ABSTRACT

Locked thumb metacarpophalangeal (MCP) joint has been known as a relatively rare condition. We present a case of a 25-year-old male presented with a traumatic locked thumb MCP joint of the right hand. Clinical presentation showed the joint was in a hyperextention position and resistant to active flexion and extension. The radial sesamoid was distally displaced on X-ray film and entrapped into the joint on CT scan. The mechanism of the locked MCP joint was thought as a "pull-hook lock" between the pointed proximal edge of the sesamoid and the deformed volar groove on the cartilaginous surface of the metacarpal head. Surgical treatment included partially releasing the insertion of the abductor pollicis brevis (APB) and flexor pollicis brevis (FPB) muscles to move the entrapped radial sesamoid, and resurfacing the deformed metacarpal head.

ABSTRACT

INTRODUCTION AND IMPORTANCE: Peripheral nerve sheath tumors are common neoplasm with different biological features ranging from benign to malignant. The majority of these tumors are smaller than 5 cm, whereas those larger are termed giant schwannomas. When localized in the lower legs, the maximum length of the schwannoma is less than 10 cm. We report a case of giant schwannoma of the leg and its management. CASE PRESENTATION: A 11-year-old boy presented with a 13 cm × 5 cm firm, smooth, well-defined margin mass in the posterior-medial aspect of right leg. The tumor was fusiform, well capsulated, multi-lobulated soft tissue with 13 cm × 4 cm × 3 cm in size at the biggest region. On MRI the tumor was low signal, isointense with adjacent tissue on T1S, hyper-intense on T2-FS sequences and surrounded by a thin fat-like intense rim. Biopsy findings were considered most consistent with Schwannoma (Antoni A). Tumor resection was performed. The mass appeared capsulated, white, and glistening with 132 mm × 45 mm × 34 mm in size. Postoperative course was uneventful without neurological deficit. CLINICAL DISCUSSION AND CONCLUSION: Schwannomas are the most common peripheral nerve sheath tumors that derived almost entirely from Schwann cells. Schwannomas usually affect the head and neck region, localization in the lower extremity is rare. When located in lower extremity, the maximum diameter of 5 cm is described in most studies. Clinical presentation of schwannomas is unclear and unspecific. Diagnosis is based on ultrasound, MRI, and histology. The recommended treatment for schwannoma is surgical enucleation or resection without damaging the involved nerve.

ABSTRACT

The absorption of CO2 from CO2-N2 gas mixtures using water and monoethanolamine (MEA) solution in polypropylene (PP) hollow-fiber membrane contactors was experimentally and theoretically examined. Gas was flowed through the lumen of the module, whereas the absorbent liquid was passed counter-currently across the shell. Experiments were carried out under various gas- and liquid-phase velocities as well as MEA concentrations. The effect of pressure difference between the gas and liquid phases on the flux of CO2 absorption in the range of 15-85 kPa was also investigated. A simplified mass balance model that considers non-wetting mode as well as adopts the overall mass-transfer coefficient evaluated from absorption experiments was proposed to follow the present physical and chemical absorption processes. This simplified model allowed us to predict the effective length of the fiber for CO2 absorption, which is crucial in selecting and designing membrane contactors for this purpose. Finally, the significance of membrane wetting could be highlighted by this model while using high concentrations of MEA in the chemical absorption process.

ABSTRACT

With better quality of life, obesity is becoming a worldwide disease due to over-eating and sedentary lifestyle. Therefore, daily monitoring of the glucose and body fat percentage (%) is vital to keep track of one's health. Currently, separated devices are required to monitor each parameter at home and some are still invasive to measure the glucose level. In this study, a portable band-shaped bioimpedance system is proposed to measure both parameters. The system is battery run with two main modules: the current source and the voltage recording, with minimal design to fit into a band of 150 mm x 40 mm in dimension. The impedance is measured at the frequency of 1 kHz at 30 kHz sampling frequency and in 1000 signal cycles to flatten noises. The final average impedance is calculated and evaluated in correlation with the body fat and the fasting glucose. The system was tested on 21 volunteers and 4 locations were picked for the impedance measurement: the arm under the triceps, the side of the belly, the back on one side and the thigh under the bicep femoris. The results show promising results with the arm being the best location for predicting the body fat (correlation coefficient: 0.89, 95% CI: 0.73-0.95), while the thigh impedance best correlated with the fasting glucose (correlation coefficient: 0.92, 95% CI: 0.81-0.97). These preliminary results indicate the feasibility and capacity of the proposed system as a home-based, portable and convenient system in monitoring the body fat and glucose. The system's performance will be verified and replicated in a future larger study.

ABSTRACT

In this study, the ZnO nanoparticles were doped with Ag and then hybridized on graphite (GP) layer (Ag-ZnO/GP) by a hydrothermal method, which was used as photocatalysts to remove metronidazole (MNZ) antibiotic from aqueous solutions. The fine structure, morphologies, and optical properties of the synthesized composites were first examined. The incorporation of Ag would readily reduce the rate of the recombination of electron-hole pairs and enhance the photocatalytic activity in a wide range of light wavelength. The graphite surface also acted as an electron sink to efficiently inhibit the photocorrosion of ZnO, thereby improving the photostability of the composites. The composition of the composite was optimized to be 0.5 wt% GP/ZnO and 1.0 wt% Ag/ZnO according to the extent of the enhancement of photocatalytic activity. In a solution containing 30 mg L-1 of MNZ and 0.5 g L-1 of Ag-ZnO/GP composite, it was shown that 88.5% and 97.3% of MNZ was removed after 60 min of 100-W UV and 180-min solar irradiation, respectively. Moreover, six over a total of eleven transformation products formed during UV photocatalysis were ascribed to the roles of reactive holes (h+), all which were detected and identified by high-resolution liquid chromatography-mass spectrometry (LC-MS). Finally, the pathways of MNZ degradation over Ag-ZnO/GP composite were proposed.
