ABSTRACT

BACKGROUND: Schistosomiasis is endemic in Nigeria, and the treatment is largely concentrated on children enrolled in schools. Consequently, the coverage of non-enrolled school-aged children is often neglected. Ajagba and Awosan are two communities in Nigeria that have never had any control intervention. Hence, this survey was designed to determine the endemicity of urogenital schistosomiasis and to evaluate the efficacy of a single-dose praziquantel in the communities. METHODS: Urine sample (10 mL) of each participant from Ajagba and Awosan communities was filtered through 12µm polycarbonate filter. The filter was placed on a microscope slide, and stained with a drop of 1% Lugol iodine solution. The stained slides were examined under the microscope and the numbers of S. haematobium eggs were counted. Water contact sites were searched for snail hosts and the snails collected were shed for Schistosoma cercariae. Data were analyzed using SPSS version 24.0 and the significance level was set at 95%. RESULTS: The overall prevalence of infection in the Ajagba community was 45.6% with a mean intensity of 61.1 ± 144.5 eggs/10 mL of urine, while the prevalence of infection in the Awosan community was 5.7% with a mean intensity of 1.4 ± 6.8 eggs/10 mL of urine. The school-aged children had a prevalence and mean intensity of infection of 73.1% and 111.6 ± 177.9 eggs/10 mL of urine, respectively. Following treatment, women had a higher egg reduction rate than men (p = 0.0283). Bulinus globosus were found in Ajagba but not in Awosan, with 5.7% shedding Schistosoma spp, cercariae. CONCLUSION: Urogenital schistosomiasis was hyperendemic in the Ajagba community, and hypoendemic in the Awosan community. The presence of Bulinus globosus supported the transmission of the schistosomiasis in the Ajagba community. Communities where schistosomiasis is still actively transmitted in Nigeria should be identified for effective intervention through the MDA programs.

ABSTRACT

Chemical investigation of the antimalarial medicinal plant Clerodendrum polycephalum led to the isolation of five new diterpenoids, including ajugarins VII-X (1-4) and teuvincenone K (5), along with four known compounds, namely, 12,16-epoxy-6,11,14,17-tetrahydroxy-17(15 → 16)-abeo-5,8,11,13,15-abietapentaen-7-one (6), methyl pheophorbide A (7), loliolide (8), and acacetin (9). The chemical structures of the new compounds were elucidated using NMR spectroscopy, mass spectrometry, circular dichroism, as well as density functional theory calculations. All compounds were evaluated for in vitro activity against Plasmodium falciparum 3D7 malaria parasites with methyl pheophorbide A (7) showing the strongest activity (IC50 4.49 µM). Subsequent in vivo testing in a Plasmodium berghei chemosuppression model showed that compound 7 significantly attenuated peripheral blood parasitemia, leading to 79% and 87% chemosuppression following oral doses at 10 and 20 mg/kg, respectively.

ABSTRACT

Lymnaea natalensis is the only snail intermediate host of Fasciola gigantica, the causative agent of fascioliasis, in Nigeria. The species also serves as intermediate host for many other African trematode species of medical and veterinary importance, and it is found throughout the country. However, there is no detailed information on the factors that influence its distribution and seasonal abundance in the tropical aquatic habitats in Nigeria. This study used the geographic information system and remotely sensed data to develop models for predicting the distribution of L. natalensis in South-Western Nigeria. Both land surface temperature (LST) and normalised difference vegetation index (NDVI) were extracted from Landsat satellite imagery; other variables (slope and elevation) were extracted from a digital elevation model (DEM) while rainfall data were retrieved from the European Meteorology Research Programme (EMRP). These environmental variables were integrated into a geographic information system (GIS) to predict suitable habitats of L. natalensis using exploratory regression. A total of 1410 L. natalensis snails were collected vis-à-vis 22 sampling sites. Built-up areas recorded more L. natalensis compared with farmlands. There was no significant difference in the abundance of snails with season (p  0.05). The regression models showed that rainfall, NDVI, and slope were predictors of L. natalensis distribution. The habitats suitable for L. natalensis were central areas, while areas to the north and south were not suitable for L. natalensis.Contribution: The predictive risk models of L. natalensis in the study will be useful in mapping other areas where the snail sampling could not be conducted.

