ABSTRACT

Antibiotic resistance (AR) is a major global health concern, but current surveillance efforts primarily focus on healthcare settings, leaving a lack of understanding about AR across all sectors of the One Health approach. To bridge this gap, wastewater surveillance provides a cost-effective and efficient method for monitoring AR within a population. In this study, we implemented a surveillance program by monitoring the wastewater effluent from two large-scale municipal treatment plants situated in Isfahan, a central region of Iran. These treatment plants covered distinct catchment regions and served a combined population about two million of residents. Furthermore, the effect of physicochemical and microbial characteristics of wastewater effluent including biological oxygen demand (BOD), chemical oxygen demand (COD), total suspended solids (TSS), temperature, total coliforms and Escherichia coli concentration on the abundance of ARGs (blaCTX-M, tetW, sul1, cmlA, and ermB) and class 1 integron-integrase gene (intI1) were investigated. Sul1 and blaCTX-M were the most and least abundant ARGs in the two WWTPs, respectively. Principal Component Analysis showed that in both of the WWTPs all ARGs and intI1 gene abundance were positively correlated with effluent temperature, but all other effluent characteristics (BOD, COD, TSS, total coliforms and E. coli) showed no significant correlation with ARGs abundance. Temperature could affect the performance of conventional activated sludge process, which in turn could affect the abundance of ARGs. The results of this study suggest that other factors than BOD, COD and TSS may affect the ARGs abundance. The predicted AR could lead to development of effective interventions and policies to combat AR in the clinical settings. However, further research is needed to determine the relationship between the AR in wastewater and clinical settings as well as the effect of other influential factors on ARGs abundance.

ABSTRACT

Indoor air pollution (IAP) has been associated with various adverse health effects. However, the evidence regarding such an association with leukocyte telomere length (LTL) in cord blood samples is still scarce. Therefore, the present study aimed to assess the relationship between exposure to indicators of IAP and LTL in umbilical cord blood samples. This cross-sectional study was based on 188 mother-newborn pairs who participated in our study between 2020 and 2022 in Isfahan, Iran. Umbilical LTL was measured by quantitative real-time polymerase chain reaction (qRT-PCR) technique. Linear mixed-effect models were used to assess the relationship between IAP indicators and umbilical LTL, adjusted for relevant covariates. The median (interquartile range (IQR)) of umbilical LTL was 0.92 (0.47). In fully adjusted models, frequency of using degreasing spray during pregnancy (times per month) (ß = -0.047, 95% CI:0.09, -0.05, P-value = 0.02), using air freshener spray during pregnancy (ß = -0.26, 95% CI: -0.5, -0.02, P-value = 0.03) and frequency of using insecticides during pregnancy (times per month) (ß = -0.025, 95% CI: -0.047, -0.003, P-value = 0.02) were significantly associated with shorter umbilical LTL. There was a positive significant relationship between the frequency of using cleaning spray during pregnancy (times per month) with umbilical LTL (ß = 0.019, 95% CI: 0.005, 0.033, P-value = 0.01). Furthermore, the direct connection of the parking with home and the frequency of using barbecue (times per week) were marginally associated with shorter umbilical LTL. For other indicators of IAP, we did not observe any statistically significant associations. Overall, this study suggested a negative association between prenatal exposure to IAP during pregnancy and umbilical LTL.

Cardiovascular disease is prevalent globally and is the most common complication of metabolic syndrome (MetS). Previous studies have suggested that curcumin and probiotics may improve the lipid profile, so we aimed to investigate the effects of the edible powder enriched with these substances on lipid profile level and atherogenic indices such as Atherogenic Coefficient (AC), Castelli Risk Index-I (CRI-I), Castelli Risk Index-II (CRI-II), and Atherogenic Index of Plasma (AIP). In the present parallel randomized double-blinded placebo-controlled clinical trial, 124 people with MetS with overweight or obesity were randomly allocated to 4 groups and were followed up for 8 weeks. The participants received a low-calorie diet and a daily sachet of enriched powder drink. The sachets contained either 109 CFU of probiotics or 1 g of curcumin, or probiotic + curcumin (pro + cur), or placebo, respectively. The fasting lipid profile and atherogenic indices were measured at the beginning and end of the study. One hundred and fourteen participants completed the study. At the end of the study, the within- and between-group comparisons showed no significant differences in lipid profile and atherogenic indices (p > .05). Based on the results of the current study, taking an oral powder containing 1 g curcumin and 109 CFU probiotics for 8 weeks had no effect on the lipid profile level and atherogenic indices; however, more studies are recommended.

