ABSTRACT

The aim of this study was to evaluate the effect of the month of oocyte harvesting and the temperature-humidity index on oocyte number and quality harvested from Holstein cows and heifers, oocyte developmental competence, and total embryos produced in an area of intense ambient temperature for most of the year. A total of 5064 multiparous lactating cows and 2988 nulliparous heifers were used as oocyte donors and distributed across the months of the year. Overall, total oocytes per collection did not differ (p > 0.05) between cows (16.6 ± 2.7) and heifers (15.1 ± 1.8), but oocyte developmental competence was lower (p < 0.05) in cows (21.3 ± 5.4) than heifers (25.5 ± 4.0). For cows, the total number of oocytes harvested was two-fold higher (p < 0.05) in November than in May. For heifers, the total number of oocytes harvested was highest in April (17.19 ± 0.53) and lowest in May (10.94 ± 0.32; p < 0.05). For cows, total embryos were highest in November (2.58 ± 0.42) and lowest in August (1.28 ± 0.10; p < 0.05). Thus, taken together, these results indicate that severe heat stress impaired the number and quality of oocytes harvested from donor Holstein multiparous cows and heifers, oocyte developmental competence, and total embryos produced in this area of intense ambient temperature for most of the year.

BACKGROUND: Heart valve replacement surgery with mechanical or biological prostheses entails a risk of thromboembolism and bleeding complications. OBJECTIVE: To determine the complications related to complementary anticoagulation therapy and the probability of risk. METHODS: One-hundred and sixty-three patients who underwent heart valve replacement between 2002 and 2016 with either mechanical or biological prostheses, and who received vitamin K antagonists after hospital discharge, were studied. Anticoagulation therapy was categorized into optimal and non-optimal according to INR values prior to the development of complications. Patients with comorbidities and other risk factors for thrombosis and/or bleeding were excluded. RESULTS: In total, 68.7 % of patients received mechanical prostheses, and 31.3 %, biological prostheses (p ≤ 0.001); 25.2 % experienced the complications that motivated the study (p ≤ 0.001), which were hemorrhagic in 48.8 %, thromboembolic in 26.8 %, and of both types in 24.4 % (relative risk = 4.229). Among the patients with complications, 95.1 % received mechanical prostheses, and 4.9 %, biological (p = 0.005); non-optimal INR was identified in 49.7 % (p ≤ 0.001). CONCLUSIONS: Given the high risk of thromboembolic and hemorrhagic complications, valve prostheses must be carefully chosen, and care priorities should include prevention and follow-up, especially in those patients who require anticoagulation therapy.

ABSTRACT

Genomic data and machine learning approaches have gained interest due to their potential to identify adaptive genetic variation across populations and to assess species vulnerability to climate change. By identifying gene-environment associations for putatively adaptive loci, these approaches project changes to adaptive genetic composition as a function of future climate change (genetic offsets), which are interpreted as measuring the future maladaptation of populations due to climate change. In principle, higher genetic offsets relate to increased population vulnerability and therefore can be used to set priorities for conservation and management. However, it is not clear how sensitive these metrics are to the intensity of population and individual sampling. Here, we use five genomic datasets with varying numbers of SNPs (NSNPs = 7006-1,398,773), sampled populations (Npop = 23-47) and individuals (Nind = 185-595) to evaluate the estimation sensitivity of genetic offsets to varying degrees of sampling intensity. We found that genetic offsets are sensitive to the number of populations being sampled, especially with less than 10 populations and when genetic structure is high. We also found that the number of individuals sampled per population had small effects on the estimation of genetic offsets, with more robust results when five or more individuals are sampled. Finally, uncertainty associated with the use of different future climate scenarios slightly increased estimation uncertainty in the genetic offsets. Our results suggest that sampling efforts should focus on increasing the number of populations, rather than the number of individuals per populations, and that multiple future climate scenarios should be evaluated to ascertain estimation sensitivity.

