters (mean defect - MD and loss variance - LV), in a nonrandom sample of 70 eyes from patients with glaucoma or OH. Statistical analysis was performed using Statistical Package for Social Sciences®. The correlation coefficient used was determined by Spearman correlation and the value of p < 0.05 was considered statistically significant. RESULTS: A significant correlation was seen between VF parameters and decrease in MT (MD: r = -0.363, p = 0.002; LV: r=-0.378, p = 0.001). The results were more significant when we compared the LV in the group with average MT 270 to 300 µm (r = -0.413, p = 0.015). Asymmetry between the superior macula and inferior macula correlated with LV (r = 0.432, p = 0.019) in the group with MT < 270 µm. There was also a significant correlation between thinning of superior-temporal and inferior-temporal RNFL and the decrease of the superior and inferior MT respectively (p < 0.001). CONCLUSION: Spectral domain optical coherence tomography measurements of retinal thickness in the macula correlate with VF parameters and RNFL parameters in glaucoma patients. This relationship was first demonstrated with static computerized perimetry made with Octopus 101®. These results can be a valuable aid for evaluating and monitoring of glaucoma patients, establishing a correlation between structure and function. Measurements of retinal thickness in the macula may be an additional instrument for early detection of structural changes and its correlation with functional defects. RESUMO Introdução: Na sarcoidose, as manifestações oculares são comuns e podem constituir a manifestação inicial da doença ou mesmo a única. O objetivo deste trabalho foi analisar os parâmetros demográficos, manifestações clínicas, terapêutica e principais complicações oculares em doentes com sarcoidose ocular. Métodos: Estudo descritivo e retrospectivo que incluiu doentes com o diagnóstico de sarcoidose ocular, observados nas consultas de Inflamação Ocular e de Doenças Auto-Imunes do Hospital Prof. Doutor Fernando Fonseca, no período entre 2009 e 2015. Resultados: Foram identificados 11 doentes com o diagnóstico de sarcoidose ocular, com predomínio do sexo feminino (54,5%) e caucasianos. A média da idade ao diagnóstico foi de 45±14 anos. A sarcoidose manifestou-se de forma exclusivamente ocular em 36% dos casos. O envolvimento ocular foi a manifestação inicial em 90,9% dos casos. Identificaram-se 9 casos de uveíte, 1 de esclerite anterior nodular e 1 de queratite intersticial. O tratamento com corticoterapia tópica foi realizado em 100% dos casos, sendo o tratamento único em apenas 1 doente. Nos restantes, foi necessário associar corticoterapia oral. Em 4 desses doentes, pela gravidade da doença e atingimento binocular, utilizou-se também corticoterapia pulsada endovenosa. O tratamento adjuvante imunossupressor mais frequentemente utilizado foi o metotrexato (45%). Um doente necessitou de terapia biológica com infliximabe para controle da doença. Conclusão: A manifestação ocular mais comum foi a uveíte, com predomínio da panuveíte. O tratamento mais utilizado e com maior taxa de controle da doença foi a corticoterapia sistêmica em associação com o metotrexato.


ABSTRACT Purpose: In sarcoidosis, ocular manifestations are common and can be the initial or even the only clinical manifestation. The aim of this study was to analyze the demographic parameters, clinical manifestations, treatment and the major ocular complications in patients with ocular sarcoidosis. Methods: We conducted a descriptive and retrospective study that included patients with the diagnosis of ocular sarcoidosis, followed by inflammatory ophthalmology and immune-mediated disease consults at the Prof. Doutor Fernando Fonseca Hospital, between 2009 and 2015. Results: Eleven patients with the diagnosis of ocular sarcoidosis were identified, with a predominance of females (54,5%) and Caucasians. The average age at diagnosis was 45 ± 14 years. Sarcoidosis was exclusively ocular in 36%. The first manifestation of sarcoidosis was eye disease in 90.9 % of cases. Nine cases of uveitis, one of nodular scleritis and one of interstitial keratitis were observed. Topical corticoid treatment was applied in 100% of cases, with only one achieving remission of the disease. Oral corticoid treatment was necessary in 10 cases, four of which needed a high dose methylprednisolone induction. Methotrexate was the adjunctive immunosuppressive treatment of choice in 45% of cases. There was one refractory case for conventional immunosuppressive therapy, having achieved remission with biologic agent infliximab. Conclusion: Uveitis was the commonest ocular manifestation, and there was a predominance of panuveitis. Systemic corticoid and methotrexate were the most used immunosuppressive treatments for maintaining the controlled stated of the disease.

