ABSTRACT

CONTEXT: Fragility fractures increase risks for future fractures, morbidity, and mortality. Available pharmacotherapy for underlying osteoporosis is safe and effective but underused. OBJECTIVE: To improve pharmacotherapy rate representing secondary prevention of osteoporotic fractures. METHODS: This single-center, observational, follow-up study included patients with fragility fractures admitted to the Massachusetts General Hospital between February 2016 and December 2019. For patients admitted to the orthopedics service with fragility fracture, the Massachusetts General Hospital Fracture Liaison Service (FLS) was systematically consulted. Initial outpatient follow-up with FLS was established in conjunction with the orthopedic postoperative follow-up visit. Patients at risk for failing timely outpatient follow-up were administered zoledronic acid (ZA) during the index fracture hospitalization. The main outcome measures were percentage of patients with fragility fracture(s) started on pharmacotherapy for osteoporosis and average length of stay and 30-day readmission rate of patients treated with ZA. RESULTS: Compared with baseline (8-11%) and reference (5-20%) rates, integration of FLS to the orthopedics service, along with appropriate inpatient administration of ZA, increased the pharmacotherapy rate to 70% (412/589) among eligible patients with verified treatment status. Inpatient ZA administration neither affected the average length of stay nor 30-day readmission rate. Treatment status of 37.9% (471/1240) of the study patients remained unknown due to lack of or unknown follow-up. CONCLUSION: Integration of a FLS and orthopedics services along with inpatient ZA administration improved the osteoporosis pharmacotherapy rate among patients with fragility fracture(s) who often had obstacles for outpatient follow-up.

ABSTRACT

Akkermansia muciniphila is a Gram-negative anaerobic mucus-layer-degrading bacterium that colonizes the intestinal mucosa of humans and rodents. Metagenomic data have shown an inverse correlation between the abundance of A. muciniphila and diseases such as inflammatory bowel disease (IBD), obesity, and diabetes. Thus, in recent decades, the potential of this bacterium as an immunomodulatory probiotic for autoimmune and chronic inflammatory diseases has been explored in experimental models. Corroborating these human correlation data, it has been reported that A. muciniphila slows down the development and progression of diabetes, obesity, and IBD in mice. Consequently, clinical studies with obese and diabetic patients are being performed, and the preliminary results are very promising. Therefore, this mini review highlights the main findings regarding the beneficial roles of A. muciniphila and its action mechanisms in autoimmune and chronic inflammatory diseases.

ABSTRACT

Pharmaceutical industry-led access programs are growing in number globally and are increasingly adopting a hybrid approach intended to generate commercial and social value in parallel. We developed and applied a new conceptual framework in a descriptive analysis of observable indicators measuring commercial and social value for 91 programs registered in the Access Observatory. We found that most programs had features consistent with the generation of commercial value, directly through revenue generation (50.0%), or indirectly by creating competitive advantage (70.3%). We also found that most programs were implemented in countries where the company has commercial products registered (85.5%). While many programs had features consistent with the generation of social value, it was difficult to ascertain the level of that value because most did not share data (83.5%) and had not been evaluated (74.7%). Future efforts by the global health community and the pharmaceutical industry should focus on strengthening measurement and reporting on commercial and social indicators of industry-led access programs.

Resumo Este artigo tem como objetivo identificar quais sistemas estaduais de ensino preveem a organização do ensino em ciclos escolares e o regime de progressão continuada dos alunos. Pretende-se, ainda, investigar em que medida essas previsões têm sido implantadas nas escolas. Para tanto, além de consulta direta às secretarias de educação e da análise da legislação estadual, recorre-se a dados do Censo Escolar. Os resultados nos permitem concluir que, na educação básica das redes estaduais brasileiras, há predomínio da organização seriada e dos regimes de progressão regular ou parcial dos alunos. Não obstante, há sistemas de ensino que apresentaram maior adesão às políticas de não repetência de forma consistente ao longo do tempo, como os dos estados de Minas Gerais, São Paulo e Mato Grosso.