ABSTRACT

Nail technology, including the application of artificial nails and nail care, is a developing sector of the global beauty industry. Nail technicians are exposed to a variety of chemical substances through inhalation, as they spend extended periods of time in close proximity to these materials. This study aimed to evaluate the semi-quantitative health risk of dust-containing heavy metals among nail technicians. This analytical descriptive study employed the risk assessment method provided by the Singapore Occupational Health Department to evaluate the health hazards of lead, cadmium, nickel, chromium, and manganese. Dust samples from nail filing were collected from the respiratory zone of 20 nail technicians following the NIOSH 7300 method. The samples were analyzed using ICP-OES instrumentation. Monte Carlo simulation was utilized to characterize the risk and its uncertainties. Manganese and cadmium had the highest and lowest mean concentrations, respectively. The risk scores of the metals ranked from highest to lowest were as follows: Ni>Cr>Cd>Mn>Pb. All five metals had risk rankings below 2.8, signifying a minimal risk level. Sensitivity analysis using Spearman's correlation coefficient demonstrated a positive relationship between concentration, daily hours of exposure, and the number of workdays per week with the risk score (RR) and exposure level (ER). Conversely, the variable of weekly working hours (W) showed a negative correlation with these parameters. Despite the low-risk level of the examined metals, continuous exposure and potential long-term effects on nail technicians warrant preventive measures. Recommendations include implementing local exhaust ventilation systems, using table fans, establishing work-rest cycles, wearing N95 dust masks, and using reputable and high-quality nail polishes.

ABSTRACT

OBJECTIVE: Nail salons offer a developing and diverse occupation for many women, especially the new generation. Due to the increasing apprehension surrounding heavy metals in dust caused by filing nails containing dried nail polish, the present study was designed aimed to health risk assessment of heavy metals in breathing zone of nail salon technicians (NSTs). METHODS: This is a cross-sectional study that was conducted in NSTs. The concentration of Cadmium (Cd), Lead (Pb), Nickel (Ni), Chromium (Cr) and Manganese (Mn)in breathing zone of 20 NSTs was determined using ICP-OES. RESULTS: The metal concentrations were in the following order: Mn > Pb > Ni > Cr > Cd with corresponding arithmetic mean values of0.008, 0.0023, 0.0021, 0.001 and 0.0006 mg m-3, respectively, which are exceeded the recommended levels stated in the indoor air guidelines. The average lifetime carcinogenic risk (LCR) for Cr, Cd, Ni and Pb was calculated 0.0084, 0.00054, 0.00026 and 1.44 E - 05, respectively. The LCR values of all metals (except Pb) exceeded the acceptable level set by the USEPA. The mean of Hazard quotients (HQ) for Mn, Cd, Cr, Ni and Pb were calculated to be23.7, 4.74, 2.19, 0.51 and 0.0.24, respectively. The sensitivity analysis showed that, the exposure frequency (EF) for Cr and Ni had the strong effects on generation of both LCR and HQ. Furthermore, the concentrations of Mn, Cd and Pb had strong impacts on the HQ generation and the concentration of Cd and Pb had main effects on LCR generation. CONCLUSION: To effectively reduce pollutant concentration, it is recommended to install a ventilation system near nail salon work tables and conduct continuous monitoring and quality control of nail products.