OBJECTIVE: To identify the prevalence of iron-deficiency anemia in children aged less than 60 months and the conditioning factors in Minas Gerais, Brazil. METHODS: This cross-sectional study conducted from February 2007 to July 2008 in Minas Gerais included a random sample of 725 male and female children aged less than 60 months. A household survey was administered to the children's parents or guardians to collect socioeconomic, demographic, food security, and the child's anthropometric data. The hemoglobin level was determined by a portable hemoglobinometer. Anemia was considered present when hemoglobin levels were below 11.0 g/dL. Statistical analyses estimated the prevalence of anemia and its association with socioeconomic, demographic, and food security-related variables. The model was adjusted by multivariate logistic regression. The significance level was set at 5% (p<0.05). RESULTS: The prevalence of anemia was 37.4%. The greatest prevalence occurred in the age group 6 to 24 months (43.0%). The associated variables were being female (PR=1.43; p<0.026); age (PR=1.53; p<0.024); and not frequenting a daycare center (PR=2.41; p<0.001). In the multivariate regression model, the variables being female and not frequenting a daycare center remained in the model. CONCLUSION: The present population-based study found that in Minas Gerais, one-third (37.5%) of the children aged less than 60 months have iron-deficiency anemia. The results show that not frequenting a daycare center and being female are risk factors for anemia. .


OBJETIVO: Identificar a prevalência de anemia em crianças menores de 60 meses e seus fatores condicionantes, em Minas Gerais. MÉTODOS: Estudo transversal com amostra probabilística de 725 crianças menores de 60 meses de idade, de ambos os sexos, realizado entre fevereiro de 2007 e julho de 2008, em Minas Gerais. Por meio de inquérito domiciliar aplicado aos responsáveis pelas crianças, foram obtidas informações sobre as condições socioeconômicas, demográficas e situação de segurança alimentar, bem como obtidas as medidas antropométricas. A dosagem de hemoglobina foi analisada em hemoglobinômetro portátil, considerando-se como anemia valor <11,0 g/ dL. A análise estatística estimou a prevalência da anemia e sua associação com condições socioeconômicas, demográficas e segurança alimentar. O modelo foi ajustado por regressão logística multivariada. Adotou-se p<0,05 como nível crítico para definir significância estatística. RESULTADOS: A prevalência de anemia foi de 37,4%. A maior prevalência ocorreu na faixa etária de 6 a 24 meses (43,0%). As variáveis associadas foram: sexo feminino (RP=1,43; p<0,026); idade (RP=1,53; p<0,024) e não frequentar creche (RP=2,41; p<0,001). No modelo de regressão multivariada, permaneceram as variáveis sexo feminino e não frequentar creche. CONCLUSÃO: O presente estudo, de base populacional, mostrou que em Minas Gerais um terço das crianças (37,5%) com menos de 60 meses de idade apresentava anemia ferropriva. Os resultados revelaram que não frequentar creche e ser do sexo feminino configuram situações de risco à ocorrência de anemia. .

ABSTRACT

UNLABELLED: We report the case of an 83-year-old man who had had cataract surgery in both eyes and was being treated with timolol and dorzolamide in the right eye. Goldmann tonometry was 28 mm Hg in the right eye and 14 mm Hg in the left eye. Biomicroscopy of the right eye revealed pigment dispersion on the corneal endothelium, trabecular meshwork, and intraocular lens (IOL), as well as iris transillumination defects. After pupil dilation, a ciliary sulcus-implanted 1-piece IOL and a ruptured posterior capsule could be seen. Optical coherence tomography showed a diminished nerve fiber layer. Exchange for a 3-piece IOL was performed, but the intraocular pressure (IOP) remained high. For that reason, a trabeculectomy was performed. The final IOP was 12 mm Hg, and the visual fields were stable. This case highlights the importance of IOL choice for sulcus implantation. ABSTRACT

OBJECTIVE: To report the clinical presentation of malarial retinopathy in an adult, emphasizing the importance of this diagnosis for the clinical suspicion and prognosis of cerebral malaria. METHODS: A 39-year-old caucasian man presented with hemolytic anemia, thrombocytopenia, acidemia and acute renal failure, developing severe encephalopathy. The diagnosis of Plasmodium falciparum malaria was done and after systemic stabilization, the patient noticed a central scotoma in the left eye. Ophthalmological examination revealed retinal features of malarial retinopathy. RESULTS: At one-month follow-up, the patient had improved his systemic condition and the left eye scotoma had disappeared. Visual acuity was 20/20 in both eyes and on examination almost all lesions had regressed. CONCLUSION: Malarial retinopathy is a diagnostic factor and a prognosis indicator of severe P. falciparum infection, usually with brain involvement. The knowledge of the ophthalmological features associated with severe malaria, which is more frequent in children but can also occur in adults, becomes imperative in order to reduce the risk of neurologic sequelae and associated mortality.

