ABSTRACT

PURPOSE: To correlate the functional performance and impact of dysphagia on the quality of life of cancer patients in palliative care. METHODS: This cross-sectional, quantitative study was conducted at the outpatient clinic and oncology ward of a university hospital. Inclusion criteria required patients to respond positively to the question: "Do you have difficulty or problems swallowing?". Patients were excluded if they had been diagnosed with head and neck cancer, were unable to answer questionnaires due to actively dying status, were in a state of drowsiness, experienced extreme pain and systemic instability, or if data collection instruments were incomplete. Two instruments were used in their Brazilian Portuguese versions: the Palliative Performance Scale (PPS) and the M. D. Anderson Dysphagia Inventory (MDADI). The variables were analyzed using descriptive and inferential statistics, with Pearson's correlation used at a 5% significance level. RESULTS: The sample consisted of 39 participants, with an average age of 65.3 years, of whom 24 (61.5%) were women. The most frequent neoplasm sites were the pancreas and stomach. The results of the PPS indicated that the average patient had reduced ambulation and inability to work, but maintained independence in self-care, with a complete level of swallowing and consciousness. The MDADI showed an average degree of limitation. Outpatients exhibited a moderate correlation between the MDADI result and the level of functionality according to the PPS. CONCLUSION: Cancer patients at the palliative care outpatient clinic demonstrated a correlation between functional performance and the impact of dysphagia on quality of life.


OBJETIVO: Correlacionar a performance funcional e impacto da disfagia na qualidade de vida de pacientes oncológicos em cuidados paliativos. MÉTODO: Estudo transversal e quantitativo realizado no ambulatório e enfermaria de oncologia de um hospital universitário. Os critérios de inclusão exigiram que os pacientes respondessem positivamente à pergunta: "você tem dificuldade ou problema para engolir?". Foram excluídos os pacientes que tivessem diagnóstico de câncer de cabeça e pescoço, incapacidade de responder questionários devido a estarem em processo ativo de morte, estado de sonolência, dor extrema e instabilidade sistêmica, bem como os instrumentos de coleta que não foram concluídos. Foram aplicados dois instrumentos em suas versões para o português brasileiro: a Palliative Performance Scale (PPS) e M. D. Anderson Dysphagia Inventory (MDADI). A análise das variáveis foi realizada com base na estatística descritiva e inferencial, por meio da correlação de Pearson, em nível de significância de 5%. RESULTADOS: A amostra foi composta por 39 participantes, com média de 65,3 anos, dos quais 24 (61,5%) eram mulheres. As localizações mais frequentes de neoplasia foram: pâncreas e estômago. O resultado da PPS indicou que a média dos pacientes apresentou deambulação reduzida, incapacidade para trabalhar, porém com independência no autocuidado, nível de ingesta e consciência completos e o MDADI obteve grau médio de limitação. Pacientes ambulatoriais apresentaram correlação moderada entre o resultado do MDADI e nível de funcionalidade pela PPS. CONCLUSÃO: Pacientes oncológicos do ambulatório de cuidados paliativos apresentaram correlação entre performance funcional e o impacto da disfagia na qualidade de vida.

ABSTRACT

PURPOSE: To map scientific evidence on the variability of quantitative parameters extracted by instrumental swallowing assessment tests in adults, using the coefficient of variation. RESEARCH STRATEGIES: The methodological procedures recommended by the Joanna Briggs Institute and the extension for scoping reviews of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA-ScR) were followed. SELECTION CRITERIA: The search was carried out in the Pubmed/Medline, Lilacs, Cochrane Library, Embase, Web of Science, Scopus and CINAHL databases, as well as in Google Scholar to consult the gray literature. DATA ANALYSIS: Two blind and independent reviewers screened the articles by title and abstract. Subsequently, the articles were read in full and selected according to the eligibility criteria. Data were extracted according to a standardized instrument. RESULTS: 363 studies were found, 13 of which were eligible. Most studies had a sample size of less than 30 participants and were made up of healthy individuals. The instrumental exams used were diverse: videofluoroscopy, electrical impedance tomography, laryngeal sensors, high-resolution manometry and surface electromyography. The studies searched for intra-individual variability and the coefficient of variation ranged from low to high variability, as the instruments, parameters and collection procedures were very heterogeneous and non-standardized. CONCLUSION: Intra-individual variability of the quantitative outcomes of instrumental swallowing assessments in adults ranged from low to high according to the exam, outcome, presence or absence of underlying disease, consistency and volume of the bolus.


