ABSTRACT

The usefulness of Contingent Negative Variation (CNV) potential as a biomarker of neurocognitive disorders due to possible Alzheimer's disease, is based on its possible physiological correlates. However, its application in the diagnostic evaluation of these disorders is still incipient. The aim of this study is to characterize the patterns of cognitive processing of information in the domain of nonspecific global attention, by recording potential CNV in a group of patients with neurocognitive disorders due to possible Alzheimer's disease. An experimental study of cases and controls was carried out. The sample included 39 patients classified according to DSM-5 with a neurocognitive disorder subtype possibly due Alzheimer's disease, and a Control Group of 53 subjects with normal cognitive functions. CNV potential was registered using standard protocol. The analysis of variance obtained significant differences in mean values and confidence intervals of total CNV amplitude between the three study groups. The late CNV segment amplitudes makes it possible to discriminate between the level of mild and major dysfunction in the group of patients. The CNV total amplitudes of potential allows for effective discrimination between normal cognitive functioning and neurocognitive disorders due to possible Alzheimer's disease.

ABSTRACT

INTRODUCTION: Post-Coronavirus disease 2019 (Post-COVID-19) syndrome has neurological symptoms related to the dysfunction of the autonomous nerve system. However, a pathogenic relationship between post-COVID-19 syndrome and dysautonomia still remains to be demonstrated. Establishing a pathogenic relationship between paresthesia and the presence of cardiac dysautonomia in patients with post-COVID-19 syndrome is the objective of this study. PARTICIPANTS AND METHODS: This observational study was carried out in the neurophysiology service wing of the Juan Bruno Zayas Hospital, Santiago de Cuba, in Cuba. The patients were recruited through a post-COVID-19 clinic at the same hospital. A variability study of cardiac frequency and a test of autonomic cardiovascular reflexes was carried out, which is composed of deep breathing, orthostatism, and the Valsalva maneuver. RESULTS: The variability parameters of the cardiac frequency, the expiration-inspiration ratio between deep breaths, and the Valsalva Index showed no statistically significant differences between healthy participants and those with post-COVID-19 syndrome. During the Valsalva maneuver, there was a greater cardiac frequency response in participants with post-COVID-19 syndrome than in healthy subjects. The difference in supine and standing blood pressure was significantly minor in patients with post-COVID-19 syndrome. The logarithm of high frequency (log HF) increased significantly in patients with paresthesia when compared to patients without paresthesia. CONCLUSIONS: In the autonomic function tests, no signs of dysautonomia were found in patients with post-COVID-19 syndrome. The presence of paresthesias is associated with differences in cardiac vagal activity, which may suggest that damage to peripheral sensory nerve fibers could be associated with an affectation to autonomic fibres.

ABSTRACT

Objective: This work aimed to determine the relationship between the autonomic nervous system and reactive hyperemia (RH) in type 2 diabetes patients with and without cardiovascular autonomic neuropathy (CAN). Methodology: A systematic review of randomized and nonrandomized clinical studies characterizing reactive hyperemia and autonomic activity in type 2 diabetes patients with and without CAN was performed. Results: Five articles showed differences in RH between healthy subjects and diabetic patients with and/or without neuropathy, while one study did not show such differences between healthy subjects and diabetic patients, but patients with diabetic ulcers had lower RH index values compared to healthy controls. Another study found no significant difference in blood flow after a muscle strain that induced reactive hyperemia between normal subjects and non-smoking diabetic patients. Four studies measured reactive hyperemia using peripheral arterial tonometry (PAT); only two found a significantly lower endothelial-function-derived measure of PAT in diabetic patients than in those without CAN. Four studies measured reactive hyperemia using flow-mediated dilation (FMD), but no significant differences were reported between diabetic patients with and without CAN. Two studies measured RH using laser Doppler techniques; one of them found significant differences in the blood flow of calf skin after stretching between diabetic non-smokers and smokers. The diabetic smokers had neurogenic activity at baseline that was significantly lower than that of the normal subjects. The greatest evidence revealed that the differences in RH between diabetic patients with and without CAN may depend on both the method used to measure hyperemia and that applied for the ANS examination as well as the type of autonomic deficit present in the patients. Conclusions: In diabetic patients, there is a deterioration in the vasodilator response to the reactive hyperemia maneuver compared to healthy subjects, which depends in part on endothelial and autonomic dysfunction. Blood flow alterations in diabetic patients during RH are mainly mediated by sympathetic dysfunction. The greatest evidence suggests a relationship between ANS and RH; however, there are no significant differences in RH between diabetic patients with and without CAN, as measured using FMD. When the flow of the microvascular territory is measured, the differences between diabetics with and without CAN become evident. Therefore, RH measured using PAT may reflect diabetic neuropathic changes with greater sensitivity compared to FMD.

