ABSTRACT

Nanotechnology offers many potential solutions for sustainable agroecosystem, including improvement in nutrient use efficiency, efficacy of pest management, and minimizing the adverse environmental effects of agricultural production. Herein, we first highlighted the integrated application of nanotechnology and precision agriculture for sustainable productivity. Application of nanoparticle mediated material and advanced biosensors in precision agriculture is only possible by nanochips or nanosensors. Nanosensors offers the measurement of various stresses, soil quality parameters and detection of heavy metals along with the enhanced data collection, enabling precise decision-making and resource management in agricultural systems. Nanoencapsulation of conventional chemical fertilizers (known as nanofertilizers), and pesticides (known as nanopesticides) helps in sustained and slow release of chemicals to soils and results in precise dosage to plants. Further, nano-based disease detection kits are popular tools for early and speedy detection of viral diseases. Many other innovative approaches including biosynthesized nanoparticles have been evaluated and proposed at various scales, but in fact there are some barriers for practical application of nanotechnology in soil-plant system, including safety and regulatory concerns, efficient delivery at field levels, and consumer acceptance. Finally, we outlined the policy options and actions required for sustainable agricultural productivity, and proposed various research pathways that may help to overcome the upcoming challenges regarding practical implications of nanotechnology.

ABSTRACT

Introduction and importance: Coronavirus disease 2019 (COVID-19) is a disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and various vaccines against it have been developed. Acute disseminated encephalomyelitis (ADEM) is a disease of the central nervous system that cause inflammation and demyelination and manifests as a multi-symptom acute neurological condition. Although infections are usually the cause of ADEM, vaccines may cause 5-10% of cases. Case presentation: A 40-year-old woman had received a second dose of the Sinopharm COVID-19 vaccine 4 months before her visit and experienced sudden gait imbalance and vertigo a day after her vaccination, which lasted for more than a month. On examination, no signs of skin bruising or bleeding were observed, and her vital signs were within the normal range. On neurological assessment, the patient had a Glasgow Coma Scale score of 14/15 (E4V5M5), had normal pupil size and light reaction, normal fundus, normal deep tendon reflexes and bilateral extensor plantar response. Meningeal symptoms were absent, and SARS-CoV-2 RNA tests using NAAT (Nucleic Acid Amplification Test) were negative. Development of central nervous system (CNS) manifestations during the recovery phase of fever, along with typical MRI findings; the diagnosis of para-infectious ADEM with COVID-19 vaccination was made. After the treatment with methylprednisolone sodium succinate injection, the patient showed improvement. Clinical discussion: ADEM associated with post-vaccinations is a rare condition. There has been growing evidence that shared epitopes between neuronal proteins and SARS-CoV-2 antigens may trigger autoimmune reactions against the CNS through molecular mimicry as its pathogenesis. Conclusion: We suggest the need for a strict vaccine safety monitoring system and post-vaccine monitoring and surveillance.

ABSTRACT

In this study, water levels resulting from the dynamic interaction of tide and surge are estimated by solving a 2-D vertically integrated shallow water equations numerically. To solve the equations on the specific 2-D grid, the explicit Leapfrog scheme is implemented, adopting a staggered Arakawa C-grid. The domain's complex land-sea interface is approximated through the stair-step method in order to employ the finite difference technique. To incorporate the complexity of the domain with a considerably high accuracy and to reduce computational cost, one-way nested grid models are embraced. The Meghna River freshwater discharge is incorporated into the innermost child model. A stable tidal regime over the region of interest is generated by applying the four vital tidal constituents, namely M2 (principal lunar semidiurnal), S2 (principal solar semidiurnal), O1 (principal lunar diurnal) and K1 (luni-solar diurnal) in the southern open boundary of the outermost model. This previously effectuated tidal regime is used as the initial state of the sea in getting total water levels due to the dynamic interaction of tide and surge. Numerical experiments are performed with the storm AILA that hit the coast of Bangladesh on May 25, 2009. The simulated results are found to closely match observed and reported data.