ABSTRACT

With increasing levels of air pollution, air quality prediction has attracted more attention. Mathematical models are being developed by researchers to achieve precise predictions. Monitoring and prediction of atmospheric PM2.5 levels, as a predominant pollutant, is essential in emission mitigation programs. In this study, meteorological datasets from 9 years in Isfahan city, a large metropolis of Iran, were applied to predict the PM2.5 levels, using four machine learning algorithms including Artificial Neural |Networks (ANNs), K-Nearest-Neighbors (KNN), Support Vector |Machines (SVMs) and ensembles of classification trees Random Forest (RF). The data from 7 air quality monitoring stations located in Isfahan City were taken into consideration. The Confusion Matrix and Cross-Entropy Loss were used to analyze the performance of classification models. Several parameters, including sensitivity, specificity, accuracy, F1 score, precision, and the area under the curve (AUC), are computed to assess model performance. Finally, by introducing the predicted data for 2020 into ArcGIS software and using the IDW (Inverse Distance Weighting) method, interpolation was conducted for the area of Isfahan city and the pollution map was illustrated for each month of the year. The results showed that, based on the accuracy percentage, the ANN model has a better performance (90.1%) in predicting PM2.5 grades compared to the other models for the applied meteorological dataset, followed by RF (86.1%), SVM (84.6%) and KNN (82.2%) models, respectively. Therefore, ANN modelling provides a feasible procedure for the managerial planning of air pollution control.

ABSTRACT

Human viral pathogens, including SARS-CoV-2 continue to attract public and research attention due to their disruption of society, global health, and the economy. Several earlier reviews have investigated the occurrence and fate of SARS-CoV-2 in wastewater, and the potential to use such data in wastewater-based epidemiology. However, comprehensive reviews tracking SARS-CoV-2 and other viral pathogens in the wastewater-water-drinking water continuum and the associated risk assessment are still lacking. Therefore, to address this gap, the present paper makes the following contributions: (1) critically examines the early empirical results to highlight the occurrence and stability of SARS-CoV-2 in the wastewater-source water-drinking water continuum, (2) discusses the anthropogenic and hydro(geo)logical processes controlling the circulation of SARS-CoV-2 in the wastewater-source water-drinking water continuum, (3) discusses the risky behaviour, drivers and high-risk settings in the wastewater-source water-drinking water continuum, (4) uses the available empirical data on SARS-CoV-2 occurrence in the wastewater-source water-drinking water continuum to discuss human health risks from multiple exposure pathways, gendered aspects of SARS-CoV-2 transmission via shared on-site sanitation systems, and (5) develops and risk mitigation strategy based on the available empirical evidence and quantitative human risk assessment data. Finally, it presents a comprehensive research agenda on SARS-CoV-2/COVID-19 to guide the mitigation of future similar outbreaks in low-income settings.

In this study, the occurrence and environmental risks related to triclosan (TCS) in the two wastewater treatment plants (WWTPs) were investigated in Isfahan, Iran. Influent and effluent samples were collected and analyzed by dispersive liquid-liquid microextraction (DLLME)-GC-MS method with derivatization. Moreover, the risk of TCS exposure was conducted for aquatic organisms (algae, crustaceans, and fishes) and humans (males and females). TCS mean concentrations in influent and effluent of WWTPs were in the range of 3.70-52.99 and 0.83-1.09 µg/L, respectively. There were also no differences in the quantity of TCS and physicochemical parameters among the two WWTPs. The mean risk quotient (RQ) for TCS was higher than 1 (in algae) with dilution factors (DFs) equal to 1 in WWTP1. Moreover, the RQ value was higher than 1 for humans based on the reference dose of MDH (RFDMDH) in WWTP1. Furthermore, TCS concentration in wastewater effluent was the influential factor in varying the risk of TCS exposure. The results of the present study showed the risk of TCS exposure from the discharge of effluent of WWTP1 was higher than WWTP2. Moreover, the results of this study may be suitable for promoting WWTP processes to completely remove micropollutants.

