ABSTRACT

This study was carried out to compare the impact of choline supplementation (available from two sources synthetic and natural) on various dosages in broilers. The mode of choline supplementation, via diet and additional sources, synthetic and natural, and the data of performance, carcass quality, blood parameters, and hepatic steatosis were compared. A total of 1050 day-old male Cobb 500 broiler chicks were randomly assigned to 10 treatments, using a completely randomized design model in a factorial scheme, with 6 replicates per treatment and 25 birds per replicate. Choline was supplemented using three sources: synthetic choline chloride 60% (CC), and two sources of natural choline A (NCA), and B (NCB). The Control treatment did not receive any choline supplementation. The diets were supplemented with low, intermediate and high doses of choline sources (400g/t, 800g/t, and 1200g/t of CC; 100g/t, 200g/t, and 300g/t of both NCA and NCB). Data analysis was performed using a factorial model to investigate the effects of choline supplementation (CC, NCA, NCB) and doses on the measured variables. Overall, the results indicated that the the performance of NCA was better than CC & NCB, specifically the dose of 100g/t of NCA outperformed MAR at 100g/t & CC at 400g/t, leading to a significant increase in body weight gain (85.66g & 168.84g respectively), and a noteworthy (9- & 12-point respectively) improvement in feed conversion ratio. Furthermore, NCA contributed to a reduction in steatosis when contrasted with various NCB & CC doses, likely due to the presence of curcumins and catechins in the natural choline source. These findings demonstrated that NCA supplementation yielded superior results compared to CC and NCB across both performance and liver health aspects in broilers aged 1 to 42 days. In conclusion, NCA can be used to replace the CC 60% without compromise on the zootechnical performance in broilers.

ABSTRACT

Morphological study of the tongue is an interesting way of understanding evolutionary processes associated with feeding habits. Therefore, the aim of the present study was to describe the tongue morphology of the Antillean manatee and to understand possible morphological relationships with its way of capturing food. Macroscopic dissections and light and scanning electron microscopy analyses of seven manatee tongues were performed. The tongue in Antillean manatees is a muscular and robust organ, divided into apex, body, and root. It is firmly adhered to the floor of the oral cavity. Lingual papillae were distributed over the entire tongue surface. They were identified as filiform papillae concentrated in the apex. Fungiform papillae were present on the apex and lateral regions. Foliate papillae were located on the dorsolateral portion of the root. Lentiform papillae were located across the dorsal tongue surface. The mucosa was lined by a keratinized stratified squamous epithelium presenting compound tubuloacinar glands and taste buds in the foliate papillae. The tongue of the Antillean manatee is similar to other Sirenia species, both of which share a completely herbivorous diet.

Resumo Introdução: Avaliação e manejo adequados da dor têm se tornado uma preocupação cada vez maior dos profissionais de saúde. Segundo a definição de cuidados paliativos da Organização Mundial da Saúde, são importantes a identificação precoce, a avaliação e o tratamento da dor. Objetivo: Este estudo teve como objetivos desenvolver e avaliar um aplicativo para dispositivos móveis voltado para o ensino de abordagem da dor em cuidados paliativos. Método: Foi desenvolvido um aplicativo móvel, seguido de sua utilização por médicos residentes e de avaliação qualiquantitativa prospectiva. Aplicou-se o teste de conhecimento. Obtiveram-se a usabilidade do aplicativo, por meio da System Usability Scale (SUS), e a satisfação, por meio do Net Promoter Score (NPS). Os dados foram tabulados no Microsoft Office Excel® e exportados para o software Statistical Package for the Social Sciences (SPSS) versão 20.0 (IBM): na realização das análises, adotou-se uma confiança de 95%. Para a análise qualitativa das avaliações, o conteúdo das respostas foi transcrito e organizado em nuvens de palavras para destacar as ideias centrais. O estudo foi aprovado pelo Comitê de Ética do Centro Universitário Christus. Resultado: Desenvolveu-se o PaliPain App, aplicativo constituído de casos clínicos de pacientes em cuidados paliativos com manifestações de dor, além de questões, vídeos e imagens, disponibilizado nas plataformas Android e iOS. Utilizaram o aplicativo 13 médicos residentes. A média de acertos de questões no pré-teste sobre dor em cuidados paliativos aumentou de 6,08 para 7,54 após manuseio do aplicativo. Na avaliação sobre a usabilidade do aplicativo, obtivemos na SUS uma média de 89,2, acima do nível considerado adequado de 70,0. Na avaliação de satisfação, 54% dos médicos responderam com pontuação 10 à pergunta: "Em uma escala de 0 a 10, quanto você recomendaria o aplicativo PaliPain a um amigo ou colega?". Conclusão: O PaliPain App, desenvolvido e disponibilizado nas plataformas Android e iOS, foi bem avaliado pelos médicos residentes, apresentando ótimos escores de usabilidade e de satisfação. Identificamos que houve ganho de conhecimento, com pontuação média na resolução de questões aumentando de 6,08 para 7,54 após livre uso da aplicação. O PaliPain App pode ser uma ferramenta útil para ensino de abordagem de dor para médicos residentes.