Resumen Este artículo tiene el propósito de identificar qué sistemas estaduales de educación prevén la organización de la enseñanza en ciclos escolares y el régimen de progresión continuada de los estudiantes. Résumé Cet article a pour but identifier quels systèmes d'enseignement publique1 prevoient l'organisation de l'enseignement en cycles scolaires et le régime de progression continue des élèves. On souhaite encore examiner dans quelle mesure ces prévisions sont mises en place dans les écoles. Pour le faire, en plus de la consultation directe des sécrétariats d'éducation et de l'analyse de la législation de l'État, on s'appuie sur les données du Recensement Scolaire. Les résultats nous permettent de conclure que dans l'éducation de base des réseaux publiques au Brésil, domine l'organisation par séries et les régimes de progression régulière ou partielle des élèves. Cependant, certains systèmes d'enseignement ont présenté une plus grande adhésion aux politiques de non redoublement régulièrement au long des années, comme les États de Minas Gerais, São Paulo et Mato Grosso.


Abstract This paper aims to identify the state education systems whose regulation establishes non-repetition as a rule and emphasizes educational cycles (units longer than a grade) as the main subdivisions of basic education. It also investigates the extent to which those regulations have been implemented. To that end, we analyzed Brazilian states' legislation on education and data from the School Census. We found that repetition and an emphasis on grades over cycles are characteristics of state education systems in most Brazilian states. However, non-repetition policies were found to be adopted to a greater extent in the states of Minas Gerais, São Paulo, and Mato Grosso.1

ABSTRACT

OBJECTIVE: This study investigated the efficacy of GnRH agonists concomitantly with transient aromatase inhibitor and tranexamic acid to treat women with uterine arteriovenous malformations (AVMs) associated with abnormal uterine bleeding (AUB) to preserve fertility and determine reproductive outcome. METHODS: This was a prospective cohort study in a tertiary centre. Doppler ultrasound demonstrated AVM in 19 women with AUB 1-28 weeks following spontaneous or therapeutic abortion and in one woman 4 years after normal pregnancy while taking an oral contraceptive. On the basis of experience from the first three cases, 17 women were treated with tranexamic acid (1 g three times daily orally for 5 days), a GnRH agonist (3.75-11.25 mg, for 1-3 months), plus an aromatase inhibitor (letrozole 2.5 mg once daily for 5days) with the initial injection of GnRH agonist. Two women required blood transfusion, and one required uterine tamponade with Foley catheter balloon in the first 48 hours to control heavy bleeding (Canadian Task Force Classification II-2). RESULTS: All 20 AVMs resolved within 1-3 months of treatment. Of 16 women who attempted pregnancy, all (100%), including two who had uterine artery embolization (one after hysteroscopic septoplasty), conceived spontaneously with 18 live births. Two women are using contraception (one taking an oral contraceptive, one using a levonorgestrel intrauterine system), and one 40-year-old is not using contraception. One woman had hysteroscopic endometrial ablation followed by vaginal hysterectomy for AUB at 1 and 2 years later. CONCLUSION: A GnRH agonist in combination with transient aromatase inhibitor and tranexamic acid is an effective management strategy to treat and maintain reproduction in women with AVMs associated with AUB.

OBJECTIVE: A 4.5-year follow-up study was conducted to characterize baseline verbal episodic memory (VEM) and its behavior and to assess the effects of relapsing-remitting multiple sclerosis (RRMS) on this domain. METHODS: Twenty-nine patients with RRMS underwent two neuropsychological assessments performed an average of 4.5 years apart. Twenty-six control participants underwent a single neuropsychological assessment. A significance level of p < 0.005 was adopted to denote a significant difference between the groups on the Mann Whitney and Wilcoxon paired statistical analyses. RESULTS: No statistical difference was found in the results of the VEM tests between the first and second neuropsychological assessments of the patients. However, a statistical difference was evident between the patient and control groups in the results of the VEM tests. CONCLUSION: The patient group showed changes in the VEM relative to the control group. After approximately 4.5 years of disease, the patient performance on the VEM stabilized or improved.

RESUMO Objetivo: Neste estudo, propomos a caracterização da Memória Episódica Verbal (MEV) basal e o seu comportamento após o período de 4,5 anos de doença, a fim de avaliar o efeito da EMRR neste domínio. Métodos: Vinte e nove pacientes com EMRR foram submetidos a duas avaliações neuropsicológicas realizadas entre um intervalo de tempo médio de 4,5 anos. Vinte e seis controles foram submetidos à avaliação neuropsicológica única. Considerou-se nível de significância p <0,005 para delinear diferença significante entre os grupos nas análises estatísticas Mann Whitney e Wilcoxon pareado. Resultados: Não houve diferença estatística nos resultados dos testes de MEV entre a primeira e segunda avaliação neuropsicológica realizada pelos pacientes. Houve discrepância estatística nos resultados dos testes de MEV entre o grupo dos pacientes e controles. Conclusão: O grupo de pacientes apresentou alterações de MEV quando comparado aos controles. Após 4,5 anos aproximadamente os pacientes estabilizaram ou melhoraram seu desempenho em MEV.