ABSTRACT

Reproductive biology is an important topic that is well explored in many vertebrates, but information about frogs' reproductive mechanisms could be improved. Therefore, this review aims to provide organized and specific information on frog reproduction. First, we developed schemes that illustrate the general information regarding reproductive biological mechanisms in frogs in a specific way. Then, we described the physiological, histological, and morphological mechanisms of each organ of the reproductive system of male and female frogs. Finally, this manuscript may contribute to a broader understanding of anuran reproductive biology. Since, understanding frogs' reproductive system permits one to make a comparison with reproduction with other anurans.

ABSTRACT

Fundamento: la promoción de salud debe comenzar en edades tempranas del desarrollo, para que los niños adquieran estilos de vida saludables, que les resulten beneficiosos en el futuro. Objetivo: implementar un programa educativo de salud bucal, dirigido a las familias de los niños/ñas de tres a cinco años matriculados en el Programa Educa a tu hijo, de los consultorios ocho y nueve del Consejo Popular Ramón Balboa, del municipio Lajas. Métodos: se realizó un estudio de intervención, de septiembre del 2020 a julio del 2021, en la sede del Programa Educa a tu hijo, con 35 miembros de familia y 35 niños/as entre tres cinco años matriculados en dicho programa. Se analizaron las variables: conocimiento sobre higiene bucal, hábitos dietéticos y hábitos deformantes, medidas antes y después de la intervención educativa. Resultados: predominó el sexo femenino y edad de cuatro años. Los familiares tenían entre 20 y 34 años. Los principales factores de riesgo identificados fueron higiene bucal deficiente, cepillado dental incorrecto y dieta cariogénica. Antes de la intervención predominó el conocimiento regular sobre higiene bucal, un conocimiento malo sobre hábitos dietéticos y un mal nivel de conocimiento sobre hábitos deformantes. Después de la intervención se observó un mayor porcentaje de conocimiento bueno sobre higiene bucal, hábitos dietéticos y buen nivel de conocimiento sobre hábitos deformantes. Conclusiones después de aplicada la intervención educativa se evidenció una notable mejoría en los conocimientos sobre salud bucal.


Background: health promotion should begin at an early age of development, so that children acquire healthy lifestyles that will be beneficial in the future. Objective: to implement an educational oral health program, aimed at the families of children from three to five years old enrolled in the Educate your child Program, from clinics eight and nine of the Ramón Balboa neighborhood, in Lajas municipality. Methods: an intervention study was carried out, from September 2020 to July 2021, at the headquarters of the Educate your child program, with 35 family members and 35 children between three and five years old enrolled in this program. The analyzed variables were: knowledge about oral hygiene, dietary habits and deforming habits, measured before and after the educational intervention. Results: the female sex and age of four years prevailed. The relatives were between 20 and 34 years old. The main risk factors identified were poor oral hygiene, incorrect tooth brushing and cariogenic diet. Before the intervention, regular knowledge about oral hygiene, poor knowledge about dietary habits, and a poor level of knowledge about deforming habits prevailed. After the intervention, a higher percentage of good knowledge about oral hygiene, dietary habits and a good level of knowledge about deforming habits was observed. Conclusions: after applying the educational intervention, a notable improvement in knowledge about oral health was evidenced.

ABSTRACT

INTRODUCTION: Hartmann's procedure (HP) is the conventional treatment in patients with complicated diverticulitis. Segmental resection with primary anastomosis (PA) is a treatment alternative for those patients. Our aim was to compare the postoperative results of HP and PA in patients with complicated diverticulitis (Hinchey stage III). METHODS: A case-control study was conducted on patients operated on for purulent Hinchey stage III diverticulitis, within the time frame of 2000 and 2019. RESULTS: Twenty-seven patients that underwent PA were compared with 27 that underwent HP. The patients that underwent HP had a greater probability of morbidity at 30 days (OR 3.5; 95% CI 1.13-11.25), as well as a greater probability of major complications (OR 10.9; 95% CI 1.26-95.05). CONCLUSION: The patients that underwent segmental resection and PA presented with lower morbidity rates and higher stoma reversal rates than the patients that underwent HP.