Abstract Introduction: Pain evaluation has become an increasingly relevant concern for healthcare workers. According to the World Health Organization's definition of Palliative Care, early identification, evaluation, and treatment of pain are important. Objective: Developing and rating an app for mobile devices that focuses on teaching the subject of pain management in Palliative Care. Research Methodology: The app was developed as a learning tool and tested by resident physicians while undergoing a qualitative and quantitative prospective evaluation. Mock tests were taken. The usability of the app was rated through the System Usability Scale (SUS), and satisfaction with the app was rated through the Net Promoter Score (NPS). The data was charted by using Microsoft Office Excel ® and was exported to the Statistical Package for the Social Sciences (SPSS) version 20.0 (IBM) software: an analysis was run with a 95% confidence interval. As for the qualitative analysis of the resident physicians' evaluations, all available content was transcribed and properly organized in word clouds in order to highlight the most important pieces of feedback that the physicians presented regarding the app. The study was approved by the Ethics Committee. Main Results: The PaliPain App was developed as a database filled with clinical cases of patients undergoing palliative care who were experiencing varying levels of pain, and also presents analysis of test questions, videos and images. It was made available for the Android and iOS platforms. The average correct answers in a mock test about pain management during palliative care rose from 6.08 to 7.54. Thirteen resident physicians made use of the app. In the evaluation of the app's usability, the average SUS score was 89.2. In the evaluation of user satisfaction, 54% of the resident physicians answered the question "from a scale of 0 to 10, what is the likelihood of you recommending the PaliPan app to a friend or colleague?" with a score of 10. Conclusion: The PaliPain App, developed and available on Android and iOS platforms, was well evaluated by resident physicians, presenting excellent scores for both usability and satisfaction. We identified that there was a gain in knowledge, with an average score in the resolution of questions increasing from 6.08 to 7.54 after free use of the application. The PaliPan app could be a useful tool to teach the subject of pain to resident physicians.

ABSTRACT

BACKGROUND: The use of fluids is the most frequently used therapy for circulatory shock. Its inadequate use has adverse effects, requiring hemodynamic goals, highlighting the use of pulse pressure variation due to its high accuracy. One of the problems related to this method is the difficulties in measuring the pulse pressure variation (PPV) in most monitors for clinical use. We assessed the qualitative aspects of perceived usability of a smartphone application (app), which, based on a photograph of the patient's arterial pulse wave, can help measure PPV and help in the diagnosis and management of shock cases. METHODS: To assess the software perceived usability, we used the System Usability Scale (SUS) applied to 30 physicians in 2 tertiary hospitals in Brazil. The software accuracy was measured using a sequence of 3 images with different values ​​of pulse pressure variation, comparing the obtained values ​​ with the gold standard. The educational interface of the app was evaluated qualitativelyfrom the spontaneous testimonies of the selected test participants.The project was approved by the Research Ethics Committee of Centro Universitário Christus. RESULTS: The analysis showed an average SUS of 86.3 points on a scale of 1-100 (above 80.3 is considered the best in terms of interface). The assessment of the application's accuracy when evaluating pulse pressure variation showed that the average variation of the measurements taken by the participants was small, with a good measure of repeatability and reproducibility. The app's educational interface was qualitatively evaluated, being praised by the users. CONCLUSIONS: It can be concluded that the developed mobile application showed excellent qualitative aspects of perceived usability results. More studies with this app will be required to evaluate the potential to help professionals with hemodynamic evaluation in emergency and intensive care settings.

ABSTRACT

The current investigation was conducted to test the potential effects of in ovo feeding of creatine monohydrate (CMH) on hatchability, embryonic mortality, hatching weight, and development of heart and gastrointestinal tract (main organs and regions) of breeder chick embryos. Rhode Island Red fertile eggs were randomly distributed into seven experimental treatments: untreated egg (control), a sterile buffered solution (0.50% NaCl), and five solutions containing increased levels of CMH (0.50, 1.00, 1.50, and 2.00%) + 0.50% NaCl, being separated in four groups/replicates (three with 15 eggs and one with 16 eggs), totaling 61 eggs/treatment and a total of 427 fertile eggs used. All-in ovo injected groups with CMH decreased the hatchability and increased the intermediary embryonic mortality. At hatching, all-in ovo injected groups with CMH also increased the hatching weight and stimulated the development of the heart and the total length of the gastrointestinal tract, especially important organs for digestion of nutrients (yolk sac, pro-ventricle and gizzard) and regions for nutrient absorption (jejunum + ileum and colon + rectum). Conclusively, the in ovo feeding using CMH showed positive impacts on hatching weight and the development of gastrointestinal tract of chicks. However, caused negative impacts on hatchability.

