ABSTRACT

Retroníquia é uma onicopatia inflamatória caracterizada pelo crescimento desordenado e empilhamento de lâminas ungueais, associados ao desequilíbrio na produção matricial de placas, que costumam se sobrepor. Este quadro pode estar associado a trauma, evoluindo com reação inflamatória junto à dobra ungueal proximal e formação de tecido de granulação. Relatamos o caso de um paciente com retroníquia no hálux esquerdo, submetido a tratamento cirúrgico após falhas de respostas terapêuticas a tratamentos clínicos prévios. A abordagem cirúrgica levou à avulsão das lâminas ungueais sobrepostas e à exérese de volumosos tecidos de granulação, com adequada evolução no pós-operatório


Retronychia is an inflammatory onychopathy characterized by the disordered growth and stacking of nail plates, associated with an imbalance in the matrix production of plaques, which tend to overlap. This condition may be associated with trauma, evolving with an inflammatory reaction along the proximal nail fold and granulation tissue formation. We report the case of a patient with retronychia in the left hallux, submitted to surgical treatment after failure of therapeutic responses with previous clinical treatments. The surgical approach led to the avulsion of the overlapping nail plates and the excision of voluminous granulation tissues, achieving adequate postoperative evolution.

ABSTRACT

A onicocriptose é uma doença ungueal frequente, que afeta mais comumente os pododáctilos e apresenta algumas variantes, dentre as quais a pouco conhecida "unha em arpão". O diagnóstico costuma ser clínico, e o tratamento cirúrgico emprega diferentes técnicas descritas na literatura. Relatamos o caso de um paciente masculino, 25 anos, com unha em arpão no hálux esquerdo, e a técnica cirúrgica utilizada para o tratamento com excelente resultado


Onychocryptosis is a frequent nail disease, which most commonly affects the toes. It has some variants, including the little known "Harpoon Nail". The diagnosis is usually clinical, and the treatment is surgical, with different techniques described in the literature. We report the case of a 25-year-old man with a harpoon nail on the left hallux and the surgical method used for treatment with excellent results

ABSTRACT

Zoon vulvitis or vulvitis chronica plasmacellularis is a rare, chronic benign inflammation of the vulvar mucosa, diagnosed histologically, with variable therapeutic responses. It is important to be diagnosed because it mimics the presentation of other genital conditions, such as lichen planus and squamous cell carcinoma, which require specific treatment. We report a case of a female patient with three asymptomatic shallow ulcers on the labia minora. Laboratory tests ruled out infectious diseases and the biopsy was consistent with Zoon Vulvitis.

ABSTRACT

BACKGROUND: Chromoblastomycosis is a deep fungal infection characterized by a complex cellular granuloma. The aim of this study was to analyze the arrangement of cells responsible for the granuloma configuration of this disease by semiquantification of the cellular components of chromoblastomycosis skin biopsies. METHODS: The cells of cutaneous biopsies slides from 100 patients with untreated chromoblastomycosis were stained with hematoxylin-eosin and the granuloma cells were evaluated by microscopic examination of the elements of each granuloma and semiquantified the number of cells through its expressivity in crosses and the histopathological variables. Their presence were coded in degrees of intensity and classified in two categories: low expression and high expression. The cells that constituted the granulomas were separated into three groups: A, B and C. RESULTS: The chromoblastomycotic granuloma analyzed by this semiquantification of its cellular components showed that there was high expression of the elements setting up a mixed organized mycotic granuloma. It was observed that mononuclear phagocytic system (A), polymorphonuclear phagocytic system (B) and lymphoplasmocytic infiltrate (C) were located around the fungus. CONCLUSION: The results indicated that the granuloma present in the cutaneous lesion of chromoblastomycosis is a mixed organized mycotic granuloma with high expression of the cellular components.

Giant cell tumor of the tendon sheath is a benign soft tissue tumor most frequent between the third and fifth decades of life. It can mimic and make differential diagnoses with several hand tumors. Definitive diagnosis and the treatment of choice are reached with complete resection and histopathological examination. Here we describe a case with clinical presentation similar to that of a myxoid cyst.

ABSTRACT

Melanocytic nevi result from the proliferation of melanocytes at the dermal-epidermal junction and/or in the dermis and very rarely affect the nail matrix and bed. Intradermal nevi are tumors often diagnosed in routine clinical dermatological practice, with typical clinical and histopathologic aspects, and found in a wide variety of skin sites. We report a case of intradermal melanocytic nevus of the proximal nail fold with uncommon intraoperative gross findings and without involvement of the nail bed or matrix, showing intradermal component only, which draws our attention to the possible different presentation of melanocytic lesions. No description of intradermal nevi at this site has been reported in the literature so far.

ABSTRACT

Relata-se o caso de uma paciente com diagnóstico de lúpus eritematoso sistêmico (LES) feito há 11 anos e que, posteriormente, apresentou artrite gotosa aguda. Na revisao bibliográfica, viu-se que, a coexistência de gota e LES é rara, sendo o primeiro relato feito em 1981. A maioria dos casos descritos foi acompanhada de nefropatia, terapia com diuréticos por longo tempo, hipertensao arterial sistêmica (HAS) e terapia com corticóides, semelhante ao presente caso. O aparecimento de gota nestes casos de LES pode ser interpretado como atrite lúpica. A gota deve ser lembrada em paciente que apresentam artrite aguda e/ou nódulos subcutâneos

ABSTRACT

Os autores relatam o caso de recém-nascido do sexo feminino com crises convulsivas e lesöes vesiculosas no nariz e lábio inferior, desde o quinto dia de vida. O exame do LCR mostrou alteraçöes compatíveis a encefalite. Houve deterioraçäo do quadro neurológico e respiratório, com morte da paciente. A autópsia paracial do crânio revelou cérebro edemaciado com área necro-hemorrágica envolvendo ambos os lobos temporais. O exame histopatológico revelou encefalite necro-hemorrágica, sem a presença de inclusöes intranucleares. O exame imuno-histoquímico, realizado pela técnica da avidina-biotina-peroxidase utilizando anticorpos policlonais contra vírus Herpes simplex tipo 1 e tipo 2, mostrou-se positivo em numerosas células neuronais, astrocíticas e, principalmente, oligodendrogliais para o anticorpo contra o vírus Herpes simples tipo 2