ABSTRACT

This case report documents the unusual co-occurrence of immune thrombocytopena (ITP) and chickenpox in a 15-year-old girl. Initial symptoms included shortness of breath, chest pain, and heavy menstrual bleeding. Laboratory results revealed significant anemia and thrombocytopenia. Treatment involved blood transfusions, prednisolone, and iron supplementation. The patient's vesicular skin rash emerged 8 weeks later, prompting the combined diagnosis of ITP and chickenpox. Antiviral treatments, blood transfusions, and supportive care were used in the course of treatment, leading to full recovery. This case emphasizes the importance of prompt diagnosis, appropriate management, and regular follow-up for patients with both chickenpox and ITP. The coexistence of chickenpox and ITP poses a clinical challenge due to the complex interaction between the viral infection and the immune system. The exact mechanism linking these two conditions remains unclear, making it a baffling case that warrants investigation and further understanding. As low is the occurrence of hemorrhagic chickenpox, the presentation of simultaneous chicken pox with or following ITP was found to be rarer, and thus is this enigmatic case presented. Healthcare providers should remain vigilant about such co-occurrences to prevent complications. In order to improve treatment for instances with comparable clinical presentations and advance our collective knowledge, further study is required to better understand the mechanisms relating viral infections and ITP.

ABSTRACT

Floating Treatment Wetland (FTW) is a cost-effective and easy-to-retrofit device for stormwater treatment. Its treatment efficiency largely depends on the fraction of inflow entering FTW and the residence time within it. Thus hydrodynamics play a crucial role, which is affected by the design configurations of FTW and stormwater pond. Despite a spike in research on FTWs, very little is known about how various design configurations affect treatment efficiency by an FTW. Our study hypothesizes that relative positions of FTW geometry, FTW position and pond inlet-outlet have impact on the hydrodynamics and as a consequence, treatment efficiency. To explore these design features, we employed computational fluid dynamics (CFD) modeling conducted in ANSYS Fluent, validated by experimental data to examine the impact of the aforementioned design features. The results revealed that circular FTW geometry positioned near inlet coupled with center inlet-side outlet configuration achieved the highest removal (94.8%) for a non-dimensional removal rate of krtHRT = 20 (kr is the first order removal rate in per day, tHRT is the nominal hydraulic residence time of the pond in days). Far side inlet-side outlet configuration performed the worst due to profound promotion of short-circuiting. FTW positioned near inlet performed better (61.8% mass removal on an average) than center (42.7%) and near outlet positions (54.1%) for krtHRT = 20. Sensitivity analysis revealed that the treatment efficiency is most sensitive to inlet-outlet configurations. The design implications of this study will help practitioners achieving better water quality and ecological improvement goals.

ABSTRACT

Adsorption has been considered as a promising remediation technology to separate organic and inorganic agrochemicals from contaminated soil and water. Low-cost adsorbents, including waste derived materials, clay composites, biochar, and biochar modified materials, have attracted enormous attention for the removal of organic contaminants, including pesticides. In this study, iron-modified base-activated biochar (FeBBC) was prepared by pyrolysis (at 400 °C for 1 h) of iron-doped base (KOH) activated sugarcane bagasse for the removal of a widely used insecticide, namely imidacloprid (IMI) from water. The maximum adsorption capacity of the adsorbent (FeBBC) was calculated as 10.33 (± 1.57) mg/g from Langmuir isotherm model. The adsorbents could remove up to ~ 92% of IMI from aqueous solution at 23.8 mg/L IMI. Experimental data fitted well with the Freundlich model and pseudo-second-order model, demonstrating physisorption, as well as chemosorption, contributed to the sorption process. Even at highly acidic/basic solution pH, the FeBBC could remove substantial amount of IMI demonstrating hydrophobic interaction and pore diffusion play vital role for removal of IMI. The slight improving of IMI sorption with increasing solution pH indicated the sorption was also facilitated through ionic interaction alongside physical sorption. However, physical sorption including hydrophobic interaction and pore-filling interaction plays a vital role in the sorption of IMI.

