ABSTRACT

A infecção pelo vírus da imunodeficiência humana (HIV) tornou-se um problema de saúde pública em todo o mun-do nas últimas décadas. A principal característica do HIV é a supressão do sistema imunológico pelo ataque aos linfócitos T CD4+ que enfraquece o sistema imunológico e torna o indivíduo suscetível a infecções oportunistas, neoplasias secundárias e doenças neurológicas. Este estudo objetiva relatar e discutir o caso de um paciente HIV positivo que apresentou concomitantemente Sarcoma de Kaposi (SK), sífilis e neurocriptococose, todas doenças relacionadas ao HIV. Trata-se de um paciente masculino, 31 anos, que procurou o serviço do hospital de referência com lesões cutâneas violáceas em face, membros superiores e tórax, com três meses de evolução. Ao exame dermatológico exibiu placas eritematovioláceas infiltrativas, com bordas regulares, elevadas, descamativas e com diâmetros variáveis. Obteve sorologia positiva para anticorpos anti-HIV e VDRL, iniciando protocolos de terapia antirretroviral (TARV) e de tratamento para sífilis. O paciente retornou ao serviço 30 dias após alta hospitalar, com queixa de cefaleia de forte intensidade, refratária à analgesia com opioides, associada a vômitos persistentes. Re-alizada tomografia computadorizada de crânio, sem alterações, e, posteriormente, punção liquórica que evidenciou a presença de criptococo. Iniciado esquema terapêutico para neurocriptococose e realizadas outras duas punções liquóricas para alívio do quadro álgico. Este relato está de acordo com o que presume a literatura médica, reafirmando que pacientes HIV positivos apresentam maior predisposição para condições como o SK, a sífilis e a neurocriptococose. Dessa forma, o estudo ilustra com ineditismo a ocorrência simultânea de complexas manifestações clínicas no mesmo paciente imunossuprimido (AU).


Human immunodeficiency virus (HIV) infection has become a worldwide public health problem in recent decades. The main characteristic of HIV is the suppression of the immune system by attacking CD4+ T lymphocytes, which weakens the immune system and makes the individual susceptible to opportunistic infections, secondary neoplasms, and neurological diseases. This study aims to report and discuss the case of an HIV-positive patient who presented concomitantly Kaposi's Sarcoma (KS), primary syphilis, and neurocryptococcosis, all HIV-related. This is a 31-year-old male patient who sought care at the reference hospital with violaceous skin lesions on the face, upper limbs and chest, with a three-month evolution. Dermatological examination showed infiltrative erythematous-violet plaques, with regular, elevated, scaly edges and varying diameters. He obtained positive serology for anti-HIV and VDRL antibodies, initiating antiretroviral therapy (ART) and treatment protocols for primary syphilis. The patient re-turned to the service 30 days after hospital discharge, complaining of severe headache, refractory to analgesia with opioids, associated with persistent vomiting. Cranial computed tomography was performed and did not demonstrate alterations; later CSF puncture showed the presence of cryptococcus. A therapeutic scheme for neurocryptococcosis was started, and two other CSF punctures were performed to relieve the pain. This report agrees with the medical literature, reaffirming that HIV-positive patients present a greater predisposition to conditions such as KS, syphilis, and neurocryptococcosis. Thus, the study illustrates with uniqueness the simultaneous occurrence of complex clinical manifestations in the same immunosuppressed patient (AU),

ABSTRACT

Hexachlorobenzene (HCB) is a pollutant still found in the environment despite being widely banned. Considering that basidiomycetes are useful to degrade a variety of organochlorinated pollutants, we therefore report the influence of HCB on the ligninolytic enzymatic system of Deconica castanella. The inoculum was prepared with sugarcane bagasse and soybean flour and was added in soil with and without HCB (2000 mg kg soil-1 ), 5% emulsion containing soybean oil and Tween 20 at proportion 9:1, v:v; with 70% moisture at 25°C. Fungal biomass was quantified by widely acknowledged growth biomarker ergosterol. The extraction of the enzymatic complex was performed and laccase, Mn-dependent peroxidase (MnP), and lignin peroxidase (LiP) activities were determined. Furthermore, HCB and its metabolites were quantified by gas chromatography and chlorides by potentiometric titration. Results evidenced that HCB did not interfere in fungal growth, though the only detected enzymatic activity was laccase. MnP and Lip were not detected during D. castanella growth in soil. The peak of laccase enzymatic activity occurred in the presence of HCB. In addition, the laccase exhibited thermostability. Therefore, we hereby shed light on the role of laccase in the degradation of HCB by an efficient low-cost and environmentally safe detoxification mechanism.

