ABSTRACT

The widespread application of innovative thermal enhanced façade solutions requires an adequate durability evaluation. The present work intends to assess the durability of a new aerogel cement-based rendering system through the adaptation of different accelerated aging cycles, such as heating-freezing, freeze-thawing, and heat-cold. Several mechanical properties and also capillary and liquid water absorptions were tested for uncoated and coated specimens. A decrease in the mechanical strength, especially after freeze-thaw cycles, was observed. However, the water action promoted the late hydration of the cement paste contributing to the densification of the matrix and, consequently, the increase of the adhesive strength. Additionally, a decrease in the dynamic modulus of elasticity and an increase in the Poisson's ratio were observed after aging, which indicates a higher capacity of the render to adapt to substrate movements, contributing to a reduction of cracking.

ABSTRACT

Quarrying activities cause profound modifications on ecosystems, such as removal of vegetation cover, biodiversity loss and depletion of ecosystem services. Ecological restoration stands as a solution to revert such effects. Concomitantly, awareness is currently being given on ecosystem services and ecological processes to evaluate restoration efficiency. The objective of the study was to assess restoration success in a quarry subjected to restoration practices for the last 40 years involving the plantation of native Mediterranean vegetation and the non-native Aleppo pine Pinus halepensis. The study was carried out by assessing the effectiveness of seed dispersal service provided by birds in the restored quarry by comparing this service to neighbouring natural (shrubland) and other semi-natural areas (oak-pine mixed open and Aleppo pine forest) present at the landscape. For this purpose, we explored bird composition structure and seed dispersal networks using point counts and faecal samples of mist-netted birds. We also collected vegetation structure data and explored its effect on bird community composition. Our results showed that bird abundance in the restored quarry was significantly lower, and its bird community was compositionally different than natural shrubland and semi-natural areas. For instance, seed-dispersing birds, woody and shrub/ground foragers and partially migrators were the most affected groups at the restored area. Bird community composition and their traits were likely driven by vegetation characteristics, with higher native vegetation cover and fruit richness promoting higher bird abundance and Aleppo pine cover negatively influencing seed-dispersing birds. Concurrently, seed dispersal network in the restored quarry was less complex than in other areas. Seed dispersal services in the restored quarry were below the reported values for neighbouring natural and semi-natural areas and are likely driven by the low abundance of seed-dispersing birds. We consider that the causes affecting this group's low abundance can be related to revegetation measures favouring Aleppo pine, combined with a shallow soil depth and poor soil quality, which may have constrained native vegetation development. We conclude that seed dispersal services at the quarry are depleted, which may suggest a low restoration success concerning ecosystem functioning. Our results strengthen that quarry revegetation with non-native species must be avoided, since it alters bird community composition, and consequently, affects seed dispersal service provided by birds.

ABSTRACT

During deceased donor procurement, the heart procurement team may cut the supra-hepatic inferior vena cava (IVC) too close to the liver surface, depriving the liver allograft from having enough supra-hepatic IVC to perform the anastomosis with the recipient's IVC or hepatic veins. In such instances, liver grafts usually are deemed as non-appropriate for transplantation, being discarded. Here we report a technique for reconstruction of damaged supra-hepatic IVCs through the use of a segment of the infra-hepatic IVC of the liver graft.

BACKGROUND: Esophagogastric junction outflow obstruction (EGJOO) is a rare syndrome, characterized by an elevation of the integrated relaxation pressure of the lower esophageal sphincter, not accompanied by alterations in esophageal motility that may lead to the criteria for achalasia. We were unable to find any prior report of the combination of Heller myotomy with anterior partial fundoplication (Dor) as the treatment for EGJOO. We herein report a case of EGJOO treated with laparoscopic Heller myotomy combined with Dor fundoplication. CASE SUMMARY: A 26-year-old man presented with a 3-year history of solid dysphagia and a 30-kg weight loss. He was treated with oral nifedipine, isosorbide, and omeprazole, without resolution of symptoms. An upper gastrointestinal series (barium swallow) revealed a "bird's beak" sign. Esophagogastroduodenoscopy was positive for Los Angeles grade A peptic esophagitis. High-resolution esophageal manometry was compatible with EGJOO. Esophageal pH monitoring showed pathological acid reflux both in orthostatic and decubitus position. An 8-cm laparoscopic Heller myotomy combined with an anterior 220° Dor fundoplication was performed. Solid diet was introduced on postoperative day 2, and the patient was discharged home the same day. At 17-mo follow-up, he reported no symptoms. Barium swallow was compatible with complete radiologic resolution. Both esophageal manometry and upper endoscopy showed normal findings 9 mo after the operation. CONCLUSION: Surgical treatment with Heller myotomy and Dor fundoplication is a potential treatment option for EGJOO refractory to medical treatment.

BACKGROUND: Self-medication is an important component of health care. To optimize pharmacists' over-the-counter counseling, there are several guidelines and protocols used in practice. In a self-care environment, protocols should comprise items related to patients' autonomy. The structure of self-medication protocols, among other elements, should present steps to facilitate patients' participation. OBJECTIVE: To analyze structural differences between existing community pharmacy minor ailment protocols, including those related to patient autonomy and empowerment in self-medication conditions. METHODS: The study design followed a cross-sectional descriptive approach. Self-medication protocols were systematically collected from 3 different professional sources (Pharmaceutical Society [OF], National Pharmacies Association [ANF], and Grupo Holon [GH]). A structural-based analysis, by comparison with the general self-medication OF standard protocol, produced outcome measures such as frequencies of flowchart critical steps, active pharmaceutical substances, dosage forms, and posologies. Simple scores were computed to assess protocols' structural quality, as well as differences between protocols, produced by each professional organization. RESULTS: Forty-four protocols presented on average 8 counseling steps toward 10 different active substances, 7 dosages forms, and 14 posologic schemes. From a maximum of 30 critical items, 1 protocol scored 24, 7 scored 23, and 5 scored less than 15 items. Significant differences were found between protocols' structural components from different sources, particularly between GHs' protocols compared with those produced by OF and ANF. CONCLUSIONS: In general, all protocols matched the OF standard for ailment characterization but fell short on steps related to medicine information and selection. Steps for patient participation and agreement were absent, as was pharmacists' expected role of outcomes monitoring. It might be appropriate to redesign self-medication protocols, preferably through a consensus process that includes not only professionals' but also patients' preferences, starting from the prevalent conditions in Portuguese pharmacy practice.