Malaria, helminthiasis and HIV are widespread in developing countries taking a heavy toll on pregnant women. Due to similar environmental and human factors of transmission, they co-exist. The epidemiology and pathology of these diseases have been extensively studied but data on serum cytokine profile changes which is crucial in pregnancy is limited. The aim of this study was to evaluate the co-infections and their impact on peripheral blood cytokines. Blood and stool samples were collected from recruited 18-45-year-old pregnant women in different trimesters who were apparently healthy with no obvious complications in pregnancy. Pretested questionnaires were administered for personal and socio-demographic details. Malaria parasitemia in Giemsa-stained thick blood films was examined microscopically. Stool samples were screened for helminths using Kato-Katz method. Cytokine levels of TNF-α, IFN-γ, IL-1α, IL-2, IL-4, IL-6, IL-10, IL-12p70, IL-13 and IL-17 in 121 serum samples were determined using ELISA. Data were analysed using descriptive statistics and Mann-Whitney U test at α0.05. Relative to the single infections, there were significant reductions in IFN-γ and IL-13 in second and third trimesters respectively in those with Plasmodium and helminth co-infection. IFN-γ and IL-17 were elevated while IL-1α and IL-12p70 were reduced in co-infection of helminths and HIV. Co-infection of Plasmodium and HIV in second and third trimesters showed significant elevations in IL-1α, IL-10 and IL-17 while TNF-α, IL-4 and IL-12p70 were significantly reduced. HIV in pregnancy and its co-infection with Plasmodium resulted in significant distortions in the cytokine profile. However, helminth and its co-infection with Plasmodium or HIV produced less changes in the cytokine profile.

BACKGROUND OR OBJECTIVES: Human Immunodeficiency Virus/Acquired Immune Deficiency Syndrome (HIV/AIDS) is widespread in sub-Saharan Africa with similarity in geographical distribution of major pathogens of public health interest. The aim of this study was to assess the effect of malaria and helminths on CD4 count, hematocrit values and viral load among HIV-infected pregnant women. METHODS: One hundred and ninety-seven HIV-infected pregnant women aged 18-45 years were recruited from a registered HIV clinic and questionnaires were administered for socio-demographic details. Screening for malaria parasites in blood was through microscopy while helminths were identified in stool using Kato-Katz method. Hematocrit levels were determined through centrifugation of blood collected in capillary tubes. At the time of recruitment, most recent CD4 count and viral load was obtained from the patients' case notes. RESULTS: About three-quarters (73.6%) of the women had above primary school level of education while more than half (60.2%) were petty traders. The prevalence of malaria parasites in the blood samples was 24.9%, while 3% were infected with helminths. There was only a single case of malaria, helminths and HIV co-infection in the study group. Prevalence of anemia was 75.6% with eight cases (4.1%) of severe anemia. About 86.6% of the women with anemia had low CD4 count (χ2= 8.801, p=0.032). The mean CD4 count was significantly lower among those with co-infection of malaria and HIV. CONCLUSION AND GLOBAL HEALTH IMPLICATIONS: Malaria or helminth infection among HIV-infected women lowers the CD4 count and increases the viral load with little changes in hematocrit values. Routine screening of HIV-infected women for probable multiple infections will aid in improving their overall health and well-being.

BACKGROUND: Iron deficiency is a dominant source of anaemia in many settings. To evaluate the key cause of anaemia in the study area, the prevalence of anaemia due to major public health diseases was compared with anaemia due to iron deficiency. METHODS: Pregnant women were recruited from ante-natal (n=490) and HIV clinics (n=217) with their personal data documented using a questionnaire. Microscopy of Giemsa-stained thick smears was used for detection of malaria parasites while helminths in stools were detected using direct smear method. Haematocrit values were determined by capillary method. Serum ferritin levels were determined using enzyme-linked immunosorbent assay. Data was analysed using SPSS version 22.0. RESULTS: The mean age of the recruited women was 28.6±5.4 years old. There were 68.1% cases of anaemia of which 35.5% was due to infections only predominantly HIV and malaria, 14.9% from unknown sources while anaemia due to iron deficiency only was 7.1%. CONCLUSION: It can safely be inferred that malaria and HIV predispose to anaemia than iron deficiency in the study area. Although pregnant women are dewormed and given IPTp for helminths and malaria treatment respectively, there should be complementary routine malaria screening at ANC visits for those with HCT values <33% and those infected with HIV.