ABSTRACT

Transcription factors (TFs) bind to DNA in a highly sequence-specific manner. This specificity can manifest itself in vivo at heterozygous loci as a difference in TF occupancy between the two alleles. When applied on a genomic scale, functional genomic assays such as ChIP-seq typically lack the statistical power to detect allele-specific binding (ASB) at the level of individual variants. To address this, we propose a framework for benchmarking sequence-to-affinity models for TF binding in terms of their ability to predict allelic imbalances in ChIP-seq counts. We show that a likelihood function based on an over-dispersed binomial distribution can aggregate evidence for allelic preference across the genome without requiring statistical significance for individual variants. This allows us to systematically compare predictive performance when multiple binding models for the same TF are available. We introduce PyProBound, an easily extensible reimplementation of the ProBound biophysically interpretable machine learning framework. Configuring PyProBound to explicitly account for a confounding sequence-specific bias in DNA fragmentation rate yields improved TF binding models when training on ChIP-seq data. We also show how our likelihood function can be leveraged to perform de novo motif discovery on the raw allele-aware ChIP-seq counts.

ABSTRACT

: O aleitamento materno, de acordo com a Organização Mundial da Saúde, deve ser o único alimento para os bebês até 6 meses de idade e o principal para as crianças de até 24 meses de idade. Entre suas inúmeras vantagens destaca-se menores índices de morbidade infantil por diarreia, infecções respiratórias, otite média, proteção contra sobrepeso e diabetes. Objetivo: O objetivo do estudo foi analisar a prevalência e os fatores condicionantes ao aleitamento materno das gestantes e lactantes investigadas, notando os padrões à implementação na rotina materna e suas dificuldades à essa prática. Metodologia: Trata-se de um estudo descritivo, quantitativo com delineamento transversal. A coleta de dados foi realizada através de um questionário semiestruturado, para conhecimento materno em aleitamento, realizado com gestantes e puérperas cadastradas em uma Unidade de Saúde localizada no município de Ubá, Minas Gerais, caracterizando uma amostragem não-probabilística de 118 mulheres. Resultados: Considerando o conhecimento do termo Aleitamento Materno Exclusivo, 56,7% das mulheres entrevistadas relataram não saber, no momento da entrevista, seu significado; 23,7% relataram achar que existem situações em que o bebê não deva ser amamentado ou que a prática deva ser interrompida; 19,4% das mulheres relataram achar que existe "leite fraco", enquanto 100% acreditam ser um alimento adequado ao bebê. Conclusão: Neste estudo foi verificado que a disseminação de informações a respeito do aleitamento materno, apesar de já ter alcançado muitas mulheres, o número de mães que amamentam de forma correta, mesmo tendo recebido informações a respeito e as que amamentam até a idade adequada, ainda é baixo


Breastfeeding, according to the World Health Organization, should be the only food for babies up to 6 months of age and the main food for children up to 24 months of age. Among its many advantages, there are lower rates of infant morbidity due to diarrhea, respiratory infections, otitis media, protection against overweight and diabetes. Objective: The aim of the study was to analyze the prevalence and conditioning factors for breastfeeding among the investigated pregnant and lactating women, noting the standards for implementation in the maternal routine and their difficulties with this practice. Methods: This is a descriptive, quantitative study with a cross-sectional design. Data collection was carried out through a semi-structured questionnaire for maternal knowledge about breastfeeding, carried out with pregnant and postpartum women registered at a Health Unit located in the city of Ubá, Minas Gerais, featuring a non-probabilistic sample of 118 women. Results: Considering the knowledge of the term Exclusive Breastfeeding, 56.7% of the women interviewed reported not knowing, at the time of the interview, its meaning; 23.7% reported thinking that there are situations in which the baby should not be breastfed or that the practice should be interrupted; 19.4% of the women reported that they thought there was "weak milk", while 100% believed that it was a suitable food for the baby. Conclusion: In this study, it was verified that the dissemination of information about breastfeeding, despite having already reached many women, the number of mothers who breastfeed correctly, even having received information about it and those who breastfeed until the appropriate age, is still low