OBJETIVO: Mapear as evidências científicas sobre a variabilidade dos parâmetros quantitativos extraídos por exames instrumentais de avaliação da deglutição em adultos, mediante o coeficiente de variação. ESTRATÉGIA DE PESQUISA: Foram seguidos os procedimentos metodológicos recomendados pelo Joanna Briggs Institute e a extensão para revisões de escopo do Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA-ScR). CRITÉRIOS DE SELEçÃO: A busca foi realizada nas bases de dados Pubmed/Medline, Lilacs, Cochrane Library, Embase, Web of Science, Scopus e CINAHL, assim como no Google Scholar para consultar a literatura cinzenta. ANÁLISE DOS DADOS: Dois revisores cegos e independentes fizeram o rastreamento dos artigos por título e resumo. Posteriormente, os artigos foram lidos na íntegra e selecionados de acordo com os critérios de elegibilidade. Os dados foram extraídos de acordo com um instrumento padronizado. RESULTADOS: Foram encontrados 363 estudos, sendo 13 elegíveis. A maioria dos estudos teve amostra menor que 30 participantes e foi composta por indivíduos saudáveis. Os exames instrumentais utilizados foram diversos: videofluoroscopia, tomografia de impedância elétrica, sensores laríngeos, manometria de alta resolução e eletromiografia de superfície. Os estudos investigaram principalmente a variabilidade intraindividual e os valores do coeficiente de variação oscilaram entre baixa e alta variabilidade, pois os instrumentos, parâmetros e procedimentos de coleta foram heterogêneos e não padronizados. CONCLUSÃO: A variabilidade intraindividual dos parâmetros quantitativos da deglutição obtidos por meio de exames instrumentais em adultos oscila entre baixa e alta conforme o exame, parâmetro testado, presença ou não de doença de base, consistência e volume do bolo alimentar.

ABSTRACT

OBJECTIVE: To report the process of translation and cross-cultural adaptation of theYale Pharyngeal Residue Severity Rating Scale into Brazilian Portuguese. METHODS: Methodological study approved by the Ethics Committee of the Institution (nº 5.166.256). The English original scale was translated into Brazilian Portuguese following suggested in the literature guidelines and recommendations after authorization from the authors of the original instrument, and involved the following reported steps of (1) Translation, (2) Synthesis of translations, (3) Determination of the applicability of the translated version 4) Back-translation, (5) Synthesis of the back-translated versions, and (6) Final synthesis. The translations and back-translations were performed by two bilingual translators. The research committee constituted three specialists who considered whether the linguistic, semantics, conceptual, idiomatic, and contextual equivalence of the translations and back-translations were. In Step 3, the raters consisted of three Speech-Language Pathologists and five ENT physicians divided into two subgroups (less than 5 years of professional expertise, and more than 5 years of professional expertise). RESULTS: Step 1 was carried out properly by the translators, in Step 2 the translated version was prepared after minor adjustments. In Step 3, the raters reported that they found no difficulties in applying the scale. The Cronbach's Alpha coefficient was 0.995, demonstrating high internal consistency of the instrument, and the analysis of the Intraclass Correlation Coefficient (ICC) among the eight raters was 0.994 with a confidence interval between 0.990 and 0.998, demonstrating excellent agreement, regardless of experience. The research committee judged the last to be adequate and not to require adjustments. CONCLUSION: The Brazilian Portuguese version of theYale Pharyngeal Residue Severity Rating Scale is presented in this study. This is a methodological study - No level of evidence.