ABSTRACT

Fusarium wilt is a significant disease in radish, but the genetic mechanisms controlling yellows resistance (YR) are not well understood. This study aimed to identify YR-QTLs and to fine-map one of them using F2:3 populations developed from resistant and susceptible radish parents. In this study, two high-density genetic maps each containing shared co-dominant markers and either female or male dominant markers that spanned 988.6 and 1127.5 cM with average marker densities of 1.40 and 1.53 cM, respectively, were generated using Genotyping by Random Amplicon Sequencing-Direct (GRAS-Di) technology. We identified two YR-QTLs on chromosome R2 and R7, and designated the latter as ForRs1 as the major QTL. Fine mapping narrowed down the ForRs1 locus to a 195 kb region. Among the 16 predicted genes in the delimited region, 4 genes including two receptor-like protein and -kinase genes (RLP/RLK) were identified as prime candidates for ForRs1 based on the nucleotide sequence comparisons between the parents and their predicted functions. This study is the first to use a GRAS-Di for genetic map construction of cruciferous crops and fine map the YR-QTL on the R7 chromosome of radish. These findings will provide groundbreaking insights into radish YR breeding and understanding the genetics of YR mechanism.

ABSTRACT

As an edible oil, palm oil is also safe and reliable in dyeing, and the residual palm oil after dyeing can be recycled and used continuously, which is green and environmentally friendly and has great research prospects. In this research, raw ramie yarn, used for traditional grass cloth, was dyed in a palm oil medium using Reactive Blue 194. Studying the adsorption and diffusion behaviour in the dyeing process is necessary. Additionally, the kinetics and isotherm model of dyeing raw ramie yarn with Reactive Blue 194 in palm oil is studied, and the adsorption behaviour between them is discussed. For a better understanding, the raw ramie yarn dyeing adsorption behaviour was also carried out in a water medium. It was found that the dyeing rates in palm oil are distinctly faster than in water. Kinetics data suggested that the pseudo-second-order model fitted for both dyeing mediums (palm oil and water) of the adsorption of the Reactive Blue 194 dye onto raw ramie yarn. Afterward, the adsorption isotherms' results denote that the Langmuir model was suitable for palm oil dyeing medium while the Freundlich model was suited for water medium. Overall, this study has demonstrated that raw ramie yarn dyeing in a palm oil medium could be a sustainable colouration route for textile fibres with a greater dye exhaustion percentage.

BACKGROUND: As the 2016 Global Strategy on Human Resources for Health: Workforce 2030 (GSHRH) outlines, health systems can only function with health workforce (HWF). Bangladesh is committed to achieving universal health coverage (UHC) hence a comprehensive understanding of the existing HWF was deemed necessary informing policy and funding decisions to the health system. METHODS: The health labour market analysis (HLMA) framework for UHC cited in the GSHRH was adopted to analyse the supply, need and demand of all health workers in Bangladesh. Government's information systems provided data to document the public sector HWF. A national-level assessment (2019) based on a country representative sample of 133 geographical units, served to estimate the composition and distribution of the private sector HWF. Descriptive statistics served to characterize the formal and informal HWF. RESULTS: The density of doctors, nurses and midwives in Bangladesh was only 9.9 per 10 000 population, well below the indicative sustainable development goals index threshold of 44.5 outlined in the GSHRH. Considering all HWFs in Bangladesh, the estimated total density was 49 per 10 000 population. However, one-third of all HWFs did not hold recognized roles and their competencies were unknown, taking only qualified and recognized HWFs into account results in an estimated density 33.2. With an estimate 75 nurses per 100 doctors in Bangladesh, the second area, where policy attention appears to be warranted is on the competencies and skill-mix. Thirdly, an estimated 82% of all HWFs work in the private sector necessitates adequate oversight for patient safety. Finally, a high proportion of unfilled positions in the public sector, especially in rural areas where 67% of the population lives, account only 11% of doctors and nurses. CONCLUSION: Bangladesh is making progress on many of the milestones of the GSHRH, notably, the establishment of the HWF unit and reporting through the national health workforce accounts. However, particular investment on strengthening the intersectoral HWF coordination across sectors; regulation for assurance of patient safety and adequate oversight of the private sector; establishing accreditation mechanisms for training institutions; and halving inequalities in access to a qualified HWF are important towards advancing UHC in Bangladesh.