BACKGROUND: The incidence of colorectal cancer (CRC) has increased in Iran, and determining the dietary patterns that can contribute to reducing or increasing the risk of CRC will help better control this disease. Therefore, in the current study, we assessed the association between the empirical lifestyle index for hyperinsulinemia (ELIH) and the empirical dietary index for hyperinsulinemia (EDIH) with the CRC odds. METHODS: The present case (n = 71)-control (n = 142) study was carried out in several CRC surgical units of hospitals in Tehran, Iran. A semi-quantitative food frequency questionnaire containing 168 items was used to assess participants' dietary intakes. The EDIH and ELIH scores were calculated by food groups and some variables such as body mass index and physical activity. Logistic regression models were applied to evaluate the association between the EDIH and ELIH scores with CRC odds. RESULTS: According to baseline features of the study participants, there were significant differences between the controls and cases in ELIH score, fiber intake, taking aspirin, and family history of CRC in first- and second-degree relatives. Also, we found that the odds of CRC increased significantly in the last tertile compared to the first tertile in EDIH and ELIH in the adjusted model (odds ratio (OR) = 3.12; 95% confidence interval (CI): 1.30-7.48 and OR = 4.72; 95% CI: 1.15-19.39, respectively). CONCLUSIONS: In conclusion, the result of this study indicated that CRC odds was significantly greater in subjects with higher EDIH and ELIH scores. Also, according to the results of this study, lifestyle and diet with insulinemic potential can influence the CRC risk.

ABSTRACT

The presence of biofilms in drinking water distribution systems (DWDS) in healthcare settings poses a considerable risk to the biological security of water, particularly when the biofilm bacteria demonstrate antimicrobial resistance characteristics. This study aimed to investigate the occurrence of antibiotic-resistant bacteria (ARB) in biofilms within DWDS of hospitals. The chlorine resistance of the isolated ARB was analyzed, and then chlorine-resistant bacteria (CRB) were identified using molecular methods. Additionally, the presence of several antibiotic resistance genes (ARGs) was monitored in the isolated ARB. Out of the 41 biofilm samples collected from hospitals, ARB were detected in 32 (78%) of the samples. A total of 109 colonies of ARB were isolated from DWDS of hospitals, with ß-lactam resistant bacteria, including ceftazidime-resistant and ampicillin-resistant bacteria, being the most frequently isolated ARB. Analyzing of ARGs revealed the highest detection of aac6, followed by sul1 gene. However, the ß-lactamase genes blaCTX-M and blaTEM were not identified in the ARB, suggesting the presence of other ß-lactamase genes not included in the tested panel. Exposure of ARB to free chlorine at a concentration of 0.5 mg/l showed that 64% of the isolates were CRB. However, increasing the chlorine concentration to 4 mg/l decreased the high fraction of ARB (91%). The domi||nant CRB identified were Sphingomonas, Brevundimonas, Stenotrophomonas, Bacillus and Staphylococcus with Bacillus exhibiting the highest frequency. The results highlight the potential risk of biofilm formation in the DWDS of hospitals, leading to the dissemination of ARB in hospital environments, which is a great concern for the health of hospitalized patients, especially vulnerable individuals. Surveillance of antimicrobial resistance in DWDS of hospitals can provide valuable insights for shaping antimicrobial use policies and practices that ensure their efficacy.

ABSTRACT

This research quantitatively evaluated the diversity of plants to protect vulnerable species. To measure vegetation information, the appropriate sampling plot size was determined based on the canopy cover of the dominant species of the study area (1 m2). Then, in each unit, sampling was done along 3 transects of 100 m. Along each transect, 10 plots with dimensions of one square meter were placed at a distance of 10 m from each other. In each plot, the type, life forms, frequency of plant species, and species density were recorded. Species diversity indices were calculated using Ecological Methodology software. The values obtained from these indicators were analyzed in SPSS 24 statistical software and using the F test. The results of the Analysis of variance (ANOVA) showed that the highest values of the species diversity indices are in the middle altitudes (ecotone) class. ANOVA of the richness, evenness, and heterogeneity indices in different altitude classes showed that the values of the richness indices were not significant, but among the indices related to the heterogeneity, the Hill index and all the evenness indices were significant. Comparing the numerical indices of our communities enables us to determine the impact of environmental stress in a single community to choose the best habitat among a similar group for conservation. A community that has high diversity and richness is important for conservation. Therefore, the authorities must prevent the destruction of the vegetation of the study area in connection with the implementation of principled and correct management by the potential of the region, but also to reduce the pressure of livestock grazing and carry out corrective and restoration operations, to turn these rangelands towards rich diversity.