ABSTRACT Objective: Evaluate the influence of the most used surgical positioners for lumbar lordosis (LL) in asymptomatic individuals. Methods: Cross-sectional study based on demographic data and radiographic parameters of asymptomatic individuals. For this study, 16 volunteers, 15 males, and one female were selected, and the average age was 24.6 years. They were submitted to lateral radiographs of the lumbar spine in orthostasis in use of the following positioners: gel cushion, gel cushion with hip extension, four-point Relton-Hall and Wilson-type positioner. Results: The mean LL in the orthostatic position was 58.76º, whereas in the gel cushion positioner it was 52.51; on the gel cushion with hip extension of 58.23º, Relton-Hall/4points 37.63º and, finally, on the Wilson-type positioner of 40.87º. An average reduction of 5.42º of the LL was observed when positioning on the gel cushion in relation to the orthostasis. In the linear regression analysis, the data presented statistically significant results (p<0.05), demonstrating that the L4-S1 segment influences 60% in LL. Conclusion: The positioner with gel cushion and hip extension reproduces an LL similar to physiological values. Relton-Hall and Wilson-type positioners with hip flexion promote hypolordotic positioning compared to basal lordosis in orthostasis. Hip extension alone generated a 5.96º increase in the subject's lordosis. The L4-S1 segment has a 60% influence on the LL when the individuals are in the positioners. RESUMO: Objetivo: Avaliar a influência dos posicionadores cirúrgicos na lordose lombar (LL) em indivíduos assintomáticos. Métodos: Estudo transversal com dados demográficos e parâmetros radiográficos de indivíduos assintomáticos. Utilizamos 16 voluntários, sendo 15 do gênero masculino e uma do gênero feminino, com idade média de 24,6 anos. Foram submetidos a realização de radiografias em perfil da coluna lombar em ortostase nos seguintes posicionadores: coxim em gel, coxim em gel com extensão do quadril, Relton-Hall em quatro pontos e posicionador tipo Wilson. Resultados: A média de LL na posição ortostática foi de 58,76º, já no posicionador coxim em gel de 52,51; no coxim em gel com extensão dos quadris de 58,23º, Relton-Hall/4pontos 37,63º e, por último, no posicionador tipo Wilson, de 40,87º. Houve redução média de 5,42º da LL ao posicionar no coxim em gel em relação a ortostase, na análise de regressão linear os dados apresentaram resultados estaticamente significativos (p<0,05), demostrando que o seguimento L4-S1 apresenta uma influência de 60% na LL. Conclusão: O posicionador coxins em gel e extensão do quadril reproduz uma LL semelhante à fisiológica. Posicionadores do tipo Relton-Hall e Wilson com flexão do quadril promovem um posicionamento hipolordótico comparada a lordose basal em ortostase. A extensão do quadril por si só foi capaz de gerar um aumento de 5,96º na lordose do indivíduo. O seguimento L4-S1 apresenta uma influência de 60% na LL quando os indivíduos estão nos posicionadores. Métodos: Estudio transversal con datos demográficos y parámetros radiográficos de individuos asintomáticos. Utilizamos 16 voluntarios, 15 hombres y una mujer, edad media de 24,6 años. Sometidos a radiografías laterales de la columna lumbar en ortostasis en los siguientes posicionadores: almohadilla de gel, almohadilla de gel con extensión de cadera, posicionador de cuatro puntos y posicionador tipo Wilson. Resultados: El promedio de LL en posición ortostática fue de 58,76º, en el posicionador de almohadilla de gel fue de 52,51º; en almohadilla de gel con extensión de cadera de 58,23º, 4 puntos 37,63º y, por último, en posicionador tipo Wilson de 40,87º. Se observó una reducción promedio de 5,42º de LL al posicionarse sobre la almohadilla de gel en relación a ortostasis. En el análisis de regresión lineal, los datos presentaron resultados estadísticamente significativos (p<0,05), demostrando que el segmento L4-S1 tiene una influencia de 60% en LL. Conclusión: El posicionador con almohadilla de gel y con extensión de cadera reproduce una LL similar a la fisiológica. Relton-Hall y Wilson con flexión de cadera promueven el posicionamiento hipolordótico en comparación con la lordosis basal. La extensión de la cadera por sí sola fue capaz de generar un aumento de 5,96º en la lordosis. El segmento L4-S1 tiene una influencia del 60% en la LL cuando los individuos están en los posicionadores. Nivel de evidencia III; Estudio Transversal Controlado.

