ABSTRACT

Objectives This study evaluated the radiographic progressions of scoliotic curves higher than 40° in patients with adolescent idiopathic scoliosis (AIS). These subjects waited for the surgical procedure while elective surgeries were on hold during the COVID-19 pandemic. In addition to radiographic progressions, this study described the quality of life of these patients. Methods This study is a retrospective cohort assessing 29 AIS patients with surgical indications registered in the Brazilian public healthcare service. We compared the scoliotic radiographic measurements in two moments: at the beginning of the interruption of elective surgeries due to the COVID-19 pandemic and when these procedures resumed. Results When comparing the radiographic measurements before and after the suspension of assessments for elective surgeries, we observed a significant increase in main curve angles ( p < 0.001), with variations ranging from 0 to 68° and a median value of 10°. In secondary curves, we observed an increase in angles from the proximal thoracic ( p < 0.001) and lumbar ( p = 0.001) regions. However, the increase in the main thoracic region was not significant ( p = 0.317). Conclusion The suspension of elective surgeries for AIS resulted in a significant increase in the radiographic values of patients' spine deformities. This increase harmed the quality of life of these subjects and their families.

Resumo Objetivos Neste estudo foram avaliadas as progressões radiográficas das curvas escolióticas, acima de 40 graus em pacientes com escoliose idiopática do adolescente (EIA). Os indivíduos analisados aguardavam o procedimento cirúrgico, em período de suspensão das cirurgias eletivas, durante a pandemia de covid-19. Além das progressões radiográficas, nestapesquisafoi descritaaqualidadedevidadestes pacientes. Métodos O artigo refere-se a um estudo de coorte retrospectivo, que avaliou 29 pacientes com EIA com indicação cirúrgica, todos cadastrados no serviço público de saúde brasileiro. As medidas radiográficas escolióticas dos pacientes foram comparadas em dois momentos: no início do período da interrupção de cirurgias eletivas, devido à pandemia de covid-19, e logo após a liberação destas. Resultados A partir da comparação das medidas radiográficas entre as avaliações pré e pós suspensão das cirurgias eletivas, observamos o aumento significativo dos valores angulares da curva principal (p < 0,001), com variações entre 0 e 68°, e mediana de 10°. Em relação às curvas secundárias, observamos um aumento dos valores angulares da região torácica proximal (p < 0,001) e lombar (p = 0,001). Entretanto, o aumento da região torácica principal não foi considerado significativo (p = 0,317). Conclusão A suspensão das cirurgias eletivas, para a correção da EIA resultou em um aumento significativo no valor radiográfico das deformidades das colunas dos pacientes, fator que promoveu um impacto negativo na qualidade de vida de pacientes e familiares.

ABSTRACT

BACKGROUND: Core myopathies are a clinically and genetically heterogeneous group of congenital myopathies with the common defined histopathological feature of focally reduced oxidative activity on muscle biopsy. It has a low incidence, however, recent articles show broad clinical spectrum, suggesting that the real incidence should be considerably larger than previously described. Due to the important association between scoliosis and paravertebral muscle imbalance, numerous authors study, by biopsy of the spinal rotator muscles, potential changes that may elucidate the etiology of adolescent idiopathic scoliosis. CASE PRESENTATION: Two patients have been followed at Spine Group of Department of Orthopedics at Federal University of São Paulo, with an initial diagnosis of idiopathic scoliosis. Both patients had clinical and radiological findings compatible with it. The patients authorized, through the Term of Consent, intraoperative biopsy of muscle multifidus from the apex of the thoracic curve on concave and convex sides. After muscle biopsy was performed a histopathological analysis. As regard to the histopathological features: in both patients were identified, the presence of core structures in extensive areas with reduced oxidative activity running along the muscle fiber. CONCLUSIONS: All patients with 'idiopathic' scoliosis deserve a careful neurological evaluation, even if they have minimal muscle symptoms in the extremities. The frequent occurrence of scoliosis in patients with CORE Myopathies, supports the thesis that the change in the paravertebral muscle fiber must be the underlying pathogenic factor in scoliosis and may help us understand the onset and progression of curves in patients previously diagnosed with idiopathic scoliosis.

ABSTRACT

Morphological, biochemical, and histopathological alterations in the paraspinal skeletal muscle of patients with adolescent idiopathic scoliosis (AIS) have been extensively reported. We evaluated rotator muscle fibers from the apex vertebra of AIS patients through histological and immunohistochemical analysis. A population of 21 female AIS patients who underwent corrective surgery between 2010 and 2013 had biopsies taken from the paraspinal muscle in the convex and concave sides of the thoracic curve apical vertebra. Serial sections were stained following routine protocols for hematoxylin and eosin (HE), Sudan red, Gomori trichrome, NADH, ATPase, and cytochrome oxidase. We assessed muscular atrophy and hypertrophy, fatty proliferation, endomysial and perimysial fibrosis, the presence of hyaline fibers, mitochondrial proliferation, muscular necrosis, nuclear centralization, and inflammation. Two independent professionals evaluated the slices. The thoracic curves had an average Cobb angle of 68 degree. Comparative analysis of the concave and convex sides was performed with McNemar test at a significance level of 5%. Results showed significant differences in both endomysial and perimysial fibrosis and fatty involution between the two sides of the apex vertebra. Paraspinal muscles in the concave side of the scoliosis apex had significantly more fibrosis and fatty involution. However, both sides showed signs of myopathy, muscular atrophy due to necrosis, presence of hyaline fibers, and mitochondrial proliferation.

ABSTRACT

Objetivo: O polimorfismo da enzima conversora de angiotensina (ECA) I/D e da α-actinina 3 (ACTN3) R577X está relacionado a variações na função do musculoesquelético. O objetivo deste trabalho foi avaliar a distribuição destes polimorfismos em uma família com múltiplos membros com escoliose idiopática do adolescente. Métodos: avaliação de 25 indivíduos de uma família, com múltiplos membros com escoliose idiopática, por meio da coleta de 10 ml de sangue para extração de DNA. A genotipagem do polimorfismo I/D do gene da ECA e R577X do gene da ACTN3 foi realizada utilizando sistema de 2 iniciadores (‘‘primers’’) específicos, para classificar os indivíduos em homozigotos ou heterozigotos. Resultados: Em relação ao polimorfismo da ECA encontrou-se 19 indivíduos DD (76%) e 6 ID (24%). A prevalência do alelo D foi de 88% e do alelo I foi de 12%. Quanto ao polimorfismo da ACTN3 observou-se seis indivíduos RR (24%), 11 RX (44%) e 8 XX (32%). A prevalência do alelo R foi 23 (46%) e do alelo X foi 27 (54%). Conclusão: observou-se diferença entre a distribuição do polimorfismo da ECA e da ACTN3 na família estudada. Ao avaliar o polimorfismo da ECA notou-se maior prevalência do alelo D em relação ao alelo I. Nível de Evidência III, Estudo Clinico, Transversal.