OBJECTIVE: The Trail Making Test (TMT) and Stroop Test (ST) are attention tests widely used in clinical practice and research. The aim of this study was to provide normative data for the adult Brazilian population and to study the influence of gender, age and education on the TMT parts A and B, and ST cards A, B and C. METHODS: We recruited 1447 healthy subjects aged ≥18 years with an educational level of 0-25 years who were native speakers of Portuguese (Brazilian). The subjects were evaluated by the Matrix Reasoning and Vocabulary subtests of the Wechsler Adult Intelligence Scale-III, along with the TMTA, TMTB and ST A, B and C. RESULTS: Among the participants, mean intellectual efficiency was 103.20 (SD: 12.0), age 41.0 (SD: 16.4) years and education 11.9 (SD: 5.6) years. There were significant differences between genders on the TMTA (p=0.002), TMTB (p=0.017) and STC (p=0.024). Age showed a positive correlation with all attention tests, whereas education showed a negative correlation. Gender was not found to be significant on the multiple linear regression model, but age and education maintained their interference. CONCLUSION: Gender did not have the major impact on attentional tasks observed for age and education, both of which should be considered in the stratification of normative samples.


OBJETIVO: Os testes de atenção Trail Making Test (TMT) e Stroop Test (ST) são largamente usados na prática clínica e em pesquisas. O objetivo deste estudo foi fornecer informação normativa para a população brasileira de adultos e estudar a interferência de gênero, idade e educação no TMT parte A e B e no ST cartão A, B e C. MÉTODOS: Recrutamos 1447 sujeitos saudáveis com idade ≥18 anos, nível educacional de 0-25 anos, falantes nativos do Português (Brasil). Os sujeitos foram avaliados pelos subtestes do Wechsler Adult Intelligence Scale-III Raciocínio Matricial e Vocabulários, além do TMTA, TMTB e ST A, B e C. RESULTADOS: Entre os participantes a média de eficiência intelectual foi de 103,20 (SD: 12,0), de idade 41,0 (SD: 16,4) anos e de escolaridade 11,9 (SD: 5,6) anos. Houve diferenças significantes por gênero em TMTA (p=0,002), TMTB (p=0,017) e STC (p=0,024). Idade se correlacionou de modo positivo com todos os testes de atenção, enquanto a escolaridade correlacionou-se de modo negativo. Após o modelo de regressão linear múltipla o gênero não manteve correlação significativa, mas idade e escolaridade mantiveram sua interferência. CONCLUSÃO: O gênero não mostrou grande impacto nas tarefas atencionais como a idade e escolaridade que devem, portanto, ser consideradas na estratificação de amostras normativas.

Este trabalho propôs-se a estudar como portadores de diabetes relacionam-se com sua doença e demandas de cuidado, visando identificar, descrever e analisar criticamente os fatores que compõem essa relação e como se articulam no modo pessoal de o sujeito lidar com a doença. Para tanto, foram realizadas entrevistas em profundidade com portadores de diabetes da Associação de Diabéticos da Lagoa, Rio de Janeiro, analisadas pelo Método de Interpretação de Sentidos. Foram contemplados: o contexto do diagnóstico e a reação dos sujeitos; os fatores que, segundo os entrevistados, afetavam sua disposição para adotar cuidados de saúde; a percepção dos sujeitos sobre a relação com os profissionais / equipes de assistência e os serviços de atenção à saúde; assim como a influência de aspectos sociais da contemporaneidade na prática de cuidados. As análises tecidas sobre as entrevistas foram orientadas pela perspectiva da Lógica do Cuidado, de Annemarie Mol e da Modernidade Tardia, de Anthony Giddens...