RESUMEN Introducción: Actualmente se ha observado que la hipovitaminosis D y la obesidad pueden influir en el desarrollo de enfermedad cardiovascular en el futuro. Objetivo: Identificar la asociación entre deficiencia de vitamina D y factores de riesgo cardiometabólicos en los pacientes pediátricos del noroeste de México. Materiales y métodos : se incluyeron niños de 6 a 15 años, se les midieron variables somatométricas, niveles séricos de colesterol total, C-HDL, C-LDL, triglicéridos, glucosa, insulina, vitamina D e índice HOMA-IR. Se identificó la asociación de factores de riesgo cardiometabólicos y la deficiencia de vitamina D, mediante la prueba de Chi cuadrado. Resultados: De los 114 pacientes evaluados, se detectó eutrofia en 42.1%, sobrepeso en 12.3%, obesidad en 41.2% y desnutrición en 4.4%. La prevalencia de deficiencia en vitamina D, insuficiencia y suficiencia fueron 18.4%, 27.2% y 54.4% respectivamente. La deficiencia de vitamina D mostró mayor predominio en pacientes con obesidad (27 %). La hipertrigliceridemia se asoció estadísticamente con deficiencia de vitamina D (p 0.041). Se observaron correlaciones inversas entre niveles de vitamina D con HOMA (r=-0.191; p=0.41), score Z IMC (r=-0.210; p=0.025) e insulina (r=-0.227; p=0.015). Conclusiones: La deficiencia de vitamina D se asocia en niños con un IMC elevado y resistencia a la insulina, lo cual puede acelerar el desarrollo de síndrome metabólico, diabetes mellitus tipo 2 y enfermedad cardiovascular.


ABSTRACT Introduction: It has currently been observed that hypovitaminosis D and obesity can influence the future development of cardiovascular disease. Objective: To identify the association between vitamin D deficiency and cardiometabolic risk factors in pediatric patients from northwestern Mexico. Materials and methods: Children aged 6 to 15 years were included, somatometric variables, serum levels of total cholesterol, HDL-C, LDL-C, triglycerides, glucose, insulin, vitamin D and HOMA-IR index were measured. The association of cardiometabolic risk factors and vitamin D deficiency was identified using the Chi square test. Results: Of the 114 patients evaluated, normal body-mass index was detected in 42.1%, 12.3% were overweight, 41.2% were obese and 4.4% were malnourished. The prevalence of vitamin D deficiency, insufficiency and sufficiency were 18.4%, 27.2% and 54.4% respectively. Vitamin D deficiency was more prevalent in obese patients (27%). Hypertriglyceridemia was statistically associated with vitamin D deficiency (p = 0.041). Inverse correlations were observed between vitamin D levels with HOMA (r = -0.191; p = 0.41), BMI Z score (r = -0.210; p = 0.025) and insulin (r = -0.227; p = 0.015). Conclusions: Vitamin D deficiency is associated with a higher BMI and insulin resistance in children, which can accelerate the development of metabolic syndrome, type 2 diabetes mellitus and cardiovascular disease.

ABSTRACT

A fast PCR-assisted impedimetric biosensor was developed for the selective detection of the clbN gene from the polyketide synthase (pks) genomic island in real Escherichia coli samples. This genomic island is responsible for the production of colibactin, a harmful genotoxin that has been associated with colorectal cancer. The experimental protocol consisted of immobilizing the designated forward primer onto an Au electrode surface to create the sensing probe, followed by PCR temperature cycling in blank, positive, and negative DNA controls. Target DNA identification was possible by monitoring changes in the system's charge transfer resistance values (Rct) before and after PCR treatment through electrochemical impedance spectroscopy (EIS) analysis. Custom-made, flexible gold electrodes were fabricated using chemical etching optical lithography. A PCR cycle study determined the optimum conditions to be at 6 cycles providing fast results while maintaining a good sensitivity. EIS data for the DNA recognition process demonstrated the successful distinction between target interaction resulting in an increase in resistance to charge transfer (Rct) percentage change of 176% for the positive DNA control vs. 21% and 20% for the negative and non-DNA-containing controls, respectively. Results showed effective fabrication of a fast, PCR-based electrochemical biosensor for the detection of pks genomic island with a calculated limit of detection of 17 ng/µL.

