ABSTRACT

Autosomal dominant epilepsy with auditory features (OMIM 600512) is characterised by focal seizures with distinctive auditory auras and/or ictal aphasia. We describe a 17-year-old girl with recurrent attacks of ictal aphasia and rare nocturnal convulsions. She had a four-generation paternal family history of epilepsy. Her father and aunt perceived bells ringing at the onset of seizures. Sequence analysis of the leucine-rich glioma-inactivated 1 (LGI1) gene identified a novel heterozygous variant in the proband and her father. LGI1-related genetic epilepsy has a benign clinical course with a favourable response to anti-seizure medications. Auditory or vertiginous seizures may be mistaken for peripheral audio-vestibular symptoms, while complex auditory ictal symptoms may be misattributed to primary psychiatric disorders. Recognising this distinctive inherited syndrome should prompt targeted analysis of the LGI1 gene.

ABSTRACT

Campylobacter jejuni infection is well-known to precipitate Guillain-Barré syndrome through an immune-mediated attack on the peripheral nervous system. Molecular mimicry between C. jejuni lipo-oligosaccharides on the surface of infectious agents and human gangliosides in the peripheral nerves induces cross-reactive immune responses. Although gangliosides also occur in the central nervous system (CNS), autoimmune CNS disorders rarely follow Cjejuni infections. However, longitudinally extensive transverse myelitis commonly has a parainfectious cause, triggered by a wide range of micro-organisms including viruses and bacteria. We report a patient who developed longitudinally extensive transverse myelitis after Cjejuni enteritis associated with antiganglioside antibodies.

ABSTRACT

INTRODUCCIÓN: una sólida educación a los pacientes en temas relacionados con la diabetes y la monitorización de la glucosa, se han asociado con mejor controlmetabólico, mayor entendimiento de la enfermedad, auto confianza y mejor cuidado clínico.MATERIALES Y MÉTODOS: estudio de corte transversal, en la jornada No más ciegos por diabetes en Antioquia 2005. Se diligenció un cuestionario indagando acerca de la monitorización, tipo de diabetes, tratamiento y complicaciones; se midieron la glucometría, el peso, la talla y el perímetro abdominal.RESULTADOS: se incluyeron 210 pacientes, de los cuales 36,6 por ciento no saben que tipo de diabetes tienen, 59,7 por ciento no saben que es la hemoglobina glicosilada y 25,8 por ciento saben que este examen debe realizarse cada 3 a 4 meses, sin embargo, solo se le había realizado a siete pacientes (3,3 por ciento) con la frecuencia ideal. El 36,4 por ciento tienen glucómetro y 60,6 por ciento de los pacientes saben que se deben realizarse una evaluación oftalmológica anual.En los pacientes con Diabetes Mellitus tipo 2 el 26,2 por ciento tuvo glucometrías postprandial menores de 140 mg/dl.Las complicaciones informadas con mayor frecuencia fueron el compromiso renal y descompensación aguda en los Diabéticos tipo 1, la dislipidemia y la neuropatía periférica en los diabéticos tipo 2.