ABSTRACT: Objective: To describe the epidemiological profile of the list of patients with pediatric scoliosis (0 to 18 years old) treated at a tertiary public hospital in the Midwest Region. Methods: A cross-sectional analytical study of patients with scoliosis from the orthopedic service of a reference center was carried out, and data collected on age, sex, date of menarche, weight, height, etiology, curve classification, form of referral, treatment performed before and after care and waiting time for surgery after indication. The sample consisted of 60 patients randomly selected among those treated. Results: 60 patients were evaluated, 44 (73.3%) were female, and 16 (26.7%) were male, with a mean age of 13.4 years. Until the moment of the study, six patients underwent surgery after being treated at the reference center. Idiopathic scoliosis was the most frequent in 38 (63%) patients. No conduct had been performed in 47 (78.3%) patients before referral to the reference center. Aftercare at the reference center, surgical treatment was indicated in 44 (73.3%) patients. The mean waiting time for surgery after the indication was 22 months, ranging from 6 to 40 months. Conclusions: The list of patients with pediatric scoliosis treated at the reference center in the Midwest region is composed of young girls with idiopathic scoliosis who, for the most part, did not have the indication of using a brace before being referred to the reference center, and most of them had an indication for surgery for adequate treatment. RESUMO: Objetivo: Descrever o perfil epidemiológico da lista de pacientes com escoliose pediátrica (0 a 18 anos) atendidos em hospital público terciário da Região Centro-Oeste. Métodos: Foi realizado estudo analítico transversal de pacientes com escoliose do serviço de ortopedia do centro de referência e coletados dados de idade, sexo, data da menarca, peso, altura, etiologia, classificação da curva, forma de encaminhamento, tratamento realizado antes e após o atendimento e tempo de espera para cirurgia após indicação. A amostra foi composta por 60 pacientes selecionados randomicamente. Resultados: Dos 60 pacientes avaliados, 44 (73,3%) eram do sexo feminino e 16 (26,7%) do sexo masculino, com idade média de 13,4 anos. Até o momento do estudo, seis pacientes foram submetidos a cirurgia após o atendimento no centro de referência. A escoliose idiopática foi a mais frequente em 38 (63%) pacientes. Nenhuma conduta havia sido realizada em 47 (78,3%) pacientes antes do encaminhamento ao centro de referência. Após o atendimento no centro de referência, o tratamento cirúrgico foi indicado em 44 (73,3%) pacientes. O tempo médio de espera pela cirurgia após a indicação foi de 22 meses, variando de 6 a 40 meses. Conclusões: A lista de pacientes com escoliose pediátrica atendidos no único centro de referência da região Centro-oeste é composta por meninas jovens, com escoliose idiopática, que em grande parte não tiveram a indicação do uso de colete antes do encaminhamento ao centro de referência, e tiveram em sua maioria indicação de cirurgia para tratamento adequado. Métodos: Se realizó un estudio analítico transversal de pacientes con escoliosis del servicio de ortopedia del centro de referencia y se recogieron datos sobre edad, sexo, fecha de la menarquia, peso, talla, etiología, clasificación de la curva, forma de derivación, tratamiento realizado antes y después de la atención y el tiempo de espera para la cirugía después de la indicación. La muestra estuvo constituida por 60 pacientes seleccionados aleatoriamente. Resultados: De los 60 pacientes, 44 (73,3%) eran mujeres y 16 (26,7%) hombres, con edad media de 13,4 años. Hasta el momento del estudio, seis pacientes fueron intervenidos quirúrgicamente. La escoliosis idiopática fue la más frecuente, en 38 (63%) pacientes. En 47 (78,3%) pacientes no se había realizado ninguna conducta antes de la derivación al centro de referencia. Tras la atención en el centro de referencia, se indicó tratamiento quirúrgico en 44 (73,3%) pacientes. El tiempo medio de espera de la cirugía fue de 22 meses, con un rango de 6 a 40 meses. Conclusiones: La lista de pacientes con escoliosis pediátrica atendidos en el único centro de referencia de la región Centro Oeste está compuesta por niñas jóvenes, quienes en su mayor parte no tenían indicación de uso de ortesis, y la mayoría tenía indicación de cirugía para un adecuado tratamiento. ABSTRACT

Neste artigo são delineadas reflexões teórico-etnográficas a partir de pesquisa de campo realizada em três grupos do WhatsApp formados por pessoas que vivem com HIV/aids (PVHA). Tem como objetivo compreender as experiências afetivo-sexuais dessas pessoas, construídas por meio de diálogos, interações e socialidades vividas nesses grupos pesquisados. A partir da descrição das socialidades nos grupos e dos modos como, por meio da biscoitagem, as pessoas fazem circular afetos e desejos sem, necessariamente, constituírem relacionamentos afetivo-sexuais, argumenta-se que, em suas interações sociais, os interlocutores conectavam pessoas, artefatos sociotécnicos, lugares, situações, emoções, relações, projetos de vida etc., de tal maneira que produziam mais que simplesmente apoio, suporte, ajuda ou comunicação sobre o HIV, visto que teciam seus próprios mundos sociais. Estes achados ganham relevância especial ao se considerar a experiência de PVHA que mantêm algum grau de segredo sobre sua sorologia positiva para o HIV


