ABSTRACT

PURPOSE: The prevalence of homologous recombination repair gene mutations (HRRm) in patients with metastatic castration-resistant prostate cancer (mCRPC) in Latin America and the Caribbean (LAC) is unknown. Prevalence of homologous Recombination repair (HRR) gene mutatiOns in patientS with metastatic castration resistant ProstatE Cancer in LaTin America (PROSPECT) aimed to determine this prevalence and to describe the demographic and clinical characteristics of the participants. MATERIALS AND METHODS: This was a prospective, cross-sectional, multicenter study across 11 cancer centers in seven LAC countries. After informed consent, all eligible participants underwent genomic testing by provided blood samples for germline HRR testing; they also provided PC tissue blocks if available for somatic HRR testing. RESULTS: Between April 2021 and April 2022, 387 patients (median age, 70 years [49-89], 94.3% Eastern Cooperative Oncology Group 0-1) with mCRPC were enrolled in the study. Almost 40% of them had a family history of cancer, and the overall time from their initial PC and mCRPC diagnosis was 3 years and 1 year, respectively. The overall prevalence of germline HRRm was 4.2%. The mutations detected included the genes CHEK2 (n = 4, 1%), ATM (n = 3, 0.8%), BRCA2 (n = 3, 0.8%), BRIP1 (n = 2, 0.5%), RAD51B (n = 2, 0.5%), BRCA1 (n = 1, 0.3%), and MRE11 (n = 1, 0.3%). The prevalence of somatic HRRm could not be assessed because of high HRR testing failure rates (79%, 199/251) associated with insufficient DNA, absence of tumor cells, and poor-quality DNA. CONCLUSION: Despite the study's limitations, to our knowledge, PROSPECT was the first attempt to describe the prevalence of HRRm in patients with PC from LAC. Notably, the germline HRRm prevalence in this study was inferior to that observed in North American and European populations. The somatic HRR testing barriers identified are being addressed by several projects to improve access to HRR testing and biomarker-based therapies in LAC.

ABSTRACT

OBJETIVO Comparar los resultados funcionales obtenidos en dos cohortes de pacientes ancianos tratados de forma quirúrgica (artroplastia reversa de hombro) versus tratamiento conservador. MATERIAL Y METODOS Estudio ambispectivo, no aleatorizado. Se incluyeron pacientes de 75 años o más, 15 tratados de forma quirúrgica y 16 siguieron tratamiento conservador. Seguimiento mínimo de 12 meses. A los 3, 6 y 12 meses de la fractura evaluamos las escalas ASES, Constant-Murley and Disabilities of the Arm, Shoulder and Hand score (DASH) y EVA. Además evaluamos los resultados radiológicos y las complicaciones. RESULTADOS No encontramos diferencias significativas para las escalas ASES, DASH, ni EVA .El grupo tratado de forma quirúrgica obtuvo a los 12 meses una puntuación media en la escala Constant mayor, diferencia estadísticamente significativa(75.1 þ/-10.3 vs. 51.9 þ/-12.4 p » 0.001). Además presentaban mayor rango de movilidad para flexión y rotación externa (128.9 þ/-17 versus 99.3 þ/-20.1 p » 0.001, and 35.7 þ/-13.9 vs. 23.4 þ/-15.5 p » 0.032). El 43% de los pacientes tratados mediante artroplastia reversa presentaban tuberosidades normoposicionadas y mejores resultados en las escalas versus pacientes con tuberosidades ausentes o malposicionadas. El grupo sometido a cirugía no presentó mayor tasa de complicaciones.CONCLUSIONES El tratamiento conservador en las fracturas de EPH en tres y cuatro fragmentos en pacientes ancianos ofrece buenos resultados en cuanto a dolor y funcionalidad en la mayoría de pacientes. Falta por definir qué pacientes por tener alta demanda funcional serían candidatos a tratamiento quirúrgico de entrada


OBJECTIVE This study compared functional results in two cohorts of older adults with three- and four-part proximal humeral fractures (PHFs) per Neer's classification treated with reverse shoulder arthroplasty (RSA) or nonoperative management. MATERIALS AND METHODS Ambispective, non-randomized study with two cohorts of patients aged 75 or older treated with RSA (n » 15) or nonoperative management (n » 16) with a minimum follow-up period of 12 months. We analyzed the American Shoulder and Elbow Surgeons (ASES), Constant-Murley, and Disabilities of the Arm, Shoulder, and Hand (DASH) scores and the visual analog scale (VAS) for pain at 3, 6, and 12 months. In addition, we recorded radiological findings and surgical complications. RESULTS The mean Constant-Murley score at 12 months was significantly higher for the RSA group (75.1 þ/- 10.3 vs. 51.9 þ/- 12.4, p » 0.001). There were no differences in ASES, DASH, and VAS scores. Statistically significant differences for flexion and external rotation in abduction favored the RSA group (128.9 þ/- 17.0 versus 99.3 þ/- 20.1, p » 0.001, and 35.7 þ/- 13.9 vs. 23.4 þ/- 15.5, p » 0.032, respectively). For the RSA group, tuberosity positioning was correct in 43% of subjects. These patients presented better scores than those with malpositioned or absent tuberosities but with no statistical significance. Complications in the surgical group were not higher. CONCLUSIONS Nonoperative treatment is a valid option regarding pain and functionality in elderly patients with three- and four-part PHFs. Characteristics of patients with high demands who may be candidates for the initial surgical treatment remain to be defined