ABSTRACT

This scoping review aimed to identify methodological procedures for acquiring and analyzing ultrasound images related to swallowing in adults and older adults. The inclusion criteria were based on the PCC strategy (participants, concept, and context), as follows: population (adults and older adults), concept (ultrasound assessment), and context (swallowing assessment). The review included observational, experimental, descriptive, and analytical studies and excluded those that were not available in full, animal studies, in vitro studies, letters to the editor, errata, study protocols, and those that used ultrasound for purposes other than swallowing. There were no language and time restrictions. Two independent blinded professionals selected 81 articles that met the inclusion criteria from different databases. The most evaluated parameters included tongue morphology and movements, hyoid bone displacement, swallowing muscle area, and pharyngeal residue detection, such as pharyngeal wall mobility. They used convex and linear transducers (3 MHz to 8 MHz) positioned in the submental, laryngeal, and lateral regions of the neck. The subjects were seated and instructed to eat different food volumes and consistencies. The literature mapping showed that ultrasound is a promising diagnostic tool, helping clinicians understand swallowing disorders, as it provides static and dynamic images in different modes and positions. Also, patients receive real-time biofeedback of changes related to dysphagia.

ABSTRACT

The aim of this study was to investigate the measures of displacement, time and velocity of hyoid bone movement in female thyroidectomy patients. Fifty-eight ultrasound videos of 29 women during swallowing were analyzed. The sample was divided into experimental group (EG), composed of 12 women following total or partial thyroidectomy; and control group (CG) of 17 healthy women. The kinematic measures of displacement, time and velocity of hyoid bone displacement were tracked during swallowing of 10 ml of liquid (water) and 10 ml of thickened liquid (honey) in both groups for comparisons. Additional analysis included bolus consistency effect and relationship with clinical characteristics. Ultrasound videos were analyzed according to a standardized protocol using ImageJ software. Displacement, time and velocity of hyoid movement during swallowing of 10 ml of liquid or thickened liquid were not statistically different between female thyroidectomy patients and healthy women. There is no bolus consistency effect on kinematic measures in both groups, but among thyroidectomy patients, velocity of hyoid bone movement is significantly faster in those with swallowing complaints.

ABSTRACT

OBJECTIVE: The objective of this study was to describe the occurrence of self-reported olfactory and taste disorders in non-hospitalized Brazilian adults who presented severe acute respiratory syndrome-related coronavirus 2 infection symptoms and attended primary health care. METHODS: This cross-sectional study was based on a routine standardized diagnostic screening questionnaire applied in a Brazilian primary care facility. The olfactory and taste disorder occurrence was compared between severe acute respiratory syndrome-related coronavirus 2-positive and severe acute respiratory syndrome-related coronavirus 2-negative cases and described by age and sex. RESULTS: Severe acute respiratory syndrome-related coronavirus 2-positive patients had a higher proportion of self-reported olfactory and taste disorders, as compared with severe acute respiratory syndrome-negative (50.7%, vs. 20.6%, p<0.0001). Of all individuals with self-reported olfactory and taste disorder cases, 69% presented both olfactory and taste impairments, 13% olfactory only, and 17% taste only. In severe acute respiratory syndrome-related coronavirus 2-positive cases, the frequency of olfactory and taste disorders was significantly higher among females as compared with males (71% vs. 34%). Additionally, people with olfactory and taste disorders were significantly younger in the severe acute respiratory syndrome-related coronavirus 2-positive group. CONCLUSION: Self-reported olfactory and taste disorders are highly common among non-hospitalized severe acute respiratory syndrome-related coronavirus 2-positive Brazilian people who attended the Family Health Care Unit. The co-occurrence of both self-reported olfactory and taste disorders was more frequent than self-reported olfactory or taste disorders alone.