BACKGROUND: Urine analysis is one of the recommended antenatal guidelines for early diagnosis of pregnancy-associated complications. While in practice, urine analysis by dipstick had been used to provide useful information on other urinary tract infections, its applications for early detection of urogenital schistosomiasis in pregnant women is often times not given due attention in most endemic areas. Our study therefore assessed the performance of some common urinalysis parameters in the diagnosis of maternal urogenital schistosomiasis in endemic rural communities of Nigeria. METHODOLOGY/PRINCIPAL FINDINGS: The cross-sectional epidemiologic survey of urogenital schistosomiasis was conducted among pregnant women in Yewa North Local Government, Ogun State, Nigeria. The women were microscopically examined for infection with Schistosoma haematobium, visually observed for macrohematuria, and screened for microhematuria and proteinuria using standard urine chemical reagent strips. Of 261 volunteered participants, 19.9% tested positive for S. haematobium infection. The proportion of microhematuria (23.8%) was significantly higher than that of macrohematuria (3.8%) and proteinuria (16.8%) (P<0.05). Microhematuria with sensitivity (82.7%) and specificity (89.0%) was the best diagnostic indicator of urogenital schistosomiasis. Macrohematuria with the least sensitivity (11.8%) was however the most specific (98.1%) for diagnosing urogenital schistosomiasis in pregnant women. Maximum microhematuria sensitivity (100.0%) was observed in women between 15-19 years but sensitivity was consistently low in older age groups. Maximum sensitivity, specificity and predictive values (100.0%) were recorded for microhematuria in first trimester women. Diagnostic efficiency of proteinuria and macrohematuria was also better in the first trimester women except the 25.0% specificity recorded for proteinuria. The overall diagnostic performance of microhematuria and proteinuria was better in secundigravidae. CONCLUSIONS/SIGNIFICANCE: Microhematuria can be used for early detection of urogenital schistosomiasis in endemic areas especially in younger women. However because microhematuria is a condition that occurs during pregnancy and in several other diseases, it is necessary to compliment the diagnosis with other diagnostic tools such as microscopy and serology. Treatment with praziquantel is recommended for the women in their late trimesters after follow up test in order to avert associated adverse pregnancy outcomes.

ABSTRACT

Schistosoma transmission is influenced by the interplay between various factors ranging from parasite to host associated factors. While many studies have focused on mass chemotherapy to reduce transmission in other populations, no study has examined the impact of social factors that favour transmission in pregnant women in Nigeria. The study was conducted to assess the impact of knowledge, attitudes and sociodemographic factors on schistosomiasis burden in pregnant women of rural communities of Nigeria. A cross sectional community-based field study was conducted to assess the association between Schistosoma haematobium burden and the associated risk factors among pregnant women in rural endemic communities of Nigeria. Structured questionnaire was used to gather information on participants' socio-demographic data, knowledge on schistosomiasis and water contact activities. Of the 237 respondents examined microscopically for infection, 50 (21.1 %) were infected with overall mean infection intensity of 69.6 ± 165.2 eggs/10 mL urine. Multivariate logistic analysis showed occupation of the women to be associated with infection with the artisans having the highest risk (OR 3.34, CI 1.67-6.69, P = 0.022). Contact with water and water usage patterns are also associated with prevalence of disease with fetching (OR 2.04, CI 0.19-3.51, P = 0.003) and multipurpose water usage (OR 4.31, CI 2.17-8.57, P = 0.0002) being the most predisposing variables respectively. Awareness about water borne diseases showed no association with infection (P = 0.382) with typhoid (23.7 %) and fever (2.6 %) constituting the most and least common water borne diseases mentioned by the women. Health education and provision of good water supply should be integrated into the control strategies in order reduce transmission in endemic areas.

ABSTRACT

Transmission of schistosomiasis is aided by human behaviour. Globally, about 800 million people are at risk of schistosomiasis infection. Data exist on biomedical understanding of the disease transmission; there is a dearth of information from the social science perspective. Hence, this study explored the social and cultural context of schistosomiasis transmission among Yewa People in Nigeria. Qualitative methods were employed with purposive sampling, using the key informant interviews and focus group discussions, among 57 participants aged 17 to 54 years. The data were content-analyzed. River water was the most reported source of water supply among others. Participants drew from the cultural milieu the use of river water for "drinking" and "swimming" as part of the continual transmission of schistosomiasis. Transmission of schistosomiasis may not be abated without behavioural change.