ABSTRACT

A Síndrome de DRESS (do inglês, Drug Rash with Eosinophilia and Systemic Symptoms) é uma patologia rara que consiste em uma severa reação medicamentosa mediada por células T. O presente relato de caso retrata uma paciente do sexo feminino, 59 anos, que apresentou icterícia, febre não termometrada, acolia, colúria, mialgia, placas hipercrômicas e lesões pruriginosas. Referiu uso recente de alopurinol, paracetamol e nimesulida, apresentando melhora importante e espontânea após a suspensão das medicações. A extensão do tempo de exposição ao medicamento agressor ocasiona um maior período de internação e risco de mortalidade. Além disso, os dados restritos sobre a Síndrome de DRESS impõe desafios ao seu diagnóstico. Sendo assim, este estudo busca destacar a importância do diagnóstico clínico precoce, a suspensão do medicamento agressor e a instituição da terapêutica adequada para um prognóstico favorável


The Drug Rash with Eosinophilia and Systemic Symptoms (DRESS) Syndrome is a rare pathology that consists of a severe drug reaction mediated by T cells. The present case report depicts a female patient, 59 years old, who presented jaundice, non thermometered fever, acholia, choluria, myalgia, hyperchromic plaques and pruritic lesions. She mentioned recent use of allopurinol, paracetamol and nimesulide, showing significant and spontaneous improvement after discontinuation of medications. The extension of time of exposure to the offending drug causes a longer period of hospitalization and risk of mortality. In addition, the restricted data on DRESS Syndrome poses challenges to its diagnosis. Therefore, this study seeks to highlight the importance of early clinical diagnosis, suspension of the offending drug and the institution of appropriate therapy for a favorable prognosis

ABSTRACT

The amount of solid waste produced across the planet in the past decade was 1.3 billion tons (1.2 kg/year per person). Also, the significant number of publications on solid waste management (SWM) draws attention to the importance of discussing the topic to improve public health and to mitigate environmental impacts. The objectives of this article are to identify the state of the art and the scientific gaps on SWM and to propose a framework to promote it in the coming years. For this, a content analysis was carried out with the support of a bibliometric study, considering articles published in the Scopus database. The field of SWM study was classified into 12 different themes, and from this division, it was sought to identify the evolution of each of them between the 2005 and 2018 period. Content analysis and bibliometric study indicate that thermal and biological treatments are a promising trend to improve the performance of SWM. Its most important applied contribution is the generation of qualified information about SWM that can support the decision-making of public and private managers to reduce environmental impacts and improve life in urban spaces. The main academic contribution of the paper is the articulation of the most important themes on SWM, identifying the individual impact of each one of them in this field of study and the indication of the scientific trends that should guide the development of future research.

ABSTRACT

BACKGROUND: Although Africa accounts for 15% of the global volume of neurosurgical disease, African hospitals and health care networks have access to <1% of the neurosurgeon community. Health care and neurosurgical care are particularly scarce in sub-Saharan Africa due to long periods of imperialism, civil war, poverty, and famines that have plagued the area. METHODS: We provide a historical perspective and initial experience of the development of neurosurgery in Angola. RESULTS: The Republic of Angola has the seventh largest territory in Africa and a population of approximately 29 million people; however, it has a severe deficit of neurosurgery specialists. Between 1975 and 2012, only 5 neurosurgeons practiced in the country. We report the initial experience of the first Angolan physician who completed a full neurosurgical residency in Brazil through the Hospital das Clínicas-University of São Paulo Medical School international medical training program. The challenges of fulfilling the need for neurosurgical services and the future perspectives of building a sustainable program in Angola are discussed. CONCLUSIONS: Partnerships between developing countries and sub-Saharan African countries may be the starting point in supplying neurosurgical specialists to the countries in need.