This article outlines theoretical-ethnographic reflection based on a field research about three WhatsApp groups formed by people living with HIV/AIDS (PLWHA). It aims to understand the affective-sexual experiences of these people constructed through dialogues, interactions and socialities on these WhatsApp groups. From the description of sociality in these groups and the ways in which, through biscoitagem, people circulated affections and desires without necessarily constituting affective-sexual relationships, it is argued that, in their social interactions, the interlocutors connected people, socio-technical artefacts, places, situations, emotions, relationships, life projects etc., in a way that produced more than support, mutual help or communication about HIV, insofar as they were weaving their own social worlds. These findings gain special relevance when we consider the experience of the PLWHA who maintain some degree of secrecy about their positive HIV serology


Este artículo esboza reflexiones teórico-etnográficas a partir de una investigación de campo en tres grupos en WhatsApp formados por personas viviendo con VIH/SIDA (PVVS). El objetivo es comprender las experiencias afectivo-sexuales de estas personas construidas a través de diálogos, interacciones y socialidades vividas en estos grupos. A partir de la descripción de la sociabilidad en los grupos investigados y de las formas en que, a través de la biscoitagem, las personas circulaban afectos y deseos sin constituir necesariamente relaciones afectivo-sexuales, se argumenta que, en sus interacciones sociales, los interlocutores conectaron personas, artefactos sociotécnicos, lugares, situaciones, emociones, relaciones, proyectos de vida etc., de manera que produjeron más que solamente apoyo, soporte, ayuda o comunicación sobre VIH, ya que tejian sus propios mundos sociales. Estos hallazgos adquieren especial relevancia cuando se considera la experiencia de las PVVS que mantienen cierto grado de secreto sobre su serología positiva para el VIH

ABSTRACT

Idiopathic hypertrophic pachymeningitis is rare cause of neurological symptoms with myelopathy due to spinal cord compression. We report a case of pachymeningitis, which was manifested primarily by tetraparesis after low-energy trauma and recurrence the myelopathy symptoms after 5 years of surgery. The patient, a 19-year-old woman, was subjected to extensive investigation without evidence of any underlying disease. A meningeal biopsy was performed and showed an unspecific inflammatory process with extensive fibrosis of the dura mater. These findings, associated with the exclusion of other causes, suggest idiopathic hypertrophic pachymeningitis.

Resumo A paquimeningite hipertrófica idiopática é uma causa rara de sintomas neurológicos apresentando mielopatia por compressão da medula espinhal. Relatamos um caso de paquimeningite com manifestação primária de tetraparesia após trauma de baixa energia e recorrência dos sintomas de mielopatia 5 anos após a cirurgia. A paciente, uma mulher de 19 anos, foi submetida a extensa investigação sem evidências de qualquer doença de base. Uma biópsia da meninge revelou processo inflamatório inespecífico com extensa fibrose da dura máter, também visualizado no período perioperatório. Esses achados, associados à exclusão de outras causas, sugerem o diagnóstico de paquimeningite hipertrófica idiopática.

ABSTRACT

Protein-ligand interactions are increasingly profiled at high throughput using affinity selection and massively parallel sequencing. However, these assays do not provide the biophysical parameters that most rigorously quantify molecular interactions. Here we describe a flexible machine learning method, called ProBound, that accurately defines sequence recognition in terms of equilibrium binding constants or kinetic rates. This is achieved using a multi-layered maximum-likelihood framework that models both the molecular interactions and the data generation process. We show that ProBound quantifies transcription factor (TF) behavior with models that predict binding affinity over a range exceeding that of previous resources; captures the impact of DNA modifications and conformational flexibility of multi-TF complexes; and infers specificity directly from in vivo data such as ChIP-seq without peak calling. When coupled with an assay called KD-seq, it determines the absolute affinity of protein-ligand interactions. We also apply ProBound to profile the kinetics of kinase-substrate interactions. ProBound opens new avenues for decoding biological networks and rationally engineering protein-ligand interactions.