INTRODUCTION: Poliovirus, an enterovirus, still persists in Nigeria despite the global efforts tailored towards its eradication. This study aimed to assess the impacts of poliovirus and other enteroviruses on the susceptibility of individuals to intestinal parasite infections. METHODOLOGY: A cross-sectional study on the prevalence of intestinal parasites was conducted on two-sample stool specimens of 717 Nigerian children (between 1 and 19 years of age) whose poliovirus/other enteroviruses infection status had been determined. RESULTS: The overall prevalence of Sabin poliovirus and other related enteroviruses infections were 6.6% and 13.8%, respectively. The prevalence of Ascaris lumbricoides was significantly higher than that of other intestinal parasites (p < 0.05), with children in the 0-4 year age group being the most predisposed age group to intestinal parasitic infection (OR = 11.7, CI = 9.2-15.0). While the prevalence of all species of parasites except S. mansoni showed no significant variations in children with Sabin poliovirus (p > 0.05), the prevalence of hookworms and Taenia spp. was significantly higher in children with other enteroviral infections (p < 0.05). CONCLUSIONS: The high risk of children of acquiring enteroviral infection through some intestinal parasites is an indication of possible association of the parasites in a more poliovirus-endemic population. A combined intervention approach for the two infections is advocated.

ABSTRACT

Schistosomiasis remains one of the most important tropical parasitic infections threatening millions of lives in endemic areas. Cases of infections due to Schistosoma spp, the diecious digenetic trematodes have been on the increase over the last decades. While considerable efforts have been made to reduce infections and morbidities in most endemic areas, these efforts seem to be tailored only towards a specific group (school-based resources). This bias towards school children in epidemiological studies has also been observed in various research efforts in sub-Saharan Africa, thus making it difficult to produce a reliable estimate of the extent of infection in other strata of the population at risk. In recent times, attention has been drawn to Schistosoma spp infections in infants and preschool children, while studies on epidemiology of maternal schistosomiasis still suffer neglect. Considering the potential morbidity of Schistosoma infections on the mothers, fetuses, and neonates, as evidenced in some animal models and human case studies, more attention is solicited in all areas of observational studies and clinical trials, for maternal schistosomiasis with the aim of providing relevant data and information for effective management of the disease during pregnancy.

OBJECTIVE: The study evaluates the prevalence of urogenital schistosomiasis and diagnostic performance of chemical reagent strips used for disease diagnosis in preschool-aged children (≤ 5 years) in Nigeria rural communities. PATIENTS AND METHODS: Urine samples from 419 children were observed microscopically for Schistosoma haematobium and screened for hematuria using standard urine chemical reagent strips. RESULTS: Prevalence and intensity of infection were 9.8% and 14.4 eggs/10 ml of urine, respectively. Prevalence of infection was similar in girls (10%) and boys (9.6%) (p > 0.05). The intensity of infection was higher in boys (17.1 eggs/10 ml of urine) than in girls (12.8 eggs/10 ml of urine); however, this was not gender dependent (p > 0.05). The occurrence of hematuria was not associated with gender (p > 0.05), but was associated with prevalence of infection (p < 0.05). CONCLUSION: Infection with S. haematobium occurs early in life in the communities and although intensity of infection is low, it could have serious implications in disease transmission. Hematuria, although moderately sensitive to infection, is an important morbidity indicator of urogenital schistosomiasis in the study population.

OBJECTIVE: To assess the epidemiology of urogenital schistosomiasis among pregnant women in rural communities of southwestern Nigeria. METHODS: The present cross-sectional epidemiologic survey of urogenital schistosomiasis was conducted during 2010-2011 among pregnant women in Yewa North Local Government, Ogun State, Nigeria. The women were microscopically screened for infection with Schistosoma haematobium. RESULTS: Of 313 volunteer participants, 20.8% tested positive for S. haematobium infection. The prevalence of infection was highest (31.5%) among women aged 20-24years. The infection intensity did not differ significantly between age groups (t=1.848, P=0.71). Primigravidae and women in the first trimester of pregnancy had the highest intensity of infection with 33.1 and 27.7 eggs/10mL of urine, respectively. There was an association between disease prevalence and parasite intensity across the age groups (χ(2)=68.82, P=0.02). The prevalence of S. haematobium was not associated with age or pregnancy trimester (P=0.06), but associations existed between intensity of infection and gravidity (P=0.001). CONCLUSION: The prevalence of urogenital schistosomiasis among pregnant women in Nigeria was high, with younger women and primigravidae at the greatest risk. These data can be used to develop a schistosomiasis control program among pregnant women in the study area.

