ABSTRACT

Postoperative gastrointestinal disorder (POGD) was a common complication after surgery under anesthesia. Strategies in combination with Traditional Chinese Medicine and Western medicine showed some distinct effects but standardized clinical practice guidelines were not available. Thus, a multidisciplinary expert team from various professional bodies including the Perioperative and Anesthesia Professional Committees of the Chinese Association of Integrative Medicine (CAIM), jointly with Gansu Province Clinical Research Center of Integrative Anesthesiology/Anesthesia and Pain Medical Center of Gansu Provincial Hospital of Traditional Chinese Medicine and WHO Collaborating Center for Guideline Implementation and Knowledge Translation/Chinese Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) Center/Gansu Provincial Center for Medical Guideline Industry Technology/Evidence-based Medicine Center of Lanzhou University, was established to develop evidence-based guidelines. Clinical questions (7 background and 12 clinical questions) were identified through literature reviews and expert consensus meetings. Based on systematic reviews/meta-analyses, evidence quality was analyzed and the advantages and disadvantages of interventional measures were weighed with input from patients' preferences. Finally, 20 recommendations were developed through the Delphi-based consensus meetings. These recommendations included disease definitions, etiologies, pathogenesis, syndrome differentiation, diagnosis, and perioperative prevention and treatment.

ABSTRACT

Transcription factor 7-like 2 (TCF7L2) polymorphism plays an essential role in the occurrence and development of patients living with diabetes, but the current conclusions are inconsistent on the relationship between TCF7L2 polymorphism and the risk of diabetic nephropathy. This meta-analysis aims to explore the exact association between TCF7L2 rs7903146 locus polymorphism and susceptibility to diabetic nephropathy. PubMed, Embase, Web of Science, Cochrane Library, China National Knowledge Infrastructure (CNKI), and China Wanfang databases were searched for studies on the relationship between single nucleotide polymorphism at TCF7L2 rs7903146 locus and susceptibility to diabetic nephropathy until January 10, 2022. The data were analyzed by Stata 15.0 software. A total of 7 articles were included, covering 1443 patients with diabetic nephropathy and 2129 diabetic non-nephropathy patients. The results showed that allele C at TCF7L2 rs7903146 locus, compared to allele T, the pooled odds ratio (OR)=0.69 (95% CI: 0.56-0.85, p≤0.05). In the dominant gene inheritance model, recessive gene inheritance model, homozygous genetic model, and heterozygous genetic model, the pooled OR was 0.47 (95% CI: 0.36-0.61), 0.63 (95% CI: 0.54-0.73), 0.39 (95% CI: 0.29-0.51), and 0.59 (95% CI: 0.45-0.78), respectively, and the differences were statistically significant. In conclusion, TCF7L2 rs7903146 polymorphism is associated with susceptibility to diabetic nephropathy. Allele T and genotype TT can increase the risk of diabetic nephropathy.

ABSTRACT

OBJECTIVE: To study the relationship between serum vitamin D and fasting blood glucose in children and adolescents, and its influence on overweight and obesity. METHODS: Using a multi-stage stratified cluster sampling method, 13 districts and counties in Shandong Province were selected as survey points, and a total of 26 elementary schools, 26 junior high schools and 13 high schools were selected. Questionnaire surveys and physical examination were conducted on children and adolescents aged 6-17.Serum vitamin D and fasting blood glucose were uniformly measured. The "BMI Classification Standard for Overweight and Obesity Screening for Chinese School-age Children and Adolescents" was used to determine overweight and obesity. Statistical method adopt t-test and generalized linear regression model for analysis. RESULTS: A total of 3562 children aged 6-17 years were investigated, 2666 were healthy, 446 were overweight, 450 were obese, including 131 peripheral obesity and 319 abdominal obesity. The fasting blood glucose level of the 13-17-year-old group was significantly higher than that of the 6-12-year-old group(t=-3.13, P=0.002), and the male was significantly higher than that of the female(t=7.87, P& lt; 0.001). In the healthy and obesity group, there was a negative correlation between serum vitamin D and fasting blood glucose(P& lt; 0.05). In overweight group, serum vitamin D was not significantly related to fasting blood glucose(P& gt; 0.05). Serum vitamin D and fasting blood glucose were negatively correlated with healthy bodies in female group and 13-17 years old group, and the abdominal obesity bodies in the 6-12 year old group(P& lt; 0.05). CONCLUSION: The fasting blood glucose of adolescents aged 13-17 is significantly higher than that of children aged 6-12, and the fasting blood glucose of men is significantly higher than that of women. Different gender, age, and overweight and obesity status have an impact on the relationship between serum vitamin D and fasting blood glucose.

ABSTRACT

OBJECTIVE: To understand the gender characteristics, HIV/AIDS related knowledge awareness and behaviors of transgender women. METHODS: A questionnaire survey was conducted among the transgender women recruited through snowball sampling in Jinan in 2014, and descriptive epidemiologic analysis was conducted on the survey results. RESULTS: A total of 55 transgender women were surveyed, all of them were male physically and female psychologically. Serious gender conflict occurred in 27 subjects (49.1%), and very serious gender conflict occurred in 8 subjects (14.5%). Thirty subjects dressed up as a man in social life, accounting for 54.5%; 25 subjects dressed up as a women in social life, accounting for 45.5%. The average awareness rate of HIV/AIDS related knowledge was 57.9%(22/38). The awareness of knowledge about AIDS associated behaviors, such as multi sex partner and anal sex, was poor. For the lovers or sexual partners, 58.2% of the subjects (32/55) would choose males and 50.9% of the subjects (28/55) had chosen males, and for the sex partner at latest sex, 63.6%(35/55) of the subjects had chosen males. Up to 56.3% of the subjects had sex with casual sexual partners (net friends and partners of one-night stand) at latest sex behavior. Among the subjects surveyed, 18(32.7%) never used condoms; 29(52.7%) used condoms occasionally; 4(7.3%) used condoms frequently and 4(7.3%) used condoms at each sex. CONCLUSIONS: AIDS associated high risk behaviors were common among the transgender women, such as unprotected anal sex, multiple sexual partners, frequent sex and poor condom use. It is necessary to conduct the study of the HIV infection prevention in transgender women.