ABSTRACT

Analyzing fiberoptic endoscopic evaluation of swallowing (FEES) is challenging and requires training to ensure the proficiency of health professionals and improve reliability. This scoping review aims to identify and map the available evidence on training health professionals to analyze FEES functional parameters. The method proposed by the Joanna Briggs Institute and the PRISMA-ScR guidelines were followed. The search was performed in MEDLINE, Cochrane Library, Embase, Web of Science, Scopus, CINAHL databases, and in the gray literature. Two blinded independent reviewers screened articles by title and abstract. Then, they read the full text of the included reports, considering the eligibility criteria. Data were extracted using a standardized form. Six studies met the established eligibility criteria, published between 2009 and 2022, with few participants. All these studies addressed training as part of the process to validate a rating scale. No standardized criteria were observed regarding the selection of experts and participants, training structure, and outcome measures to assess participants' competence. The reviewed literature indicates that training must be developed to equip students and health professionals who treat dysphagia, enabling them to analyze the functional parameters of the FEES, considering variables that may influence the participants' performance.

ABSTRACT

PURPOSE: To synthesize the scientific knowledge on which measurements of hyoid bone movement during swallowing are obtained by ultrasonography and how to extract these measures. RESEARCH STRATEGIES: The PECO question and combinations of descriptors and keywords were formulated in the electronic databases Medline/PubMed, EMBASE, Web of Science, Scopus and Lilacs. SELECTION CRITERIA: Articles that used ultrasonography to analyze measurements of hyoid bone movement during swallowing were included, regardless of language, year of publication, or presence of deglutition disorders. DATA ANALYSIS: The included articles were analyzed for: year, study site, study design, population, sample size, equipment used, transducer positioning, measurements obtained, method of extraction, and reliability of measurements. RESULTS: Twenty-six articles met the eligibility criteria. The most frequent measurement was hyoid movement maximum amplitude, followed by time and velocity. There was great variability in the study population, equipment used, positioning of the transducer and method of extraction of the measurements. Thus, it was not possible to find a standard model to extract the measures. The level of reliability was investigated in only eight articles. CONCLUSION: Amplitude, time and velocity are the measures of hyoid bone movement during swallowing that can be obtained by ultrasonography. There is no standardization on how to extract these measurements.


OBJETIVO: Sintetizar o estado do conhecimento científico sobre quais medidas do movimento do osso hioide durante a deglutição são obtidas pela ultrassonografia e como extraí-las. ESTRATÉGIA DE PESQUISA: A pergunta PECO e as combinações de descritores e palavras-chave foram formuladas nas bases de dados eletrônicas Medline/PubMed, EMBASE, Web of Science, Scopus and Lilacs. CRITÉRIOS DE SELEçÃO: Foram incluídos os artigos que utilizaram a ultrassonografia para analisar as medidas de movimento do osso hioide durante a deglutição, independentemente do idioma, ano de publicação ou presença de alteração na deglutição. ANÁLISE DOS DADOS: Os artigos incluídos foram analisados quanto: ano, local do estudo, desenho do estudo, população, tamanho da amostra, equipamento utilizado, posicionamento do transdutor, medidas obtidas, método de extração e confiabilidade das medidas. RESULTADOS: Vinte e seis artigos cumpriram os critérios de elegibilidade. A medida mais frequente foi a de amplitude máxima do movimento, seguida de tempo e velocidade. Houve grande variabilidade quanto à população de estudo, equipamentos utilizados, posicionamento do transdutor e método de extração das medidas, não sendo possível estabelecer padronização. O nível de confiabilidade foi investigado em apenas oito artigos. CONCLUSÃO: Amplitude, tempo e velocidade são as medidas do movimento do osso hioide durante a deglutição que podem ser obtidas por ultrassonografia. Não há padronização dos métodos de extração dessas medidas.