This work aimed to study how people with diabetes are related to their illness and care demands in order to identify, describe and critically analyze the factors that make this relationship and how articulate with each other in the personal mode of coping with the disease. Thus, we conducted in-depth interviews with diabetes patients of Associação dos Diabéticos da Lagoa, Rio de Janeiro, analyzed by the Método de Interpretação de Sentidos. It were contemplated: the context of the diagnosis and the reaction of subjects; the factors that, according to the interviewees, affected their willingness to adopt health care; subjects perception of the relationship with professionals / teams for assistance and services for health care; as well as the influence of social aspects of contemporaneity in care practices. The analyses woven about the interviews were guided by perspective of the Logic of Care, Annemarie Mol and Late Modernity, Anthony Giddens...

ABSTRACT

Objetivos: determinar o perfil dos pacientes encaminhados do Serviço Especial de Genética do Hospital das Clínicas da UFMG para avaliação oftalmológica no Hospital São Geraldo, no período de julho de 2008 a janeiro de 2010; determinar as principais causas desses encaminhamentos, as alterações encontradas nos pacientes e os achados mais comuns em algumas das doenças encontradas. Métodos: estudo descritivo baseado em dados dos pacientes atendidos no Setor de Retina do Hospital São Geraldo, encaminhados do Serviço Especial de Genética do Hospital das Clínicas da UFMG, no período de julho de 2008 a janeiro de 2010. Foram coletadas informações sobre gênero, idade, motivo do encaminhamento, principais características clínicas e suspeita diagnóstica. Para cada paciente foram realizados biomicroscopia do segmento anterior e exame do fundo de olho. Resultados: no período foram avaliados 100 pacientes. As principais suspeitas diagnósticas foram retardo mental e dismorfismos sem diagnóstico estabelecido (21%), síndrome de Marfan (12%), síndrome de Cohen (11%), erro inato do metabolismo (9%), neurofibromatose tipo 1 (5%) e síndrome de Stickler (4%). A avaliação oftalmológica contribuiu para o esclarecimento diagnóstico em 66% dos casos. As principais alterações encontradas foram: palidez de disco óptico 10%; estrabismo, iridodonese e alteração da pigmentação da retina, 7% cada; aumento da escavação do disco óptico 6%, disco óptico hipoplásico 5%, coloboma eptose palpebral, 4% cada. Conclusões: as alterações oftalmológicas são características importantes em diversas doenças genéticas. Quando avaliadas adequadamente podem contribuir para o diagnóstico e para estabelecer o prognóstico das síndromes genéticas...


Objectives: To identify the profile of the patients referred from the Special Genetic Service of UFMG University Hospital to ophthalmologic evaluation at São Geraldo Hospital from July 2008 through January 2010; to determine the major causes of such referrals, the patients? disorders, and the most common findings for some of the diseases. Methods: This is a descriptive study based on data of patients at the Retina Sector of São Geraldo Hospital that were referred from the Special Genetic Service of UFMG University Hospital from July 2008 through 2010. The collection included data on gender, age, reason for referral, main clinical characteristic, and suspected diagnosis. Biomicroscopy of the anterior segment and fundoscopy were carried out for all patients. Results: A total of 100 patients was assessed in the period. The main suspected diagnoses were mental retardation and dimorphisms without established diagnosis (21 %), Marfan syndrome (12 %), Cohen syndrome (11 %), innate errors of the metabolism (9 %), neurofibromatosis type 1 (5 %), and Sitckler syndrome (4 %). The ophthalmologic examination contributed to clarifying diagnosis in 66 % of the cases. The major disorders found were: pale optic disc (10 %); strabismus, irregular retinal pigmentation, and iridodonesis (7 % each); increased optic disc cupping (6 %); hypoplastic optic disc (5 %); and coloboma and ptosis (4 % each). Conclusions: Ophthalmologic disorders are important characteristics inherent to several genetic disorders. ABSTRACT

The corpus callosum is the largest fiber bundle in the central nervous system and it takes part in several cognitive pathways. It can be affected by multiple sclerosis (MS) early in the disease. DTI is capable of infering the microstructural organization of the white matter. The vectorial analysis of the DTI offers the more specific indices of axial diffusivity (AD) and radial diffusivity (RD), which have shown to be useful to discriminate myelin damage from axon loss, respectively. This study presents DTI results (mean diffusivity (MD), fractional anisotropy (FA), RD, and AD) of 23 relapsing-remitting MS patients and its correlation with cognitive performance. There were 47.8% of cognitive impaired patients (MS CI). We found signs of demyelination, reflected by increased RD, and incipient axon loss, reflected by AD increase, which was slightly higher in the MS CI. The cognitive changes correlated with the DTI parameters, suggesting that loss of complexity in CC connections can impair neural conduction. Thus, cognitive impairment can be related to callosal disconnection, and DTI can be a promising tool to evaluate those changes.