ABSTRACT

Tuberous stem of kohlrabi is an important agronomic trait, however, the molecular basis of tuberization is poorly understood. To elucidate the tuberization mechanism, we conducted a comparative transcriptomic analysis between kohlrabi and broccoli at 10 and 20 days after germination (DAG) as tuberous stem initiated between these time points. A total of 5580 and 2866 differentially expressed transcripts (DETs) were identified between genotypes (kohlrabi vs. broccoli) and growth stages (10 DAG vs. 20 DAG), respectively, and most of the DETs were down-regulated in kohlrabi. Gene ontology (GO) and KEGG pathway enrichment analyses showed that the DETs between genotypes are involved in cell wall loosening and expansion, cell cycle and division, carbohydrate metabolism, hormone transport, hormone signal transduction and in several transcription factors. The DETs identified in those categories may directly/indirectly relate to the initiation and development of tuberous stem in kohlrabi. In addition, the expression pattern of the hormone synthesis related DETs coincided with the endogenous IAA, IAAsp, GA, ABA, and tZ profiles in kohlrabi and broccoli seedlings, that were revealed in our phytohormone analysis. This is the first report on comparative transcriptome analysis for tuberous stem formation in kohlrabi at early growth periods. The resulting data could provide significant insights into the molecular mechanism underlying tuberous stem development in kohlrabi as well as in other tuberous organ forming crops.

ABSTRACT

Structural, mechanical, spin-dependent electronic, magnetic, and optical properties of antiperovskite nitrides A3InN (A = Co, Ni) along with molecular orbital diagram are investigated here by using an ab initio density functional theory (DFT). The mechanical stability, deformation, damage tolerance and ductile nature of A3InN are confirmed from elastic calculations. Different mechanical anisotropy factors are also discussed in detail. The spin dependent electronic properties such as the band structure and density of states (DOS) of A3InN are studied and, the dispersion curves and DOS at Fermi level are different for up and down spins only in case of Co3InN. These calculations also suggest that Co3InN and Ni3InN behave as ferromagnetic and nonmagnetic, respectively. The induced total magnetic moment of Co3InN is found 2.735 µB/cell in our calculation. Mulliken bond population analysis shows that the atomic bonds of A3InN are contributed by both ionic and covalent bonds. Molecular orbital diagrams of A3InN antiperovskites are proposed by analyzing orbital projected band structures. The formation of a molecular orbital energy diagram for Co3InN is similar to Ni3InN with respect to hybridization and orbital sequencing. However, the orbital positions with respect to the Fermi level (E F) and separations between them are different. The Fermi surface of A3InN is composed of multiple nonspherical electron and hole type sheets in which Co3InN displays a spin-dependent Fermi surface. The various ground-state optical functions such as real and imaginary parts of the dielectric constant, optical conductivity, reflectivity, refractive index, absorption coefficient, and loss function of A3InN are studied with implications. The reflectivity spectra reveal that A3InN reflects >45% of incident electromagnetic radiations in both the visible and ultraviolet region, which is an ideal feature of coating material for avoiding solar heating.