ABSTRACT

In the last decade, significant advances have been made towards the rational design of proteins, DNA, and other organic nanostructures. The emerging possibility to precisely engineer molecular structures resulted in a wide range of new applications in fields such as biotechnology or medicine. The complexity and size of the artificial molecular systems as well as the number of interactions are greatly increasing and are manifesting the need for computational design support. In addition, a new generation of AI-based structure prediction tools provides researchers with completely new possibilities to generate recombinant proteins and functionalized DNA nanostructures. In this study, we present Catana, a web-based modelling environment suited for proteins and DNA nanostructures. User-friendly features were developed to create and modify recombinant fusion proteins, predict protein structures based on the amino acid sequence, and manipulate DNA origami structures. Moreover, Catana was jointly developed with the novel Unified Nanotechnology Format (UNF). Therefore, it employs a state-of-the-art coarse-grained data model, that is compatible with other established and upcoming applications. A particular focus was put on an effortless data export to allow even inexperienced users to perform in silico evaluations of their designs by means of molecular dynamics simulations. Esta pesquisa qualitativa objetivou compreender as relações entre acesso a serviços de saúde e experiências de sofrimento social entre pessoas trans. Foram conduzidas entrevistas semiestruturadas com cinco interlocutores e observações participantes. Os dados foram analisados por meio da codificação temática, originando duas categorias: "A negação do nome" e "Acesso à saúde e transfobia institucionalizada no Sistema Único de Saúde (SUS)". As narrativas das/os interlocutores permitiram localizar tais relações na sociogênese de experiências de sofrimento social. A negação do nome implica negação da humanidade da pessoa trans, bem como patologização de sua identidade e prejuízos no acesso à saúde, colocando a automedicação como uma possibilidade de agência. Concluiu-se que a transfobia institucionalizada no setor de saúde reproduz a precarização da cidadania de pessoas trans, destacando quanto a ação estatal com vistas a mitigar o sofrimento social pode, por vezes, intensificá-lo. The aim of this qualitative study was to understand the relationships between access to health services and experiences of social suffering among trans people. We conducted semi-structured interviews with five participants and participant observation. The data were analyzed using thematic coding, giving rise to two categories: "Name denial" and "Access to health care and institutionalized transphobia in Brazilian National Health System". The participants' narratives allowed the researchers to situate these relationships within the sociogenesis of social suffering experiences. Name denial implies the negation of the humanity of trans people and pathologization of their identity, and compromises access to health care, making self-medication a possibility of agency. We conclude that institutionalized transphobia in the public health care system reproduces the precariousness of the citizenship of trans people, highlighting how government actions aimed at mitigating social suffering can sometimes intensify it. Esta investigación cualitativa tuvo el objetivo de comprender las relaciones entre acceso a servicios de salud y experiencias de sufrimiento social entre personas trans. Se realizaron entrevistas semiestructuradas con cinco interlocutores y observaciones participantes. Los datos se analizaron por medio de la codificación temática originando dos categorías: "La negación del nombre" y "Acceso a la salud y transfobia institucionalizada en el Sistema Único de Salud". Las narrativas de los interlocutores permitieron localizar tales relaciones en la sociogénesis de experiencias de sufrimiento social. La negación del nombre implica la negación de la humanidad de la persona trans, así como la patologización de su identidad y prejuicios en el acceso a la salud, planteando la automedicación como una posibilidad de agencia. Se concluyó que la transfobia institucionalizada en el sector de la salud reproduce la precarización de la ciudadanía de personas trans, destacando hasta qué punto la acción estatal dirigida a mitigar el sufrimiento social puede, algunas veces, intensificarlo. Esta revisão narrativa procura discutir aspectos concernentes ao processo gestacional de mulheres negras, quais sejam: se existem diferenças de tratamento entre mulheres brancas e negras durante a gravidez e nos momentos do parto e pós-parto, como essas diferenças são influenciadas pelos aspectos fisiológicos de cada grupo étnico e como isso afeta as taxas de morbimortalidade. Para esta revisão, quatro bases de dados foram usadas (SciELO, LILACS, PubMed e MEDLINE) e 23 artigos foram lidos na íntegra, depois de selecionados por data de publicação, língua, país da pesquisa e análise dos títulos e resumos. Como principais resultados, os autores encontraram diferenças claras entre mulheres brancas e negras quanto ao acesso à saúde, sendo as negras mais propensas a usar os sistemas públicos e ter menos consultas pré-natal. Também foi observado que as mulheres negras reportaram maus-tratos mais vezes, tinham maiores chances de serem proibidas de ter um acompanhante durante o parto e recebiam menos anestesia para episiotomias. As características fisiológicas também foram apontadas várias vezes. Nesse sentido, altas taxas de anemia ferropriva e hipertensão durante a gravidez foram mais comuns entre as negras. Além disso, em se tratando de taxas de morbimortalidade, mulheres negras tinham uma chance consideravelmente maior de serem readmitidas pós-parto e maiores taxas de mortalidade, quando comparadas com mulheres brancas.(AU)


This review aims to discuss aspects related to the gestational process of black women, namely: if there is a difference in how black and white women are treated throughout pregnancy, partum and postpartum moments, how this difference is influenced by the physiological aspects of each ethnical group and how it affects their morbidity and mortality rates. For this review, four databases were used (SciELO, LILACS, PubMed and MEDLINE) and 23 articles were fully read, after being selected by publishing date, language, country of research, title and abstract analysis. The authors found as the main results clear differences between black women's and white women's access to health care, as black women are more likely to use public health care systems and have fewer prenatal appointments. It was also noticed that black women reported maltreatment more frequently, had a higher chance of being prohibited from keeping a companion during labor and suffering from less local anesthesia for episiotomy. The physiological characteristics were also pointed out several times, with high rates of iron deficiency anemia and hypertension during pregnancy being more common among black women. Moreover, when it comes to morbidity and mortality rates, black women had an extremely higher chance of being readmitted postpartum, and a higher mortality rate, when compared to white women.(AU)