OBJECTIVE: The relationship between brain tumors and cognitive deficits is well established in the literature. However, studies investigating the cognitive status in low and high-grade gliomas patients are scarce, particularly in patients with average or lower educational level. This study aimed at investigating the cognitive functioning in a sample of patients with low and high-grade gliomas before surgical intervention. METHOD: The low-grade (G1, n=19) and high-grade glioma (G2, n=8) patients underwent a detailed neuropsychological assessment of memory, executive functions, visuo-perceptive and visuo-spatial abilities, intellectual level and language. RESULTS: There was a significant impairment on verbal and visual episodic memory, executive functions including mental flexibility, nominal and categorical verbal fluency and speed of information processing in G2. G1 showed only specific deficits on verbal and visual memory recall, mental flexibility and processing speed. CONCLUSION: These findings demonstrated different levels of impairments in the executive and memory domains in patients with low and high grade gliomas.

This study aimed at investigating the cognitive functioning in a sample of patients with low and high-grade gliomas before surgical intervention. METHOD: The low-grade (G1, n=19) and high-grade glioma (G2, n=8) patients underwent a detailed neuropsychological assessment of memory, executive functions, visuo-perceptive and visuo-spatial abilities, intellectual level and language. RESULTS: There was a significant impairment on verbal and visual episodic memory, executive functions including mental flexibility, nominal and categorical verbal fluency and speed of information processing in G2. G1 showed only specific deficits on verbal and visual memory recall, mental flexibility and processing speed. CONCLUSION: These findings demonstrated different levels of impairments in the executive and memory domains in patients with low and high grade gliomas.


OBJETIVO: A associação entre tumores cerebrais e déficits cognitivos é bem estabelecida na literatura. No entanto, estudos sobre a cognição de pacientes com gliomas de baixo e alto grau são escassos, especialmente, em sujeitos com baixa escolaridade. Este estudo investigou o funcionamento cognitivo de uma amostra de pacientes com gliomas de baixo e alto grau antes da intervenção cirúrgica. MÉTODO: Os pacientes com glioma de baixo grau (G1, n=19) e alto grau (G2, n=8) foram avaliados quanto à memória, funções executivas, habilidades visuo-perceptivas e visuo-espaciais, nível intelectual e linguagem. RESULTADOS: Houve prejuízo significativo em G2 na memória episódica verbal e visual, funções executivas incluindo flexibilidade mental, fluência verbal nominal e categórica e velocidade de processamento de informações. G1 demonstrou apenas déficits específicos de evocação verbal e visual, flexibilidade mental e velocidade de processamento. CONCLUSÃO: Estes achados demonstraram níveis diferenciados de comprometimento nos domínios executivos e mnésticos de pacientes com gliomas de baixo e alto grau.

ABSTRACT

BACKGROUND: Real time PCR has become the most common technique to monitor BCR-ABL transcript levels of patients treated with kinase inhibitors. The aim of this study was to evaluate BCR-ABL levels of chronic myeloid leukemia patients treated with imatinib in the chronic phase and correlate the response to therapy and event-free survival. METHODS: BCR-ABL levels were measured in peripheral blood cell samples using Real time PCR at diagnosis and then every 3 months after starting therapy with imatinib. Major molecular response was defined as a three-log reduction from the standardized baseline value. Major molecular response values were adjusted to international scale using a conversion factor of 1.19. The results are reported as a BCR-ABL/ABL ratio ( percent). RESULTS: Hematological, major cytogenetic and complete cytogenetic responses were achieved by 57 (95 percent), 45 (75 percent) and 38 (63 percent) patients, respectively. Twenty-four out of sixty patients achieved a major molecular response (40 percent) in a median time of 8.5 months. Overall survival and event free survival were higher for patients with (100 percent) versus patients without (77 percent) a complete cytogenetic response (p-value = 0.01) at 48 months. Patients with complete cytogenetic response and major molecular response had a higher event free survival compared to patients with complete cytogenetic response but without major molecular response (p-value = 0.007). CONCLUSION: In conclusion, the prognostic impact of achieving complete cytogenetic response and a major molecular response and also the importance of molecular monitoring in the follow-up of chronic myeloid leukemia patients were demonstrated.