ABSTRACT

Objective: In this study, we investigated the efficacy and safety of remdesivir and tocilizumab combination therapy against dexamethasone for the management of severe COVID-19 patients. Methods: This was a multicenter study. Cases were randomly chosen and divided into two groups using an odd-even ratio of 1:1 applied to the hospital registration number. Group A received remdesivir [5 mg/kg (<40 kg) or 200 mg (>40 kg) on day 1 and then 2.5 mg/kg (<40 kg) or 100 mg (>40 kg) daily] + tocilizumab [8 mg/kg up to 800 mg highest 12 h apart], and group B was the control and received dexamethasone 6 mg/day. In addition, a broad-spectrum antibiotic and other essential treatments were received by all patients. To evaluate the mortality risk, the sequential organ failure assessment (SOFA) score was calculated on day-1. Treatment outcomes were measured as time to clinical improvement; mortality rate; duration of ICU stay; total period of hospitalization; the rate of (Supplementary Material) oxygen use; time to clinical failure; National Early Warning Score-2 (NEWS), and the percentage of lung recovery on CT of chest on discharge. Clinical trial registration ID: NCT04678739. Results: Remdesivir-Tocilizumab group had a lower mortality rate (25.49%) than the control (30.77%). The time to clinical improvement (Group A-9.41; B-14.21 days), NEWS-2 on discharge (Group A-0.89; B-1.2), duration of ICU stay (Group A-7.68; B-10.58), and duration of hospitalization (Group A-9.91; B-14.68) were less in the treatment group. Group A had a better percentage of lung recovery on chest CT than the control (Group A-22.13; B-11.74). All these differences were statistically significant (p= <0.05) in a t-test. However, no significant survival benefit was found among the study groups in Kaplan-Meier survival analysis, p = 0.739. Conclusion: The remdesivir-tocilizumab combination had preferable outcomes compared to the dexamethasone therapy for the treatment of severe COVID-19 concerning mortality rate and clinical and pulmonary improvement, although it did not demonstrate a significant survival benefit. BACKGROUND: Bangladesh's Health system is characterized by severe shortage and unequitable distribution of the formally trained health workforce. In this context, government of Bangladesh uses fixed staffing norms for its health facilities. These norms do not always reflect the actual requirement in reality. This study was conducted in public sector health facilities in two selected districts to assess the existing staffing norms with the purpose of adopting better norms and a more efficient utilization of the existing workforce. METHODS: To carry out this assessment, WHO's Workload Indicators of Staffing Need (WISN) method was applied. Selection of the two districts out of 64 and a total of 24 health facilities were made in consultation with the formally established steering committee of the Ministry of Health. Health facilities, which were performing well in serving the patients during 2016-2017, were selected. This assessment examined staffing requirement of 20 staff categories. RESULTS: Based on the computer-generated WISN results, most of the staff categories were found to have a workload pressure of Very High (seven out of 20 staff categories), followed by Extremely High (five staff categories). Two staff categories had high, three had moderately high, two normal, and one low workload. Nurses were found to be predominantly occupied with support activities (50-60% of working time), instead of actual nursing care. Regarding vacancy, if all the vacant posts were filled, understandably, the workload would reduce, but not yet sufficient to meet the existing staff requirements such as consultants, general physicians and nurses at the district and sub-district/upazila-based hospitals. CONCLUSION: The existing staffing norms fall short of the WISN staffing requirement. The results provide evidence to prompt a revisit of the staffing policies and adopt workload-based norms. This can be supplemented by reviewing the scope of practice of the staff categories in their respective health facilities. In the short term, government might consider redistributing existing workforce as per workload. In the long term, revision of staffing norms is needed to provide quality health services for all.