ABSTRACT Objective: The objective of this study is to describe a new, extremely simple method for measuring pelvic incidence (PI) using computed tomography (CT) and to compare those measurements with measurements derived from whole spine and lumbosacral X-rays in a Brazilian population. Methods: Patients who had whole spine and lumbosacral X-rays and whole abdomen, pelvis, or lumbar spine CT performed within a period of less than three months were selected. Image overlay was used to measure PI from the CT. The PI was calculated by two independent examiners, and the PI for each exam was calculated twice, with an interval of two months between the assessments. The intra- and interexaminer reliability and reproducibility were evaluatedusing the intraclass correlation coefficient (ICC) and the repeatability coefficient, considering a 95% confidence interval. Results: Fifty-five patients of both sexes with a mean age of 58.7 years (±19) were analyzed. The mean PI angles in the analyses of both examiners at both evaluations were 54.85° (±13.73) for the whole spine X-ray, 54.06° (±11.67) for the lumbosacral spine X-ray, and 49.96° (±9.85) for the CT. There was good intra- and interexaminer reliability and reproducibility. There was also high concordancewith the whole spine and lumbosacral X-rays. Conclusion: CT is a reliable and reproducible alternative for measuring PI. Level of Evidence III; Prospective comparative.


RESUMO: Objetivo: O objetivo deste estudo é descrever um novo método extremamente simples para medir a incidência pélvica (IP) usando tomografia computadorizada (TC) e comparar essas medidas com as medidas derivadas de radiografias de coluna totale lombossacral em uma população brasileira. Métodos: Foram selecionados pacientes que realizaram radiografias de coluna totale lombossacral e tomografia computadorizada de abdome, pelve ou coluna lombar em intervalo inferior a três meses.A sobreposição de imagens foi usada para medir a IP na TC.AIP foi calculada por dois examinadores independentes e aIP de cada exame foi calculada duas vezes, com intervalo de dois meses entre as avaliações. A confiabilidade e a reprodutibilidade intra e interexaminadores foram avaliadas por meio do coeficiente de correlação intraclasse (ICC) e do coeficiente de repetibilidade, considerando um intervalo de confiança de 95%. Resultados: Foram analisados 55 pacientes de ambos os sexos com média de idade de 58,7 anos (±19). Os ângulos IP médios na análise de ambos os examinadores em ambas as avaliações foram de 54,85° (±13,73) na radiografia de coluna total, 54,06° (±11,67) na radiografia de coluna lombossacral e 49,96° (±9,85) na TC. Houve boa confiabilidade e reprodutibilidade intra e interexaminadores. A concordância também foi alta nasradiografias da coluna total e lombossacral. Conclusões: A TC é uma alternativa confiável e reprodutível para mensuração da IP Nível de evidência III; Estudo prospectivo comparativo.


RESUMEN: Objetivo: El objetivo de este estudio es describir un método nuevo y extremadamente simple para medir la incidencia pélvica (IP) mediante tomografía computarizada (TC) y comparar esas medidas con las derivadas de radiografías de columna vertebral total y lumbosacra en una población brasileña. Métodos: Se seleccionaron pacientes a los que se les realizaron radiografías de columna vertebraltotal, lumbosacra y TC de abdomen, pelvis o columna lumbar con un intervalo inferior a tres meses. Se utilizó la superposición de imágenes para medir laIP en la TC. LaIPfue calculada por dos examinadores independientes, y en cada examen se calculó dos veces, con un intervalo de dos meses entre las evaluaciones. La confiabilidad y reproducibilidad intra e interexaminadores fueron evaluadas mediante el coeficiente de correlación intraclase (CCI) y el coeficiente de repetibilidad, considerando un intervalo de confianza del 95%. Resultados: Se analizaron 55 pacientes de ambos sexos con una edad media de 58,7 años (±19). Los ángulos medios deIP en el análisis de ambos examinadores en ambas evaluaciones fueron de 54,85° (±13,73) en la radiografía de la columna vertebral total, de 54,06° (±11,67) en la radiografía de columna lumbosacra y de 49,96° (±9,85) en la TC. Hubo buena confiabilidad y reproducibilidad intra e interexaminadores. La concordancia también fue alta enlas radiografías de columna vertebral total y lumbosacra. Conclusiones: La TC es una alternativa confiable y reproducible para medir laIP. Nivel de Evidencia III; Estudio prospectivo comparativo.
