ABSTRACT

BACKGROUND: Age-related macular degeneration (AMD) is a significant retinal disease that leads to irreversible low vision, particularly in developing countries. The variation in AMD prevalence among different racial groups and highlighted role of inflammation on disease pathology from previous studies which yielded in inconsistent findings, It seems to be of great importance to do more investigation in this field. METHODS: This case control study involved 204 participants, divided into four groups of equal size (51 individuals per group). Three groups represented AMD cases of varying severity according to Beckman classification (3 groups) and one healthy control group. Sampling was conducted exhaustively until the desired sample size was reached. The control group comprised healthy individuals without any infectious or inflammatory systemic, ophthalmic disease. Blood samples were collected to measure inflammatory factors, including lymphocytes, monocytes, neutrophils, neutrophil-to-lymphocyte ratio (NLR), and C-reactive protein (CRP). Collected data were analyzed by statistical methods in SPSS version 21. RESULTS: Of the participants, 51% were women, and their ages ranged from 47 to 89 years (62.2 ± 8). According to multiple logistic regression analysis, age exhibited a statistically significant positive association with AMD severity (P = 0.038, odds ratio [OR] = 1.034). ANOVA results indicated a significant association between neutrophil count and AMD severity (P < 0.001). As the disease severity increased, the number of neutrophils decreased. The mean ± SD neutrophil counts for early, intermediate and advanced AMD were 3849 ± 800, 3702 ± 734, and 3342 ± 823, respectively. No statistically significant associations were found between lymphocyte count, monocyte count, neutrophil-to-lymphocyte ratio, CRP, and AMD. CONCLUSION: There was a significant relationship between the number of neutrophils in peripheral blood and the severity of AMD in study participants which needs more evaluation for the potential utility of this factor in the prognosis of AMD. There was not any significant relationship among the other factors and AMD.

ABSTRACT

This study aimed to specify Pentacam indices in patients who suffered from different types of refractive error and underwent photorefractive keratectomy (PRK) surgery. It is a descriptive cross-sectional study carried out on 1125 patients (2215 eye samples) who underwent PRK surgery in the Noor Surgical Center of Ardabil, Iran, over a 5 year period (2014-2018). A particular checklist was provided to patients, which consisted of demographic data, pachymetry test, keratometry, refractive error, corneal-thickness indices, and corneal surface area indices. The data were analysed using the statistical analysis package of IBM® V25. The mean age of the participants in this study was 28.48±6.82 years, and the ratio of women to men was 66.4%. It was observed that the differences between angle, volume, the depth of the anterior chamber, IVA, and ISV were significant (P=0.00) when compared to each other in all types of refractive errors. High myopes had significantly higher Kmax front than low myopes (P=0.00). In astigmatism patients, the Kmax in front of the cornea in extreme type was significantly higher than in moderate (P=0.00) and high (P=0.01) types. High myopes had significantly lower Rmin than mild myopes (P=0.02), and extreme astigmatism had significantly lower Rmin than high (P=0.014) and moderate types (P=0.013). The data from this study revealed that in patients undergoing PRK surgery, some Pentacam indices could be related to some types of refractive error, and in some of these indices, there are statistically significant differences between different severities of refractive errors. Therefore, their preoperative evaluation is very important.

ABSTRACT

Background: Since the relation between ocular manifestation and coronavirus disease 2019 (COVID-19) has been not elucidated well, thus this study was performed to assess the transmission to patients and healthcare providers via ocular secretions, and to decide if there is a link between ocular symptoms and COVID-19. Methods: A total of 101 patients who have been diagnosed with COVID-19 based on the clinical symptoms, radiological studies and confirmed by reverse transcriptase-polymerase chain reaction (RT-PCR), and hospitalized in Corona ward of Imam Khomeini Hospital in Ardabil from April 15, 2020 to September 15, 2020. Ocular examination was done and patients' demographical data and ocular symptoms were recorded. Also, ocular secretion specimens were obtained to evaluate the COVID-19 RT-PCR test to detect the virus in ocular secretion specimen. Results: In this study, we observed that 15.8% of patients had positive RT-PCR test for COVID-19 in their ocular secretion specimens. In addition, we showed that there is no difference between male and female as well as age in patients with positive RT-PCR and negative tests obtained from ocular secretion. In addition, we observed that 27 patients (26.7%) were symptomatic patients for ocular manifestations including conjunctivitis, epiphora, injection, discharge, etc., but only 2 patients (1.98%) had follicular conjunctivitis during examination. Besides, nonsymptomatic patients (for ocular manifestations) had significant lower rate of positive RT-PCR test from their ocular secretion. Conclusion: This study showed that ocular secretion might be a source of COVID-19 infection even in nonsymptomatic patients. Thus, self-protection from ocular secretion should be observed by ophthalmologists from all patients (even nonsymptomatic patients).