ABSTRACT

BACKGROUND: Non-alcoholic fatty liver disease (NAFLD) is associated with metabolic factors including obesity, dyslipidemia, insulin resistance, oxidative stress, and elevated inflammatory factors. Zinc (Zn) supplementation has been investigated as a potential adjunctive therapy in managing NAFLD outcomes. METHODS: In this randomized, double-blinded, controlled clinical trial, 50 overweight or obese participants with NAFLD were randomized into 2 groups of 25 and received either 30 mg of daily Zn or a placebo for 8 weeks. Both groups were invited to follow a balanced energy-restricted diet and physical activity recommendations. RESULTS: Based on the between-group comparison, Zn supplementation caused a significant increase in the Zn level (P < 0.001) and a significant decrease in weight (P = 0.004), body mass index (BMI) (P = 0.002), waist circumference (P = 0.010), aspartate transaminase (AST) (P = 0.033), total cholesterol (TC) (P = 0.045), and low-density lipoprotein cholesterol (LDL-C) (P = 0.014), but it had no significant effect on alanine transaminase (ALT), fasting blood sugar (FBS), insulin, homeostasis model assessment of insulin resistance (HOMA-IR), high-density lipoprotein (HDL), triglyceride (TG), high-sensitivity C-reactive protein (hs-CRP), malondialdehyde (MDA), and total antioxidant capacity (TAC) (P > 0.05). CONCLUSION: The results of the present study indicated that 8-week supplementation of 30 mg daily Zn may increase the Zn serum level and decline anthropometric parameters, AST, TC, and LDL-C in NAFLD patients, so further research is suggested in the future. ABSTRACT

In the past few decades, there has been a significant focus on detecting steroid hormones in aquatic environments due to their influence on the endocrine system. Most compounds of these pollutants are the natural steroidal estrogens, i.e., estrone (E1), 17ß-Estradiol (E2), and the synthetic estrogen 17α-Ethinylestradiol (EE2). The Moving-Bed Biofilm Reactor (MBBR) technique is appropriate for eliminating steroid hormones. This study centers on creating a model to estimate the effectiveness of the MBBR system regarding its ability to eliminate E1, E2, and EE2. The results were modeled with artificial neural networks (ANNs). The Particle Warm Optimization (PSO) and Levenberg Marquardt (LM) algorithms were selected for network training. The models incorporated five input parameters, encompassing the COD loading rate, initial levels of E1, E2, and EE2 steroid hormones, and Hydraulic Retention Time (HRT). The optimum removal conditions (three steroid hormones and COD) were determined using the optimized ANN based on both PSO and LM algorithms. The optimal transfer functions for the hidden and output layers were identified as tan-sigmoid and linear, respectively. The best ANN structures (Neurons in input, hidden, and output layers) and correlation coefficients (R) were 5:9:4, with R = 0.9978, and 5:10:4, with R = 0.9982 for the trained networks with LM and PSO algorithms, respectively. Eventually, the input parameters' importance was ranked using sensitivity analysis (SA) through Pearson correlation and developed ANNs.

Triclosan (TCS) has been increased in the water during the COVID-19 pandemic because it cannot remove by conventional water treatment. In addition, it can accumulate in the human body over time through long-term exposure. Therefore, the occurrence of TCS in the water treatment plant (WTP) and tap water, and its human health risk assessment through tap water ingestion, dermal absorption, and inhalation routes in Isfahan, Iran, were investigated. Moreover, spatial regression methods were used for the prediction of water quality parameters, TCS concentration, and total hazard quotient (HQ). The average TCS concentration in the influent and effluent of WTP and tap water was 1.6, 1.4, and 0.4 µg/L, respectively. Conventional WTP has low efficiency in the removal of TCS (12.6%) from water. The average values of total HQ for males were 7.79×10-5, 4.97×10-4, and 4.97×10-5 and for females were 3.31×10-5, 2.11×10-4, and 2.11×10-5 based on RfDEPA, RfDMDH, and RfDRodricks, respectively that were in the low-risk levels (HQ<1). Furthermore, TCS concentration in tap water and the ingestion rate of drinking water had the highest effect on the risk of TCS exposure from tap water. The non-carcinogenic health risk of TCS in water was low. The results of this study may be useful for promoting WTP processes to remove emerging pollutants.