OBJECTIVE: • Seventy-four (33.18%) municipalities in the state of Paraíba registered live births with microcephaly. • The highest proportion of cases (23.03%) was concentrated in the capital, João Pessoa. • Number of inhabitants, number of cases of Zika virus, water supply, and average household income were associated with a higher proportion of new cases. To analyze the relationship between microcephaly and social inequality indicators in the state of Paraíba during the biennium January 2015 and December 2016. METHODS: Ecological study with data from newborn microcephaly records and municipal socioeconomic, environmental, and demographic indicators was conducted using two health information systems from the Brazilian Ministry of Health (SINASC and SINAN) and the Brazilian Institute of Geography and Statistics. A Poisson multiple regression model was applied with a significance level of 5%. RESULTS: Among 223 municipalities in Paraíba, 74 registered new cases of microcephaly. The number of Zika virus cases, number of inhabitants, number of households without adequate water supply, and household income were predictor variables of the number of new cases of microcephaly in Paraíba. CONCLUSION: Microcephaly is associated with indicators of social inequality in Paraíba. Zika virus cases, water supply, and family income are the indicators that best explain the increase in microcephaly cases. Therefore, these variables must be monitored by health professionals and authorities.

ABSTRACT

ABSTRACT Purpose: to conduct a scoping review to identify procedures to obtain and analyze ultrasound images related to swallowing in adults and older adults. Methods: the strategy (participants, concept, and context) was employed to determine inclusion criteria - population (adults and older adults), concept (ultrasound assessment), and context (assessment of swallowing). The review will analyze observational, experimental and/or quasi-experimental, descriptive, analytical, and qualitative studies, and opinion texts and articles. It will exclude studies unavailable in full text, in animals, or in vitro, letters to the editor, errata, study protocols, and studies using ultrasound with purposes other than swallowing. There will be no restriction on the language. Two independent professionals will select the studies in the databases (MEDLINE, EMBASE, LILACS, Web of Science, and Scopus). The review will analyze the year of publication, study design, sample, age, and procedures to assess swallowing with ultrasound. Data will be presented in diagrams, tables, and narrative. Final Considerations: the literature has described ultrasound as an assessment instrument to analyze biomechanical swallowing movements. This scoping review will describe methods to acquire ultrasound images to assess swallowing.


RESUMO Objetivo: realizar uma revisão de escopo para identificar os procedimentos de obtenção e análise de imagens ultrassonográficas relacionadas à deglutição de adultos e idosos. Métodos: a estratégia (participantes, conceito e contexto) foi empregada para determinar os critérios de inclusão - população (adultos e idosos), conceito (avaliação ultrassonográfica) e contexto (avaliação da deglutição). Serão analisados os estudos observacionais, experimentais e/ou quase experimentais, descritivos, analíticos, qualitativos, textos e artigos de opinião; e eliminados os que não estiverem disponíveis na íntegra, estudos em animais, in vitro, cartas ao editor, errata, protocolos de estudo e os que usaram a ultrassonografia para fins que não contemplam a deglutição. Não haverá restrições a idiomas. Os estudos serão escolhidos por dois profissionais, independentemente, a partir das bases de dados (MEDLINE, EMBASE, LILACS, Web of Science e Scopus). Serão analisados o ano do estudo, delineamento, amostra, idade e os procedimentos para avaliação ultrassonográfica da deglutição. Os dados serão expostos por meio de diagramas, narrativas e tabelas. Considerações Finais: a ultrassonografia vem sendo descrita na literatura como um instrumento de avaliação que permite a análise dos movimentos biomecânicos da deglutição. Esta revisão de escopo irá permitir a descrição dos métodos de aquisição da ultrassonografia na avaliação da deglutição.