Resumen: La atención y manejo del paciente quemado en México representa un reto para el personal médico, la estancia intrahospitalaria del quemado generalmente es prolongada, por lo que los pacientes pueden desarrollar complicaciones psicológicas derivadas del cambio en su aspecto físico y los tratamientos que este tipo de patología requiere, por lo que la aparición de ansiedad es común. El propósito de este trabajo es plantear una propuesta de terapia audiovisual como tratamiento alternativo y/o complementario en el control de la ansiedad en el paciente durante su estancia en la unidad de cuidados intensivos.


Abstract: The care and management of the burn patient in Mexico represents a challenge for the medical staff, the hospital stay of the burn is generally prolonged so that patients can develop psychological complications derived from the change in their physical appearance and the treatments that this type of pathology requires, so the appearance of anxiety is common. The purpose of this work is to propose a proposal of audiovisual therapy as an alternative and/or complementary treatment in the control of anxiety in the patient during his stay in the intensive care unit.


Resumo: O cuidado e o manejo do paciente queimado no México representam um desafio para a equipe médica, a internação do paciente queimado é geralmente prolongada para que os pacientes possam desenvolver complicações psicológicas derivadas da mudança em sua aparência física e dos tratamentos que este tipo de a patologia exige, então o aparecimento de ansiedade é comum. O objetivo deste trabalho é propor uma proposta de terapia audiovisual como alternativa e/ou tratamento complementar no controle da ansiedade do paciente durante sua permanência em unidade de terapia intensiva.

ABSTRACT

A total of 1338 high-yielding cows with five lactations from a large well-managed commercial dairy operation in a hot environment of northern Mexico were used to evaluate milk yield and reproductive performance of cows undergoing unplanned extended lactations (EL; > 450 days). The null hypothesis that cumulative milk yield during five consecutive lactations is the same in cows not having unplanned EL and cows with several EL was tested. All EL resulted from cows failing to become pregnant with ≥ 4 services. Cows were allocated to 1 of 6 groups according to the number of EL (EL0 for cows with no EL to EL5 for cows with 5 EL). Cumulative milk yield was highest (P < 0.01) for EL5 cows (95,499 kg) and lowest for EL0 cows (56,236 kg). Likewise, cumulative days in milk were greatest (P < 0.01) in EL5 (2968 days) and lowest in EL0 (1857 days) cows. Mean milk yield per lactation increased linearly with the increase in the number of extended lactations with a maximum of 19,099 kg of milk/lactation for EL5 cows. Mean daily milk yield/productive life was lowest (P < 0.01) for EL0 (30.2 kg) and highest for EL5 (32.1 kg). The percentage of days in milk relative to days from birth to the end of the fifth lactation was highest for EL5 (78.1%) and lowest for EL0 (59.6%), with EL2 to EL4 cows intermediate. Length of calving interval was longest (P < 0.01) for EL5 (24.7 months) and shortest for EL0 (15.5 months) cows. It was concluded that it is worthwhile maintaining high-yielding cows incapable to conceive before 230 days in milk because they did conceive again and produced considerable amounts of milk during their productive life, which substantially prolonged their permanence time in the herd.