BACKGROUND: The absence of antibodies specific for the 19 kDa C-terminal domain of merozoite surface protein 1 (MSP119) has been associated with high-density malaria parasitaemia in African populations. The hypothesis that a high prevalence and/or level of anti-MSP119 antibodies that may inhibit erythrocyte invasion would be present in apparently healthy individuals who harbour a sub-microscopic malaria infection was tested in this study. METHODS: Plasma samples were collected from residents in a region in Nigeria hyperendemic for malaria, who had no detectable parasitaemia by microscopy. Using a competition-based enzyme-linked-immunosorbent assay with two invasion-inhibitory monoclonal antibodies (mAbs) 12.10 and 12.8, the levels and prevalence of specific antibodies were measured. The minimum multiplicity of infection was determined using PCR. The prevalence of anaemia was also measured. RESULTS: Plasma samples from 85% of individuals contained antibodies that bound to MSP119. The inhibition of mAb 12.10 binding was strongly correlated with the prevalence (Spearman correlation test, p < 0.0001) and mean titre of anti-MSP119 antibodies (Spearman correlation test, p < 0.001) in the samples. Comparing samples from individuals with multiple infection (group M) and single infection (Group S), group M contained a higher (p = 0.04) prevalence of anti-MSP119 antibodies that competed with mAb 12.10. Using a logistic regression model, it was found that the presence of antibodies competitive with mAb 12.10 was affected negatively by anaemia (p = 0.0016) and positively by the carriage of multiple parasite genotypes (p = 0.04). CONCLUSIONS: In the search for correlates of protection against malaria, which will be essential to evaluate clinical trials of malaria vaccines based on MSP1, this study examines some potential assays and the factors that need to taken into account during their evaluation, using samples from individuals naturally exposed to malaria infection.

ABSTRACT

This study evaluated, using replicated laboratory bioassays, the toxicities of the crude aqueous and ethanolic extracts of Dalbergia sissoo Roxb. 1832 (family Leguminosae) fruits, leaves, roots and stem bark against egg masses of Biomphalaria pfeifferi (Krauss, 1848), the snail intermediate host of Schistosoma mansoni (Sambon, 1907) in Nigeria. Viable 0-24 hr-old embryonated egg masses were separately exposed to five different concentrations (7.81-2000 mg/l) of extracts for 24 hrs, washed in dechlorinated tap water and incubated at room temperature for a maximum of 4 weeks. The LC(50) and LC(90) values of test extracts for egg masses were calculated by probit analysis. The activities of the tested extracts were concentration-dependent. However, only the ethanolic extract of the fruits demonstrated significant activity (24 hr-LC(90) value < 100 mg/l: 89.29 mg/l). Mortalities of eggs were manifested at the gastrula/exogastrula and or the prehatch snail stage of development. The percentage of dead embryos at the prehatch snail stage decreased while the deaths of embryos at the gastrula/exogastrula stage increased, with increasing concentration of extract. Lethality of the ethanolic extract of D. sissoo fruits to embryonated egg masses of B. pfeifferi is an added advantage to its potential development for use as a plant molluscicide, as the overall efficacy of a molluscicide is greatly enhanced if it also shows significant toxicity towards snail eggs.

BACKGROUND: In the continued search for molluscicidal compounds from plants, crude aqueous and ethanolic extracts from different parts of Dalbergia sissoo Roxb. 1832, were evaluated against egg masses and adults of Biomphalaria pfeifferi (Krauss, 1848), the snail intermediate host of Schistosoma mansoni (Sambon, 1907) in Nigeria. METHODS: Laboratory-bred adult B. pfeifferi and their viable 0-24 h old egg masses were separately exposed to five different concentrations (7.81-2000 mg l(-1)) each, of the crude aqueous and ethanolic extracts of the fruits, leaves, roots and stem bark of D. sissoo, for 24 h. The LC50 and LC90 values of each extract for the target organisms were calculated using probit analysis. RESULTS: Only the ethanolic extracts of the fruits and roots showed significant activities against the adult snails (24 h LC90<100 mg l(-1): 74.33 and 93.93 mg l(-1), respectively) and their egg masses (LC90: 89.29 and 114.29 mg l(-1), respectively) while all other extracts demonstrated weak molluscicidal and ovicidal activities (24 h LC90 > 100 mg l(-1)). There were concentration-dependent behavioural changes in snails exposed to test extracts, while egg mortalities, manifested at the gastrula/exogastrula stage and or the prehatch snail stage of development, were similarly concentration-dependent. CONCLUSIONS: The crude ethanolic extracts of D. sissoo fruits and roots exhibited promising molluscicidal activities (LC90 values<100 mg l(-1)) against adult B. pfeifferi with additional toxicities towards its 0-24 h-old egg masses.

ABSTRACT

Malaria merozoite surface protein 1 (MSP1) is cleaved in an essential step during erythrocyte invasion. The responses of children to natural malaria infection included antibodies that inhibit this cleavage and others that block the binding of these inhibitory antibodies. There was no correlation between the titer of the antibody to the 19-kDa fragment of MSP1 and its inhibitory activity. These findings have implications for the design of MSP1-based vaccines.