ABSTRACT Objective To analyze the relationship between microcephaly and social inequality indicators in the state of Paraíba during the biennium January 2015 and December 2016. Methods Ecological study with data from newborn microcephaly records and municipal socioeconomic, environmental, and demographic indicators was conducted using two health information systems from the Brazilian Ministry of Health (SINASC and SINAN) and the Brazilian Institute of Geography and Statistics. A Poisson multiple regression model was applied with a significance level of 5%. Results Among 223 municipalities in Paraíba, 74 registered new cases of microcephaly. The number of Zika virus cases, number of inhabitants, number of households without adequate water supply, and household income were predictor variables of the number of new cases of microcephaly in Paraíba. Conclusion Microcephaly is associated with indicators of social inequality in Paraíba. Zika virus cases, water supply, and family income are the indicators that best explain the increase in microcephaly cases. Therefore, these variables must be monitored by health professionals and authorities.

ABSTRACT Purpose: to investigate the profile of speech-language-hearing therapists from Northeastern Brazil assisting children presented with congenital Zika syndrome and their perception of these children's orofacial myofunctional aspects and speech-language-hearing intervention procedures. Methods: a cross-sectional observational study with quantitative/qualitative analysis. The data collection instrument was an online questionnaire with 34 items on the speech-language-hearing therapists' sociodemographic information and perception of the study topic. The sample comprised 23 speech-language-hearing therapists from the nine Northeastern states. Results: altogether, 96% of the sample were females. Most professionals had more than 3 years of experience with congenital Zika syndrome. Concerning stomatognathic system characterization, the professionals described a greater degree of change in aspects of orofacial muscle posture and mobility. Regarding oral functions, they perceived greater changes in speech and swallowing liquids and solid foods. There was a greater degree of change in masticating solids than in swallowing liquids. Conclusion: most speech-language-hearing therapists in the research had a specialization degree, especially in dysphagia and oral-motor control. Most of them had more than 3 years of experience following up with children with congenital Zika syndrome, which indicates a satisfactory time working with this population to help identify orofacial myofunctional changes in them. The professionals stated that orofacial muscle posture and mobility, speech, and swallowing liquids and solid foods were the most changed aspects.


RESUMO Objetivo: investigar o perfil e a percepção de fonoaudiólogos do Nordeste do Brasil, que atuam com crianças com Síndrome Congênita pelo Zika vírus, quanto aos aspectos miofuncionais orofaciais e aos procedimentos de intervenção fonoaudiológica. Métodos: trata-se de estudo transversal, observacional, com análise quanti-qualitativa. O instrumento de coleta de dados tratou-se de um questionárioon-line, que continha 34 questões abrangendo: dados sociodemográficos e questões voltadas para a percepção de fonoaudiólogos no que se refere ao assunto estudado. A amostra foi composta por 23 fonoaudiólogos dos nove estados do Nordeste. Resultados: da amostra, 96% foram mulheres. A maioria dos profissionais apresentava mais de 3 anos de experiência com a Síndrome Congênita pelo Zika Vírus. Na caracterização do sistema estomatognático, os profissionais descreveram maior grau de alteração nos aspectos de postura e mobilidade da musculatura orofacial. Em relação às funções, observou-se maior percepção de alteração na fala e deglutição para líquido e sólido. Conclusão: a maioria dos fonoaudiólogos da pesquisa possuem especialização, com destaque nas áreas de Disfagia e Motricidade Orofacial. Os profissionais afirmaram que a postura e mobilidade da musculatura orofacial são os aspectos mais alterados, assim como as funções de fala e deglutição de sólido e líquido.

ABSTRACT Purpose: to describe feeding characteristics of children with microcephaly, encompassing the feeding route, food consistencies, and utensils used to feed them. Methods: a descriptive study approved by the Research Ethics Committee, including 34 caregivers of children diagnosed with microcephaly. They answered a structured interview on the children's feeding route, breastfeeding, utensils used to feed them, and the food consistencies, besides sociodemographic and overall development data. Descriptive statistical analyses were performed, presenting absolute and percentage frequencies and measures of central tendency and dispersion. Results: the 34 caregivers in the sample were responsible for 34 children born between 2015 and 2018, diagnosed with microcephaly, due to various causes. Of these, 33 (97.02%) were on oral feeding in their first year of life - 26 (76.44%) on breastfeeding, nine of which (26.46%) exclusively so until 6 months old. The children were served food in baby bottles, cups, spoons, and syringes. Pureed food was the consistency most accepted by the children. Conclusions: there was a low prevalence of exclusive breastfeeding until they were 6 months old, delayed progress of consistencies, use of utensils inadequate to their age, and necessary adaptations to feed children, orally.