INTRODUCTION AND AIMS: More than 20% of healthcare-associated infections correspond to those at surgical sites, and there is a higher incidence of infections in colorectal surgery due to the associated bacterial load. Surgical wound protectors are designed to prevent contamination and mechanical trauma. Our aim was to demonstrate the usefulness of a circumferential wound retractor/protector for the prevention of surgical site infections (SSIs) in emergency colorectal surgery. METHODS: Forty-one patients that underwent emergency open surgery at a tertiary care hospital were randomized into 2 groups: 20 cases without the retractor (group A) and 21 cases with the retractor (group B). Subjects were assigned to a group in a 1:1 randomization allocation ratio. The chi-square and Fisher's exact tests were employed for the quantitative variables, and the statistical analysis was performed using the IBM Statistical Package for the Social Sciences software for Mac, version 16.0 (IBM SPSS Inc., Chicago, IL, USA). RESULTS: The SSI rate was 17%. Six group A patients developed SSI versus one group B patient. The use of a circumferential wound retractor/protector was statistically significant for the prevention of surgical wound infections, with a P=.031 and an OR of 8.5. In addition, preoperative blood glucose levels below 200mg/dl provided a 3.2-times higher protective effect, compared with glucose levels above 200mg/dl. CONCLUSIONS: In the present prospective randomized pilot study, the use of the circumferential wound retractor/protector significantly decreased the likelihood of SSI in emergency colorectal surgery.

ABSTRACT

Introducción. En octubre 2018 inició la estrategia de atención integral de pacientes con tuberculosis y diabetes mellitus en seis unidades comunitarias de salud familiar de la región metropolitana. Dicha estrategia brinda atención integral a pacientes con estas enfermedades. Objetivo. Establecer la asociación entre tuberculosis y diabetes mellitus en establecimientos que han implementado el plan de atención integral para tuberculosis y diabetes mellitus. Metodología. Estudio transversal analítico donde se estudiaron 12 casos nuevos con tuberculosis y diabetes mellitus, mayores de 10 años, sin otro factor de riesgo. Resultados. La prevalencia de diabetes mellitus en los casos de tuberculosis en los establecimientos donde se ha implementado el plan, es mayor que en la población general de El Salvador (p = 0.02). No existe diferencia significativa entre la prevalencia obtenida en los establecimientos donde se ha implementado el plan y los resultados a nivel nacional para ambos sexos (femenino 0.09 y masculino 0.07). La probabilidad de padecer tuberculosis es 11.4 veces mayor entre los diabéticos. Conclusiones. Existe una asociación significativa entre la diabetes mellitus y tuberculosis en los establecimientos donde se ha implementado el plan de atención integral para tuberculosis y diabetes mellitus. La probabilidad de enfermar de tuberculosis en los diabéticos es mayor que en los no diabéticos.


Introduction. In October 2018, the strategy of comprehensive care for patients with tuberculosis and diabetes mellitus began in 6 community family health units in the metropolitan region, a strategy that provides comprehensive care for patients with these diseases. Objective. Establish the association between tuberculosis and diabetes mellitus, in establishments that have implemented the comprehensive care plan for tuberculosis and diabetes mellitus. Methodology. This study is an analytical cross-section where 12 new cases with tuberculosis and diabetes mellitus, older than 10 years, with no other risk factor were studied. Results. The prevalence of diabetes mellitus in cases of tuberculosis in the establishments where the plan has been implemented is higher than in the general population of El Salvador (p = 0.02). There is no significant difference between the prevalence obtained in the establishments where the plan has been implemented and the results at the national level for both sexes (female 0.09 and male 0.07). The probability of suffering from tuberculosis among diabetics is 11.4. Conclusions. The association of diabetes mellitus and tuberculosis is greater in the establishments where the comprehensive care plan for tuberculosis and diabetes mellitus has been implemented in comparison to the general population. The probability of getting tuberculosis in diabetics is higher than in non-diabetics.