MATERIALS AND METHODS: This descriptive-survey study was conducted on the students and professors of Isfahan and Kashan Universities of Medical Sciences in fields related to social determinants of health and at different educational levels in the years 2020-2021. Data were collected by a researcher-made questionnaire of awareness and attitude. Data were reported by descriptive statistics using Statistical Package for Social Sciences (SPSS) 20. RESULTS: The average percentage of correct answers to awareness questions was 44% by professors and 33.3% by students. The overall mean score of professors' attitude toward social determinants of health was 2.48 and students' attitudes were 2.65 out of 5. Professors' awareness about social determinants of health was more than students and their attitude was less positive than students. CONCLUSION: Considering that a significant percentage of effective factors in health outcomes are social determinants of health and bearing in mind that universities, especially medical universities in providing healthcare, maintaining, improving, and promoting the health of the community, and also training the necessary human resources in the health sector to provide services play an important role, officials and decision-makers in the Ministry of Health and universities should decide to define this issue in the educational curriculum as well as holding related workshops.

ABSTRACT

Serious health issues can result from exposure to the nitrogenous pollutant like 2,4,6-trinitrotoluene (TNT), which is emitted into the environment by the munitions and military industries, as well as from TNT-contaminated wastewater. The TNT removal by extended aeration activated sludge (EAAS) was optimized in the current study using artificial neural network modeling. In order to achieve the best removal efficiency, 500 mg/L of chemical oxygen demand (COD), 4 and 6 h of hydraulic retention time (HRT), and 1-30 mg/L of TNT were used in this study. The kinetics of TNT removal by the EAAS system were described by the calculation of the kinetic coefficients K, Ks, Kd, max, MLSS, MLVSS, F/M, and SVI. Adaptive neuro fuzzy inference system (ANFIS) and genetic algorithms (GA) were used to optimize the data obtained through TNT elimination. ANFIS approach was used to analyze and interpret the given data, and its accuracy was around 97.93%. The most effective removal efficiency was determined using the GA method. Under ideal circumstances (10 mg/L TNT concentration and 6 h), the TNT removal effectiveness of the EAAS system was 84.25%. Our findings demonstrated that the artificial neural network system (ANFIS)-based EAAS optimization could enhance the effectiveness of TNT removal. Additionally, it can be claimed that the enhanced EAAS system has the ability to extract wastewaters with larger concentrations of TNT as compared to earlier experiments.

ABSTRACT

Widespread use of benzophenones (BPs), a group of environmental phenolic compounds, is suspected of interfering with human health. The association of prenatal exposure to benzophenone derivatives with birth outcomes including birth weight and length, head, arm and thoracic circumference, abnormalities, corpulence index and anterior fontanelle diameter (AFD) was investigated. Mother-infant pairs of 166 within PERSIAN cohort population in Isfahan, Iran, in the 1st and 3rd trimesters of pregnancy were assessed. Four common benzophenone metabolites including 2,4-dihydroxy benzophenone (BP-1), 2-hydroxy-4-methoxy benzophenone (BP-3), 4-hydroxy benzophenone (4-OH-BP) and 2,2'-dihydroxy-4-methoxy benzophenone (BP-8) were measured in maternal urine samples. The median concentration of 4-OH-BP, BP-3, BP-1 and BP-8 were 3.15, 16.98, 9.95 and 1.04 µg/g Cr, respectively. In the 1st trimester, 4-OH-BP showed a significant correlation with AFD in total infants, decreasing 0.034 cm AFD per a log unit increase of 4-OH-BP. Within the male neonates, 4-OH-BP in the 1st and BP-8 in the 3rd trimester were significantly associated with head circumference and AFD increase, respectively. Among female neonates in the 3rd trimester, increasing 4-OH-BP and BP-3 concentration was correlated with a decrease in birth weight and AFD, respectively. This study demonstrated that all the target BP derivatives can influence normal fetal growth at any age of the pregnancy, nevertheless, to support these findings further studies are needed in a large and different group population.