ABSTRACT

This study aimed to examine the effectiveness of photorefractive keratectomy (PRK) in treating patients with cycloplegic hyperopia from +1.00 to +7.00 diopter using Allegretto wave Eye Q 400. This study was conducted on 25 patients with cycloplegic astigmatism ≤1 diopter and cycloplegic hyperopia between +1.00 and +7.00 diopters in 47 eyes, who successively entered into the study within 6 months and underwent PRK. Prior to PRK surgery, all the patients were examined for cycloplegic refraction (astigmatism and hyperopia), slit lamp, keratometry, fundus, and best-corrected (BCVA) and uncorrected visual acuity (UCVA) testing. These examinations were repeated after 1 week, 1 month, 3 months, 6 months, and 1 year postoperatively. The mean preop UCVA of patients was 0.76±0.28 (ranging from 0.00 to 1.3), which reached 0.19±0.22 (ranging from 0.00 to 0.78) one year after the surgery (P=0.000). There was a significant correlation between increasing astigmatism and preop cycloplegic hyperopia >5 diopters (P=0.000), corneal ring haziness at 12th months (P=0.000), and 12 months cycloplegic residual hyperopia ≥2.00 diopters (P=0.000). 53.2% of the eyes (with a mean grade of 2.34) were detected with corneal ring haziness at 12th months, which was significantly correlated with 12 months residual cycloplegic hyperopia of ≥2.00 diopters (P: 0.000) and cycloplegic sphere above 5 diopters (P=0.006). Although the use of photorefractive keratectomy (PRK) with Allegretto Eye Q 400 is associated with a decrease in the mean cycloplegic and improved UCVA and BCVA, its use is not recommended in cases with preop cycloplegic hyperopia above 5 diopters due to the high rate of induction of astigmatism, corneal haziness, and regression of hyperopia.

ABSTRACT

Retinopathy of prematurity (ROP), which affects the retina of premature infants, is a leading cause of blindness in premature infants worldwide. The aim of this study was to evaluate the frequency and risk factors of retinopathy of prematurity infants referred to Alavi hospital between October 2018 and October 2019. In the present study, 400 infants with gestational age less than 34 weeks and or birth weight of 2000 g or less were enrolled in the study. Required information including sex, gestational age, maternal age, birth weight, type of delivery, oxygen therapy, septicemia, multiple gestations, consanguineous marriage, respiratory problem, and blood exchange were extracted from their hospital records and then included in the data collection form. These infants were also examined for ROP, stage and area of involvement by an experienced ophthalmologist and then classified into two groups including retinopathy and healthy group. Finally, the data were analyzed in SPSS 25 software using chi-square, fisher exact test, t-test, binary logistic regression, and ROC curve analysis. Of the 400 preterm infants studied (57.2% male and 42.8% female), 107 (26.8%) infants had ROP of whom 23 (21.5%) needed treatment (i.e. 5.8% of all infants need treatment). There were 4 (3.7%), 29 (27.1%), and 74 (69.2%) infants with zone I, II, and III, respectively. There were 91 (85%), 11 (10.3%), and 5 (4.7%) infants with stage I, II, and III, respectively. Multivariate logistic regression analysis showed that parental consanguinity (OR=2.263, 95%CI: 1.240-4.130, P=0.008), gestational age less than 32.5 weeks (OR=4.067, 95%CI: 2.340-7.069, P<0.001), and birth weight less than 1725 g (OR=4.522, 95%CI: 2.677-7.637, P<0.001) were associated with a 2.3-fold, 4.1-fold, and 4.5-fold increased risk of ROP, respectively. ROP had no significant relationship with other variables. In summary, the findings of the present study showed that one quarter of the premature infants had ROP. Furthermore, birth weight less than 1725 g, gestational age less than 32.5 weeks, and parental consanguinity were risk factors for ROP, in addition, one-fifth of the infants with ROP needed treatment.