ABSTRACT

ABSTRACT

Introduction: Due to the need for an early diagnosis of neurodegenerative disorders, attempts have been made to harmonize diagnostic criteria using morphometric methods based on neuroimaging techniques, but conclusive results have not yet been obtained. Objective: To determine the ventricular volume due to its wide use as a marker of cerebral atrophy and to identify the effect of sex on these structures, according to the type of skull, estimated from multislice computed tomography imaging techniques. Methods: An observational and descriptive study was developed in 30 subjects with normal neurocognitive functions and neuropsychiatric examination, aged between 45 and 54 years, who underwent a simple multislice CT scan of the skull. An image segmentation method based on homogeneity was used. Results: The ventricular volumes showed a significant and positive correlation between them, except between the third and fourth ventricles and the third and the right ventricular volume. The statistics in the multivariate linear model applied showed that they were only significant in terms of sex and type of skull. No significant differences were found regarding sex in any volume except in the third ventricle (p= 0.01). The same occurred by type of skull (p= 0.005). Conclusions: The morphometry method of the encephalic ventricular system from Computed Tomography images / Segmentation by homogeneity, allowed to quantify the cerebral volumetric changes associated with normal aging and can be used as a biomarker of the relationship between brain structure and cognitive functions.

ABSTRACT

Texture analysis applications and their extraction of features are considered research trends in neuroscience. Texture as a method of image analysis has shown promising results in the detection of visible and non-visible lesions, and in computed tomography (CT) studies they are scarce. The present research aims to determine the applicability of the automatic processing of homogeneous texture indices in the volumetric estimation of brain gray matter in cranial CT images. For this, artificial images with predefined regions and the selection of CT images were used in patients with previous indications for CT of the skull. Two fundamental steps are taken for the implementation of this approach. As a result, an automatic windowless pattern recognition method was obtained by means of the extraction of homogeneous texture characteristics through the co-occurrence matrix(AU)

ABSTRACT

Introduction: Alzheimer disease is presented with different neuropsychological phenotypes and it is considered that the memory disorder is its most important one; however, recent studies suggest that the dysfunctions in the executive functions can constitute very early markers of the disease. Objectives: To characterize the clinical and neuropsychological profiles that constitutes phenotypical markers of the neurocognitive disorders possible Alzheimer subtype. Methods: A descriptive and cross-sectional study of 39 sick persons with ages between 57-85 years, classified according to the DSM-5 criteria, with cognitive mild and major disorder, classified from the etiological point of view in the possible Alzheimer subtype was carried out. Multiple cognitive domains were evaluated (complex attention, memory and learning, executive functions, language, motor and perceptual functions) and a detailed evaluation of the executive functions was made by means of a group of specific neuropsychological tests. Results: The statistical analysis demonstrated that comparing the two study groups with cognitive disorder, significant differences exist as for the level of cognitive performance in all the explored domains, except in the perception and motor functions. Likewise, the results demonstrated that the yielding level for each function evaluated within the domain of the executive functions is significantly different between both groups with cognitive disorder, except for the space orientation. Conclusions: The neuropsychological profile of the cognitive disorder Alzheimer possible subtype was characterized by a disorder in multiple domains, with predominance of the focused executive dysfunction in the areas of the mental flexibility and inhibition of automatisms.

ABSTRACT

ABSTRACT

ABSTRACT

A sample of 35 healthy individuals over 18 years chosen at random, was also included. Significant differences between the biomechanical phenomenum variables extracted from the evaluation of gait in sick and healthy people were observed. The method used allowed to determine that the course of the disease influences in the decrease of gait speed and increases the step width and the hip oscillations

ABSTRACT

ABSTRACT

Introduction: During surgery a patient under general anesthesia must remain unconscious and insensitive to pain. However, cases have been reported of intraoperative awareness. Due to the incidence of this phenomenon and the adverse effects it causes, the Center for Neuroscience and Image and Signal Processing Studies of the University of Oriente, Cuba, is developing a prototype for an anesthesia monitor allowing detection of changes in anesthetic status based on automated recognition of Anesthetic Depth Levels in electroencephalographic signals. Objective: Automatically detect anesthetic sedation states in electroencephalographic signals as a support system for intraoperative monitoring. Methods: Recording was conducted of electroencephalographic signals from 27 patients undergoing general abdominal surgery. The channel selected for the study was F4. Detection of anesthetic depth levels was performed using Artificial Intelligence computer methods. Results: The anesthetic depth scale was reduced to three levels. Recognition effectiveness was 90.24 percent for the light level, 90.06 percent for the moderate level, and 12 percent for the deep level. Conclusions: Three anesthetic depth levels are proposed, which were detected with above 90 percent accuracy in electroencephalographic signals. The daily work of anesthesiologists will be improved with the use of the monitor being developed at the above mentioned study center. Results show that F4 derivation is representative of the effect of anesthetics upon brain activity(AU)

ABSTRACT

Digital image processing and computer vision are frequently used in medicine at present and the proposals of new methods of automatic analysis of digital images or the efficiency improvement of the existing are of great interest. In this work new methods to computationally study sickle cell disease through blood samples images are developed, an illness with high incidence in the world and in Cuba, mainly in the eastern region. New shape analysis methods obtained from classical results of integral geometry and new computer vision proposals for evaluate neuro physiological disorders associated with this illness through the study of the facial expressions of the patient were proposed. The statistical validation realized confirmed the superiority of these methods on previous proposals, which is why they are valid to be introduced in support software to improve the quality of the medical attention(AU)