ABSTRACT

This study investigated the sedative, cardiopulmonary, and gastrointestinal effects produced by buprenorphine and xylazine given in combination to horses. Six healthy adult horses underwent 4 randomized treatments, with an interval of 1 wk between treatments. A control group was given a saline solution intravenously (IV) and the experimental groups received buprenorphine [10 µg/kg bodyweight (BW)] in combination with 1 of 3 different doses of xylazine: 0.25 mg/kg BW (BX25), 0.50 mg/kg BW (BX50), or 0.75 mg/kg BW (BX75), all of them by IV. Cardiopulmonary parameters were evaluated for 120 min after the drugs were administered and intestinal motility was observed for 12 h after treatment. Sedation was found to be dose-dependent in all groups receiving buprenorphine and xylazine and it was observed that the heart rate decreased in the first 5 min and increased at the end of the sedation period. Arterial blood gas tension analyses showed minimal alterations during the experiment. Gastrointestinal hypomotility was observed for up to 8 h. The combination of buprenorphine and 0.50 mg/kg BW of xylazine (BX50) provided a 30-minute period of sedation without intense ataxia and maintained cardiopulmonary parameters within acceptable limits for the species.

ABSTRACT

The objective was to determine the cardiopulmonary effects and eyeball centralization time obtained with 15 or 30µg kg-1 of atracurium in anesthetized dogs under spontaneous breathing. Eighteen healthy adult mixed-breed dogs were used, which received 0.1mg kg-1 acepromazine and 0.5mg kg-1 morphine IM, followed by 4mg kg-1 propofol IV and maintained on isoflurane anesthesia with spontaneous breathing. Animals received 1mL 0.9 percent NaCl IV (CG), 15µg kg-1 (G15) or 30µg kg-1 (G30) of atracurium IV. Eyeball centralization time was measured; heart rate (HR), systolic (SAP), mean (MAP) and diastolic (DAP) arterial pressures, respiratory rate (RR), tidal volume (Vt) and minute volume (Vm) were determined every 5min, and pH, arterial CO2 pressure (PaCO2 ), arterial O2 pressure (PaO2 ), hemoglobin oxygen saturation (SaO2 ), bicarbonate (HCO3-) and base excess (BE) every 15min until 60min. Both doses of atracurium produced a similar period of eyeball centralization. Vt in groups treated with atracurium was lower than in CG up to 15min. Vm in G15 differed from CG up to 10min and in G30 up to 25min. No differences were observed for cardiovascular parameters, RR, SaO2, PaO2, HCO3- and BE. pH decreased in CG between 30 and 60min and in G15 and G30 at 15min. G30 differed from CG between 15 and 30min. PaCO2 in GC differed from baseline between 30 and 60min and in G15 differed at 15min. Atracurium at the dose of 15µg kg-1 is adequate for short corneal procedures in inhalant-anesthetized dogs under spontaneous breathing.


Objetivou-se determinar os efeitos cardiopulmonares e o tempo de centralização do globo ocular obtidos com 15 ou 30µg kg-1 de atracúrio em cães anestesiados sob ventilação espontânea. Dezoito cães adultos foram pré-medicados com 0,1mg kg-1 de acepromazina e 0,5mg kg-1 de morfina IM, anestesiados com 4mg kg-1 de propofol IV e mantidos sob anestesia inalatória com isofluorano em ventilação espontânea. Os animais receberam 1mL de NaCl 0,9 por cento IV (GC), 15µg kg-1 (G15) ou 30µg kg-1 de atracúrio IV. Mensuraram-se o tempo de centralização do globo ocular, a frequência cardíaca (FC), as pressões arteriais sistólica (PAS), média (PAM) e diastólica (PAD), a frequência respiratória (f), o volume corrente (Vt) e o volume minuto (Vm) a cada 5min, e o pH, as pressões arteriais de CO2 (PaCO2) e O2 (PaO2), a saturação de oxigênio nas hemoglobinas (SaO2), o bicarbonato (HCO3-) e o excesso de base (BE) a cada 15min, até 60min. Ambas as doses de atracúrio produziram similar período de centralização. Os grupos tratados com atracúrio apresentaram menores valores para Vt até 15min. O Vm, no G15, diferiu do GC até 10min e do G30 até 25min. Não foram observadas alterações nos parâmetros cardiovasculares f, SaO2, PaO2, HCO3- e EB. O pH diminuiu no GC entre 30 e 60min e em G15 e G30 aos 15min. A PaCO2 elevou-se no GC entre 30 e 60min e no G15 aos 15min. A dose de 15µg kg-1 de atracúrio demonstrou ser adequada para centralização do globo ocular, podendo ser utilizada em procedimentos corneanos de curta duração em cães anestesiados com isoflurano e mantidos sob ventilação espontânea.