ABSTRACT

PURPOSE: To evaluate the effect of opposite clear corneal incisions (OCCI) with 4 mm incisions on the steep meridian on postoperative astigmatism. METHODS: This study was performed on 64 eyes of 55 patients with keratometric astigmatism of ≥ 1 diopter (D) undergoing phacoemulsification. Patients were divided into two groups, with-the-rule (WTR) astigmatism and against-the-rule (ATR) astigmatism. Initial incisions in the WTR group were performed on the temporal side with 3.2 mm keratome and paired stab incisions were performed on the steep meridian. At the end of the surgery, stab incisions were enlarged to 4 mm. Follow-up visits were scheduled at 1, 3, 6, and 12 months postoperatively, which included refraction and keratometry. RESULTS: It was found that the mean preoperative keratometric astigmatism was 2.06 ± 0.86 D. The postoperative mean keratometric astigmatism was 1.3 ± 0.7 D after 1 month and 1.2 ± 0.7 D after 12 months. The mean astigmatism correction between the preoperative measure and that taken at 1 month was statistically significant (P = 0.001), but there was no significant change in the severity of astigmatism afterward. The mean surgically-induced astigmatism was found to be 1.99 ± 0.9 D. The 12-month changes of mean absolute astigmatism were: 1.06 ± 0.7 D in the WTR group, and 0.53 ± 0.7 D in the ATR group. The difference between the two groups was statistically significant (P = 0.02). CONCLUSION: Based on our findings, we posit that paired OCCI on the steep axis, using 4 mm incisions is an effective technique to correct preoperative astigmatism.

ABSTRACT

Refractive eye surgeries are one of the most non-emergent ophthalmic surgeries due to the effect on the reduction of refractive errors, increasing visual acuity, enhancing the quality of vision, and indirectly increasing the quality of life of patients. The aim of this study was to determine Pentacam indices in the patients who underwent photorefractive keratectomy (PRK) during 2014-2018, as well as to show their correlation with the type of refractive error. This descriptive cross-sectional study was performed on 2215 eyes of 1125 patients undergoing PRK surgery. The patients' checklist, including demographic information, refractive index, keratometry, pachymetry, corneal surface zone indices, and progressive corneal thickness indices, was provided. All data were analyzed using the IBM SPSS software, version 25. The findings showed that there was a significant association between posterior corneal astigmatism (PCA) and anterior corneal astigmatism (ACA) (p=0.00). The mean Kmax front was recorded as 44.844 ± 1.58 D, which was significantly correlated with the type of refractive errors (p=0.00). According to the findings, there was a significant relationship between anterior chamber indices and refractive error types and their severity (p=0.00). There was also a significant correlation between the surface zone and keratoconus indices (i.e., index of surface variance - ISV, index of vertical asymmetry - IVA, index of height asymmetry - IHA, and minimum radius of curvature - Rmin) with refractive errors (p=0.00). The findings showed that some of the Pentacam indices could be related to the types of refractive errors in patients undergoing PRK surgery. Therefore, their evaluation is of great importance in this regard.

ABSTRACT

BACKGROUND: Zinc (Zn) acts as a cofactor of matrix metalloproteinases (MMPs) and is vital for their activity and controlling their expression. Alteration of Zn in the body could affect the expression, activity, and destructive impacts of MMPs. OBJECTIVE: This systematic review aimed to summarize existing evidence on the effects of Zn treatment on the expression and activity of MMPs. METHOD: International sources from Pub Med, Scopus and Google Scholar were searched for the original and English-language studies, published up to the end of May 2018. RESULTS: During the initial search, 179 records were found, and 135 articles of them remained after the exclusion of duplicate articles. 47 studies met the inclusion criteria, after multiple stages of screening and critical reviews of articles. CONCLUSION: Approximately 62% of the included studies (29 of 47) showed an inhibitory impact of Zn on MMPs production and activities. The inhibitory or stimulatory effect of Zn on MMPs seems to depend on physiological conditions of the cells or animals used, dose of Zn used, and duration of treatment.

