ABSTRACT

The results of surgical treatment of complex recurrent inguina hernias in 138 patients (all of them are men), ageing 30-82 yrs old, were analyzed. In 75 patients, suffering unilateral inguinal hernia and destroyed inguinal ligament, the preperitoneal alloplasty method, elaborated in the clinic, was applied, using operative access through inguinal channel with prolene mesh fixation to lacunar ligament. In 18 patients the mesh was sutured, using herniostapler with spiral-like fixation devices. In 47 patients the modified operation according to Stopa method was used for bilateral inguinal hernia recurrences, and in 16 patients--transabdominal preperitoneal laparoscopic hernioplasty for bilateral recurrences. The pain syndrome after the operation for unilateral recurrent inguinal hernia was observed in 5.9% patients, and for bilateral hernias - in 11.4% patients. The recurrences had occurred in 3.1% patients, suffering one-side inguinal hernia recurrence, and in 3.9% patients with bilateral recurrent inguinal hernias, in whom the net was sutured using prolene threads.

ABSTRACT

The results of surgical treatment of 257 patients aged from 10 to 50 years with the oblique inguinal hernia were analyzed. The structure of inguinal channel was studied up using anatomical dissection and measurement on 35 male cadavers aged from 12 to 60 years old. There was substantiated the expediency of surgical correction of the intrachannel and not large external oblique inguinal hernia in the presence of the three-wall-fissure-like inguinal channel using the improved method of the hernial sac mobilization from its neck.

ABSTRACT

In experiment on 57 animals the method of plasty of anterior wall of inguinal channel by forming of the aponeurosis of m. abdominis obliqus ext. and suturing together the aponeurotic edges was simulated. The results of surgical correction of inguinal hernia in 257 patients were analysed. The correlational connection of immunological changes in organism with the degree of destructive changes of hematotesticular barrier (HTB) and spermatogenic epithelium was established. Considering the testicle autoimmunity the application of protective operative methods of pharmacological defense of HTB and spermatogenic epithelium are proposed.

ABSTRACT

Luminescent investigation data of the tissue blood circulation and microcirculation in the limbs in 29 patients (in 25 of them with the main arteries occlusion) were adduced. Significance of the luminescent investigation of the limbs blood circulation in the diagnosis of the tissue blood circulation and microcirculation infringement degree was determined.