OBJECTIVE: To investigate the links between depression and cognitive functioning in patients with Hepatitis C and other chronic liver diseases with and without the use of alcohol on the waiting list for liver transplantation and their associations with the MELD classification. METHOD: 40 patients were evaluated on a waiting list for liver transplant by a battery of neuropsychological tests, depression scales and interview at the Liver Transplant Service, of the Hospital das Clínicas University of São Paulo Medical School. RESULTS: After splitting the sample according to the education, the results showed statistical significance in the comparisons between groups of MELD > 15 and <15 in the following functions: estimated IQ, visual-spatial delayed recall and recognition as part of episodic memory and short term memory. CONCLUSION: These findings, usually found in hepatic encephalopathy, corroborated with the literature and emphasized the need to investigate in more detail the cognitive functions of these patients in order to facilitate the adoption of different conducts.

Objetivo: Investigar as relações entre depressão e funcionamento cognitivo em pacientes portadores de hepatite C e demais doenças hepáticas crônicas com e sem uso de álcool em fila de espera para transplante hepático e suas relações com a classificação MELD. Método: Foram avaliados 40 pacientes em lista de espera para transplante hepático por bateria de testes neuropsicológicos, escalas de depressão e entrevista no Serviço de Transplante do Fígado do HC-FMUSP. Resultados: Após divisão da amostra por escolaridade os resultados mostraram significância estatística nas comparações entre grupos de MELD > 15 e <15 nas funções: QI estimado, memória episódica de evocação tardia e de reconhecimento visuo-espacial e memória de curto prazo. Conclusão: As dificuldades encontradas, comuns ao quadro de encefalopatia hepática, corroboram a literatura pesquisada e enfatizam a necessidade de se investigar de maneira mais detalhada o funcionamento cognitivo destes pacientes, uma vez que diferentes condutas podem ser adotadas.

ABSTRACT

Introdução: Atualmente inúmeras pesquisas vêm sendo desenvolvidas sobre envelhecimento e o declínio da memória de longo prazo, sendo que tais alterações são mais frequentes quando comparadas a outras áreas da cognição. Objetivo: Descrever o desempenho de 15 idosas sem comorbidades, em testes neuropsicológicos que avaliam a memória de longo prazo (episódica de evocação imediata) para material verbal e visuo-espacial. Método: Foi realizada avaliação neuropsicológica de 15 idosas provenientes da comunidade saudável de São Paulo, com idade igual ou superior a 65 anos, sem história prévia de transtorno psicótico ou esquizofrenia, quadros neurodegenerativos, institucionalização e sem tratamento neurológico para alterações cognitivas. Utilizou-se os testes HVLT-R e BVMT-R para avaliar a memória de longo prazo. Resultados: Identificou-se que o declínio de memória acentua-se com o passar da idade, principalmente, na memória de longo prazo explícita. Conclusão:O comprometimento da memória torna-se mais relevante quando envolvido conteúdo visuo-espacial se comparado a conteúdo verbal(AU)


Introduction: At present many researches are being developed about aging and the decline of long term memory. Such changes are more frequent when they are compared to other areas of cognition. Objective: To describe the performance of 15 old ladies without comorbidities in neuropsychological tests which evaluate long term memory (episodic, with immediate recall) for a verbal and visual-spatial material. Method: A neuropsychological evaluation was carried out among 15 old ladies from a healthy community in São Paulo. They were 65 or older, with no previous history of psychotic disturbance, schizophrenia, neurodegenerative condition, institutionalization or neurologic treatment for cognitive alterations. The HVLT-R e BVMT-R tests were used to evaluate long term memory. Results: It was noted that the decline of memory gets more severe along the years, mainly with long term explicit memory. Conclusion: The deterioration of memory becomes more relevant when a visual-spatial content is involved when compared to verbal contents(AU)