BACKGROUND: Astigmatism is the leading complication in visual recovery after penetrating keratoplasty (PKP) and deep anterior lamellar keratoplasty (DALK); in this study, we evaluated the outcome of femtosecond laser arcuate keratotomy (FLAK) after DALK and PKP in Iranian keratoconic patients. MATERIALS AND METHODS: In this prospective interventional case series, refractive and keratometric predictability, efficacy, and complications of FLAK for postkeratoplasty astigmatism in keratoconus were evaluated; 23 eyes of 23 consecutive patients (mean age of 32.43 ± 9.11 years) with high astigmatism were enrolled. The femtosecond laser performed paired 90°-angled arcuate incisions. RESULTS: Mean logarithm of the minimum angle of resolution of corrected and uncorrected visual acuity improved from preoperative values of 0.30 ± 0.18 and 0.85 ± 0.32 to 6-month values of 0.19 ± 0.17 and 0.65 ± 0.33, respectively (P < 0.05). Mean subjective astigmatism was 7.79 ± 2.64 diopter (D) preoperatively and 3.69 ± 2.25D at 6-month after surgery (P < 0.05). Surgically induced astigmatism was 9.27 ± 5.00D. Mean refractive spherical equivalent showed no significant (P = 0.69) hyperopic shift from - 4.21 ± 4.84D preoperatively to - 2.16 ± 6.09D postoperatively. Two (8.7%) microperforations were observed. CONCLUSION: FLAK is a relatively safe and effective method for the treatment of postkeratoplasty astigmatism.

ABSTRACT

Background and Purpose. Trauma is the leading cause of death for youth in developing countries. Given the prevalence of head trauma (HT) in society and its complication and burden, the epidemiologic study of head trauma is necessary and is the main aim of this study. Materials and Methods. This retrospective population-based survey describes the epidemiology of head injury in a defined population in Ardabil city. It includes all 204 patients with head injury referred to the University Hospital of Ardabil, Iran, during 2013-2014. Data were collected by a checklist and analyzed by statistical methods in SPSS.19. Significance level p < 0.05 was considered. Results. Of all registered cases, 146 (71.6%) were male and the rest of them were female. Most of HT patients lived in Ardabil city (60.8%). The mean age of patients was 22.6 ± 25.9 and most of victims were young. 24.5% of traumatic patients have injuries in severe to critical level (grade 3-4). The most cause of trauma was accidents (41.7%). Most of injuries occurred in night (55.9%) and in summer season (42.2%). Causes were traffic accident in 41.7%. Conclusion. Results showed that the leading cause of head trauma especially in the warm seasons is accidents and so, designing programs to reduce road accidents can dramatically reduce the rate of trauma in the future in Ardabil province.

ABSTRACT

BACKGROUND: The aim of this study was to evaluate the results of amblyopia screening in Ardabil Province in three examination levels by kindergarten teacher, optometrist, and ophthalmologist. METHODS: In a cross-sectional study, the results of the national amblyopic prevention program in 2-6 years old children in Ardabil Province were investigated in 2012. The results pertained to the examinations of children participating in this research were collected in the national approved forms. The data were entered into the computer and were analyzed using statistical methods in SPSS 18. RESULTS: Around 38,844 children (51.7%) out of 75173 with 2-6 years old qualified children participated in the screening program in Ardabil Province. In the first stage of screening, 1068 children (33.1%) are visually impaired in one eye and 2160 children (66.9%) are visually impaired in two eyes. In the second stage, the results related to the examinations by optometrists indicated that the prevalence of refractive errors, strabismus, and others were 70%, 27.8%, and 2.2%, respectively. Refractive errors problem was most prevalent in Ardabil city (72.6%).The prevalence of refractive errors, strabismus, and other reasons in amblyopic children was 51.3%, 23.9%, and 24.8%; respectively. CONCLUSIONS: The present investigation showed that coverage of amblyopia screening program was not enough in Ardabil Province. To increase the screening accuracy, standard instruments and examination room must be used; more optometrists must be involved in this program and increasing the validity of obtained results for future programming.