INTRODUCTION: This study describes cases of syphilis in pregnancy (SiP) and congenital syphilis (CS) and identifies factors associated with mother-to-child transmission in patients in Itapeva from January 1, 2010 to December 31, 2014. METHODS: Using SiP and CS notification forms and medical records, a cross-sectional study involving 149 pregnant women was conducted. RESULTS: Annual SiP detection rates ranged from 16.3-31.4/1000 live births. Most women had prenatal care started at the first trimester of pregnancy with ≥6 visits. Mother-to-child transmission rate was 69.7%. CS incidence rates varied from 9.1-22.3/1000 live births. CONCLUSIONS: The results suggest low quality of prenatal care.

ABSTRACT

Traumatic brain injury (TBI) is responsible for high rates of morbidity and mortality, constituting an important public health problem throughout the world. Improving medical research on systemic trauma is a critical issue to understand its impact and develop strategies for prevention and treatment. This paper presents an overview of medical research performed in Brazil about TBI comparing it with the production on the same topic with other countries and with publications about different neurological and non-neurological diseases. It is possible to notice that Brazil has a deficiency in the scientific production on TBI given its importance. Greater integration between the research centers could help meliorate the production and quality of the papers and encourage further studies on the theme, in the quest to minimize the shortage of publications that can be seen today.


O trauma cranioencefálico (TCE) é responsável por altas taxas de morbidade e mortalidade, constituindo um importante problema de saúde pública em todo o mundo. Melhorar a investigação médica sobre trauma sistêmico é uma questão crítica para compreender seu impacto e desenvolver estratégias para a sua prevenção e tratamento. Este artigo apresenta uma visão geral da pesquisa médica realizada no Brasil sobre TCE comparando-a com a produção sobre o mesmo tema com outros países e com publicações sobre diferentes doenças neurológicas e não neurológicas. É possível notar que o Brasil tem uma deficiência na produção científica sobre TCE dada a sua importância. Maior integração entre os centros de pesquisa poderia ajudar a aperfeiçoar a produção e a qualidade dos trabalhos e incentivar novos estudos sobre o tema, na busca de minimizar a escassez de publicações vista atualmente.

ABSTRACT

Dermoid account for 0.04­0.06% of intracranial tumors. The rupture of these slowgrowing lesions are a rare event, generally taking place spontaneously. Their presentation are clinically variable according to cyst topography and integrity. Surgery remains the first-line therapy and gross total resection should be attempted if feasible. We report on a case of a 22-year-old male with a 2-year history of seizures and cognitive impairment and a temporal mesial dermoid cyst successfully treated with gross total resection microsurgery.


Os Cistos dermoides compreendem 0,04­0,06% dos tumores intracranianos. É uma lesão de crescimento lento, e sua ruptura é um evento raro e espontâneo. A variabilidade clínica vai de acordo com a topografia do cisto e sua integridade. A cirurgia continua a ser a terapia de primeira linha, e a ressecção total é a opção sempre que for possível. Os autores relatam um caso de paciente com 22 anos de idade com histórico de 2 anos de convulsões e comprometimento cognitivo e diagnosticado com um cisto dermoide mesial temporal, tratado com sucesso com ressecção microcirúrgica.

ABSTRACT

O câncer sistêmico é a segunda maior causa de mortalidade na população adulta. Destes pacientes, 20% desenvolvem sintomas neurológicos em algum momento de sua doença. A incidência do câncer sistêmico vem crescendo com a evolução dos exames diagnósticos, melhora das terapias e ao aumento da sobrevida da população geral. Isto reflete, conseqüentemente, no aumento da incidência das metástases intracranianas o que representa, por sua vez, uma considerável fonte de morbimortalidade. Neste artigo os autores revisam os princípios de diagnóstico e conduta a cerca das metástases encefálicas.

