ABSTRACT

Nipple hypertrophy is a rare deformity that affects women's self-esteem. Several surgical techniques have been described for its treatment, but there is no gold standard technique, leaving the choice to the surgeon based on preference. A systematic review was performed using PRISMA guidelines on the surgical treatment of nipple hypertrophy over the period available. An illustrative case series was also included with 24 women, mean age 34.5 years, with nipple hypertrophy, treated with a functional preservation technique. This consisted of the removal of a circular segment of nipple tissue between the base and the tip, preserving its central structures, in conjunction with breast surgeries. The nipple-areola complex also underwent a procedure to achieve a 1:3 ratio. Postoperative follow-up was 12 months for the metric assessment, occurrence of complications, and tactile and pain sensitivities. A total of 1,064 articles were found, but only 19 were eligible. Most studies were classified with a low evidence level, homogeneous series, without the possibility of meta-analysis. Regarding the case series, the measurements of the vertical and horizontal axes varied, on average, 1.4 and 1.25 cm, respectively, in the preoperative period. No changes in the sensitivity were reported. No women had any of local or systemic complications. All of them were followed for at least 12 months, with preservation of the nipple-areola metrics. There is a need for better scientific evidence regarding the surgical treatment of nipple hypertrophy. The present surgical technique demonstrated the long-term preservation of nipple metrics without sensory or functional impairment.

ABSTRACT

O Capítulo de Cirurgia do Contorno Corporal da Sociedade Brasileira de Cirurgia Plástica realizou em 2012 um amplo estudo dos parâmetros de segurança para a realização de uma lipoaspiração, antecedendo ao curso do Capítulo, realizado em 2013, no Rio de Janeiro, durante o 50º Congresso Brasileiro de Cirurgia Plástica, com a presença do ilustre Dr. Yves Gérard Illouz e grande número de participantes. Os autores realizaram uma revisão e atualização destes parâmetros, incorporando recentes avanços e sistematizando de forma prática as informações relevantes para a realização de uma lipoaspiração mais segura. Com a colaboração de anestesiologista, intensivista e cirurgião vascular, experientes no suporte ao procedimento, são apresentadas novas orientações práticas para o pré-operatório, trans e pós-operatório de uma lipoaspiração mais segura.


In 2012, the Research Group on Body Contouring Surgery of the Brazilian Society of Plastic Surgery conducted an extensive study on the safety of liposuction before the initiation of the course of the Group, held in 2013 in Rio de Janeiro during the 50th Brazilian Congress of Plastic Surgery, with the presence of the illustrious Dr. Yves Gérard Illouz and a large number of participants. The authors performed a review and update of the safety parameters, incorporating recent advances and systematizing relevant information for the execution of a safer liposuction. With the collaboration of anesthesiologist, intensive care physician, and vascular surgeon, all with extensive experience in supporting the procedure, new practical guidelines were presented for a safer liposuction on the preoperative, transoperative, and postoperative periods.

ABSTRACT

Introdução: As cirurgias para aumento das mamas com uso de implantes de silicone ocupam posição de destaque nas estatísticas mundiais. O advento das próteses mamárias recobertas com poliuretano trouxe um grande avanço médico nestes procedimentos, já que sua principal característica é o baixo índice de contratura capsular, proporcionando, assim, resultados mais estáveis e duradouros. Objetivo: O propósito deste estudo é demonstrar a experiência do autor com este tipo de implante nos últimos 14 anos. As vantagens e desvantagens de seu uso são analisadas, assim como as complicações, intercorrências e resultados. Alguns detalhes táticos da técnica cirúrgica, como escolha do volume e demarcação prévia da extensão da loja receptora, também são apresentados. Método: Um levantamento retrospectivo de 350 casos, divididos em três grupos de tempo pós-operatório, demonstra a evolução das pacientes com relação a estas intercorrências e complicações imediatas e tardias. Resultados: Os resultados pós-operatórios, tanto precoces como a longo prazo, são altamente satisfatórios, de rápida recuperação e baixo índice de complicações. Conclusão: O uso de próteses mamárias preenchidas com silicone coesivo e revestidas com película de poliuretano, apesar da maior dificuldade de manipulação, tem se mostrado uma excelente indicação para uso em aumento mamário, reconstruções, mastopexias e cirurgias secundárias, com contraturas capsulares graves.


Introduction: The breast augmentation surgeries using silicone implants have an important position in the global statistics about a esthetic and reconstructive procedures. The use of poliurethane covered breast implants represents a medical advance, since its use presents very low rates of capsular contracture and provides more satisfactory long lasting outcomes. Objective: The purpose of this study is to demonstrate the author’s experience in the last 14 years. The study of the advantages and disadvantages of using this kind of prothes is are analysed as well as its complications and results. Some surgical details like volume choice and size pocket previous demarcation are both commented. Method: This paper is a retrospective analysis of 350 cases divided in three groups in various post operative fases showing the evolution of these patients and their relation regarding to complications. Results: The immediate and long lasting outcomes are highly satisfactory with fast recovery and low incidence of complications. Conclusion: The use of poliurethane coated silicone implants despite of a more laborious manipulation is an excellent indication for primary breast augmentation, mastopexies, reconstructions and secondary cases in patients with severe capsular contractions.