RESUMO Objetivo: descrever características relacionadas à alimentação de crianças com microcefalia, considerando via de alimentação, consistência e utensílios utilizados. Métodos: estudo descritivo, aprovado pelo Comitê de Ética em Pesquisa. Participaram 34 cuidadores de crianças com diagnóstico de microcefalia. Foi realizada entrevista estruturada com os cuidadores quanto à via de alimentação, amamentação, utensílios para oferta e consistência do alimento, além de aspectos sociodemográficos e de desenvolvimento global. Foi realizada análise estatística descritiva, apresentada por meio de frequências absoluta e percentual, e medidas de posição e dispersão. Resultados: os 34 cuidadores que compuseram a amostra eram responsáveis por 34 crianças nascidas entre os anos de 2015 e 2018 com diagnóstico de microcefalia de causas diversas. Dessas, 33 (97,02%) alimentaram-se por via oral durante o primeiro ano de vida, sendo que 26 (76,44%) foram amamentadas, nove delas (26,46%) de forma exclusiva até o sexto mês. Para oferta do alimento foram utilizados mamadeira, copo, colher e seringa. A consistência alimentar mais aceita pelas crianças foi a pastosa. Conclusões: observa-se baixa prevalência de aleitamento materno exclusivo até o sexto mês, atraso na progressão de consistências, uso de utensílios inadequados para a idade e necessidade de adaptações para as crianças que se mantiveram em alimentação por via oral.

RESUMO Objetivo Sintetizar o estado do conhecimento científico sobre quais medidas do movimento do osso hioide durante a deglutição são obtidas pela ultrassonografia e como extraí-las. Estratégia de pesquisa A pergunta PECO e as combinações de descritores e palavras-chave foram formuladas nas bases de dados eletrônicas Medline/PubMed, EMBASE, Web of Science, Scopus and Lilacs. Critérios de seleção Foram incluídos os artigos que utilizaram a ultrassonografia para analisar as medidas de movimento do osso hioide durante a deglutição, independentemente do idioma, ano de publicação ou presença de alteração na deglutição. Análise dos dados Os artigos incluídos foram analisados quanto: ano, local do estudo, desenho do estudo, população, tamanho da amostra, equipamento utilizado, posicionamento do transdutor, medidas obtidas, método de extração e confiabilidade das medidas. Resultados Vinte e seis artigos cumpriram os critérios de elegibilidade. A medida mais frequente foi a de amplitude máxima do movimento, seguida de tempo e velocidade. Houve grande variabilidade quanto à população de estudo, equipamentos utilizados, posicionamento do transdutor e método de extração das medidas, não sendo possível estabelecer padronização. O nível de confiabilidade foi investigado em apenas oito artigos. Conclusão Amplitude, tempo e velocidade são as medidas do movimento do osso hioide durante a deglutição que podem ser obtidas por ultrassonografia. Não há padronização dos métodos de extração dessas medidas.