OBJECTIVE: To analyze the tip-apex distance (TAD), cervicodiaphyseal angle and Garden angle in stable and unstable extracapsular fractures of the femur treated with a plate and sliding screw. METHOD: Hip radiographs in anteroposterior (AP) and lateral view on 117 patients were evaluated. The fractures were classified as stable or unstable, using the AO classification, and the reduction achieved was assessed in accordance with the following criteria: TAD > 3 cm; Garden alignment index (AP) < 160°; and AP cervicodiaphyseal varus angle < 125°. When two or more criteria were present, the quality of the osteosynthesis was classified as "not ideal". RESULTS: The patients with unstable fractures presented AP cervicodiaphyseal angles that were significantly greater (p = 0.05) than in those with stable fractures. The patients with unstable fractures presented lateral cervicodiaphyseal angles that were significantly smaller (p = 0.05) than in those with stable fractures. There were no significant differences in the remainder of the criteria evaluated. CONCLUSION: This study did not find any significant differences in the measurements evaluated, except in relation to the cervicodiaphyseal angle. Satisfactory reduction was achieved both for the stable and for the unstable fractures, when we used a plate and sliding screw to treat proximal extracapsular fractures of the femur.

The Stevens-Johnson syndrome (SJS) is a severe variant of erythema multiforme (EM) that occasionally can rise to systemic upset and possibly compromise life. In this report, we described important oral and dermatologic aspects of a case of SJS triggered by Mycoplasma pneumoniae infection.

ABSTRACT

The authors present a case of a hemorrhagic adrenal cyst, one of the tumors known in literature as incidentalomas, emphasizing the clinical characteristics, since adrenal cysts or pseudocysts are generally rare and observed by chance during imaging procedures. Traditionally they are classified as pseudocysts, endothelial, epithelial or parasitic cysts. Laparoscopic adrenalectomy has been considered the treatment of choice for benign, functioning or non-functioning adrenal lesions. Small cystic adrenal tumors can be managed conservatively by laparoscopic decortication or marsupialization, but larger cysts should be treated by total or partial adrenalectomy.

ABSTRACT

Dorstenia stellaris is a new species from southeastern Brazil. This species is endemic to the region and differs from the others by its star shaped coenanthium and cordiform leaves. A description and illustration of this species is presented here. Dorstenia stellaris is found in moist and shady places, in small populations within the type locality, thus we recommend its inclusion in the endangered (EN) status of conservation.

ABSTRACT

OBJECTIVE: To compare the existence of agreement between measurements of anteversion of the femoral neck and the degrees of osteoarthritis and abnormality of the neck-shaft angle in patients with unilateral idiopathic hip osteoarthritis, among three evaluators. METHODS: Forty-two patients with unilateral hip osteoarthritis were evaluated by means of simple radiography and computed tomography. RESULTS: It was observed that there was no significant variation in femoral anteversion between the diseased and healthy hips. There was strongest agreement between observers 1 and 2 in relation to both the diseased hips (cases) and the healthy hips (controls). Moreover, no significant agreement was found between observers 1 and 3 (p = 0.13) and between observers 2 and 3 (p = 0.12), in relation to the neck-shaft angle of the control hips. CONCLUSION: Although there was no relationship between femoral anteversion and the neck-shaft angle in the patients with unilateral hip osteoarthritis, the present study showed that there was also no relationship with these angular deviations.

OBJETIVO: Comparar, entre três avaliadores, a existência de concordância das medidas da anteversão do colo femoral com o grau da osteoartrite e a alteração do ângulo cervicodiafisário em pacientes com osteoartrite idiopática unilateral do quadril. MÉTODOS: Foram avaliados 42 pacientes com osteoartrite unilateral do quadril por meio de radiografia simples e tomografia computadorizada. RESULTADOS: Observou-se que não ocorreu variação significativa na anteversão femoral entre o quadril doente e o sadio. Houve uma concordância mais forte entre os observadores 1 e 2, tanto em relação ao quadril doente (caso) quanto ao sadio (controle). Além disso, não foi verificada concordância significativa entre os observadores 1 e 3 (p = 0,13) e entre 2 e 3 (p = 0,12) em relação ao ângulo cervicodiafisário do quadril controle. CONCLUSÃO: Apesar de não ter ocorrido relação entre a anteversão femoral e o ângulo cervicodiafisário nos pacientes com osteoartrite unilateral do quadril, este estudo demonstrou não haver relação com estes desvios angulares.
