ABSTRACT

Field cancerization (FC) is a chronic disease involving multiple clinical and subclinical actinic keratoses (AK) on large photo-exposed surfaces with multifocal areas of dysplasia and precancerous changes. Patients and treatment must be properly monitored and managed to avoid aggravation and progression of the disease. Management of actinic keratoses includes lesion-directed treatments, such as cryotherapy and field-directed therapies. Field-directed therapies may have the potential to address subclinical damage, reduce AK recurrence rates and potentially reduce the risk of squamous cell carcinoma development. Multiple studies have demonstrated the efficacy of field-directed treatments, including 5-fluorouracil, photodynamic therapy, imiquimod, chemical exfoliation with trichloroacetic acid and diclofenac gel, for multiple AK and FC. The choice of therapy should be based on multiple factors, such as efficacy, tolerability, patient risk profile, costs and cosmetic results. Management of AK includes not only treatment but also prevention. Medical devices, such as sunscreens containing liposome-encapsulated DNA repair enzymes, can repair DNA damage associated with chronic UV radiation and reduce the number of new AK lesions. Here we provide therapeutic pearls and expert opinions on the treatment of AK and FC (as monotherapy or in combination) with the overall aim to achieve better, faster, and well-tolerated clinical responses.

ABSTRACT

Medullary carcinoma of the thyroid gland accounts for only 5-10% of thyroid carcinomas. Also, metastases to the skin of malignant tumors are infrequently (2-9% of patients). In the case herein reported in a 64-year old woman, a metastatic nodule on the scalp was the presenting clinical manifestation of a medullary thyroid carcinoma. A comprehensive review of the literature was conducted for similar cases using PubMed. Only 18 cases of cutaneous metastases of medullary thyroid carcinoma have been previously reported in the literature, but skin lesions were the presenting complaint of the thyroid neoplasm in only three.

ABSTRACT

Introducción: con la denominación 'cara roja' diferentes autores se han referido a dermatitis de la cara con una hipersensibilidad, reactividad e intolerancia que o no tiene diagnóstico definitivo o es una superposición de diferentes cuadros clínicos. Su manejo terapéutico es difícil. El objetivo de este estudio es confirmar la evidencia de recientes estudios que apuntan la eficacia de la aplicación tópica de un preparado cosmético cuyo principal componente es furfurilglucitol con actividad antioxidante y antiinflamatoria. Métodos: se incluyeron 30 pacientes con dermatitis de la cara leve o moderada en un estudio abierto, prospectivo, de 30 días de seguimiento, que siguieron tratamiento con un preparado de furfurilglucitol aplicado dos veces al día. Se valoró la evolución de las lesiones a los 15 y 30 días de iniciarse el tratamiento. Resultados: aunque la muestra de 28 pacientes no es muy numerosa permite sacar algunas conclusiones. Sólo un paciente abandonó el tratamiento por falta de efectividad y un caso por intolerancia. En general la valoración del investigador fue muy positiva, tanto en la evolución de cada uno de los síntomas valorados como en la impresión clínica global de la enfermedad. No hubo una respuesta parecida por parte del paciente, apreciando un resultado estético escaso. Conclusiones: considerando la levedad del proceso y la exigencia de este tipo de pacientes se consiguen bajos porcentajes de cumplimiento de la pauta prescrita por parte del paciente en cuanto se consigue algo de mejoría (AU)