ABSTRACT

INTRODUCTION AND AIMS: Cholangiocarcinoma accounts for 3% of gastrointestinal tumors and is the second most frequent hepatic neoplasia after hepatocellular carcinoma. The primary aim was to evaluate the median disease-free period and survival in patients with cholangiocarcinoma diagnosis through the comparison of R0 and R1 resection margins. MATERIAL AND METHODS: A retrospective analysis was conducted on 36 patients that underwent some type of surgical resection due to cholangiocarcinoma diagnosis, within the time frame of 2000-2017, at a center specializing in hepatopancreatobiliary surgery. Population, preoperative, and oncologic variables were included. The IBM Statistical Package for the Social Sciences for Mac, version 16.0, software (IBM SPSS Inc., Chicago, IL, USA) was employed. RESULTS: Thirty-one patients underwent hepatectomy, the Whipple procedure, or bypass surgery, depending on tumor location. The statistical significance of survival between patients with positive margins and those with negative margins was evaluated through the Mann-Whitney U test, with a P<.05 as the reference value. No statistically significant difference was found. The overall morbidity rate was 58.06% (n=18) and the mortality rate was 12.9% (n=4). CONCLUSIONS: No statistically significant difference in relation to the incidence of disease recurrence or general survival resulted from the comparison of microscopically positive surgical margins (R1) and negative surgical margins (R0). There was also no correlation between preoperative CA 19-9 levels and disease prognosis.

ABSTRACT

BACKGROUND: Mexico occupies one of the first places worldwide in childhood obesity. Its Mestizo and Indigenous communities present different levels of westernization which have triggered different epidemiological diseases. We assessed the effects of a multi-component school-based intervention program on obesity, cardiovascular and diabetes risk factors. METHODS: A physical activity, health education and parent involvement (PAHEPI) program was developed and applied in six urban (Mestizo ethnic group) and indigenous (Seri and Yaqui ethnic groups) primary schools for 12 weeks. A total of 320 children aged 4-12 years participated in intervention program; 203 under Treatment 1 (PAHEPI program) and 117, only from Mestizo groups, under Treatment 2 (PAHEPI+ school meals). For Body Mass Index (BMI), cardiovascular and diabetes factors, pairwise comparisons of values at baseline and after treatments were done using Wilcoxon signed rank test. Generalized linear models were applied to assess the intervention effect by age, sex and nutritional status in relation to ethnicity and treatment. RESULTS: We observed improvements on BMI in children with overweight-obesity and in triglycerides in the three ethnic groups. The Mestizo ethnic group showed the largest improvements under Treatment 2. While Seris showed improvements only in cardiovascular risk factors, Yaquis also showed improvements in diabetes risk factors, though not in BMI. CONCLUSIONS: This study showed that the same intervention may have positive but different effects in different ethnic groups depending on their lifestyle and their emerging epidemiological disease. Including this type of intervention as part of the school curriculum would allow to adapt to ethnic group in order to contribute more efficiently to child welfare. ABSTRACT

Colorectal cancer is the third most common cancer in the world and the fourth most common cause of death related to cancer. Signet ring cell carcinoma represents an uncommon histological type for rectal cancer with less than 1% of all rectal neoplasms. It usually behaves aggressively and has an inferior prognosis. We present the case of a young man diagnosed with signet ring cell rectal carcinoma. He underwent neoadjuvant therapy with partial response, had surgery with curative intent and showed local recurrence after only 3 months. Disease progression happened only weeks after recurrence with metastasis to vertebrae, extraocular muscles, bone marrow and skin. He is currently receiving palliative chemotherapy.

ABSTRACT

Estudiar el humanismo exterioriza cualquier directriz de pensamiento que ratifique la dignidad del ser humano. Sobre esta base, se puede desentrañar al humanismo de las formas más diversas. Las grandes dimensiones del pensamiento humanista de Fidel Castro Ruz, parten de la convicción de justicia social como un medio de reivindicación, sobre la base de la igualdad. El presente trabajo, demuestra a través de discursos y reflexiones de Fidel Castro Ruz, sobre la sociedad y la medicina, su visión humanista. Se emplearon métodos del nivel teórico, particularmente el histórico lógico y del nivel empírico, la revisión de fuentes documentales. Se han referenciado 22 documentos, que refieren el legado histórico de la obra de Fidel Castro. En ella se demuestra su proyección, su lucha contra las adversidades, por lograr los cambios necesarios en la sociedad cubana, contribuir al mejoramiento de los pueblos y en particular es reconocida en ella, su preocupación por la salud de todos, la ética, la solidaridad, la fidelidad, la honestidad con la historia y sobre todo, su percepción perenne en aras del bienestar de la humanidad.