ABSTRACT

A two-dimensional lattice-gas model, supplemented by Monte Carlo simulations in the grand canonical ensemble, is applied to study the CO2/CH4 exchange process in sI clathrate hydrates. The coverage dependence of the Helmholtz free energy, chemical potential, entropy, and degree of deformation of the sI structure is given. Two different situations are considered according to the value of the intra- and inter-species' interactions. First, lateral interactions between the guest species and water molecules are introduced by following the well-known Lorentz-Berthelot mixing rules. Second, the study is restricted to an ideal clathrate hydrate, where the lateral interactions are neglected and entropy governs the exchange of CH4 by CO2. In the case of real clathrate hydrates (non-zero lateral interactions), the displacement phenomenon is clearly observed from the behavior of the chemical potential and Helmholtz free energy as functions of coverage. The guest species CO2 has an occupation of more than 80% of the cavities, and therefore displaces the CH4 species. Only 13 to 15% of CH4 remains stagnant in the sI structure. With respect to the degree of deformation, a direct relationship between cell distortion and cell occupancy is observed. Finally, the detailed analysis carried out for the ideal clathrate hydrate allows us to interpret the physical mechanism underlying the exchange process: it is entropy, not energy, that drives the displacement of CH4 by CO2 in sI clathrate hydrates.

ABSTRACT

Schwanomas originating from the brachial plexus, although rare, may be mistaken for another type of tumour. A 40 year-old woman, who had been treated years earlier for a breast adenocarcinoma, showed in the 5-year follow-up magnetic resonance examination a localized lesion in the right brachial plexus. The presumptive radiological diagnosis was a metastasis from the primary adenocarcinoma. Following surgical access via the right axilla, a well-circumscribed mass in the brachial plexus was detected. Under intraoperative electrophysiological guidance, the lesion was observed to depend on the ulnar nerve and its complete resection was possible without compromising nerve function. Histological findings indicated a schwannoma thus ruling out the presence of metastasis. The postoperative development was uneventful and six years after surgery, the patient is to date tumour-free. To the best of our knowledge, this is the first report of a brachial plexus schwannoma arising from the ulnar branch. Intraoperative electrophysiological monitoring is essential for a good surgical outcome.

ABSTRACT

Los neurinomas del plexo braquial son tumores infrecuentes que pueden confundirse con otras lesiones de índole tumoral. Se presenta el caso de una mujer de 40 años, tratada previamente de un adenocarcinoma de mama derecha en el pasado, que en el estudio de extensión realizado 5 años después se detectó una lesión localizada en el plexo braquial derecho. La paciente se encontraba asintomática. El diagnóstico radiológico de presunción fue metástasis de adenocarcinoma mamario. Se realizó un abordaje axilar derecho descubriendo una lesión bien delimitada en el plexo braquial. Con ayuda de la monitorización neurofisiológica intraoperatoria, se observó que la lesión dependía de la rama cubital y se pudo realizar una resección completa preservando la función de dicho nervio. El estudio anatomopatológico confirmó que se trataba de un neurinoma, descartando así la existencia de metástasis. La evolución postoperatoria fue satisfactoria. Seis años después de la intervención no existe recidiva tumoral. En nuestro conocimiento este es el primer caso publicado en la literatura de un neurinoma del plexo braquial dependiente de la rama cubital. La monitorización neurofisiológica intraoperatoria resulta fundamental para abordar este tipo de lesiones con baja morbilidad.

ABSTRACT

Se estudian las historias clínicas de 30 pacientes operados en el Hospital Pediátrico Docente "Centro Habana", entre los meses de julio de 1973 y julio de 1988, a quienes se les realizó esofagocoloplastia retroesternal extrapleural. El grupo de edad que prevaleció en esta variedad de sustitución esofáfica fue el comprendido entre los 5 primeros años de vida, con predominio del sexo masculino. La enfermedad básica que motivó la sustitución esofágica fue la estenosis esofágica por cástico (80%) y atresia esofágica (20%). Fue utilizado el colon derecho con mayor frecuencia como segmento de trasplante (80,4%); ocurrieron complicaciones en 21 pacientes y no hubo fallecidos en esta serie

ABSTRACT

Se hace una revisión de 108 esofacoloplastias realizadas consecutivamente en los servicios de Cirugía General de los hospitales docentes "General Calixto García" y Pediátrico de Centro Hubana; se hace énfasis en las indicaciones, los detalles fundamentales de la técnica quirúrgica, las complicaciones y la mortalidad posoperatorias y su prevención. Las complicaciones más frecuentes fueron la dehiscencia de la anastomosis cervical del colon, el neumotórax, la necrosis del segmento transpuesto y la estenosis de la anastomosis cervical. La mortalidad global fue del 23,1%, con el 39,1% para los operados por procesos malignos y el 6,5% en las afecciones benignas. El resultado funcional y estético de la esofagocoloplastia retrosternal extrapleural fue muy satisfactorio

ABSTRACT

Se realiza una encuesta nacional entre los cirujanos pediatras de nuestro país para conocer los resultados de la esofagocoloplastia realizada a niños. Se recopilan 28 operaciones en 26 pacientes intervenidos quirúrgicamente en 4 centros asistenciales, que contestaron el modelo confeccionado y enviado. Se analizan mediante tablas elaboradas, los pacientes y las operaciones realizadas en cada hospital, el sexo, el color de la piel, la enfermedad básica que motivó la operación, el segmento de colon trasplantado y el tipo de anastomosis cervical realizada, evolución y complicaciones. Se informa en las conclusiones la baja mortalidad del 3,8% en esta serie, así como los buenos resultados logrados en niños en comparación con esta técnica en adultos