ABSTRACT

BACKGROUND: Globally, many countries are adopting evidence-based workforce planning that facilitates progress towards achieving sustainable development goals for reproductive, maternal newborn and child health. We reviewed case studies on workforce planning for reproductive maternal newborn child health services at primary care level facilities using workload indicators of staffing need in five countries. METHOD: Using available workload indicators for staffing need reports from Bangladesh, Ghana, Kenya, Sultanate of Oman and Papua New Guinea, we generated descriptive statistics to explore comparable workload components and activity standards, health service delivery models with an emphasis on the primary care levels and the specific health occupations offering interventions associated with reproductive maternal, newborn and child health services. RESULTS: The health services delivery models vary from one country to another. The results showed variability in the countries, in the workload components and activity standards of each regardless of facility level or occupational groups involved. All the countries have decentralized health services with emphasis on comprehensive primary care. Reproductive, maternal and new-born child health care services include antenatal, postnatal, immunization, family planning, baby wellness clinics, delivery and management of integrated minor childhood illnesses. Only Sultanate of Oman offers fertility services at primary care. Kenya has expanded interventions in the households and communities. CONCLUSION: Since the health care services models, health services delivery contexts and the health care worker teams vary from one country to another, the study therefore concludes that activity standards cannot be adopted or adapted from one country to another despite having similar workload components. Evidence based workforce planning must be context-specific, and therefore requires that each country develop its own workload components and activity standards aligned to their local contexts.

In this paper, the distribution of thermodynamic variables in the protoplanets formed by gravitational instability in the mass range 0.3 - 10 M J ( 1 M J = 1 Jupiter mass = 1.8986 × 10 30 gm) is investigated in their initial state by solving the structure equations via the Adomian decomposition method. Concerning the heat transfer in the protoplanets, the mode of convection is taken into account. The outcomes indicate that there is a reasonably good agreement between the Adomian semi-analytical solution containing only first 8 terms and the numerical results.

ABSTRACT

Stormwater pollution causes an excessive influx of nutrients and metals to the receiving waterbodies (stormwater ponds, lakes, and rivers), which can cause eutrophication and metal toxicity. One of the most cost-effective and eco-friendly solutions to stormwater pollution is constructing Floating Treatment Islands (FTIs) within the waterbodies receiving stormwater runoff. Treatment efficiency of FTIs depends on many factors including plant species, temperature, detention time, and pollutant loading rate. Another important factor is FTI hydraulics, which determines the amount of inflow to the root zone and residence time, greatly impacting the treatment. However, only a few studies refer to the hydraulics of waterbodies retrofitted with FTIs. This paper reviews available literature on field-scale, laboratory-scale and numerical studies on the hydraulics of FTI retrofitted waterbodies. Because of limited knowledge on the factors affecting hydraulics of waterbodies retrofitted with FTIs, current practices cannot ensure maximum hydraulic performance of this system. This review paper identifies different factors affecting the FTI hydraulics, investigates knowledge gaps, and provides future research direction for hydraulically efficient design of FTIs to treat stormwater. It was found that there is a need to investigate the impact of new design parameters such as FTI shape, FTI coverage, inlet-outlet configurations, and shape of waterbody on the hydraulic performance of FTI retrofitted waterbodies. A lack of dimensional analysis on FTI retrofitted waterbodies in existing literature revealed that field-scale values were not properly scaled down in laboratory experiments. Although a few short-circuiting prevention mechanisms (SPMs) were used in different field-scale studies, those mechanisms may be vulnerable to short-circuiting in the vertical dimension. It was revealed that studying the role of eddy diffusion and gap layer for vertical short-circuiting can help designing better SPMs. This review also identified that further investigation is required to incorporate root flexibility in the current modeling approach of FTI retrofitted waterbodies.