ABSTRACT Purpose To synthesize the scientific knowledge on which measurements of hyoid bone movement during swallowing are obtained by ultrasonography and how to extract these measures. Research strategies The PECO question and combinations of descriptors and keywords were formulated in the electronic databases Medline/PubMed, EMBASE, Web of Science, Scopus and Lilacs. Selection criteria Articles that used ultrasonography to analyze measurements of hyoid bone movement during swallowing were included, regardless of language, year of publication, or presence of deglutition disorders. Data analysis The included articles were analyzed for: year, study site, study design, population, sample size, equipment used, transducer positioning, measurements obtained, method of extraction, and reliability of measurements. Results Twenty-six articles met the eligibility criteria. The most frequent measurement was hyoid movement maximum amplitude, followed by time and velocity. There was great variability in the study population, equipment used, positioning of the transducer and method of extraction of the measurements. Thus, it was not possible to find a standard model to extract the measures. The level of reliability was investigated in only eight articles. Conclusion Amplitude, time and velocity are the measures of hyoid bone movement during swallowing that can be obtained by ultrasonography. There is no standardization on how to extract these measurements.

OBJECTIVE: To characterize the clinical practice of Brazilian speech-language-hearing therapists regarding voice therapy for older adults during the COVID-19 pandemic. METHODS: Cross-sectional survey conducted remotely. Data were collected through a form shared online with approximately 1.500 speech-language-hearing therapists. The form included voice therapy practice with older adults during the COVID-19 pandemic. It was responded by 155 voice experts. RESULTS: Most respondents were females with over 21 years' experience in vocal health care, working with both in-person therapy and teletherapy. Obtaining acoustic parameters and using therapy strategies for breathing and body training were the most reported changes in remote therapy during the pandemic. The main difficulties involved wearing masks in in-person therapy and assessing the voice in teletherapy. Patient adherence and goals reached were deemed positive by most participants. Associations were found between place and format of service; between patient adherence and goals reached; and between difficulties in teletherapy and use of complementary therapeutic resources. CONCLUSION: The COVID-19 pandemic led Brazilian speech-language-hearing therapists to change their clinical practice with older adults in both remote and in-person therapy. The main changes involved wearing masks in in-person therapy and assessing the voice in teletherapy. Remote therapy proved to be a safe and effective possibility.

INTRODUCTION: Infrared thermography has the potential to help assess human head and neck muscles, as thermal variation measurements due to muscle imbalance can contribute to the evaluation and therapeutic follow-up of various clinical conditions in the field of health. OBJECTIVE: This study investigated, with a scoping review, the use of infrared thermography in complementary assessments of human head and neck muscles to determine the extent of research on the topic, what methodologies are used in thermal assessment and thermographic analysis, what regions are assessed, and what results are expected in thermography. METHODS: LILACS, MEDLINE, SciELO, and Web of Science were the databases searched to identify articles published on the topic, with no restriction of language or time of publication. Descriptive, analytical, and experimental observational studies on the assessment of the human head and neck muscle surface temperature with infrared thermography were included. Case studies, case series, methodological accuracy, literature review, animal studies, studies that assessed patients with head and neck cancer, and studies that did not assess head and neck muscle temperature with thermography were excluded. The articles were analysed with a protocol developed by the authors, with data on author, year, country, type of study, sample characterisation, muscles assessed, outcomes investigated, thermal assessment methodology, thermographic analysis methodology and thermography measurements. RESULTS: This review identified and analysed 27 articles. The studies assessed thermal distribution in normal individuals and those with pathologies related to hyper- and hypofunctional head and neck muscle conditions for diagnosis or therapeutic follow-up. The masseter, temporal, digastric, anterior cervical region, orbicularis oris, frontalis, buccinator, suprahyoid, trapezius, sternocleidomastoid and levator scapulae muscles were assessed. Quantitative analyses with area selection tool predominated, considering absolute temperatures and temperature differences. The studies investigated temperature and its relationship with myogenic pain, quantitative assessment of muscle parameters and blood flow velocity. The mean temperature ranged from 32.97°C (±2.21) to 34.90°C, and hyper-radiant and/or asymmetric regions were observed in hyperfunctional conditions and normal subjects after muscle activation. CONCLUSION: Thermography is used in complementary assessments of head and neck muscles, identifying hyper-radiant regions and thermal asymmetry related to muscle tension and activation state. Papers are limited to specific clinical conditions and few muscle groups, besides having great methodological variability.