Studying humanism externalizes any guideline of thought, which enphasize the dignity of the human being. On this basis, humanism can be unraveled in the most diverse ways. The great dimensions of humanist thought of Fidel Castro Ruz, start from the conviction of social justice as a means of vindication, on the basis of equality. The present study demonstrates, through speeches and reflections of Fidel Castro Ruz, about science and medicine, his humanistic vision. Methods of the theoretical level were used, particularly the logical historical one; and from the empirical level, the review of documentary sources. 22 documents have been referenced, which refer to the historical legacy of the work of Fidel Castro. Through his works we can see his projection, his struggle against adversity, to achieve the necessary changes in Cuban society, contributing to the improvement of the peoples and, in special, his concern for the health of all, ethics, solidarity, fidelity, honesty with history and above all, its perennial perception for the welfare of humanity.

Abstract Introduction: Acute postoperative pain is a usual symptom and a surgical challenge. Objective: To determine the frequency of pain in the postoperative period of patients undergoing elective surgery and to characterize pain management at a second-level public hospital. Material and methods: A cross-section study of 175 postop patients was conducted, analyzing variables such as level of pain 24 hours after surgery according to the visual analog scale, type of surgery, use of analgesics, and anesthetic technique. Results: The findings indicate that the frequency of moderate, severe, and excruciating pain is 66.3%. In all cases, the analgesia treatment was prescribed by the treating service, and 2 to 3 nonsteroidal anti-inflammatory drugs were used in 86.4% of the cases, with a minimal use of opioids in 13% of the patients. The anesthetic techniques used included balanced general anesthesia, neuro-axial block, and a mixed technique; the latter improved pain control. Conclusion: The frequency of postoperative pain is similar to the level reported in other trials (30%-70%), pointing to the need to review our current management, with more extensive participation and training of the staff involved in pain control.


Resumen Introducción: El dolor agudo postoperatorio es un síntoma frecuente, el cual representa un reto en el ámbito quirúrgico. Objetivo: determinar la frecuencia de dolor en el paciente postoperado de cirugía electiva y caracterizar el manejo del mismo en un hospital público de segundo nivel de atención. Material y métodos: se realizó un estudio transversal en 175 pacientes postoperados, analizando las variables de grado de dolor a las 24 horas del postoperatorio con la escala visual análoga, tipo de cirugía, uso de analgésicos, técnica anestésica. Resultados: Se encontró que la frecuencia de dolor moderado, severo o insoportable es del 66.3%. El tratamiento analgésico en todos los casos fue prescrito por el servicio tratante y en el 86.4% de los casos se emplearon AINE'S, en número de uno a tres. Con un uso mínimo de opioides en el 13% de los pacientes. Las técnicas anestésicas usadas fueron AGB, BNA y técnica mixta; con mejoría en el grado de dolor con la técnica mixta. Conclusión: Existe una frecuencia de dolor postoperatorio similar a lo reportado en otros estudios (30-70%), reflejando la necesidad de revisión del manejo actual, mayor participación y capacitación del personal involucrado en su manejo.

ABSTRACT

One of the most challenging problems when trying to recycle urine for different purposes is the removal of urea. In this project we studied an ureolysis system using the bacterium Proteus vulgaris for the transformation of urea to ammonia and its subsequent oxidation to nitrogen at a Pt working electrode. Our system was tested under different pH, microbial reaction times, and urea and bacteria concentrations. Our results indicate that a pH8 is optimal for the combined Proteus vulgaris urease activity and the ammonia oxidation reaction at a Pt electrode. The reaction time and concentration dependence on the ammonia oxidation reaction current densities was also studied. Results showed limited ammonia oxidation under high urea concentrations in ~2.5×109cfu/mL Proteus vulgaris in synthetic urine.