ABSTRACT

BACKGROUND: Access to washroom facilities and a place to dispose of menstrual waste are prerequisites for optimal menstrual hygiene management in schools. Like other low- and middle-income countries, Bangladeshi schools lack facilities for girls to change and dispose of their menstrual absorbents. We explored existing systems for disposing of menstrual absorbent wastes in urban and rural schools of Bangladesh and assessed the feasibility and acceptability of alternative disposal options. METHODS: We explored how girls dispose of their menstrual products, identified girls' preferences and choices for a disposal system and piloted four disposal options in four different schools. We then implemented one preferred option in four additional schools. We explored girls', teachers', and janitors' perspectives and evaluated the acceptability, feasibility, and potential for sustainability of the piloted disposal system. RESULTS: Barriers to optimal menstrual hygiene management included lack of functional toilets and private locations for changing menstrual products, and limited options for disposal. Girls, teachers, and janitors preferred and ranked the chute disposal system as their first choice, because it has large capacity (765 L), is relatively durable, requires less maintenance, and will take longer time to fill. During implementation of the chute disposal system in four schools, girls, teachers, and janitors reported positive changes in toilet cleanliness and menstrual products disposal resulting from the intervention. CONCLUSIONS: The chute disposal system for menstrual products is a durable option that does not require frequent emptying or regular maintenance, and is accepted by schoolgirls and janitors alike, and can improve conditions for menstrual hygiene management in schools. However, regular supervision, motivation of girls to correctly dispose of their products, and a long-term maintenance and management plan for the system are necessary.

ABSTRACT

The serial feature-positive discrimination task requires the subjects to respond differentially to the identical stimulus depending on the temporal context given by a preceding cue stimulus. In the present study, we examined the involvement of the M1 muscarinic acetylcholine receptors using a selective M1 antagonist VU0255035 in the serial feature-positive discrimination task of eyeblink conditioning in mice. In this task, mice received a 2-s light stimulus as the conditional cue 5 or 6 s before the presentation of a 350-ms tone conditioned stimulus (CS) paired with a 100-ms peri-orbital electrical shock (cued trials), while they did not receive the cue before the presentation of the CS alone (non-cued trials). Each day mice randomly received 30 cued and 30 non-cued trials. We found that VU0255035 impaired acquisition of the conditional discrimination as well as the overall acquisition of the conditioned response (CR) and diminished the difference in onset latency of the CR between the cued and non-cued trials. VU0255035 administration to the control mice after sufficient learning did not impair the pre-acquired conditional discrimination or the CR expression itself. These effects of VU0255035 were almost similar to those with the scopolamine in our previous study, suggesting that among the several types of muscarinic acetylcholine receptors, the M1 receptors may play an important role in the acquisition of the conditional discrimination memory but not in mediating the discrimination itself after the memory had formed in the eyeblink serial feature-positive discrimination learning.

ABSTRACT

OBJECTIVE: This study aimed to assess the current workload and staffing need of physicians and nurses for delivering optimum healthcare services at the Upazila Health Complexes (UpHCs) in Bangladesh. DESIGN: Mixed-methods, combining qualitative (eg, document reviews, key informant interviews, in-depth interviews, observations) and quantitative methods (time-motion survey). SETTING: Study was conducted in 24 health facilities of Bangladesh. However, UpHCs being the nucleus of primary healthcare in Bangladesh, this manuscript limits itself to reporting the findings from the providers at four UpHCs under this project. PARTICIPANTS: 18 physicians and 51 nurses, males and females. PRIMARY OUTCOME MEASURES: Workload components were defined based on inputs from five experts, refined by nine service providers. Using WHO Workload Indicator of Staffing Need (WISN) software, standard workload, category allowance factor, individual allowance factor, total required number of staff, WISN difference and WISN ratio were calculated. RESULTS: Physicians have very high (WISN ratio 0.43) and nurse high (WISN ratio 0.69) workload pressure. 50% of nurses' time are occupied with support activities, instead of nursing care. There are different workloads among the same staff category in different health facilities. If only the vacant posts are filled, the workload is reduced. In fact, sanctioned number of physicians and nurses is more than actual need. CONCLUSIONS: It is evident that high workload pressures prevail for physicians and nurses at the UpHCs. This reveals high demand for these health workforces in the respective subdistricts. WISN method can aid the policy-makers in optimising utilisation of existing human resources. Therefore, the government should adopt flexible health workforce planning and recruitment policy to manage the patient load and disease burden. WISN should, thus, be incorporated as a planning tool for health managers. There should be a regular review of health workforce management decisions, and these should be amended based on periodic reviews.