ABSTRACT

Studies have shown that including food waste as a co-digestion substrate in the anaerobic digestion of livestock manure can increase energy production. However, the type and inclusion rate of food waste used for co-digestion need to be carefully considered in order to prevent adverse conditions in the digestion environment. This study determined the effect of increasing the concentration (2%, 5%, 15% and 30%, by volume) of four food-processing wastes (meatball, chicken, cranberry and ice cream processing wastes) on methane production. Anaerobic toxicity assay (ATA) and specific methanogenic activity (SMA) tests were conducted to determine the concentration at which each food waste became toxic to the digestion environment. Decreases in methane production were observed at concentrations above 5% for all four food waste substrates, with up to 99% decreases in methane production at 30% food processing wastes (by volume).

ABSTRACT

PURPOSE: To describe a clinical case and literature review of Whipple disease. METHODS: A 65-year-old male with bilateral decreased visual acuity for 3 weeks as well as bilateral hypoacusia, vertigo, disequilibrium, headache and decreased strength in the right upper limb for 4 months. The clinical work-up revealed a bilateral panuveitis and an ischemic cerebellar stroke. RESULT: The diagnosis of Whipple disease was confirmed by histopathological analysis of adenopathy. The patient was treated with cortico-antibiotic therapy with significant clinical improvement. CONCLUSION: Although rare, Whipple disease is potentially fatal if left untreated, it must be always be taken into consideration before any panuveitis of an unknown cause, even in the absence of gastrointestinal symptoms.

ABSTRACT

Este artigo apresenta um recorte da pesquisa "1º Inventário de Saúde do Trabalhador 2009", desenvolvida pela Coordenação Geral de Saúde do Trabalhador do Ministério da Saúde. Buscou-se identificar lacunas e potencialidades da implantação da Rede Nacional de Atenção Integral em Saúde do Trabalhador (Renast), focando as ações de Vigilância em Saúde do Trabalhador (Visat) e variáveis estruturantes dos Centros de Referência em Saúde do Trabalhador (Cerest), como o financiamento, a equipe profissional e a participação dos trabalhadores. Analisaram-se as atividades de Visat e destacaram-se as articulações dos Cerests intra e intersetoriais, entre elas instâncias regionais da Previdência Social e universidades, além das internas ao SUS, sobretudo com instâncias da Vigilância em Saúde. Os resultados demonstraram que as ações de Visat estão acontecendo na maioria dos Cerests (58,4%) e que há uma adequação estrutural e da equipe, além de relativa participação dos trabalhadores nas Comissões Intersetoriais de Saúde do Trabalhador (CIST) dos Conselhos de Saúde. São apresentadas considerações para a construção de parâmetros para futuras pesquisas e recomendações visando ao fortalecimento da Renast. Destaca-se a necessidade de intensificação da capacidade de análise das informações decorrentes das ações de vigilância e do registro de casos.


This paper presents partial records of the first report on worker's health 2009 produced by the Workers' Health General Coordination (CGSAT) of the Ministry of Health. It identifies gaps and potentialities in implanting the Brazilian National Network for Worker's Health Integrated Care (Renast). It is focused on actions performed by the Worker's Health Surveillance (VISAT) and structural indexes of the Workers' Health Reference Centers (Cerest), such as its financial flow, its professional team and the workers' participation. It also presents the VISAT activities carried out and the inter and intra sector articulations of the Cerests, among then, universities as well as the regional spheres of the Brazilian Social Security Ministry. The results evidences that VISAT actions are taking place in most Cerests (58,4%), and show that the services physical structure and technical teams are appropriate and also that there is a reasonable participation of workers in the Workers' Heath Inter-sector Commissions (CIST) of the Regional Health Committees. The paper presents parameters viewing future research and recommendations aimed at empowering Renast. Finally it emphasizes the need to intensify the capacity to analyze information resulting from surveillance actions and case records.