BACKGROUND: Depression is prevalent among individuals with non-alcoholic fatty liver disease (NAFLD) and can cause poor health outcomes. Moreover, a solid bilateral association between NAFLD and depression has been shown, which may alleviate by kefir consumption. Thus, we aimed to investigate the effect of milk kefir drinks on the depression status of individuals with NAFLD. METHODS: In a secondary outcome analysis of a randomized, single-blinded, controlled clinical trial, 80 adults with grades 1 to 3 of NAFLD were included in an 8-week intervention. Participants were randomly assigned to Diet or Diet + kefir groups to either follow a low-calorie diet or a low-calorie diet along with a 500 cc milk kefir drink daily. The participants' demographic, anthropometric, dietary, and physical data were recorded before and after the study. Depression status was assessed using the Persian format of the second version of the Beck Depression Inventory (BDI-II-Persian) at the baseline and after 8 weeks of intervention. RESULTS: Overall, 80 participants aged 42.87 ± 10.67 years were included in the analysis. The data on the baseline demographic, dietary, and physical activity of the groups were not significantly different. During the study, participants in Diet + Kefir group had a significantly decreased energy (P = 0.02), carbohydrate (P = 0.4), and fat consumption (P = 0.4). However, during the study, the depression score was not significantly reduced in the Diet group, the Diet + Kefir group showed a significant reduction in depression (P = 0.02). However, between-group analyses for changes in depression were not significant (P = 0.59). CONCLUSION: Consumption of milk kefir drink for 8 weeks may not reduce depression symptoms in adults with NAFLD. ABSTRACT

This study aimed to assess the triclosan (TCS) health risk in an Iranian pregnant women sample by Monte Carlo simulation (MCS). The urinary TCS of 99 women after the 28th week of pregnancy was detected by gas chromatography/mass spectrometry detector (GC/MS), and the MCS model implemented a health risk assessment. The corresponding hazard quotient (HQ) and the sensitivity analysis were calculated. TCS was measured in 100% of the urine samples with a median concentration of 2.89 µg/L. The median of HQ was obtained at 1.93 × 10-4. The TCS exposure risk in the studied population was lower than the allowable limit. A comparison between HQ values in the two weight subgroups of pregnant women showed that the risk level is almost equal, and there was minimal health risk in pregnant women from exposure to TCS.

BACKGROUND: Cardiovascular diseases (CVDs) remain the leading cause of mortality worldwide. This underlies the need to evaluate different targets, such as diet quality. In this regard, we conducted the present study to find whether the healthy eating index-2015 (HEI-2015) score is associated with a 10-year risk of CVDs based on Framingham Risk Score (FRS) and QRISK3 in different body mass index (BMI) groups. METHODS: This cross-sectional study was performed based on Shiraz University of Medical Sciences Employees Health Cohort Study (SUMS EHCS) data in April 2020. A total of 764 participants met the inclusion criteria. An expert performed demographic, anthropometric, and dietary evaluations. A semi-quantitative food frequency questionnaire (FFQ) was applied to assess the diet quality, and FRS and QRISK3 were used to evaluate the 10-year risk of CVDs. RESULTS: Based on the results, many components of HEI-2015 indicated an increasing trend through quartiles (p < 0.001). However, the consumption of refined grains in higher quartiles showed a decreasing trend (p < 0.001). The consumption of added sugar and saturated fatty acids (SFAs) in higher quartiles revealed an increasing trend (p < 0.001). In addition, lower HEI-2015 scores and lower whole grain consumption were significantly associated with higher BMI (p < 0.05). Also, lower consumption of fruits showed a significant relationship with higher risk scores of Framingham and QRISK3 (p < 0.05). Higher added sugar and SFAs intake was significantly related to lower FRS (p < 0.05). A significant reverse association between HEI-2015 and QRISK3 and Framingham risk scores was seen (p < 0.05). CONCLUSION: Our findings support dietary recommendations to increase fruit and whole grains intake to prevent CVD and obesity. Moreover, a significant inverse association between HEI-2015 and QRISK3 and Framingham risk scores was observed. Since the results for added sugars and SFA intakes were controversial, further studies are needed.
